Two contributions from Dr Ivan Illich follow. The first, in which he sets out his primary thesis of the medicalization of life, is a section from Dr Illich's book `Medical Nemesis'. (It is reprinted with the permission of the author and his publishers, Messrs Calder and Boyars.) The second is a transcript of the paper which Dr Illich read at the conference organized by the London Medical Group on iatrogenic disease. Both are ultimately addressed to the recipients of medical (...) care, the general public, although the second paper is specifically addressed to young doctors and medical students. For Dr Illich the world is suffering from too much medical interference, and a medical edifice has been built which is one of the threats to the real life of human beings - a threat which so far has been disguised as care. JOHN WISDOM AND THE BREADTH OF PHILOSOPHY hham Dhman 1. THE ESSAYS IN THIS VOLUME The essays following the two pieces by John Wisdom have all been written by philosophers who are former students or friends of Wisdom or who have a high regard for his work. Their contributions were all written with him in mind and to be discussed at a conference honouring his work. This conference was held in August 1983 at Trinity College, Cambridge, of which Wisdom has (...) been a fellow since 1935. Wisdom is a master of discursive reasoning and one of his distinctive contributions in philosophy has been to examine its various forms and their interconnections, particularly the form it takes in philosophical inquiry and the way it advances our understanding there. His concern to bring out the links between all that is abstract in such reasoning and the concrete and particular is well known and represented in many of the essays in this volume. But Wisdom has also a deep appreciation of the kind of understanding that is advanced non-discursively. As he puts it in the first piece in this volume: However skilled a good critic 'I am sure that much of what makes "Hamlet" "Hamlet" will run between his fingers'. He has himself advanced our understanding on many questions in philosophy in this way, not simply by what he has said, but also by what he has suggested 'between the lines'. (shrink)

Islamic ethics in the light of Koranic teachings.

Building on Alfred Schütz’s work, this essay conceptually scrutinizes virtual worlds with an aim to clarify what is at stake with the virtualization of the late modern society. The diffusion of technological artifacts, devices of communication and the Internet in particular, have transformed the life-world of essentially everyone. In the past few years our everyday life, including its livelihoods, has seen a proliferation of activities within virtual worlds, such as games and virtual social networks. We can now live (...) and experience actively in different virtual realms, as compared to being mere passive receivers in the era of television and radio. This has direct implications for what is inherently relevant for people and, in consequence, we have to take such transformation into account also in conceptual terms. This article makes a major contribution to the conceptual understanding of phenomenological sociology that due to the virtualization, we can no more equate the paramount reality as the zone of primary relevance. The paramount reality as conceived as the sensorily perceivable, physical world of concrete objects is increasingly far from being equated to that part of the world within our reach which we can immediately observe and also at least partially dominate, that is, the zone of primary relevance. It is furthermore argued that the ongoing virtualization of societies urges us to conceive of virtual worlds as transforming the chief finite province of meaning, that of the world of working, instead of seeing virtual worlds merely as other sub-universes of reality. The concept of quality of life is the basis for a new paradigm of clinical medicine. Its assessment is considered to be an important instrument in determining disease severity and effectiveness of different treatment modalities. Our studies are devoted to the problem of diabetes complications especially anemia in patients with diabetic kidney disease. Anemia in subjects with diabetic nephropathy may result from various pathogenic factors including erythropoietin deficiency, iron deficiency, vitamin B12 and/or folate deficiencies, the effects of proinflammatory cytokines (...) etc. Diabetic patients with anemia are at increased risk of cardiovascular accidents and death. The aim of this study was to assess the influence of anemia on the quality of life of patients with early stages of diabetic nephropathy. We investigated 127 patients with type 2 diabetes mellitus and early diabetic nephropathy. Anemia was defined according to the World Health Organization criteria. Anemic patients were divided into three groups according to the stage of chronic kidney disease. Patients’ quality of life was assessed with SF-36 health survey. Anemic patients had lower scores of physical role functioning and emotional role functioning as compared to the patients with normal hemoglobin level, irrespective of the stage of chronic kidney disease. Patients with anemia and chronic kidney disease stage 3 had lower scores of general health perceptions, physical functioning, bodily pain, vitality and mental health than patients in the non-anemic group. Social role functioning was similar in anemic and non-anemic patients. The possible mechanisms and implications of the above mentioned findings are discussed with a medical and humanitarian perspective. The directions of further studies are proposed. (shrink)

