BackgroundRandomized controlled trials (RCTs) are often complex and expensive to perform. Less than one third achieve planned recruitment targets, follow-up can be labor-intensive, and many have limited real-world generalizability. Designs for RCTs conducted using cohorts and routinely collected health data, including registries, electronic health records, and administrative databases, have been proposed to address these challenges and are being rapidly adopted. These designs, however, are relatively recent innovations, and published RCT reports often do not describe important aspects of their methodology in (...) a standardized way. Our objective is to extend the Consolidated Standards of Reporting Trials (CONSORT) statement with a consensus-driven reporting guideline for RCTs using cohorts and routinely collected health data.MethodsThe development of this CONSORT extension will consist of five phases. Phase 1 (completed) consisted of the project launch, including fundraising, the establishment of a research team, and development of a conceptual framework. In phase 2, a systematic review will be performed to identify publications (1) that describe methods or reporting considerations for RCTs conducted using cohorts and routinely collected health data or (2) that are protocols or report results from such RCTs. An initial “long list” of possible modifications to CONSORT checklist items and possible new items for the reporting guideline will be generated based on the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) and The REporting of studies Conducted using Observational Routinely-collected health Data (RECORD) statements. Additional possible modifications and new items will be identified based on the results of the systematic review. Phase 3 will consist of a three-round Delphi exercise with methods and content experts to evaluate the “long list” and generate a “short list” of key items. In phase 4, these items will serve as the basis for an in-person consensus meeting to finalize a core set of items to be included in the reporting guideline and checklist. Phase 5 will involve drafting the checklist and elaboration-explanation documents, and dissemination and implementation of the guideline.DiscussionDevelopment of this CONSORT extension will contribute to more transparent reporting of RCTs conducted using cohorts and routinely collected health data. In seiner letzten Schrift unternimmt Husserl den Versuch, auf dem Wege einer teleologisch-historischen Besinnung auf die Ursprünge unserer kritischen wissenschaftlichen und philosophischen Situation die Notwendigkeit einer transzendentalphänomenologischen Umwendung der Philosophie zu begründen. Er geht von seinem Begriff der "Lebenswelt" aus und entwickelt eine auf diesen Zentralbegriff seiner Spätphilosophie gegründete eigenständige Einleitung in die transzendentale Phänomenologie.

This book makes available to the English reader nearly all of the shorter philosophical works, published or unpublished, that Husserl produced on the way to the phenomenological breakthrough recorded in his Logical Investigations of 1900-1901. Here one sees Husserl's method emerging step by step, and such crucial substantive conclusions as that concerning the nature of Ideal entities and the status the intentional `relation' and its `objects'. Husserl's literary encounters with many of the leading thinkers of his day illuminates both the (...) context and the content of his thought. Many of the groundbreaking analyses provided in these texts were never again to be given the thorough expositions found in these early writings. Edmund Gettier is Professor Emeritus at the University of Massachusetts, Amherst. This short piece, published in 1963, seemed to many decisively to refute an otherwise attractive analysis of knowledge. It stimulated a renewed effort, still ongoing, to clarify exactly what knowledge comprises.

Dabei stehen die Fragen, welche die Anwendung des Schemas `Auffassung - Auffassungsinhalt' auf das Zeitbewusstein und die Gefahr des unendlichen Regresses betreffen, noch immer im Vordergrund. Es ergeben sich aber auch entscheidende neue Einsichten. Diese betreffen vor allem das auf die Zukunft gerichtete `protentionale' Zeitbewusstsein in seiner Verflechtung mit der `Retention' der Vergangenheit sowie eine spezifisch noematische Bestimmung der (...) Zeitmodalitäten, die zu einer neuen Phänomenologie der Individuation führt. Auch dem Zusammenhang zwischen zeitlichem Urstrom und ichlicher Zeitigung widmet Husserl erstmals eingehende Untersuchungen. In recent years, virtue theories have enjoyed a renaissance of interest among general and medical ethicists. This book offers a virtue-based ethic for medicine, the health professions, and health care. Beginning with a historical account of the concept of virtue, the authors construct a theory of the place of the virtues in medical practice. Their theory is grounded in the nature and ends of medicine as a special kind of human activity. This eloquent 1757 treatise examines how interactions with the physical world affect formulation of ideals related to beauty and art. What nonsense might be, and what Wittgenstein thought that nonsense might be, are two of the central questions in the current debate between those—such as Cora Diamond, James Conant and Michael Kremer—who favour a “resolute” approach to Wittgenstein’s work, and those—such as P. M. S. Hacker and Hans-Johann Glock—who instead favour a more “traditional” approach. What answer we give to these questions will determine the nature and force of his criticisms of traditional philosophy, and so the very shape Wittgenstein’s work (...) has for us, as well as, to some extent, what the lesson of the Tractatus might be. My aim in this paper is to provide a detailed defence of the austere view of nonsense, that lies at the heart of the resolute approach, against a range of influentialcriticisms developed by Hans-Johann Glock and which focus on Wittgenstein’s contextualism. In so doing, I hope also to shed some light on the kind of view the austere view is, as well as how it might relate to certain other crucial aspects of Wittgenstein’s thought. (shrink)

