Mental health problems start early in life. However, the majority of adolescents fulfilling the criteria for mental health disorders do not receive treatment, and half of those who do get treatment drop out. This begs the question of what differentiates helpful from unhelpful treatment processes from the perspective of young clients. In this study, we interviewed 12 young people who entered mental health care reluctantly at the initiative of others before the age of 18. Their journeys through mental health (...) care varied significantly despite sharing the same starting point. Our analyses resulted in a model of three trajectories. We describe relational and structural facilitators and obstacles within each trajectory and have formulated narratives highlighting core experiences differentiating them. Trajectory 1 was characterized by a rapid loss of hope, leading the adolescents to conclude that mental health care was not worth the investment. Trajectory 2 was characterized by a lingering hope that never materialized into a constructive therapeutic process despite prevailing efforts by both therapists and adolescents. Trajectory 3 was characterized by genuine meetings, allowing the therapist to transform from an unsafe stranger into a safe, competent, and benevolent adult. We discuss how our results have implications for understanding agency displayed by adolescent clients in therapy, therapist flexibility and authenticity, service organization, and attributional processes influencing clinical judgment and therapeutic processes when adolescent psychotherapy has a difficult starting point. In this article several signs of life present in the Shroud of Turin are pointed out. Following the development of rigor mortis, the body posture of the image on the Shroud is analyzed. This, together with the presence of specific facial folds indicate that the person wrapped in it is alive. Therefore, the image on the Shroud of Turin shows both signs of death and life in a person whose image was imprinted when he was alive. (...) If this is a fraud case, it should be considered as an artwork performed by a genius with medical, forensic and image processing knowledge from at least the XX century. But if we read the Gospels, a remarkable symmetry is found between the data obtained from the image and the events described in the Gospels, both regarding the death and resurrection of Jesus. This article applies some conceptual tools from semiotics to better understand the disastrous impacts of the world economy on global ecology. It traces the accelerating production of material disorder and waste to the logic of the money sign, as economic production processes simultaneously increase exchange-values and entropy. The exchange of indexical and iconic signs is essential to the dynamics of ecological systems and the proliferation of biological diversity. The human species has added a third kind of sign, the symbol, (...) and more recently a fourth: all-purpose money. Money does not refer to any referent either through contiguity, similarity, or convention. It is an empty sign, capable of assuming any significance that its owner attributes to it. The article argues that the concept of symbolic reference should be restricted to cultural and linguistic phenomena. Money qualifies as a new species of sign based on its exceedingly open mode of reference. It does not refer to any object by social convention but owes its specific properties precisely to the absence of such conventions. The logic of money pivots on decontextualisation: it presupposes and encourages the detachment of exchange values, people, and concepts from the particular and the local. Selective advantage is no longer primarily about calibration within local contexts, but increasingly a matter of transcending or emancipating oneself from the specific. This drift toward decontextualisation reverses the evolution of complexity and diversity throughout the planetary biosphere. (shrink)

