ABSTRACTAnxiety and depression diagnoses are associated with suicidal thoughts and behaviours. However, a categorical understanding of these associations limits insight into identifying dimensional mechanisms of suicide risk. This study investigated anxious and depressive features through a lens of suicide risk, independent of diagnosis. Latent class analysis of 97 depression, anxiety, and suicidality-related items among 616 psychiatric outpatients indicated a 3-class solution, specifically: a higher suicide-risk class uniquely differentiated from both other classes by high reported levels of depression and anxious (...) arousal; a lower suicide-risk class that reported levels of anxiety sensitivity and generalised worry comparable to Class 1, but lower levels of depression and anxious arousal; and a low to non-suicidal class that reported relatively low levels across all depression and anxiety measures. BackgroundThe death of a loved one can trigger a range of responses, including painful thoughts and emotions, as well as positive changes, such as post-traumatic growth. To understand more about the relationship between these outcomes this study explored the co-occurrence of depression, anxiety and PTG among a group of bereaved Chinese adults.MethodsData were collected from 194 participants, who had lost a first-degree relative. Latent class analysis was used to analyze the data to identify subgroups of participants with shared symptom profiles.ResultsThree (...) classes were identified: a Growth class, a Depression/Anxiety/Growth class and a Depression/Anxiety class. Marital status, social support, close relationship with the deceased and witnessing the suffering of the deceased were identified as differential predictors of class membership.ConclusionThe findings contribute to our understanding of the potentially wide ranging impacts of bereavement and highlight the important role of stressor characteristics and support and in influencing impairment and positive outcomes. Motivational internalism (MI) holds that, necessarily, if an agent judges that she is morally obligated to ø, then, that agent is, to at least some minimal extent, motivated to ø. Opponents of MI sometimes invoke depression as a counterexample on the grounds that depressed individuals appear to sincerely affirm moral judgments but are ‘listless’ and unmotivated by such judgments. Such listlessness is a credible counterexample to MI, I argue, only if the actual clinical disorder of depression, rather than a (...) merely hypothetical example of such listlessness, is the source of this listlessness. However, empirical evidence concerning depression shows that, to the extent that the depressed are motivationally listless at all, they are abnormally listless only with respect to an important class of non-moral judgments, namely, their prudential normative judgments (i.e., those concerning their own happiness and well-being), not their moral judgments. Hence, depressed individuals do not constitute a counterexample to MI. This conclusion has important methodological implications concerning how supporters and opponents of MI can best defend their respective theses. (shrink)

In this article, I argue that depression and suicide are natural kinds insofar as they are classes of abnormal behavior underwritten by sets of stable biological mechanisms. In particular, depression and suicide are neurobiological kinds characterized by disturbances in serotonin functioning that affect various brain areas (i.e., the amygdala, anterior cingulate, prefrontal cortex, and hippocampus). The significance of this argument is that the natural (biological) basis of depression and suicide allows for reliable projectable inferences (i.e., predictions) to be made (...) about individual members of a kind. In the context of assisted suicide, inferences about the decision-making capacity of depressed individuals seeking physician-assisted suicide are of special interest. I examine evidence that depression can hamper the decision-making capacity of individuals seeking assisted suicide and discuss some implications. (shrink)

The differentiation of trait anxiety and depression in nonclinical and clinical populations is addressed. Following the tripartite model, it is assumed that anxiety and depression share a large portion of negative affectivity, but differ with respect to bodily hyperarousal and anhedonia. In contrast to the tripartite model, NA is subdivided into worry and dysthymia, which leads to a four-variable model of anxiety and depression encompassing emotionality, worry, dysthymia, and anhedonia. Item-level confirmatory factor analyses and latent class cluster analysis based on (...) a large nation-wide representative German sample substantiate the construct validity of the model. Further evidence concerning convergent and discriminant validity with respect to related constructs is obtained in two smaller nonclinical and clinical samples. Marlene van Niekerk's 1994 Triomf is a plaasroman, or farm novel, without the farm; it formally resembles a nostalgic pastoral genre initiated by the collapse of Southern African agricultural economy around the time of the Great Depression, but removes even the symbol of the farm as aesthetic compensation for material loss. In the process, van Niekerk composes a post-apartheid tragicomedy of a lumpenproletariat white supremacist family coming into long-belated class consciousness, an epiphany which, surprisingly, survives the novel's translations from Afrikaans (...) to South African English to 'international' English. Crucially, this understanding is mediated by a critical tendency to appraise Triomf in the