The nature of reality has exercised philosophers and mystics, theologians and shamans. Yet what we perceive as reality is bounded, if not defined, by the apparatus with which we perceive: our brains. Neurology has to know the capacity normal and disordered of the tool with which we see the world; neurologists use evidence of disordered perception to recognize and classify illness in the brain.

In this companion volume to their 1981 work, A Philosophical Basis of Medical Practice, Pellegrino and Thomasma examine the principle of beneficence and its role in the practice of medicine. Their analysis, which is grounded in a thorough-going philosophy of medicine, addresses a wide array of practical and ethical concerns that are a part of health care decision-making today. Among these issues are the withdrawing and withholding of nutrition and hydration, competency assessment, the requirements for valid surrogate decision-making, quality-of-life determinations, (...) the allocation of scarce health care resources, medical gatekeeping, and for-profit medicine. The authors argue for the restoration of beneficence to its place as the fundamental principle of medical ethics. They maintain that to be guided by beneficence a physician must perform a right and good healing action which is consonant with the individual patient 's values. In order to act in the patient 's best interests, or the patient 's good, the physician and patient must discern what that good is. This knowledge is gained only through a process of dialogue between patient and/or family and physician which respects and honors the patient 's autonomous self-understanding and choice in the matter of treatment options. This emphasis on a dialogical discernment of the patient 's good rejects the assumption long held in medicine that what is considered to be the medical good is necessarily the good for this patient. In viewing autonomy as a necessary condition of beneficence, the authors move beyond a trend in the medical ethics literature which identifies beneficence with paternalism. In their analysis of beneficence, the authors reject the current emphasis on rights- and duty-based ethical systems in favor of a virtue-based theory which is grounded in the physician- patient relationship. This book's provocative contributions to medical ethics will be of great interest not only to physicians and other health professionals, but also to ethicists, students, patients, families, and all others concerned with the relationship of professional to patient and patient to professional in health care today. An important philosophical issue in the study of addiction is what difference the fact that a person is addicted makes to attributions of autonomy (and responsibility) to their drug-oriented behavior. In spite of accumulating evidence suggesting the role of emotional dysregulation in understanding addiction, it has received surprisingly little attention in the debate about this issue. I claim that, as a result, an important aspect of the autonomy impairment of many addicted individuals has been largely overlooked. A widely shared assumption (...) in the philosophical literature is that for addiction to impair a person’s autonomy it has to make them (in some sense) take drugs against their will. So-called “willing addicts” are therefore usually seen as exempted from the autonomy impairment believed to characterize “unwilling addicts”, the latter being those who ‘truly want’ to stop using drugs but find their attempts repeatedly derailed by failures of self-control. In this article, I argue that the association between addiction and emotional dysregulation shows why this assumption is false. Emotional dysregulation is not only consistent with the possibility that many addicts take drugs ‘willingly’, it supports the hypothesis that they use drugs because they truly want to. The article proposes an explanation for why emotional dysregulation should nevertheless be seen as an aspect of their loss of control and an important reason why they have impaired autonomy. I end by exploring some implications of this account for addict’s decision-making capacity when they are prescribed the drugs to which they are addicted. -/- . (shrink)

ABSTRACT In a series of experiments at the National Institute of Mental Health, the animal ecologist John B. Calhoun offered rats everything they needed, except space. The resulting population explosion was followed by a series of “social pathologies”—violence, sexual deviance, and withdrawal. This essay examines the influence of Calhoun's experiments among psychologists and sociologists concerned with the effects of the built environment on health and behavior. Some saw evidence of the danger of the crowd in Calhoun's “rat cities” and fastened (...) on a method of analysis that could be transferred to the study of urban man. Others, however, cautioned against drawing analogies between rodents and humans. The ensuing dispute saw social scientists involved in a careful negotiation over the structure and meaning of Calhoun's experimental systems and, with it, over the significance of the crowd in the laboratory, institution, and city. We tend to take for granted that we know what is involved in belief in other minds, and that the real problem lies in justifying that belief. By contrast, this chapter argues that we misunderstand what belief in other minds involves, and that the problem of other minds has its source in that misunderstanding. My aim is to rethink what belief in other minds involves in terms of what Wittgenstein calls ‘an attitude towards a soul’. Doing so not only undermines (...) the problem of other minds as traditionally formulated, but also has significant consequences for our understanding of the mind itself. One such consequence is that ethical considerations have a much more fundamental role to play in thinking about the mind than is typically believed. (shrink)

