This paper begins with a simple illustration of the choice between individual and population strategies in population health policy. It describes the traditional approach on which the choice is to be made on the relative merits of the two strategies in each case. It continues by identifying two factors—our knowledge of the consequences of the epidemiological transition and the prevalence of responsibility-sensitive theories of distributive justice—that may distort our moral intuitions when we deliberate about the choice of appropriate (...) risk-management strategies in population health. It argues that the confluence of these two factors may lead us to place too much emphasis on personal responsibility in health policy. (shrink)

Through a study of the history of the concepts of wealth and poverty, this paper investigates the onset of a transition in the conceptual architecture of epidemiological research concerning social differences in mortality rates from 1858 to 1914. It raises the question as to what the concepts of wealth and poverty meant to those who used them and what objects of interventions the conceptual architecture surrounding the concepts enabled the researchers to create. It argues that a transition (...) began in the late 19th century in which an important framework for the understanding of causal relations behind the mortality patterns changed and that this change in turn influenced the scope of what was conceived as relevant objects of intervention. We study the SIS and SIRI epidemic models discussing different approaches to compute the thresholds that determine the appearance of an epidemic disease. The stochastic SIS model is a well known mathematical model, studied in several contexts. Here, we present recursively derivations of the dynamic equations for all the moments and we derive the stationary states of the state variables using the moment closure method. We observe that the steady states give a good approximation of the quasi-stationary states of the (...) SIS model. We present the relation between the SIS stochastic model and the contact process introducing creation and annihilation operators. For the spatial stochastic epidemic reinfection model SIRI, where susceptibles S can become infected I, then recover and remain only partial immune against reinfection R, we present the phase transition lines using the mean field and the pair approximation for the moments. We use a scaling argument that allow us to determine analytically an explicit formula for the phase transition lines in pair approximation. (shrink)

The nutrition transition—a process of dietary change that describes the shift to calorie-dense, higher fat and protein diets from cereal based ones—is happening in India. This paper argues that relatively little is known about the nature of nutrition transition in India. This is a result of both a lack of adequate and timely data and a consequence of national and state-level statistics, which render an incomplete and potentially misleading picture of how these processes are unfolding in local contexts. (...) This may be especially true in India where very little ethnographic research has documented the dual edges of nutrition transitions. Analyzing data collected from the Kumaon Hills, Uttarakhand in 2013, this paper suggests the ways in which aspects of the nutrition transition have developed unevenly over space and time. In particular, while new types of calorie dense foods have infiltrated these rural, remote areas, the process has been uneven and fraught with contestation due to preexisting social practices. More troubling is the evidence that though incomes are rising, the predicted increases in high value, protein rich foods may actually be declining. This paper concludes by arguing that the widely influential nutrition transition literature needs look to ethnographic and in-depth qualitative methods to form better policies relevant to the contingencies of dietary and epidemiological change. The demographic and epidemiological transitions are driving pharmaceutical expenditures up in Latin American and the Caribbean, with much of the cost falling on households. The domestic development and manufacturing of bio-similars could make medicines more affordable.

In this work, the evolution of demographic and health patterns in a Basque rural population from Spain is analysed, as they relate to progress in demographic and epidemiological transition. For this purpose, parochial record data on 13,298 births and 9215 deaths, registered during the 19th and 20th centuries (180090) resulting from cardiovascular diseases and malignant neoplasms (post-transition causes). This last point is in contrast with observations from the first four decades of the 20th century, when infectious diseases (...) and respiratory ailments were determining factors in mortality among this population. The data provided by the study of the variation over time in demographic and health patterns indicate that reducing the risk of mortality is one of the most important preconditions for fertility decline. This chapter focuses on documenting the changes in life expectancy and human lifespan and reviewing current understandings of demographic and epidemiological transitions. It allows assumption of shared understandings of the meaning of human aging and old age. A key policy indicator of changes in health and life expectancy is the concept of healthy active life expectancy and associated concept of disability‐free life expectancy. From a social gerontological perspective, aging is a concept that is less contested than the idea of (...) old age. The biomedicalization of aging has been a major influence across social spaces. Vincent has described four categories of anti‐aging medicine: symptom alleviation; life expectancy extension; lifespan extension; and abolition. The development of anti‐aging technology, science, and practice challenges us to think about the enhancement of human aging and the future of later life. Attitudes to aging remain essentially negative with little evidence to suggest that institutionalized ageism will disappear. (shrink)

