The public often believes that parents have a right to make medical decisions about their child. The idea that, in respect of children, doctors should do what parents tell them to do is problematic on the face of it. The effect of such a claim would be that a doctor who acted deliberately to harm a child would be making a morally correct decision, providing only that it is what the child’s parents said they wanted. That is so obviously nonsense (...) that it cannot be what people who claim it actually mean. In this paper, I suggest that the claim actually represents either or both of two misunderstandings. It can be a result of wrongly appealing to the principle of respect for autonomy, or a belief that doctors are not committed to acting in the interests of the child. In this paper, I show that, while neither belief is entirely justified, there are elements of truth in both. I argue that if ethically correct decisions are those that are directed to improving the quality of a child’s existence, then neither parents nor doctors are in a position to make ethically correct decisions about a child except in discussion with one another. Where such discussion is not possible, I suggest there should be a national Children’s Interests Panel to agree on the child’s interests. The panel should include, but not be limited to, paediatricians and lawyers and its decisions should be legally binding on all parties. (shrink)

Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk. Introduction The General Medical Council stipulates that doctors must be competent professionals, not merely scholars and practitioners. Medical school curricula should enable students to develop professional values and competencies. Additionally, medical schools are moving towards integrated undergraduate curricula, Cardiff's C21 being one such example. Methods We carried out an audit to determine the extent to which C21 delivers GMC professionalism competencies, and a student questionnaire to explore student perspective on ethics and professionalism. Results and discussion C21 delivers explicit or implicit (...) teaching for all major GMC competencies, though some missed opportunities remain. The questionnaire responses showed that most students value ethics and professionalism teaching, and that it is most well received when delivered in a variety of ways and contexts throughout the curriculum. We also note that some respondents confuse ethics and professionalism with the policing of student behaviour. Conclusions C21 and curricula like it offer many opportunities for nurturing ethically and professionally competent physicians. Students appear to value this, though there remains confusion between medical school discipline and ethics and professionalism which needs further explication. (shrink)

Since Socrates, and through Descartes to the present day, the problems of self-knowledge have been central to philosophy's understanding of itself. Today the idea of ''first-person authority''--the claim of a distinctive relation each person has toward his or her own mental life--has been challenged from a number of directions, to the point where many doubt the person bears any distinctive relation to his or her own mental life, let alone a privileged one. In Authority and Estrangement, Richard Moran argues (...) for a reconception of the first-person and its claims. Indeed, he writes, a more thorough repudiation of the idea of privileged inner observation leads to a deeper appreciation of the systematic differences between self-knowledge and the knowledge of others, differences that are both irreducible and constitutive of the very concept and life of the person. Masterfully blending philosophy of mind and moral psychology, Moran develops a view of self-knowledge that concentrates on the self as agent rather than spectator. He argues that while each person does speak for his own thought and feeling with a distinctive authority, that very authority is tied just as much to the disprivileging of the first-person, to its specific possibilities of alienation. Drawing on certain themes from Wittgenstein, Sartre, and others, the book explores the extent to which what we say about ourselves is a matter of discovery or of creation, the difficulties and limitations in being ''objective'' toward ourselves, and the conflicting demands of realism about oneself and responsibility for oneself. Meaning is spread across a population, inherited by each new generation of speakers from the last, and evolving through the interactions of speakers with their environment.

Has the passing of the old God paved the way for a new kind of religious project, a more responsible way to seek, sound, and love the things we call divine? Has the suspension of dogmatic certainties and presumptions opened a space in which we can encounter religious wonder anew? The humanity formulation of Kant's Categorical Imperative demands that we treat humanity as an end in itself. Because this principle resonates with currently influential ideals of human rights and dignity, contemporary readers often find it compelling, even if the rest of Kant's moral philosophy leaves them cold. What Bar-On and Simmons call 'Conceptual Deflationism' is the thesis that truth is a 'thin' concept in the sense that it is not suited to play any explanatory role in our scientific theorizing. One obvious place it might play such a role is in semantics, so disquotationalists have been widely concerned to argued that 'compositional principles', such as -/- (C) A conjunction is true iff its conjuncts are true -/- are ultimately quite trivial and, more generally, that semantic theorists have (...) misconceived the relation between truth, meaning, and logic. This paper argues, to the contrary, that even such simple compositional principles as (C) have substantial content that cannot be captured by deflationist 'proofs' of them. The key thought is that (C) is supposed, among other things, to affirm the truth-functionality of conjunction and that disquotationalists cannot, ultimately, make sense of truth-functionality. -/- This paper is something of a companion to "The Logical Strength of Compositional Principles". RICHARD RORTY has written one of the most important and challenging books to be published by an American philosopher in the past few decades. Some will find it a deeply disturbing book while others will find it liberating and exhilarating—both, as we shall see, may be right and wrong. Not since James and Dewey have we had such a devastating critique of professional philosophy. But unlike James and Dewey, who thought that once the sterility and artificiality of professional—and indeed (...) much of modern philosophy since Descartes—had been exposed, there was an important job for philosophers to do; Rorty leaves us in a much more ambiguous and unsettled state. I will examine Rorty’s book from a variety of perspectives, beginning with a general overview and then moving to more finely meshed descriptions. Horton and Gerrig outlined a memory-based processing model of conversational common ground that provided a description of how speakers could both strategically and automatically gain access to information about others through domain-general memory processes acting over ordinary memory traces. In this article, we revisit this account, reviewing empirical findings that address aspects of this memory-based model. In doing so, we also take the opportunity to clarify what we believe this approach implies about the cognitive psychology of common ground, and just (...) as important, what it does not imply. We also highlight related areas of research demonstrating how general cognitive processes can constrain access to relevant knowledge in ways that shape both language production and comprehension. A common complaint against contemporary epistemology is that its issues are too rarified and, hence, of little relevance for the everyday assessments we make of each other=s beliefs. The notion of epistemic rationality focuses on a specific goal, that of now having accurate and comprehensive beliefs, whereas our everyday assessments of beliefs are sensitive to the fact that we have an enormous variety of goals and needs, intellectual as well as nonintellectual. Indeed, our everyday assessments often have a quasi-ethical dimension; (...) we want to know, for example, whether someone has been responsible, or at least non-negligent, in forming opinions. Nevertheless, epistemology, properly conceived, is relevant to our commonplace intellectual concerns. Epistemic rationality is an idealized notion, but its idealized character makes it suitable to serve as a theoretical anchor for other notions of rationality, including notions that are less idealized and, hence, potentially more directly relevant to our everyday assessments.
