After a review of terminology, I identify—in addition to Margaret Battin’s list of five primary arguments for and against aid-in-dying—the argument from functional equivalence as another primary argument. I introduce a novel way to approach this argument based on Bernard Lonergan’s generalized empirical method. Then I proceed on the basis of GEM to distinguish palliative sedation, palliative sedation to unconsciousness when prognosis is less than two weeks, and foregoing life-sustaining treatment from aid-in-dying. I conclude that aid-in-dying must be justified on (...) its own merits and not on the basis of these well-established palliative care practices; and that societies must decide, in weighing the merits of aid-in-dying, whether or not to make the judgment that no life is better than life-like-this part of their operative value structure. The status of risk factors and disease remains a disputed question in the theory and practice of medicine and healthcare, and so does the related question of delineating disease boundaries. I present a framework based on Bernard Lonergan’s account of emergent probability for differentiating (1) generically distinct levels of systematic function within organisms and between organisms and their environments and (2) the methods of functional, genetic, and statistical investigation. I then argue on this basis that it is possible to understand (...) disease in terms of biological or higher intra-level dysfunction, risk factors—including genetic risk factors—in terms of statistical inter-level conditioning of a given stage or developmental sequence of systematic functioning, and the empirical boundaries of disease in terms of the limits of both functional categorization (from an epistemic standpoint) and upper-level integration of lower-level processes and events (from an ontological standpoint). This paper represents a preliminary investigation relating Bernard Lonergan’s thought to health science and the healing arts. First, I provide background for basic elements of Lonergan’s theoretical terminology that I employ. As inquiry is the engine of Lonergan’s method, next I specify two questions that underlie medical insights and define several terms, including health, disease, and illness, in relation to these questions. Then I expand the frame of reference to include all disciplines involved in the cycle of clinical interaction under (...) the heading health science and the healing arts. Finally, I analyze the cycle of clinical interaction in terms of Lonergan’s cognitive theory. I compare and contrast my analysis, based on Lonergan, with that of Pellegrino, Thomasma and Sulmasy as I proceed. In closing, I comment briefly on the next stage of this project regarding Lonergan’s theory of the human good in relation to the practice of the healing arts. (shrink)

Defining disease and delineating its boundaries is a contested area in contemporary philosophy of medicine. The leading naturalistic theory faces a new round of difficulties related to defining a normal environment alongside normal organismic functioning and to delineating a discrete boundary between risk factors and disease. Normative theories face ongoing and seemingly intractable difficulties related to value pluralism and the problematic relation between theory and practice. In this article, I argue for an integral—as opposed to a hybrid—philosophy of health based (...) on Bernard Lonergan’s notion of generalized empirical method that provides a way to settle these difficulties dynamically and comprehensively, both in theory, by orienting functional and statistical investigation toward an explanatory ecological viewpoint, and in practice, by framing critiques in relation to the normativity intrinsic to all human inquiry. In this introduction to a special subsection of Theoretical Medicine and Bioethics comprising separate reviews of the Springer Handbook of the Philosophy of Medicine, The Routledge Companion to Philosophy of Medicine, and The Bloomsbury Companion to Contemporary Philosophy of Medicine, I compare the three texts with respect to their overall organization and their approach to the relation between the science and the art of medicine. I then indicate two areas that merit more explicit attention in developing a comprehensive philosophy of (...) medicine going forward: health economics and systematic relations within the field as a whole. After considering two of Pellegrino’s papers that address the relation between philosophy of medicine and medical ethics, I identify several overarching problems in his account that revolve around his self-described essentialism and the lack of a systematic attempt to relate clinical medicine to biomedicine and public health. I address these from the critical realist position of Bernard Lonergan, who grounds both metaphysics and ethics on the normative structure of human inquiry and seeks to understand historical development, such as we are (...) witnessing in health science and health care, in terms of the dynamic structure of the human good. I conclude that Lonergan’s generalized empirical method and hierarchical account of world order provide a potentially dynamic framework on which to build a more comprehensive philosophy of medicine than one whose foundations rest primarily on a phenomenology of the clinical encounter and the telos of medicine. Lonergan’s treatment of transcendental method in the first chapter of <Method in Theology> presents a bit of a puzzle. Something about heightening consciousness at the level of experience is different from the reflexive operations by which we objectify this heightened experience. Lonergan’s summary statement of transcendental method makes no explicit reference to what this difference is. In this paper, I work out an interpretation of transcendental method in which I relate the problem of being explicit about heightening consciousness at the (...) level of experience to the problem of objectifying the subject-as-subject: both are a matter of performance. In this regard I identify a performative mode of subjective operation – in addition to the direct and introspective modes that Lonergan identifies in <Insight> – and develop an account of this mode of operation as it is manifested experientially in feelings and existentially in action. I relate the notion of feelings as data of consciousness to Lonergan’s account of the unity-in-tension of human consciousness, various forms and degrees of tension being the primary feeling-states of conscious experience. Finally, I note the significance of transcendental method in action with regard to understanding the subtleties of subject-to-subject communication in the encounter of patient and clinician as part of a philosophy of health on which I am working. The unfinished nature of Beauchamp and Childress’s account of the common morality after 34 years and seven editions raises questions about what is lacking, specifically in the way they carry out their project, more generally in the presuppositions of the classical liberal tradition on which they rely. Their wide-ranging review of ethical theories has not provided a method by which to move beyond a hypothetical approach to justification or, on a practical level regarding values conflict, beyond a questionable appeal to (...) consensus. My major purpose in this paper is to introduce the thought of Bernard Lonergan as offering a way toward such a methodological breakthrough. In the first section, I consider Beauchamp and Childress’s defense of their theory of the common morality. In the second, I relate a persisting vacillation in their argument regarding the relative importance of reason and experience to a similar tension in classical liberal theory. In the third, I consider aspects of Lonergan’s generalized empirical method as a way to address problems that surface in the first two sections of the paper: (1) the structural relation of reason and experience in human action; and (2) the importance of theory for practice in terms of what Lonergan calls “common sense” and “general bias.”