35 million people die annually of non-communicable diseases (NCDs), 80% of them in low- and middle-income countries — representing a marked epidemiological transition from infectious to chronic diseases and from richer to poorer countries. The total number of NCDs is projected to rise by 17% over the coming decade, absent significant interventions. The NCD epidemic poses unique governance challenges: the causes are multifactorial, the affected populations diffuse, and effective responses require sustained multi-sectorial cooperation. The authors propose a range (...) of regulatory options available at the domestic level, including stricter food labeling laws, regulation of food advertisements, tax incentives for healthy lifestyle choices, changes to the built environment, and direct regulation of food and drink producers. Given the realities of globalization, such interventions require global cooperation. In 2011, the UN General Assembly held a High-level meeting on NCDs, setting a global target of a 25% reduction in premature mortality from NCDs by 2025. Yet concrete plans and resource commitments for reaching this goal are not yet in the offing, and the window is rapidly closing for achieving these targets through prevention — as opposed to treatment, which is more costly. Innovative global governance for health is urgently needed to engage private industry and civil society in the global response to the NCD crisis. (shrink)

Indigenous populations in New World nations share the common experience of culture contact with outsiders and a prolonged history of prejudice and discrimination. This historical reality continues to have profound effects on their well-being, as demonstrated by their relative disadvantages in socioeconomic status on the one hand, and in their delayed demographic and epidemiological transitions on the other. In this study one aspect of aboriginals’ epidemiological situation is examined: their mortality experience between the early 1980s and early 1990s. (...) The groups studied are the Canadian Indians, the American Indians and the New Zealand Maori (data for Australian Aboriginals could not be obtained). Cause-specific death rates of these three minority groups are compared with those of their respective non-indigenous populations using multivariate log-linear competing risks models. The empirical results are consistent with the proposition that the contemporary mortality conditions of these three minorities reflect, in varying degrees, problems associated with poverty, marginalization and social disorganization. Of the three minority groups, the Canadian Indians appear to suffer more from these types of conditions, and the Maori the least. (shrink)

Following Michel Bitbol, I suggest one way to mitigate this crisis of faith may be for science to become more epistemically modest, renouncing some of its claims to describe reality as it objectively is, and adopting an “intransitive” rather (...) than a “transitive” approach to Nature. This was the attitude adopted by the Greco-Roman medical school of the Empirics, which may also be of assistance in combatting the urgent problem of Fake News. The epistemology of Skepticism and the scientific methodology of the closely related Medical Empiricism shows that an epistemically modest, non-interventionist approach to science is quite compatible with a robust and sophisticated proto-experimental scientific methodology. Research findings on addiction are contradictory. According to biographical records and widely used diagnostic manuals, addicts use drugs compulsively, meaning that drug use is out of control and independent of its aversive consequences. This account is supported by studies that show significant heritabilities for alcoholism and other addictions and by laboratory experiments in which repeated administration of addictive drugs caused changes in neural substrates associated with reward. Epidemiological and experimental data, however, show that the consequences of drug consumption can (...) significantly modify drug intake in addicts. The disease model can account for the compulsive features of addiction, but not occasions in which price and punishment reduced drug consumption in addicts. Conversely, learning models of addiction can account for the influence of price and punishment, but not compulsive drug taking. The occasion for this target article is that recent developments in behavioral choice theory resolve the apparent contradictions in the addiction literature. The basic argument includes the following four statements: First, repeated consumption of an addictive drug decreases its future value and the future value of competing activities. Second, the frequency of an activity is a function of its relative (not absolute) value. This implies that an activity that reduces the values of competing behaviors can increase in frequency even if its own value also declines. Third, a recent experiment (Heyman & Tanz 1995) shows that the effective reinforcement contingencies are relative to a frame of reference, and this frame of reference can change so as to favor optimal or suboptimal choice. Fourth, if the frame of reference is local, reinforcement contingencies will favor excessive drug use, but if the frame of reference is global, the reinforcement contingencies will favor controlled drug use. The transition from a global to a local frame of reference explains relapse and other compulsive features of addiction. (shrink)

