It remains unclear whether such genetic variants are associated with disease aggressiveness. The NCI-SPORE Genetics Working Group retrospectively collected clinicopathologic information and genotype data for 36 SNPs which at the time had been validated to be associated with PC risk from 25,674 cases with PC. Cases were grouped according to race, Gleason score and aggressiveness. Statistical analyses were used to compare the frequency (...) of the SNPs between different disease cohorts. After adjusting for multiple testing, only PC-risk SNP rs2735839 was significantly and inversely associated with aggressive and high-grade disease in European men. Similar associations with aggressive and high-grade disease were documented in African-American subjects. The G allele of rs2735839 was associated with disease aggressiveness even at low PSA levels in both European and African-American men. Our results provide further support that a PC-risk SNP rs2735839 near the KLK3 gene on chromosome 19q13 may be associated with aggressive and high-grade PC. Future prospectively designed, case-case GWAS are needed to identify additional SNPs associated with PC aggressiveness. (shrink)

One of the earliest and most influential treatises on the subject of this volume is Aristotle's Categories. Aristotle's title is a form of the Greek verb for speaking against or submitting an accusation in a legal proceeding. By the time of Aristotle, it also meant: to signify or to predicate. Surprisingly, the "predicates" Aristotle talks about include not only bits of language, but also such nonlinguistic items as the color white in a body and the knowledge of grammar in a (...) man's soul. (Categories I/ii) Equally surprising are such details as Aristotle's use of the terms 'homonymy' and 'synonymy' in connection with things talked about rather than words used to talk about them. Judging from the evidence in the Organon, the Metaphysics, and elsewhere, Aristotle was both aware of and able to mark the distinction between using and men tioning words; and so we must conclude that in the Categories, he was not greatly concerned with it. For our purposes, however, it is best to treat the term 'predication' as if it were ambiguous and introduce some jargon to disambiguate it. Built-in decision thresholds for AI diagnostics are ethically problematic, as patients may differ in their attitudes about the risk of false-positive and false-negative results, which will require that clinicians assess patient values.

Exceptionalism is the view that one group is better than other groups and, by virtue of its alleged superiority, is not subject to the same constraints. Here we identify national exceptionalism in the responses made by political leaders in the United States and the United Kingdom to the covid-19 pandemic in early 2020. First, we observe that responses appealed to national values and national character and were marked by a denial of the severity of the situation. Second, we suggest an (...) analogy between national exceptionalism and unrealistic optimism, i.e., people’s tendency to make rosier predictions about their future than is warranted by the evidence due to illusions of superiority and control. Finally, we argue that, at the national level, exceptionalism gave rise to an assumption of invulnerability that made for slow responses to the pandemic, and at the individual level, it served as a justification of people’s failures to adopt safety behaviors. In theMetaphysics, Aristotle pointed out that some activities are engaged in for their own sake, while others are directed at some end. The test for distinguishing between them is to ask, ‘At any time during a period in which someone is Xing, is it also true that they have Xed?’ If both are true, the activity is being done for its own sake. If not, it is being done for the sake of some end other than itself. For example, if (...) I am thinking, it is true that I have thought. But if I’m making a blouse, it is not true that I have made a blouse, at least not this particular blouse. That’s not true until I have completed the project.There have been a number of attempts to deepen our understanding of this distinction. Anthony Kenny devoted a chapter of Action, Emotion, and Will to this issue, exploring the effect tense has on implication relations, and using that as a basis for dividing verbs of action into state-verbs, activity-verbs and performance-verbs. In more recent years the trend has been to generalize these categories so as to include occurrences other than actions, i.e., occurrences which do not involve intentions. While interest in this area tends to focus primarily on linguistic issues, such as the categorization of verbs, or on the logical analysis of sentences, there has been some interest in related metaphysical issues. In 1978 Alexander Mourelatos published ‘Events, Processes, and States,’ a paper which has turned out to be quite influential, in which he proposes an ontological trichotomy of occurrences. In his view, processes and events form distinct categories within the general category of occurrences. In this paper I will examine the reasoning underlying Mourelatos’s claim, arguing that the differences between processes and events cannot provide the basis for an ontological subcategorization of occurrences. While feminist critiques of AI are increasingly common in the scholarly literature, they are by no means new. Alison Adam’s Artificial Knowing (1998) brought a feminist social and epistemological stance to the analysis of AI, critiquing the symbolic AI systems of her day and proposing constructive alternatives. In this paper, we seek to revisit and renew Adam’s arguments and methodology, exploring their resonances with current feminist concerns and their relevance to contemporary machine learning. Like Adam, we ask how new AI (...) methods could be adapted for feminist purposes and what role new technologies might play in addressing concerns raised by feminist epistemologists and theorists about algorithmic systems. In particular, we highlight distributed and federated learning as providing partial solutions to the power-oriented concerns that have stymied efforts to make machine learning systems more representative and pluralist. Conscientious objection has become a divisive topic in recent bioethics publications. Discussion has tended to frame the issue in terms of the rights of the healthcare professional versus the rights of the patient. However, a rights-based approach neglects the relational nature of conscience, and the impact that violating one’s conscience has on the care one provides. Using medical assistance in dying as a case study, we suggest that what has been lacking in the discussion of conscientious objection thus far is (...) a recognition and prioritising of the relational nature of ethical decision-making in healthcare and the negative consequences of moral distress that occur when healthcare professionals find themselves in situations in which they feel they cannot provide what they consider to be excellent care. We propose that policies that respect the relational conscience could benefit our healthcare institutions by minimising the negative impact of moral distress, improving communication among team members and fostering a culture of ethical awareness. Constructive responses to moral distress including relational cultivation of moral resilience are urged. (shrink)