The ever-increasing technological advances of modern medicine have increased physicians’ capacity to carry out a wide array of clinical interventions near the end-of-life. These new procedures have resulted in new “types” of living where a patient’s cognitive functions are severely diminished although many physiological functions remain active. In this biomedical context, patients, surrogate decision-makers, and clinicians all struggle with decisions about what clinical interventions to pursue and when therapeutic intent should be replaced with palliative goals of care. For some (...) patients and clinicians, religious teachings about the duty to seek medical care and the care of the dying offer ethical guidance when faced with such choices. Accordingly, this paper argues that traditional Sunni Islamic ethico-legal views on the obligation to seek medical care and Islamic theological concepts of human dignity and inviolability provide the ethical grounds for non-intervention at the end-of-life and can help calibrate goals of care discussions for Muslim patients. In closing the paper highlights the pressing need to develop a holistic ethics of healthcare of the dying from an Islamic perspective that brings together multiple genres of the Islamic intellectual tradition so that it can meet the needs of the patients, clinicians and Muslim religious leaders interacting with the healthcare system. This book... is a set of case studies, relating and reflecting on the stories of specific practitioners, in identified Asian contexts, struggling to act purposefully and conscientiously within their spheres of work, to meet their professional duties as they understand them. Through careful examination of these selected cases, we can learn a great deal about the kinds of moral competence practitioners require in order to act effectively and well in public life.

This paper reports two cases in Hong Kong involving two native Chinese adolescent cancer patients (APs) who were denied their rights to consent to necessary treatments refused by their parents, resulting in serious harm. We argue that the dynamics of the 'AP-physician-family-relationship' and the dominant role Chinese families play in medical decision-making (MDM) are best understood in terms of the tendency to hierarchy and parental authoritarianism in traditional Confucianism. This ethic has been confirmed and endorsed by various Chinese writers from (...) Mainland China and Hong Kong. Rather than giving an unqualified endorsement to this ethic, based more on cultural sentimentalism than rational moral reasoning, we warn that a strong familism in MDM, which deprives 'weak' family members of rights, represents the less desirable elements of this tradition, against which healthcare professionals working in this cultural milieu need to safeguard. Specifically for APs, we suggest that parental authority and family integrity should be re-interpreted in terms of parental responsibility and the enhancement of children's interests respectively, as done in the West. This implies that when parents refuse to consent to necessary treatment and deny their adolescent children's right to consent, doctors, as the only remaining advocates of the APs' interest, have the duty to inform the state, which can override parental refusal to enable the doctors to fulfill their professional and moral obligations. In so doing the state exercises its 'parens patriae' power to defend the defenseless in society and the integrity of the medical profession. (shrink)

The theory is presented that the sexual process is a repair mechanism which maintains redundancy within the sub-structure of hierarchical, self-reproducing organisms. In order to keep the problems within mathematically tractable limits , a simple model is introduced: a wheel with 6 spokes, 3 of them vital and 3 redundant, symbolizes the individual . Random accidents destroy spokes; the wheels replicate at regular cycles and engage periodically in pairing and repair phases during which missing spokes are copy-reproduced along the intact (...) spokes of the partner wheel.The hierarchical structure of such a system is analysed and an ‘autonomous unit’ is defined: this is the unit of minimal hierarchical complexity which is capable of perpetuating autonomously all higher and all lower levels of the hierarchy; this is the central unit of selection.Four basic, physical parameters are isolated which determine the essential features of any eucaryotic life cycle: 1. The number of levels of the hierarchy ; 2. the relation between the phases of replication and repair ; 3. the duration of potential repair ; 4. the position of the sexual partners within the hierarchy .The evaluation of fitness components is considered in relation to trends of reproductive patterns in evolution. Professor Guillaume's translation of the Sira of Ibn Ishaq is now reissued. The translator used Ibn Hisham's abridgement and also included many additions and variants found in the writings of early authors. The book thus presents in English practically all that is known of the life of the Prophet.In the introduction, the translator discusses the character of the Sira in the light of the opinion of early Arabian scholars, noting especially the difficulties of the poetry. As the earliest monument (...) of Arabian prose literature, the Sira remains a work of the first importance. (shrink)

Politics, public discourse, and legislation restricting abortion has settled on a moderate orthodoxy: restrict abortion, but leave exceptions for pregnancies that result from rape and incest. I challenge that consensus and suggest it may be much harder to defend than those who support the compromise think. From both Pro-Life and Pro-Choice perspectives, there are good reasons to treat all abortions as equal.