It is an orthodoxy of modern political thought that violence is morally incompatible with politics, with the important exception of the permissible violence carried out by the state. The “commonsense argument” for permissible political violence denies this by extending the principles of defensive ethics to the context of state-subject interaction. This article has two aims: First, I critically investigate the commonsense argument and its limits. I argue that the scope of permissions it licenses is significantly more limited than its proponents (...) allow. Second, I develop an alternative (and supplementary) framework for thinking about permissible political violence. I argue that under certain circumstances, subjects may violently protest their treatment, where protest is understood as an expression of rejection of those circumstances. On my view, protest, including violent protest, is permissible when it is the fitting response to those circumstances. This alternative framework accounts for an important class of cases of intuitively permissible political violence, including cases in which such violence does not serve strategic political ends or is even counterproductive towards those ends. Business ethicists should examine not only business practices but whether a particular type of business is even prima facie ethical. To illustrate how this might be done I here examine the contemporary U.S. defense industry. In the past the U.S. military has engaged in missions that arguably satisfied the just war self-defense rationale, thereby implying that its suppliers of equipment and services were ethical as well. Some recent U.S. military missions, however, arguably fail the self-defense rationale. At issue, then, is (...) whether a business supporting these latter missions may not be circumstantially unethical. No it is not, say defense industry advocates, for two principal reasons. For one, this business benefits society at large in numerous ways. And, for another, the organizer of these military missions is a superpower which by its very nature is not subject to the ethical constraints of the self-defense rationale. I dispute both reasons, argue against the second, and conclude that the U.S. military-industrial complex (MIC) is circumstantially unethical. (shrink)

The moral authority for professional ethics in medicine customarily rests in some source external to medicine, i.e., a pre-existing philosophical system of ethics or some form of social construction, like consensus or dialogue. Rather, internal morality is grounded in the phenomena of medicine, i.e., in the nature of the clinical encounter between physician and patient. From this, a philosophy of medicine is derived which gives moral force to the duties, virtues and obligations of physicians qua physicians. Similarly, an ethic specific (...) to the other healing professions, law, teaching or ministry, can be derived from the specific ends to telos of each of these professions, which like medicine, are focused on a special type of human relationship. Central to a new, or 'resolute', reading of Wittgenstein's Tractatus Logico- Philosophicus is the idea that Wittgenstein held there an 'austere' view of nonsense: the view, that is, that nonsense is only ever a matter of our failure to give words a meaning, and so that there are no logically distinct kinds of nonsense. Resolute readers tend not only to ascribe such a view to Wittgenstein, but also to subscribe to it themselves; and it is also a feature of some (...) readings which in other respects are clearly not Resolute. This paper forms part of a reply to Hans-Johann Glock's work in which he argues (in part) that Wittgenstein in the Tractatus held a view of nonsense other than the austere view. Instead, Glock argues, Wittgenstein there held that there are many logically distinct kinds of nonsense. Here, I outline and defend the austere view, together with its attribution to the early Wittgenstein, against a number of Glock's criticisms, and focussing especially on Wittgenstein's reformulation in the Tractatus of Frege's context-principle. Der Soziologe Robert Bellah hat mit seinem Forschungsteam eine eigene Version des Kommunitarismus entwickelt. Dies geschieht in zwei wichtigen Werken zu den amerikanischen „Gewohnheiten des Herzens“ und zur „guten Gesellschaft“. In seinem Spätwerk zur „Religion innerhalb der menschlichen Evolution“ wird diese Version religionsgeschichtlich unterfüttert. Behandelt wird im Beitrag zudem die kontroverse Rezeption von Bellahs Ansatz, der einen demokratischen Kommunitarismus vertritt.

Corporate social responsibility (CSR) has become a focal point for research aimed at extending business ethics to extra-corporate issues; and as a result many companies now seek to at least appear dedicated to one or another version of CSR. This has not affected the arms industry, however. For, this industry has not been discussed in CSR literature, perhaps because few CSR scholars have questioned this industry's privileged status as an instrument of national sovereignty. But major changes in the organization of (...) political communities call traditional views of sovereignty into question. With these considerations in mind I assess the U.S. arms industry on the basis of CSR requirements regarding the environment, social equity, profitability, and use of political power. I find that this industry fails to meet any of these four CSR requirements. Countering a claim that these failings should not be held against arms manufacturers because their products are crucial to national defense, I contend that many of these companies function not as dutiful agents of a nation-state but as politically powerful entities in their own right. So, I conclude, they should be held responsible for the foreseeable consequences that flow from use of their products. This responsibility should include civil liability and, in cases involving war crimes and violations of human rights, responsibility under international human rights standards. (shrink)