A central question of this chapter is how we can relate the unique ethos of Japan to the ways that influences of international bioethics, civil rights and legal reforms have shifted medical research in Japan from the legacy of the structured paternalism and impunity that allowed abuses to be committed by medical researchers in the World War II era, including in Unit 731 and in medical schools in Japan, to contemporary research agendas and policies. Throughout the twentieth century, Japan has (...) been a centre of research and innovation. The pro-technology attitudes still exist in current Japan, with the evolution of bioethics. Although we can trace the origins of bioethics in Japan back through millennia, with the Ainu indigenous practices and relationships to nature, the sophisticated hierarchical relationships expressed in the seventh century A.D. book the Kojiki, the conversion of forest to rice paddies, emergence of an integrated religious system including Shintoism and Buddhism, and the sophisticated class system supported by linguistics and social relationships, this chapter would argue that one of the most influential influence for this question is the Samurai tradition. This tradition values both autonomy and informed choice, while also allowing the legacy of tolerance of the elite classes to make decisions that may sacrifice some persons for the greater good. While principles of solidarity and social justice are a solid basis for universal health insurance they also allow sacrifices when necessary. Informed consent is now widely accepted, and bioethics is part of a transition which is transforming Japanese society from a paternalistic society to an individualistic one. Among the issues that will be used to illustrate the situation, are the functioning of research ethics committees, large gene bank and epidemiological projects, organ transplantation, truth telling, euthanasia and the public health measures such as mask wearing in the current COIVD-19 pandemic. Many European countries during the last northern summer will adopt mental health legislation that moves in the opposite direction to the United Nations Convention on Human Rights for persons with disabilities, (...) and will allow for compulsory mental health treatment. In this author’s home state of Victoria, Australia, the Mental Health Act has five criteria that must be met for someone to be made an involuntary patient in an institution or hospital, in the community, or in one’s own home (Victoria 2003, s.6). One of the criteria is that the person is judged to be at risk of danger to themselves or others. Once made an involuntary patient, one looses the right to refuse medical treatment. At present, in various parts of the world, medical practitioners are being required to assess a person’s level of dangerousness with immediate consequences for that person’s liberty.From the point of view of a historian of the human sciences, these events propel psychiatry into a dialogue with its recent past. The project of Western psychiatry is usually characterized as a breaking down of the walls of the old asylum that contained the lunatic in its dark recesses, and the subsequent unveiling of the mental patient thanks to the rational, liberatory practices of medicine. But modern psychiatry has also been shaped by the coming into being of particular conceptions of mental illness that are not all its own making. This entailed the removal over time of the mental defective and the mental deficient, the moral imbecile, the epileptic, the alcoholic, the chronically insane, and so on, from spaces in which psychiatry would seek to define it object—populations that psychiatry would determine to be “not amenable to treatment.” If we were to see this shaping in a Foucauldian sense, we would also want to include earlier eighteenth- and nineteenth-century separations of the lunatic