Healthcare organizations and workers are under pressure to produce increasingly complete and accurate data for multiple data-intensive endeavors. However, little research has examined the emerging occupations arising to carry out the data work necessary to produce “improved” data sets, or the specific work activities of these emerging data occupations. We describe the work of Clinical Documentation Integrity Specialists, an emerging occupation that focuses on improving clinical documentation to produce more detailed and accurate administrative datasets crucial for evolving data-intensive forms of (...) healthcare accountability, management, and research. Using ethnographic methods, we describe the core of CDIS’ work as a translation practice in which the language, interests, and concerns of clinicians and clinical documentation are translated via real-time “nudging” and ongoing education of clinicians into the language, interests, and concerns of medical coders, structured administrative datasets, and the various stakeholders of these datasets. Further, we show how the institutional context of CDIS’ work shapes the occupational virtues that guide CDIS’ translation practice, including financial reimbursement, quality measures, clinical accuracy, and protecting clinician’s time. Despite the existence of these multiple virtues, financial reimbursement is the most prominent virtue guiding CDIS’ limited attention. Thus, overall clinical documentation is “improved” in specific, partial ways. This research provides one of the first studies of the emergent data work occupations arising in the wake of digitization and big data opportunities, and shows how local data settings shape large scale data in specific ways and thus may influence outcomes of analyses based on such data. Given this broad theoretical framework, we will specify by looking at the case of STEM education and its variant Science, Technology, Engineering and Mathematics with Art education as an example par excellence. Without going into the technical details of the financial support of the projects, we prefer to open a philosophical debate on the way how policies on academic subjects influence a whole society and the personal life of both researchers and people/pupils involved in education. Beginning with Rachel Carson’s small book, The Sense of Wonder, I explore the moral significance of a sense of wonder—the propensity to respond with delight, awe, or yearning to what is beautiful and mysterious in the natural world when it unexpectedly reveals itself. An antidote to the view that the elements of the natural world are commodities to be disdained or destroyed, a sense of wonder leads us to celebrate and honor the more-than-human world, to care for it, to protect (...) its thriving. If this is so, then a sense of wonder may be a virtue, perhaps a keystone virtue in our time of reckless destruction, a source of decency and hope and restraint. (shrink)

This article reviews the phenomena of disordered discourse often manifested in schizophrenia. It argues that the Structure Building Framework, a model of the general cognitive processes and mechanisms underlying discourse, can be used to account for these phenomena. According to the Structure Building Framework, the goal of comprehension is to build coherent mental representations or structures. Building a mental structure involves several component subprocesses: laying a foundation, mapping relevant information onto that foundation, and shifting to initiate a new substructure. Building (...) a mental structure also involves at least two general cognitive mechanisms: enhancement of relevant activation and suppression of irrelevant or inappropriate activation. We suggest that schizophrenics who exhibit verbose disordered discourse have inefficient suppression mechanisms, are impaired in laying a foundation, and tend to shift too often. We also speculate that schizophrenics who exhibit impoverished disordered discourse have inefficient enhancement mechanisms and are impaired with the cognitive process of mapping. (shrink)

Prevailing interpretations of Aristotle’s use of syllogistic language outside the Organon hold that he offers a single, comprehensive theory of the practical syllogism spanning his ethical and biological works. These comprehensive theories of the practical syllogism are plausible neither philosophically nor as interpretations of Aristotle. I argue for a multivocal account of the practical syllogism that distinguishes (1) Aristotle’s use of syllogistic language to explain aspects of his account of animal motion in MA from (2) his use of syllogistic language (...) to explain aspects of his account of the distinctive practical cognition of the phronimos in EN. I offer a novel account of the role of syllogistic language in ethics, arguing that it elucidates a nuanced account of universals and particulars in ethics according to which acting virtuously requires an understanding of underlying universal values and a capacity to relate them to concrete, particular features of our circumstances. In the Two Treatises of Governlnent, Locke appears to hold a view of dominion which compromises humanitarian principles for economic gain. He often asserts that man has a right to accumulate property and to use land and animals for comfort and convenience. This right issues from God’s decree that men subdue the Earth and have (...) dominion over every living thing. Although abuse of the environment appears to be justified in Locke’s political works, I argue that there are many passages in this work that cast doubt on such an interpretation. Further, the view of dominion adopted in Locke's educational work is one of responsible stewardship. On the whole, his view stresses man’s duties and obligation towards all creation. (shrink)