Biobanks act as the custodians for the access to and responsible use of human biological samples and related data that have been generously donated by individuals to serve the public interest and scientific advances in the health research realm. Risk assessment has become a daily practice for biobanks and has been discussed from different perspectives. This paper aims to provide a literature review on risk assessment in order to put together a comprehensive typology of diverse risks biobanks could potentially face. (...) Methodologically set as a typology, the conceptual approach used in this paper is based on the interdisciplinary analysis of scientific literature, the relevant ethical and legal instruments and practices in biobanking to identify how risks are assessed, considered and mitigated. Through an interdisciplinary mapping exercise, we have produced a typology of potential risks in biobanking, taking into consideration the perspectives of different stakeholders, such as institutional actors and publics, including participants and representative organizations. With this approach, we have identified the following risk types: economic, infrastructural, institutional, research community risks and participant’s risks. The paper concludes by highlighting the necessity of an adaptive risk governance as an integral part of good governance in biobanking. In this regard, it contributes to sustainability in biobanking by assisting in the design of relevant risk management practices, where they are not already in place or require an update. The typology is intended to be useful from the early stages of establishing such a complex and multileveled biomedical infrastructure as well as to provide a catalogue of risks for improving the risk management practices already in place. At the core of medicine is the idea to help fellow human beings by improving or even restoring their health. Let us call this the auxiliary stance of medicine—the motivation of medical intervention by reference to a moral obligation to guide our peers in their attempt to live a healthy and productive life. In parallel, the auxiliary stance is also central to public health, with a focus on prevention and health promotion. Taken together, we can view medicine and public (...) health as the two main human auxiliary endeavors to protect individuals and populations from health risks and help them to heal when sick. BackgroundCommonly applied measures of symptoms of anxiety are not sensitive to disease-specific anxiety in patients with chronic obstructive pulmonary disease. There is a need for validated instruments measuring COPD-specific anxiety. Therefore, we translated the COPD-Anxiety Questionnaire into Danish and performed an initial validation of the psychometric properties in a sample of patients with COPD.Materials and MethodsTranslation procedures followed the World Health Organization guidelines. Participants with COPD completed questionnaires measuring COPD-specific anxiety, general psychological distress as well as variables related to COPD, (...) quality of life, and socio-demography.ResultsA total of 260 patients with COPD completed questionnaires. The Danish version of CAF-R-DK demonstrated acceptable Cronbach’s α values that were comparable with those of the original CAF. As expected, the CAF-R-DK showed positive correlations with convergent constructs and negative correlations with discriminant constructs. However, the results for specific subdomains of the CAF-R-DK indicated inconsistency in the underlying concept of disease-specific anxiety, which was also suggested based on the subsequent confirmatory and exploratory factor analyzes.ConclusionThe CAF could serve as an important supplement to generic psychological distress screening of patients with COPD in somatic health care settings, and the questionnaire is now available in Danish. Translation into other languages is needed with the purpose of obtaining data for further testing the psychometric properties of the questionnaire. (shrink)

Looking to Scripture through the eyes of contemporary medical experience, I analyze the meaning of the criteria used for determining death, specifically in the light of Jesus’ final moments and the resurrection of the Shunammite’s son in 2 Kings, chapter 4. I argue that four theses are consistent with, and informed by, these passages that can help guide Christian belief and decision-making about how death is determined in the clinical context: (1) death is neither permanent nor irreversible; (2) something like (...) the “brain dead” state is, at best, a confounding state that requires one to “pace and pray” or let go; (3) that the case for determining death by neurologic criteria depends on the “working togetherness” of the body's parts for the sake of impacting its environment; and (4) that the practice of neurologically-based death determination is a response to the problem of disaggregation of the human form into its organ systems that modern critical care medicine makes possible. I end with advice about how Christians might approach the debates over the law and practice even if they cannot come to a consensus. (shrink)

This paper offers a counterpoint to the prevailing account of waste in the human sciences. This account identifies waste, firstly, as the anomalous product of arbitrary social categorizations, or ‘matter out of place’, and, secondly, as a distinctly human way of leaving behind and interpreting traces, or a mirror of culture. Together, these positions reflect a more or less constructivist and anthropocentric approach. Most commonly, waste is placed within a framework that privileges considerations of meaning over materiality and the threat (...) of death over the perpetuity of life processes. For an alternative I turn to bio-semiotics and cross-species scholarship around the question of the animal. Specifically, the paper asks what theories of waste would look like if instead of taking ‘dirt’ as their starting point, they began with trans-species encounters with animal scat. Following bio-semiotics and efforts to deconstruct the animal/human binary, it is suggested that the objectual forms commonly referred to as ‘waste’ are not arbitrarily classified but purposefully expended, and thus symptomatic of life’s spatio-temporal continuation. Waste matter, therefore, is best construed not as anthropocentric but as semi-biotic: a sign of the form of life to which it once belonged. This alternative perspective has implications for how approaches to industrial forms of mass waste can be reconceived. (shrink)

Summary The analysis of cultural artefacts, such as cartoons and films, provides the potential to gain insights into both the professional identities of teachers and the behaviour and self-concepts of students. This paper suggests that the ostensibly banal animated cartoon characters Beavis and Butt-head offer some utility in this respect. The paper explores aspects of the stereotypes and behaviour represented in the Beavis and Butt-head series and briefly discusses some possible interpretations of these images.