and the criminal (Foucault 1965, 1979). Moreover, we might want to insist that the marking out of the modern mental health patient is first and foremost an administrative act—an act of separation and management within a bounded population—which then serves as a condition of possibility for the emergence of knowledges in psychiatric medicine, with the latter following rather than preceding the arrangement of bodies in the asylum/hospital.The psychiatrist in public medicine may well be encouraged to shape a new mission according to the requirements of an administration concerned with managing dangerousness. Let the individuals of a population be divided into two groups with different personal habits. The core group is associated with health risk behaviors; the non-core group avoids unhealthy activities. Assume that the infected individuals of the core group can spread a contagious disease to the whole population. Also, assume that cure does not confer immunity. Here, an epidemiological model written as a set of ordinary differential equations is proposed to investigate the infection propagation in this population. In the (...) model, migrations between these two groups are allowed; however, the transitions from the non-core group into the core group prevail. These migrations can be either spontaneous or stimulated by social pressure. It is analytically shown that, in the scenario of spontaneous migration, the disease is either naturally eradicated or chronically persists at a constant level. In the scenario of stimulated migration, in addition to eradication and constant persistence, self-sustained oscillations in the number of sick individuals can also be found. The transition economy of Vietnam enjoyed remarkable achievements in the first 20 years of economic renovation (Doi Moi) from 1986 to 2006. Notably, the economy grew at an average annual rate of 7.5% in 1991-2000 period. Vietnam’s Amended Constitution 1992 recognized the role of private sector in the economy. U.S.-Vietnam Trade Bilateral Agreement (US-BTA) was signed in 2001. The country's stock market made debut trading in 2000. In the unfolding debate on the prospects, challenges and viability of the imminent transition towards a ‘Bio-Based Society’ or ‘Bio-based Economy’—i.e. the replacement of fossil fuels by biomass as a basic resource for the production of energy, materials and food, ‘big’ concepts tend to play an important role, such as, for instance, ‘sustainability’, ‘global justice’ and ‘naturalness’. The latter concept is, perhaps, the most challenging and intriguing one. In public debates concerning biotechnological interactions with the natural environment, the use (...) of terms such as ‘nature’ and ‘naturalness’ is both inevitable and hazardous . Indeed, various conflicting interpretations of naturalness play a role on both sides of the current debate. This paper aims to analyse and critically assess the role of ‘nature-speak’ in the BBS transition. We will begin with a concise overview of the vicissitudes of the nature-concept so far, focussing on how modern science and technology have challenged and affected our understanding of what nature is. Subsequently, we describe how ‘naturalness’ functions in the unfolding BBS debate. Finally, we will focus on a particular case study, namely the production of rubber with the help of natural latex coming from dandelion plants rather than from rubber trees. On the one hand, this is presented as a more natural and nature-friendly way of producing rubber. On the other hand, it is a sophisticated process, involving high technology and primarily focussed on competitiveness on the global market. To what extent or in what sense can dandelion latex be regarded as more natural? And what can we learn from this case study when it comes to addressing naturalness in the broader conceptual and bio-political arena? (shrink)