In the first two volumes of this work, Paul Ricoeur examined the relations between time and narrative in historical writing, fiction, and theories of literature. This final volume, a comprehensive reexamination and synthesis of the ideas developed in volumes 1 and 2, stands as Ricoeur's most complete and satisfying presentation of his own philosophy. Ricoeur's aim here is to explicate as fully as possible the hypothesis that has governed his inquiry, namely, that the effort of thinking at work in every (...) narrative configuration is completed in a refiguration of temporal experience. To this end, he sets himself the central task of determing how far a poetics of narrative can be said to resolve the "aporias"—the doubtful or problematic elements—of time. _Time and Narrative_ builds on Paul Ricoeur's earlier analysis, in _The Rule of Metaphor_, of semantic innovation at the level of the sentence. Ricoeur here examines the creation of meaning at the textual level, with narrative rather than metaphor as the ruling concern. Ricoeur finds a "healthy circle" between time and narrative: time is humanized to the extent that it portrays temporal experience. Ricoeur proposes a theoretical model of this circle using Augustine's theory of time and Aristotle's theory of plot (...) and, further, develops an original thesis of the mimetic function of narrative. Understanding people's experience of skin ageing as it is lived can enable sensitive approaches to promoting healthy skin and to care in general. By understanding the insider perspective, what it is like for individuals, a way to sensitise practice for more humanly sensitive care is offered. Through interviews with seventeen community‐dwelling older people, the essential meaning of living within ageing skin was illuminated as a state of managed inevitability. The skin is inevitably changing, and ageing skin is a marker of (...) change over time but the person within remains. Constituents of the phenomenon comprise the experience of unfamiliar sights and sensations given by ageing skin; facing and accepting bodily changes and seeing this back and forth in family connections; taking care of the skin “to face” the world; and to present oneself to others and a different place in the world, same person, changed body. Findings point to why and how nurses can treat older people as persons by not over emphasising a view on ageing bodies or bodies with aged skin alone, but in tempering this view with deeper existential insights, meeting the older person with a skin care need as a person and not just as a physical entity. This book offers an intervention into the process of decolonization through the re-subjectification of the settler subject. The authors draw on what Deleuze and Guattari call minor threads of philosophy, pedagogy, spirituality, and healing practices rooted in neglected lineages of European thought and ceremony. The book proposes a methodology for unontologizing the settler subject, which they term 'desettlering.' Rather than fetishizing indigenous theory and practice as a mode for resubjectifying settlers to facilitate land-based decolonization, it offers a fresh approach by (...) looking towards alternative sets of traditions and identity. These alternatives are used to interrogate minoritarian European philosophies, practices, and beliefs, which the authors propose could be deployed to unontologize the settler within current historical conditions. Asserting that such a process is not volitional but a historical necessity, the book offers a novel and timely investigation into who settlers become if they intend to engage seriously in decolonization. Some of the issues raised have been (1) Is the general point of view the moral point of view? (2) Is the general point of view necessary in order for a judgment to count as (...) a moral judgment? (3) Does the general point of view provide a justificatory perspective or is it a psychological explanatory concept that explains how moral judgments are made without necessarily offering a standard or normative basis for moral judgments? (4) If a general point of view yields some notion of impartiality, then how does it do so without introducing rationalistic elements that would undermine the sentimentalist approach Hume defends?1 In this paper I offer an interpretation of Hume's general point of view in morals as, employing a photographic analogy, a kind of focusing activity. It allows for strengthening of sentiments for those remote and, through contrariety, weakening of sentiments for those near such that we are moved to focus on those effects that are typical and, through conversing with others, arrive at general principles of praise or blame. I also examine the general point of view in comparison to what Hume has to say about regulation of belief. The comparison with belief sheds new light, I think, on how production of contrariety through the general point of view is regulative in morals. On my interpretation of the general point of view, it does not undermine Hume's sentimentalist thesis in morals, but rather is a device by which sentiment is properly aroused and directed. This will be helpful in setting up the scope of regulation in morals, since the latter involves, but is not limited to, regulation of belief. Our judgments concerning cause and effect are deriv'd from habit and experience; and when we have been accustom'd to see one object united to another, our imagination passes from the first to the second, by a natural transition, which precedes reflection, and which cannot be prevented by it. (T 1.3.13.8; SBN 147) There are two tendencies that Hume identifies under "generalizing." The first is an error-prone generalizing tendency of the imagination and the second is the tendency to form corrections, in the understanding to form general rules for regulating inferences.