Modern medicine provides unprecedented opportunities in diagnostics and treatment. However, in some situations at the end of a patient’s life, many physicians refrain from using all possible measures to prolong life. We studied the incidence of different types of treatment withheld or withdrawn in 6 European countries and analyzed the main background characteristics.

“Black Star” music video was released two days before Bowie’s death. It bears various implications of dying and the notion of mortality is both literal and metaphorical. It is highly autobiographical and serves as a theatrical stage for Bowie to act both as a music performer and as a self-conscious human being. In this paper, we discuss the signs of mortality in Bowie’s “Black Star” music video-clip. We focus on video’s cinematic techniques and codes, on its motivic elements and (...) on its narrative in relation with music, lyrics, characters, and gestures. We also discuss the video’s intertextual references and the broader signification of the black star figure. We adopt a quasi-semiotic approach considering “Black Star” music video-clip as a text which can be investigated through its signs, codes, and conventions of the musical, visual, and cinematic languages as well. Our interdisciplinary tools derive from visual semiotics and audiovisual analysis models, without leaving outside Bowie’s musical-artistic and personal history. As it turns out, Bowie created a video clip that is philosophical in nature and poetic in structure, preserving the role of protagonist. With the visuals creating a psychedelic atmosphere, the lyrics often are heard as a personal confession. They both generate cognitive and emotional responses that influence the way the viewers-listeners may experience, decompose, and interpret Bowie’s artistic endeavor bridging life and death. (shrink)

Objectives: The objective of this study is to estimate the proportion of different types of end‐of‐life decisions (ELDs) of physicians in the city of Hasselt (Flanders, Belgium). The question is addressed to what degree these ELD meet legal constraints and the ethical requirements for prudent practice. Methodology: All physicians of the city of Hasselt who signed at least one death certificate in 1996 (N=166) received an anonymous self‐administered mail questionnaire per death case (max. 5/doctor) Results: the response rate was (...) 55% (N=269). In 37.3% of all cases at least one ELD was made (16.5% non‐treatment decisions; 16% potential life‐shortening by intensifying the treatment of pain and symptoms; 4.8% administration, supply or prescription of lethal drugs). In 59.5% of the cases were an ELD was made that decision was legally questionable. Patient characteristics were clearly related to the type of ELD. There was no influence of physician characteristics, except for commitment to life‐stance. In 71.3% of the cases the ELD was in no way discussed with the patient. 8.1% of the ELD‐cases were in response to a direct request from the patient Conclusions: The incidences of ELDs in Hasselt are consistent with earlier findings. The study shows that religious commitment influences the behaviour of physicians at the end of their patients' life. The patient's and her family's entitlements to participation in the decision making process were rather poorly respected. (shrink)