Among the most influential and enigmatic thinkers of the modern age, Nietzsche and Heidegger have become pivotal in the struggle to define postmodernism. In this work, Gregory Smith offers the most comprehensive examination to date of the turn to postmodernity in the writings of these philosophers. Smith argues that, while much of postmodern thought is rooted in Nietzsche and Heidegger, it has ironically attempted, whether unwittingly or by design, to deflect their philosophy back onto a modern path. Other alternative paths (...) emanating from both Nietzschean and Heideggerian thought that might more powerfully speak to postmodern culture have been ignored. Nietzsche and Heidegger, Smith suggests, have made possible a far more revolutionary critique of modernity then even their most ardent postmodern admirers have realized. Smith contends that the influences on the postmodern in the thought of Nietzsche and Heidegger are founded in a new vision of praxis liberated from theory. Ultimately, these philosophers do transcend the nihilism often found in the guise of postmodernism. Their thought is, moreover, consistent with the possibility of limited constitutional government and the rule of law. Smith's book takes the first step toward recovering these possibilities and posing the fundamental questions of politics and ethics in ways that have heretofore been closed off by late-modern thought. There is a mystery at the heart of Plato's Parmenides. In the first part, Parmenides criticizes what is widely regarded as Plato's mature theory of Forms, and in the second, he promises to explain how the Forms can be saved from these criticisms. Ever since the dialogue was written, scholars have struggled to determine how the two parts of the work fit together. Did Plato mean us to abandon, keep or modify the theory of Forms, on the strength of Parmenides' (...) criticisms? Samuel Rickless offers something that has never been done before: a careful reconstruction of every argument in the dialogue. He concludes that Plato's main aim was to argue that the theory of Forms should be modified by allowing that forms can have contrary properties. Historically, there has been little agreement between advocates of radical financial reform and socialist theoreticians. However, in the new circumstances of the twenty-first century, a productive synthesis of these two traditions might be possible. Drawing on the franchise model of credit creation elaborated by Robert C. Hockett and the dysfunctions created by the extreme concentration of private financial institutions, this article outlines a reform agenda that would both democratize finance and facilitate the flow of funds into valuable forms of investment (...) that are currently starved for resources. If the new institutions envisioned in this proposal were able to take root and expand, they could ultimately facilitate a transition to socialism, defined as the subordination of the market to democratic politics. LAS is a program that acquires augmented transition network (ATN) grammars. It requires as data sentences of the language and semantic network representatives of their meaning. In acquiring the ATN grammars, it induces the word classes of the language, the rules of formation for sentences, and the rules mapping sentences onto meaning. The induced ATN grammar can be used both for sentence generation and sentence comprehension. Critical to the performance of the program are assumptions that it makes about the (...) relation between sentence structure and surface structure (the graph deformation condition), about when word classes may be formed and when ATN networks may be merged, and about the structure of noun phrases. These assumptions seem to be good heuristics which are largely true for natural languages although they would not be true for many nonnatural languages. Women who had had breast cancer and had been enrolled in a large genetic breast cancer epidemiological study were interviewed about their experience of participation in the study, their attitudes to the confidentiality of data, and the feedback of personal and general research results. Collection of family history information seemed more salient in indicating the genetic nature of the study than the enrolment information sheet. There were no concerns about confidentiality.While participants would have welcomed general feedback about the results (...) of the study and were critical that this had not been provided, the feedback of personal information proved complicated and, sometimes, difficult. : Two major philosophers of the twentieth century, the German existential phenomenologist Martin Heidegger and the seminal Japanese Kyoto School philosopher Nishida Kitarō are examined here in an attempt to discern to what extent their ideas may converge. Both are viewed as expressing, each through the lens of his own tradition, a world in transition with the rise of modernity in the West and its subsequent globalization. The popularity of Heidegger's thought among Japanese philosophers, despite its own admitted limitation (...) to the Western "history of being," is connected to Nishida's opening of a uniquely Japanese path in its confrontation with Western philosophy. The focus is primarily on their later works (the post-Kehre Heidegger and the works of Nishida that have been designated "Nishida philosophy"), in which each in his own way attempts to overcome the subject-object dichotomy inherited from the tradition of Western metaphysics by looking to a deeper structure from out of which both subjectivity and objectivity are derived and which embraces both. For Heidegger, the answer lies in being as the opening of unconcealment, from out of which beings emerge, and for Nishida, it is the place of nothingness within which beings are co-determined in their oppositions and relations. Concepts such as Nishida's "discontinuous continuity," "absolutely self-contradictory identity" (between one and many, whole and part, world and things), the mutual interdependence of individuals, and the self-determination of the world through the co-relative self-determination of individuals, and Heidegger's "simultaneity" (zugleich) and "within one another" (ineinander) (of unconcealment and concealment, presencing and absencing), and their "between" (Zwischen) and "jointure" (Fuge) are examined. Through a discussion of these ideas, the suggestion is made of a possible "transition" (Übergang) of both Western and Eastern thinking, in their mutual encounter, both in relation to each other and each in relation to its own past history, leading to both a self-discovery in the other and to a simultaneous self-reconstitution. This paper offers a descriptive account of the transition in evolutionary ethics with reference to some major works from ethics, sociobiology, moral psychology, and primatology. The causes and nature of the transition are discussed by making a distinction between traditional and recent trends in evolutionary ethics enabling us to understand the significance of contemporary evolutionary ethics. Many advocates of evolutionary ethics and evolutionary moral psychology claim that reason plays no role or plays a very insignificant role in morality. Such assertion often leads to a rejection of Kantian ethics in particular and cognitivism in general. In this backdrop, the scope of reconciling evolutionary ethics with Kantian ethics as proposed by Frederick Rauscher is analyzed. This paper is thus an attempt to expose the significance of evolutionary ethics with reference to the relation of reason and morality. (shrink)