This chapter recounts an imaginary situation which the author confronts with while reading The Book of Life. Realizing that the book purports to be a book on his life, he decides to test it and defeat it by looking at a future entry, and turns to an entry 18 minutes hence. He closes the book, and opens it again after a few moments, and starts reading an earlier part of the book. After a period of 18 minutes, the (...) author finds that he is actually reading the particular entry itself which he wanted to avoid, in order to prove the book wrong. The entry said “He is reading this sentence”. The author realizes that the book is correct, and that he has not been able to refute the book. The chapter ends by leaving the reader to ponder over the question whether the world and our lives are determined. (shrink)

An account of desire as a species of belief may owe its appeal to the details of its proposal as to precisely what sort of beliefs desires are to be identified with, and its downfall may be due to those details it does provide. For example, it may be proposed that the desire that α is in fact the belief that it ought to be that α, or is morally good or desirable that it should be the case that α. (...) Here the appeal might be that of forging a link between the holding of a moral belief and the acknowledgment that one has a reason for acting a certain way; and the shortcoming of the suggestion is its evident implausibility: even if the ‘necessity’ direction could be established, having a desire hardly seems sufficient for the holding of any such belief. Again: it might be proposed, perhaps simply to bring some order into the realm of propositional attitudes by reducing some to others, that the belief with which we should identify a’s desire that α is a’s belief that he will or would be happy if α. This proposed identification can be seen to be incorrect by consideration of examples such as the following, due to J. Gosling. An aging and ailing parent might forego numerous pleasures in order that his children should reap the benefits of his saving and have a good start in life – perhaps by receiving an expensive education – after his impending death. Oearly he may want that they should so benefit even if he does not believe in an after-death existence in which he might come to know of, and so take pleasure in, his children’s subsequent well-being. So his wanting that they should prosper cannot consist in his believing that he will be happy if/when they do, since he has no expectation of even being in existence in that eventuality. As Gosling puts it, there is a clear difference between thinking that something’s coming about will bring one pleasure, on the one hand, and viewing the prospect of its coming about with pleasure, on the other. Aleksei Fedorovich Losev did not share the hereditary Russian prejudice that science and philosophy exclude religion . Although he did not leave any specialized works in theology, the history of religion, or the foundations of the Christian worldview, many of his books, lines from letters, and fragments of conversations and, most importantly, his whole way of life and life journey show that all his life he reflected continuously and intensely on themes of the Christian faith and, primarily, (...) of the Orthodox faith that he confessed. Not only did he reflect on them, he lived by them. For Aleksei Fedorovich Christianity was never just an object of knowledge, a preoccupation of the reflective mind, but rather the profound foundation of his relationship with the world and with people. It constituted not only his thought and its foundation, but his very life. Religion, as Aleksei Fedorovich understood it, is first of all a certain kind of life, not just a personal worldview, however religious and mystical it might be. It was not a personal morality, however lofty or religious it might be, nor personal feeling or esthetics, however ardent the feeling might be or however perfectly mystical the esthetics. Religion, in his mind, is the realization of a worldview, the material substance of morality, the real affirmation of feeling; it is the life of the person directed to the persons substantial self-affirmation, to self-affirmation in eternity . In recounting the facts of Aleksei Fedorovichs spiritual biography I shall not follow their chronological order. The basic facts can be found in the works of A.A. Takho-Godi, D. Dzhokhadze, M. _An Ethical Modernity?_ offers a new view of Hegel’s doctrine of ethical life in relation to modernity. In this collection of essays, the authors investigate various aspects of this relation and its importance for today’s world.

Banks play a unique role in society. They not only maximize profits but also consider the interests of stakeholders. We investigate whether banks consider firms’ pollution records in their lending decisions. The evidence shows that banks offer significantly higher loan spreads, higher total borrowing costs, shorter loan maturities, and greater collateral to firms with higher levels of chemical pollution. The costly effects are stronger for borrowers with greater risk and weaker corporate governance. Further, the results show that banks with higher (...) social responsibility account for their borrowers’ environmental performance and charge higher loan spreads to those with poor performance. These results support the idea that banks with higher social responsibility can promote the practice of business ethics in firms. (shrink)