. Although environmental philosophy and the human exploration of space share common beginnings, scholars from either field have not given adequate attention to the possible connections between them. In this essay, we seek to spur the rapprochement and cross-fertilization of philosophy and space policy by highlighting the philosophic dimensions of space exploration, pulling together issues and authors that have had insufficient contact with one another. We do so by offering an account of three topics: planetary exploration, planetary protection and the search (...) for extraterrestrial life, and terraforming. The resulting synthesis seeks to change our thinking about earthbound environmental ethics as it considers the philosophical dimensions of space exploration, and introduces the possible benefits of a humanities-oriented approach to space policy. Thomas Hofweber argues that the thesis of direct reference is incompatible with physicalism, the claim that the nonphysical supervenes on the physical. According to Hofweber, direct reference implies that some physical objects have object-dependent properties, such as being Jones’s brother, which depend on particular objects for their existence and identity. Hofweber contends that if some physical objects have object-dependent properties, then Local-Local Supervenience (the physicalist doctrine on which he concentrates) fails. Various philosophers have wanted to move from a (P.F.) “Strawsonian” understanding of the “practices of moral responsibility” to a non-skeptical result. I focus on a strategy moving from a “response-dependent” theory of responsibility. I aim to show that a key analogy associated with this strategy fails to support a compatibilist result. It seems clear that nothing could show that nothing we have been laughing at has really been funny. If “the funny” is similar to “the blameworthy”, then perhaps it would (...) follow that nothing could show that no one we have ever blamed has really been blameworthy. The comparison is interesting, but inconclusive: even if we grant that these properties are normatively similar, the latter has substantive empirical presuppositions, whereas the former does not. One important upshot: even if the standards operative in our practices cannot be mistaken, it could nevertheless be that no one has ever met these standards Ruling out this possibility seemingly requires re-engaging in all of the so-called "metaphysical" debates Strawsonians are keen to avoid. Moral latitude is only ever a matter of coincidence on the most popular decision procedure in the literature on moral uncertainty. In all possible choice situations other than those in which two or more options happen to be tied for maximal expected choiceworthiness, Maximize Expected Choiceworthiness implies that only one possible option is uniquely appropriate. A better theory of appropriateness would be more sensitive to the decision maker’s credence in theories that endorse agent-centred prerogatives. BackgroundWe review original papers on ovarian-hormone status in two areas of emotional processing: facial emotion recognition and emotional memory. Ovarian-hormone status is operationalized by the levels of the steroid sex hormones 17β-estradiol and progesterone, fluctuating over the natural menstrual cycle and suppressed under oral contraceptive use. We extend previous reviews addressing single areas of emotional processing. Moreover, we systematically examine the role of stimulus features such as emotion type or stimulus valence and aim at elucidating factors that reconcile the inconsistent (...) results.MethodsWe followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and included papers published until September 2020 indexed in PubMed and Web of Science databases. Search terms were MeSH terms AND AND with representing our separately searched areas, resulting in, and. To be included, articles had to be written and published in English, examine healthy, non-pregnant adult women in their reproductive age, and measure or at least estimate levels of E2 and P4. In PubMed, the search was limited to humans and to the search term present in the title or abstract.ResultsFeatures of the provided stimulus material constitute a relevant influence that interacts with E2- and P4-related ovarian-hormone status. For instance, recognition of basic emotions appears to be more related to P4- than E2-levels. Quite consistent, OC intake was accompanied by impaired recognition accuracy of basic and also complex emotions, although not in a recent large-sample study assessing complex emotions. Memory recall of negative content was mainly enhanced by P4, especially after having been stressed.Discussion and ConclusionWe document the methodological diversity in the field, presumably contributing to the heterogeneity of results. More studies explicitly contrasting the early follicular phase, mid-cycle phase, mid-luteal, and OC intake while standardizing tasks are needed. Research would take advantage of using within-subject designs and accounting for the recognition of complex emotions. (shrink)

Inductively defined structures are ubiquitous in mathematics; their specification is unambiguous and their properties are powerful. All fields of mathematical logic feature these structures prominently: the formula of a language, the set of theorems, the natural numbers, the primitive recursive functions, the constructive number classes and segments of the cumulative hierarchy of sets. -/- This dissertation gives a mathematical characterization of a species of inductively defined structures, called accessible domains, which include all of the above examples except the set of (...) theorems. The concept of an accessible domain comes from Wilfried Sieg's analysis of proof-theoretic practices, starting with his dissertation. In particular, he noticed the special epistemological character of elements of an accessible domain: they can always be uniquely identified with their build-up. Generally, the unique build-up of elements justifies the principles of induction and recursion. -/- I use category theory to give an abstract characterization of accessible domains. I claim that accessible domains are all instances of initial algebras for endofunctors. Grounded in the historical roots of Sieg's discussions, this dissertation shows how the properties of initial algebras for endofunctors and accessible domains coincide in a satisfying and natural way. -/- Filling out this characterization, I show how important examples of accessible domains fit into this broad characterization. I first characterize some accessible domains by relatively simple functors (e.g. finite, polynomial, those that preserve certain colimits) where we can see how iterating the functor can produce the accessible domain. Then I describe accessible domains that result from more involved specifications (e.g. ordinals and segments of the cumulative hierarchies associated with CZF, IZF, and ZF) by relying heavily on algebraic set theory. I end with a discussion of some of the methodological features of category theory in particular that helped characterize accessible domains. (shrink)