Abstract Energy typically is discussed in terms of science, technology, economics, and politics. Little attention has been given to fundamental religious and ethical questions surrounding the upcoming transition to renewable energy. The essays in this thematic section seek to redress that deficiency. This introductory essay raises some key questions and summarizes various presentations on energy and religion, as these were held at the 2010 conference of the Institute on Religion in an Age of Science (IRAS). Some presentations described the (...) energy landscape and provided data and perspectives needed for sound policy. Others raised ethical and religious considerations for energy decisions as the transition from nonrenewable to renewable sources is faced. Some posed the challenges of the energy transition to religion itself. Yet others offered examples of sustainable energy use and/or promising sources for meeting future needs sustainably. At the conclusion of the conference, presenters crafted a common “Statement on Energy and Climate Change” that includes a “Call to Action.” The “Energy Statement” follows as an appendix. (shrink)

Depuis quatre décennies, les organisations non gouvernementales alertent l'opinion publique face au modèle de croissance destructrice des ressources écologiques de la planète. Quelles leçons tirer de l'histoire récente? Comment accélérer l'innovation sociale afin de construire un autre monde? Autant de questions auxquelles répondent avec conviction douze auteur·e·s dans un ouvrage mosaïque. Sans doute que tout est dans le sous-titre : la société civile, moteur de la transition écologique. A côté des États et des collectivités supra-nationales, ONU, Europe entre autres, (...) les acteurs de la société civile ont joué un rôle prophétique de lanceurs d'alertes et de créateurs d'alternatives durables. Non écoutés ou à tout le moins peu pris en compte, ils prennent le relais d'acteurs politiques classiques, défaillants et timides, voire contribuant significativement à l'aggravation du constat. Crise de la gouvernance politique? Certainement. Démarquage, différences et divergences face au capitalisme mondial et existentiel? Oui encore. (shrink)

I argue that the proper way to think of the difference between A- and B-time is not as the difference between transition and the lack of transition, as is common, but as A-transition and B-transition. However, it is not evident what the difference is between these two kinds of transition. Thus, it is not evident what the difference is between A- and B-time.

This paper examines Kant’s treatment of the design argument for the existence of God, or physicotheology. It criticizes the interpretation that, for Kant, the assumption of intelligent design satisfies an internal demand of inquiry. It argues that Kant’s positive appraisal of physicotheology is instead better understood on account of its polemical utility for rebutting objections to practical belief in God upon which Kant’s ethicotheological argument rests, and thus as an instrument in the transition from theoretical to practical philosophy. Kantian (...) physicotheology plays this role a) by criticizing alternative speculative accounts of the ground of nature, and b) by analogizing from the structure of finite rational agency in order to represent more clearly the action of an ideal agent. Over the last four decades, environmental ethics has become an increasingly significant field of philosophy. Yet, many of its practitioners question its goals and effectiveness. Above all, environmental philosophers voice uncertainty about the extent to which the field has been able to influence action, behaviour, and policy in relation to the environment. What are the reasons behind this meagre influence and what kind of contrasting philosophical approach might enable transformative action? The goal of this paper is to answer these questions (...) by drawing on Schelling’s late philosophy. I argue that Schelling’s critique of philosophical logicism reveals the ways in which certain ways of doing philosophy fails to enable ethical and behavioural transformation. By contrast, I show that Schelling’s “positive philosophy” can offer important insights into our current situation and illuminate a way forward for a sustainable environmental ethics. (shrink)

Recent developments in contemporary pragmatist thought have the potential to help reshape our understandings of pragmatism in philosophy of education. We first survey the development of pragmatism as founded in experience, moving through linguistic pragmatism, to a newer actionistic approach in conduct pragmatism. Conduct pragmatism prioritises action over both experience and discursive thought in ways that can be central to educational activity and projects. Conduct pragmatism so conceived has the potential to alter and shift how philosophers of education relate to (...) pragmatist thought and in the second part of the paper we outline some of these potentials. We indicate the possibilities for pragmatist philosophy of education to engage with diverse bodies of thought, moving away from an insular focus on one or two philosophers, and a diversity of locations where philosophy is active and conducted. This study investigated students’ perceptions of ethical organizational climates, attitudes towards ethical issues, and the perceived relationship between ethical behavior and success in business organizations. Comparisons were made between the attitudes of these future managers with previously published studies of Russian managers’ attitudes. A survey of 100 business students in three Moscow universities showed that their attitudes toward ethical behavior were more negative than those of Russian managers. No significant differences were found in the perceptions or attitudes of students who (...) had attended an ethics course and those that did not.