The dialectical-theological origins of the politically- and ethically-charged concept of alterity are well-known within the philosophy of religion. Intellectual histories of this concept tie it too exclusively to the notion of distance or διάστασις in Karl Barth’s early Römerbrief, however, and so miss Barth’s Trinitarian reinterpretation of God’s otherness in his later work. Taking as my hermeneutical key a cipher, the ‘sign of Jonah,’ that emerges in Church Dogmatics IV/1, I show that Barth’s mature doctrines of temporality and filiation understand (...) alterity as a moment of divine life. Jesus’ agony in the garden of Gethsemane marks the climax of Barth’s self-reinterpretation: world history inheres within the Christological situation of paternal abandonment. The political-theological conclusions Barth draws from the ‘sign of Jonah’ dovetail with alterity discourses’ antitotalitarian aspirations but suggest that these aspirations’ structural coherence rest on the magisterial Reformers’ Christological and ecclesiological commitments. The creative imagination changes itself and the world in ways we cannot anticipate. This restless creativity gathers not just refutable facts; it hunts self-transforming revelations, semiotic prizes acclaimed and defended in the realms of inner awareness and political power. So doing, it eludes final description in any one set of signs. This means, I argue here, that sign systems must themselves give chase. Texts of this kind will not be the fixed embalmed arrays of signs and symbols that (...) have sustained our civilizations to date. These texts will be a new kind of artificial life, creative agents that rewrite and re-express themselves on-the-fly, in ways unpredictable by their founding authors. The deconstructive turn in postmodern theory sought interpretive vistas and strange loops to be teased, once upon a time, from each embalmed text. Beyond postmodernity is an aesthetics and a critical theory of empowered texts — sign arrays never quite the same and that, like time itself, sit still for no one. After an initial push by us, they self-evolve. Human sociobiology, asking how culture rewrites the agenda of replicating genes, provides a first glimpse of this new adventure in semiotics and human self-discovery. (shrink)

He shows us that the history of medicine is in no small part the history of learning that fewer and fewer things are somebody's "fault"; for example, for centuries we thought seizures were a sign of demonic possession. Yet, as he acknowledges, it's very hard, and at times impossible, to uncouple from our zeal to judge others and to judge ourselves. Sapolsky applies the new understanding of life beyond free will to some of our most essential questions around punishment, morality, and living well together. This interpretation lends itself to a functional analysis of the material put to use and whose operations jointly express the concept we are after. Modes of expression can be plotted between a rhetorical and semantic axis (Gates 48). As such, "[t]he level of connotation of the visual sign, of its contextual reference and positioning in different discursive fields of meaning and association, is the point where already coded signs intersect with the deep semantic codes of a culture and take on additional more active ideological dimensions" (Hall 56). Coordinates within this domain encode signifying practices, revitalizing the traditional concept of signification as a function of what's signified with respect to a signifier. The content of a syntagm in this coordinate system, a point in this plane, indexes its function with respect to others. As such, we will conceive of the content of modes of expression functionally.A function is defined by pairing an element from a domain and another from a sub/co-domain whereby that pair is an element of that function. In this way, we can treat functions as abstract objects. If we treat a domain as what determines the availability of an expression, and a sub/co-domain as representative of its range of application, then the function of applying some proposition maps a movement from a domain of selection to one of composition, that is, its use with respect to others. The function of an expression, then, represents an operation over the possible worlds of its appropriate application (Taylor 201–02, 269, 276). With that in mind, let us consider a coordinate system where there are two perpendicular axes, and we define positions within the space provided at the intersection of lines drawn from each. Each axis is represented by an ordered series of markers. A line drawn from each axis indicates that the coordinates indexed at their intersection is a function of these two markers. This locates an individual at a point in that domain, indexing a relation between the axes framing that space. Therefore, that coordinate-wise space, here a domain of possible expression, is not "empty" as pessimists would claim, but is filled with functional-content, albeit functions yet applied and marked by a point in that plane.Although we are currently speaking of two-dimensional space, the same follows for as many dimensions as we want. Recent advances in life-sustaining treatments and technologies, have given rise to newly-emerged, critical and sometimes, controversial questions regarding different aspects of end-of-life decision-making and care. Since religious values are among the most influential factors in these decisions, the present study aimed to examine the Islamic scholars' views on end-of-life care. A structured interview based on six main questions on ethical decision-making in end-of-life care was conducted with eight Shiite experts in Islamic studies, and was analyzed through (...) deductive content analysis. Analysis revealed certain points in Islamic views on the definition of death and the persons making decisions about end-of-life care. According to the participants, in addition to conventional criteria such as absence of heartbeat and respiration, the irreversible cessation of human voluntary acts are considered to be the criteria in establishing death. The participants also recognized physicians as the main authorities in verifying signs of death. Furthermore, it was emphasized that life preservation and continuation of care must be sensible, and the patient can request not to have death-prolonging procedures started or continued. In the view of participants, patient's autonomy cannot be the sole basis for all measures, but Islamic ethical and jurisprudential principles should be relied upon to make correct and sensible decisions whether to continue or stop terminal patients' care. Final decisions should be made by a team of experts, and physicians must be at the center of such a team. Finally, we suggest that a guideline in keeping with Islamic norms on human life and death, purpose of life, God's will, boundaries of man's authority, and the physician's ethical duties and obligations should be developed. We describe how a Community-Based Participatory Research (CBPR) process was used to develop a means of discussing end-of-life care needs of Deaf seniors. This process identified a variety of communication issues to be addressed in working with this special population. We overview the unique linguistic and cultural characteristics of this community and their implications for working with Deaf individuals to provide information for making informed decisions about end-of-life care, including completion of health care directives. Our research and our (...) work with members of the Deaf community strongly show that communication and presentation of information should be in American Sign Language, the language of Deaf citizens. We discuss how such operation like reading, writing, sign, interpretation, etc., can be applied in the realm of the living and what can be gained, by such an approach, in order to understand the phenomenon of life.