context of Faulkner and the Southern Gothic, a generic comparison which gets a lot wrong but is ultimately very revealing, less about Triomf than about the imperial world-system through which it circulates and is consecrated. Consequently, the novel stages globally what seems at first to be a parochial question: how is one supposed to imagine democratic reconciliation and integration after apartheid, when one of the classes to be reconciled lacks historical self-consciousness and has no obvious place in either the apartheid regime or the post-apartheid dispensation? By analyzing van Niekerk's novel and the institutions which consecrate it, the paper fleshes out critiques of world-literary hermeneutics, specifically for its naive handling of genre and context, and of post-apartheid 'reconciliation' under capital. Adequate measurement is an essential component of the assessment of mental health disorders and symptoms such as depression and anxiety. The present study investigated sex-specific differences in the Patient Health Questionnaire-9 and Generalized Anxiety Disorder-7. This comprehensive cross-sectional design study pursued four objectives: measurement invariance of PHQ-9 and GAD-7 between male and female; depression and anxiety prevalence differences; cross-sex differences in the relationship between depression and anxiety; and a comparison of symptom heterogeneity. A sample of 1966 students from South Africa (...) completed the PHQ-9 and the GAD-7. Data analyses for measurement invariance, latent class analysis, inter-variable correlations and group comparisons were conducted in Mplus. The two-dimensional PHQ-9 achieved scalar invariance, while the GAD-7 yielded metric invariance. The somatic and non-somatic latent dimensions of depression were compared and showed no significant difference between male and female groups. The positive relationship between depression and anxiety was also not significantly different between the two groups. While the PHQ-9 symptoms formed three classes in the male group, and four classes in the female group, the GAD-7 had the same number of classes and a similar pattern between the two groups. These findings hold implications for the measurement, assessment and understanding of symptom manifestation and distribution, as well as the treatment of depression and anxiety in South Africa. We report on an interview-based study of decision-making capacity in two classes of patients suffering from depression. Developing a method of second-person hermeneutic phenomenology, we articulate the distinctive combination of temporal agility and temporal inability characteristic of the experience of severely depressed patients. We argue that a cluster of decision-specific temporal abilities is a critical element of decision-making capacity, and we show that loss of these abilities is a risk factor distinguishing severely depressed patients from mildly/moderately (...) class='Hi'>depressed patients. In the light of a debate in which the middle class emerges as an indisputable keyword of the American political cultures, the essay points (...) out the historical origin of the fetish of middle class by showing the role that US social sciences have had in the making of the middle class as an ideological notion at the very foundation of the American century. Released by its fetish, middle class emerges as a historical category that allows to trace the formation of the liberal order and its transition to the neoliberal order, to point to the crisis and transformation of capitalism and the American state, to identify the frontier beyond which their historical legitimacy is missing. Depression has become one of the most common mental illnesses, and the widespread use of social media provides new ideas for detecting various mental illnesses. The purpose of this study is to use machine learning technology to detect users of depressive patients based on user-shared content and posting behaviors in social media. At present, the existing research mostly uses a single detection method, and the unbalanced class distribution often leads to a low recognition rate. In addition, a large number of (...) irrelevant or redundant features in high-dimensional data sets interfere with the accuracy of recognition. To solve this problem, this paper proposes a hybrid feature selection and stacking ensemble strategy for depression user detection. First, recursive elimination method and extremely randomized trees method are used to calculate feature importance and mutual information value, calculate feature weight vector, and select the optimal feature subset according to the feature weight. Second, naive bayes, k-nearest neighbor, regularized logistic regression and support vector machine are used as base learners, and a simple logistic regression algorithm is used as a combination strategy to build a stacking model. Experimental results show that compared with other machine learning algorithms, the proposed hybrid method, which integrates feature selection and ensemble, has a higher accuracy of 90.27% in identifying online patients. We believe this study will help develop new methods to identify depressed people in social networks, providing guidance for future research. (shrink)