This book presents programmatic texts on biosemiotics, written collectively by world leading scholars in the field (Deacon, Emmeche, Favareau, Hoffmeyer, Kull, Markoš, Pattee, Stjernfelt). In addition, the book includes chapters which focus closely on semiotic case studies (Bruni, Kotov, Maran, Neuman, Turovski). According to the central thesis of biosemiotics, sign processes characterise all living systems and the very nature of life, and their diverse phenomena can be best explained via the dynamics and typology of sign relations. The authors are (...) therefore presenting a deeper view on biological evolution, intentionality of organisms, the role of communication in the living world and the nature of sign systems - all topics which are described in this volume. Introduction. Cities play a key role in the development of countries and regions. The authors of the article emphasize the importance of the largest cities’ development, which is based on an industrial model that has not exhausted its potential. The authors show possibilities of urban development on the basis of the industrial sector effective functioning in order to improve the citizens’ welfare, meet their needs and improve the life quality. In this regard, the authors formulate a hypothesis that the (...) largest Russian cities developing according to the industrial model have and are able to realize their potential in order to improve the life quality, preserve and increase the population on their territory. The authors set a goal: to identify the possibilities for developing industrial-type megalopolises to improve the population’s life quality of the (PLQ). Methods. The authors use the following information base: strategic documents of different levels, statistical data, scientific literature, analytical documents, and expert assessments of the megalopolises’ development. The authors also use an industrial model for developing the largest cities; they determine conditions under which the potential of megacities can be realized to improve the citizens’ life quality and carry out an analysis of the demographic and social situation in the largest Russian industrial cities. The authors evaluate some approaches and theses of the study as exemplified by the following cities with a million-plus population: Yekaterinburg, Krasnoyarsk, Chelyabinsk, and Omsk and they group the cities according to the signs of growth and decline, which is illustrated by favorable or unfavorable demographic trends. To confirm the identified trends and problems of the largest cities, the authors provide the results of expert assessments on the population’s life quality and the state of the urban environment and emphasize the need for a systemic integrated development of the largest cities of industrial type to improve the life quality of the population of megalopolises. The scientific novelty of the study consists in substantiating the prospects for the socio-economic development of the largest industrial cities of Russia on the basis of an industrial model that provides for the technological transformation of the economy, the use of agglomeration effects, the formation of a friendly urban environment for residents and the achievement of socialization of urban space. Results and conclusions. As a result of the analysis and typology of the largest cities by functional specialization, two groups of cities were identified: one group with signs of demographic growth, the second with signs of decline. The cities of the first group have a greater economic potential, opportunities for agglomeration, provide more comfortable living and consolidation of the population on their territory. On the contrary, cities with signs of decreasing have fewer opportunities (resources), are less diversified, the urban environment in them is less comfortable, which is reflected in the citizens’ life quality and is accompanied by the population’s outflow. At the same time, it was revealed that all analyzed cities actively use digital technologies, realizing, in particular, the concept of a «smart city». At the same time, cities with signs of decreasing show high IQ indicators of cities. Taking into account the identified problems, the authors propose promising directions for the development of industrial megalopolises with signs of decline. (shrink)

Schrödinger's definition of life needs a slight modification to absorb the criticism of it. It is the comparison of the entropy level of a system before and after a process which makes one view it as living: we consider the stability of the deviation from the probable a sign of life. This explains why we do not hesitate to consider as remnants of living systems skeletons and fossils anywhere and physical culture on any archeological site.

The present paper discusses a topic often neglected by contemporary philosophy of biology: The relation between metaphorical notions of living organisms as information processing systems, the attempts to model such systems by computational means (e.g., Artificial Life research), and the idea that life itself is a computational phenomenon. This question has ramifications in theoretical biology and thedefinition of Iife, in theoretical computer science and the concept of computation, and in semiotics (the study of signs in the most (...) general sense, including information, signification, and meaning), and the concept of the interpreter. It is argued, that the theory of autopoietic systems known from theoretical biology should be integrated with a biosemiotic reflection on the natural history of signs. (shrink)

Background Nurses providing care to patients with end-of-life or terminal illnesses often encounter ethically challenging situations leading to moral distress. However, existing quantitative studies have examined moral distress using instruments that address general clinical situations rather than those specific to end-of-life care. Furthermore, qualitative studies have often been limited to participants from a single unit or those experiencing moral distress-induced circumstances. A comprehensive and integrated understanding of the overarching process of moral distress is vital to discern the unique (...) circumstances surrounding end-of-life care and its consequential impacts. Research objectives To explore the moral distress experiences of nurses who are frequently involved in caring for patients with end-of-life or terminal illnesses and apply it to two existing theories: the model of moral distress and the ecological model. Research design A qualitative descriptive approach was employed. Participants and research context Seven focus group interviews involving 30 nurses were performed. The subsequent transcriptions underwent rigorous content analysis. Ethical considerations We obtained Institutional Review Board approval from a university. Focus group interviews were conducted with nurses who agreed to participate and signed the consent form. Findings The moral distress-inducing factors and nurses’ perceived impact of moral distress were identified and categorized based on moral distress theories and ecological models. A total of 15 categories and 30 subcategories across the following 4 domains were derived: (1) intrapersonal, (2) interpersonal, (3) organizational, and (4) structural factors. Conclusions End-of-life-specific circumstances induced moral distress among nurses, with both negative and positive impacts identified. Effective organizational and policy support is essential to manage conflicts, form a healthy organizational culture, provide training, and prevent unnecessary expenses due to the negative consequences of moral distress. Sacrifice as a practice aimed at honoring deities by offering them something as a sign of propitiation or worship is usually studied from the viewpoint of numerous disciplines and religious cultures, from which equally numerous interpretations follow. However, the view of sacrifice as able to shape life in its entirety, which means that every act taken by believers may be seen in sacrificial terms, does not seem to be sufficiently considered. This is a view that I believe emerges from (...) various reflections on sacrifice, especially the ones offered by thinkers of the past such as Augustine and Aquinas. In this essay, I first focus on these reflections and more specifically on Augustine’s and Aquinas’s view that religious believers should order everything to God. I then argue—in the footsteps of Aquinas—that this view applies to important acts taken by believers. These acts are prayer, faith, and intellectual activity. (shrink)