Background: The comorbidity of posttraumatic stress disorder and depression is common among people living with the HIV. Given the high prevalence and serious clinical consequences of the comorbidity of these two disorders, we conducted a latent profile analysis to examine the co-occurrence pattern of PTSD and depression in PLWH.Methods: The data for this cross-sectional study of PLWH were collected from 602 patients with HIV in China. A secondary analysis using latent profile analysis was conducted to examine HIV-related PTSD and depression (...) symptoms.Results: A four-class solution fits the data best, with the four classes characterized as asymptomatic, mild symptoms, low to moderate symptoms, and high to moderate symptoms. The severity of PTSD and depression symptoms was comparable in this solution, and no group was dominated by PTSD or depression.Conclusion: The absence of a distinct subcluster of PLWH with only PTSD or depression symptoms supports that PTSD and depression in PLWH are psychopathological manifestations after traumatic exposures. Health care staff should pay more attention to the existence of comorbid symptoms of individuals, develop integrated interventions for the symptoms cluster, and evaluate their effectiveness in clinical practice. The dual-continua model of mental health provides a contemporary framework for conceptualising and operationalising mental health. According to this model, mental health is distinct from but related to mental illness, and not the opposite or merely the absence of psychopathology symptoms. To examine the validity of the dual-continua model, previous studies have either applied variable-based analysis such as confirmatory factor analysis, or used predetermined cut-off points for subgroup division. The present study extends this contribution by subjecting data from an African (...) sample to both CFA and latent class analysis to test the dual-continua model in Africa. We applied CFA separately for the Mental Health Continuum—Short Form and Patient Health Questionnaire—9 ; and LCA on combined item responses. College students from Ghana, Kenya, Mozambique, and South Africa completed the MHC-SF and PHQ-9. With minor modifications to the measurement models, the CFA results of this study confirm the three-factor structure of the MHC-SF, and a unidimensional solution for the PHQ-9. LCA results show the presence of three distinct latent classes: languishing with moderate endorsement of depressive symptoms, flourishing with low endorsement of depressive symptoms, and moderate mental health with high endorsement of depressive symptoms. These findings further contribute to affirming the evidence for the dual-continua model of mental health, with implications for the assessment of mental health, to inform policy, practise, and future research in community and clinical settings in Africa. In this study, the well-known CES-D depression scale was considered as a prototypical tool for assessing an individual’s inclination to respond at random. It was postulated that the set of the responses obtained from the participants of a questionnaire survey could be divided into three classes: the honest responses class; the pure random class, characterized by the same prob- ability of each admissible score; the smart random class, characterized by the same distribution of the probability of scores as in (...) the honest responses class. A two-step classifying procedure was recommended. According to this procedure, the first step should be to separate the pure random responses from the others. As needed, at the second step the smart random responses should be separated from the honest responses. The CES-D scale consists of sixteen direct questions and four reverse questions, each question with four admissible score values, 0, 1, 2, or 3. The material for the analyses includes all of the 4 294 967 296 possible arrangements of the four scores for the sixteen direct questions, and all of the 256 arrangements of the four scores for the four reverse questions, and all of the possible 49 · 13 = 637 pairs of the possible values of the mean scores from the sixteen direct questions and from the four reverse questions, respectively. It was assumed, without loss of generality, that the honest responses to the sixteen direct questions lead to the resulting distribution: P = (8/16; 5/16; 2/16; 1/16), for a score equal to 0, 1, 2, or 3, respectively. The original iterative procedure was used to compute the exact distributions of the mean scores in the pure random class, and in the smart random class. The discriminant ability to dis- tinguish between the two classes was characterized with the Receiver Operating Characteristic (ROC) curve. In result, the excellent discriminant properties of the CES-D were confirmed. The area under the ROC curve was estimated equal to AUC = 0.98 with standard error SE = 0.004 for the screening of the pure random responses from all of the others, and equal to AUC = 0.87 with standard error SE = 0.006 for the purpose of distinguishing the smart random responses from the honest ones. This article had several purposes. First, I wanted to highlight the work of Esther Bubley, an American photographer whose documentary work for the Farm Security Administration and the Office of War Information in the early 1940s is largely unknown. Second, I wanted to show how her images complicated and undermined the traditional themes of Depression era photography in the United States, Third, by looking at her images of women, my intention was to reveal how she worked against depictions of femininity (...) during the Depression, and in confrontation with one-dimensional portrayals of women as America entered the Second World Wan In conclusion, I contend that Bubley's images were fundamentally portrayals of working-class femininity represented as being an individual – rather than a symbolic – experience. Most specifically in the images I have examined, Bubley deconstructs an ideological image of female working-class identity which was central to documentary photography in 1930s America. For example, unlike in photographs by Dorothea Lange, Bubley did not portray working-class women as metaphoric sites of passive endurance which would eventually lead to the rejuvenation of American nationalism. Rather, she showed working-class women to be potentially subversive in the ways they defined themselves against the legacy of 1930s photography and in opposition to the ideological impositions of wartime propaganda. As a result, Bubley's images of working-class women waiting in bars for lonely soldiers, or looking for a future beyond the confines of their boarding house existences while remaining outside the middle-class boundaries defined by capitalist consumerism, set out a pictorial foundation for working-class female identity which exists beyond the context in which the photographs were taken. Consequently, Bubley's work highlights individual self-identity, personal empowerment and self-conscious desire in working-class women which was – and still is – confined and repressed by economic disadvantage and systematic marginalization from an American society defined from a middle-class point of view. Der Jenaer Fichte hat nicht nur auf seine Zeitgenossen am stärksten gewirkt, er war auch seinerseits damals noch am offensten für Einflüsse. Der vorliegende Kommentar – der erste, der den gesamten deutschen Text behandelt – bietet keine Paraphrase, keine Übersetzung in eine zeitgemäßere Sprache, keine „Darstellung“, die Fichtes Disposition durch eine eigene (...) ersetzt und damit – gewollt oder ungewollt – in Konkurrenz zum Original tritt. Er will vielmehr den Leser auf seinem dornigen Pfad Satz für Satz begleiten und bei keiner Schwierigkeit im Stich lassen. Die konkrete Erforschung literarischer Bezüge und Abhängigkeiten zeigt, wie sehr der „Selbstdenker“ Fichte – unbeschadet seiner Eigenständigkeit – nicht nur in der Philosophie, sondern im ganzen Geistesleben der Epoche verwurzelt ist. The present "philological commentary" is directed at those who have decided to take time for reading the original text, at least in an English translation. The first volume "Textual Criticism" is intended to meet the difficulties caused by the fact that our text editions are based on manuscripts separated from the original by more than a millennium.

With the third volume, it is invited to enter the intellectual environment of Aristotle. The most relevant sources are given in full (with English translation), so that the commentary is also a reader documenting the disputationes metaphysicae of the 4th century BC. For the undeniable contradictions in the Metaphysics, a new genetic explanation is offered.

Though since Werner Jaeger's famous Studien zur Entstehungsgeschichte der Metaphysik des Aristoteles of 1912 remarkable observations were made, the topic "Composition of the Metaphysics" almost disappeared from the agenda. As, however, neglect of this philological task results in either selective reading or anachronistic systematization, the author has resumed it, extending it to the internal structure of the singular books.

Zu diesen gehört an erster Stelle Kants Sprache; sie ist nicht mehr die unsere, was den unvorbereiteten Leser von heute schon an der Semantik und Syntax vieler Sätze scheitern lässt. Neben einer Fülle von sprachlichen Erläuterungen und textkritischen Untersuchungen bringt der Kommentar reichliche Zitate aus den von Kant benutzten Logik- und Metaphysik-Lehrbüchern, die seine Rezeption der "dogmatischen" Philosophie Christian Wolffs und seiner Nachfolger belegen; lateinische (...) Texte werden dabei sowohl im Original als auch in deutscher Übersetzung geboten. Einzelne Motive werden ideengeschichtlich bis in die Antike zurückverfolgt; hieraus eröffnen sich Perspektiven für eine "Metakritik" von einem "vorkritischen" Standpunkt aus, die im Rahmen eines philologischen Kommentars nur angedeutet werden können. When a professor from Middle Europe teaches Computer Ethics in the Middle East using a textbook from the US, cultural differences become apparent. A main challenge lies in avoiding cultural imperialism during teaching. In order to meet this challenge, personal portfolios have been used for course work. The course design as well as portfolio tasks are presented and experiences are discussed. Based on our experiences we recommend applying this approach to equally overcome effects of group dynamics in similar courses as (...) well as to bridge the generational gap that normally exists between students and professors. Dr. B.R. Ambedkar is one of the names who advocated to change social order of the age-old tradition of suppression and humiliation. He was an intellectual, scholar, statesman and contributed greatly in the nation building. He led a number of movements to emancipate the downtrodden masses and to secure human rights to millions of depressed classes. He has left an indelible imprint through his immense contribution in framing the modern Constitution of free India. He stands as a symbol (...) of struggle for achieving the Social Justice. We can assign several roles to this great personality due to his life full dedication towards his mission of eradicating evils from Indian society. The social evils of Indian society, also neglected this great personality even in intellectual sphere too. The so-called intellectuals of India not honestly discussed his contribution to Indian intellectual heritage, rather what they discussed, also smells their biases towards a Dalit literate and underestimated his great personality. This paper will attempt to discuss important facts about life and a short description of the literature written by Dr. B.R. Ambedkar. This is followed by discussion his philosophy in the five major sections i.e. Feminism and women empowerment, philosophy of education, ideas on social justice and equality, philosophy of politics and economics and philosophy of religion. ‘This class is so [insert expletive] depressing.’ I overheard a student communicating this to a friend upon exiting one of my ethics courses and I wondered how my classes could generate a sense of empowerment rather than depression, a sense of hope rather than despair. Drawing from David Hume's and Martin Hoffman's work on the psychology of empathy and sympathy, I contend that dominant Western philosophical pedagogy is inadequate for facilitating morally empowered students. Moreover, I stipulate that an adequate (...) analysis of the role emotion should play in pedagogy requires tending to the politics of emotional expression and how oppression functions. I argue that ethical educators have a moral responsibility to facilitate not only critical moral thinking but critical moral agency. Part of ethical education should involve the provision of tools for effective citizen engagement, and reasoning alone is insufficient for this goal. The present theory section deals with learned helplessness produced by pervasive experiences of failure or negative events, leading to decreased motivation and risk for depression. In their target article, Boddez, van Dessel, and de Houwer apply this concept to different forms of psychological suffering and propose a goal-directed mechanism –generalisation over similar goals. Duda and Joormann define goal similarity by action-outcome contingencies and highlight individual differences in attribution styles. Brandstätter proposes incentive classes as the organising principle for goal similarity (...) and explores outcomes such as deterioration of cognitive ability. Mikulincer and Lifshin focus on differences between goals and introduce the notion of motivated helplessness, serving as an anxiety buffer in uncontrollable life settings. Finally, potential contributions from emotion science are outlined, in particular individual differences and biases in appraising goals and coping potential likely to produce negative emotion dispositions. In conclusion, the need to elaborate a more comprehensive theoretical framework for helplessness that can guide empirical studies using multifactorial and longitudinal designs is emphasised. Given the strong increase in the prevalence of anxiety and depression worldwide, better understanding of predispositions and eliciting factors is needed for early diagnosis and the development of intervention programmes to reduce psychological suffering. Hence, we used a latent variable and a person-centered approach to identify different patterns of perfectionism, and their relationships with psychological health as outcome among early adolescents (13-14 years) attending conventional or elite sports-/performance-oriented lower secondary schools (14 schools, 832 students, 53% girls). All students completed two perfectionism (...) scales, i.e., the Child-Adolescent Perfectionism Scale (CAPS) and the Frost Multidimensional Perfectionism Scale (FMPS). The criterion-related variables of psychological health included anxiety, depression, eating disorder problems, self-worth and resilience, respectively. Exploratory and confirmatory factor analyses yielded a four-factor representation of perfectionism. Using latent class analysis extracted five profiles of perfectionism, which were related to the criterion variables. Three profiles were clear indicators of either low or high perfectionism score patterns. Two profiles showed a mixed picture of high and low scores, whereas one represented a psychological healthy subgroup. About four of ten adolescents in the ordinary schools matched the two most debilitating perfectionism profiles compared to two of ten in the elite schools. How these results align with international findings is discussed along with the relevance for early interventions aimed at preventing the potential downsides of perfectionism. Longitudinal studies are needed to explore profile trajectories as well as possible health consequences. (shrink)

This paper addresses philosophical issues concerning whether mental disorders are natural kinds and how the DSM should classify mental disorders. I argue that some mental disorders (e.g., schizophrenia, depression) are natural kinds in the sense that they are natural classes constituted by a set of stable biological mechanisms. I subsequently argue that a theoretical and causal approach to classification would provide a superior method for classifying natural kinds than the purely descriptive approach adopted by the DSM since DSM-III. My (...) argument suggests that the DSM should classify natural kinds in order to provide predictively useful (i.e., projectable) diagnostic categories and that a causal approach to classification would provide a more promising method for formulating valid diagnostic categories. Franco “Bifo” Berardi tells us that the current transformation of every domain of social life into economy has led to “the subjugation of the soul to work processes.” There is a newfound love of work and, consequently, writes Berardi, “no desire, no vitality seems to exist anymore outside of the economic enterprise.” Concerned as it once was with “fostering the soul,” and concerned as it now is with preparing students for the job market, what role might education have in Berardi’s (...) musings? Can “creativity,” “self-actualization” and “curiosity,” which are so valued in education, still speak and help foster a sense of the soul that exceeds economy and work? In this paper, I explore these questions by providing an account of what is at stake in Berardi’s conception of the soul and its subjugation to work. I draw on his ideas to critically appraise the seemingly-benign aspirations of the “creative class,” a term coined to signal a new era for human flourishing afforded by shifts towards personal creativity in the digital economy. I then move to consider how, in particular, the ubiquitous promotion of “creativity” and “curiosity” in education tends to replicate a form of learning that puts the soul to work, as it were, promoting a narrow, self-enterprising subject with a frantic instrumental orientation towards the world. I conclude with a discussion on how demands on learners, to be always curious, innovative, nimble and open to discovery lead to panic, collapse and depression: the diseases of the soul. (shrink)

_Confronting American Labor_ traces the development of the American left, from the Depression era through the Cold War, by examining four representative intellectuals who grappled with the difficult question of labor’s role in society. Since the time of Marx, leftists have raised over and over the question of how an intelligentsia might participate in a movement carried out by the working class. Their modus operandi was to champion those who suffered injustice at the hands of the powerful. From the late (...) nineteenth through much of the twentieth century, this meant a focus on the industrial worker. The Great Depression was a time of remarkable consensus among leftist intellectuals, who often interpreted worker militancy as the harbinger of impending radical change. While most Americans waited out the crisis, listening to the assurances of President Franklin D. Roosevelt, the Marxian left was convinced that the crisis was systemic. Intellectuals who came of age during the Depression developed the view that the labor movement in America was to be the organizing base for a proletariat. Moreover, many came from working-class backgrounds that contributed to their support of labor. World War II and the resultant economic recovery shattered this coherence on the left. How did radicals opposed to capitalism deal with a labor movement that was very successful in terms of membership and power but clearly capitalist in its orientation? Coker describes the marked ambivalence and confusion of the intellectual left in the postwar years—a period of frustration brought on by a misreading of labor militancy during the 1930s and an unsuccessful search for a radical proletarian movement. The result was a politically and intellectually weakened left for decades to come. C_onfronting American Labor_ examines four individuals who represent a cross section of postwar radicalism. Each came of age on the socialist left, expecting that an anticapitalist movement would emerge from the ranks of labor. Seymour Martin Lipset and C. Wright Mills were professional sociologists. Sidney Lens spent his early life working within the labor movement before becoming a political commentator for a variety of leftist magazines and journals in the postwar era. This book explores how the weather and city-form impact the mind, and how city-form and mind interact. It builds on Merleau-Ponty's contention that mind, the human body and the environment are intertwined in a singular composite, and on Walter Benjamin's suggestion that mind and city-form, in mutual interaction, through history, have set the course of civilization. Bringing together the fields of philosophy, urbanism, geography, history, and architecture, the book shows the association of existentialism with prevalence of mood disorder in Northern (...) Europe at the close of Little Ice Age. It explains the implications of city-form and traces the role of the myths and allegories of urban design as well as the history of gender projection onto city-form. It shows how urbanization in Northern Europe provided easier access to shelter, yet resulted in sunlight deprivation, and yielded increasing incidence of depression and other mental disorder among the European middle-class. The book uses the examples of Kierkegaard, Nietzsche, Dostoevsky and Kafka, to show how walking through the streets, squares and other urban voids became the informal remedy to mood disorder, a prominent trait among founders of modern Existentialism. Drawing the line on physician assistance in physician-assisted death continues to be a contentious issue in many legal jurisdictions across the USA, Canada and Europe. PAD is a medical practice that occurs when physicians either prescribe or administer lethal medication to their patients. As more legal jurisdictions establish PAD for at least some class of patients, the question of the proper scope of this practice has become pressing. This paper presents an argument for restricting PAD to the terminally ill that (...) can be accepted by defenders as well as critics of PAD for the terminally ill. The argument appeals to fairness-based paternalism and the social meaning of medical practice. These two considerations interact in various ways, as the paper explains. The right way to think about the social meaning of medical practice bears on fair paternalism as it relates to PAD and vice versa. The paper contends that these considerations have substantial force when directed against proposals to extend PAD to non-terminally ill patients, but considerably less force when directed against PAD for the terminally ill. The paper pays special attention to the case of non-terminally ill patients who suffer from treatment-resistant depression, as these patients present a potentially strong case for extending PAD beyond the terminally ill.
