Much of the discourse in the bioethics literature on what is often called “cesarean delivery on maternal request” has focused on balancing respect for patient autonomy with attention to the short- and long-term risks of this procedure to maternal and neonatal well-being. And while there has been some analysis of the social and economic costs inherent in performing cesareans, much of the clinical and ethical analysis has concluded that, given the degree of risk to the mother and neonate from a (...) primary or single repeat cesarean delivery, there is sufficient justification for pregnant women to request and obtain this procedure from obstetricians willing to meet such requests. However, these same analyses often caution that physicians should not offer these elective cesareans to all women, but only to those who initiate the request. This paper analyzes recommendations that do not promote universal access to these procedures and concludes that such a policy is ethically unjustifiable, as it treats women who do not inquire about such procedures differently. Depriving appropriate candidates—for example, healthy women who are good surgical candidates and desire only one or two children—of information about elective cesarean delivery is a form of undue influence on women's decision-making. Primordial emotions are the subjective element of the instincts which are the genetically programmed behaviour patterns which contrive homeostasis. They include thirst, hunger for air, hunger for food, pain and hunger for specific minerals etc.There are two constituents of a primordial emotion—the specific sensation which when severe may be imperious, and the compelling intention for gratification by a consummatory act. They may dominate the stream of consciousness, and can have plenipotentiary power over behaviour.It is hypothesized that early in animal evolution (...) complex reflex mechanisms in the basal brain subserving homeostatic responses, in concert with elements of the reticular activating system subserving arousal, melded functionally with regions embodied in the progressive rostral development of the telencephalon. This included the emergent limbic and paralimbic areas, and the insula. This phylogenetically ancient organization subserved the origin of consciousness as the primordial emotion, which signalled that the organisms existence was immediately threatened. Neuroimaging confirms major activations in regions of the basal brain during primordial emotions in humans. The behaviour of decorticate humans and animals is discussed in relation to the possible existence of primitive awareness.Neuroimaging of the primordial emotions reveals that rapid gratification of intention by a consummatory act such as ingestion causes precipitate decline of both the initiating sensation and the intention. There is contemporaneous rapid disappearance of particular regions of brain activation which suggests they may be part of the jointly sufficient and severally necessary activations and deactivations which correlate with consciousness [Crick, F. & Koch, C. . After more than 15 years of study, the 1/f noise or complex-systems approach to cognitive science has delivered promises of progress, colorful verbiage, and statistical analyses of phenomena whose relevance for cognition remains unclear. What the complex-systems approach has arguably failed to deliver are concrete insights about how people perceive, think, decide, and act. Without formal models that implement the proposed abstract concepts, the complex-systems approach to cognitive science runs the danger of becoming a philosophical exercise in futility. The complex-systems (...) approach can be informative and innovative, but only if it is implemented as a formal model that allows concrete prediction, falsification, and comparison against more traditional approaches. We report two experiments in which participants had to judge the time of occurrence of a stimulus relative to a clock. The experiments were based on the control condition used by Libet, Gleason, Wright, and Pearl [Libet, B., Gleason, C. A., Wright, E. W., & Pearl, D. K. . Time of conscious intention to act in relation to onset of cerebral activities : The unconscious initiation of a freely voluntary act. Brain 106, 623–642] to correct for any bias in the (...) estimation of the time at which an endogenous event, the conscious intention to perform a movement, occurred. Participants’ responses were affected systematically by the sensory modality of the stimulus and by the speed of the clock. Such findings demonstrate the variability in judging the time at which an exogenous event occurs and, by extension, suggest that such variability may also apply to the judging the time of occurrence of endogenous events. The reliability of participants’ estimations of when they formed the conscious intention to perform a movement in Libet et al.’s study is therefore questionable. (shrink)

Encounters with Alphonso Lingis is the first extensive study of this American philosopher who is gaining an international reputation to augment his national one. Using data collected from a survey document, the paper analyses the extent of the model code’s influence in an international process of harmonisation of ethical rules for accountants. Obstacles to the adoption of the model code are examined, as is the impact that government supervision of codes has had (...) on harmonisation efforts. Particular attention is paid to the relationship between professional independence and close family relationships and culture. Findings are also presented on other issues of code content, and the most appropriate body for interpreting national codes is identified. Experiential evidence shows that pain is associated with common meanings. These include a meaning of threat or danger, which is experienced as immediately distressing or unpleasant; cognitive meanings, which are focused on the long-term consequences of having chronic pain; and existential meanings such as hopelessness, which are more about the person with chronic pain than the pain itself. This interdisciplinary book - the second in the three-volume Meanings of Pain series edited by Dr Simon van Rysewyk - aims to better (...) understand pain by describing experiences of pain and the meanings these experiences hold for the people living through them. The lived experiences of pain described here involve various types of chronic pain, including spinal pain, labour pain, rheumatic pain, diabetic peripheral neuropathic pain, fibromyalgia, complex regional pain syndrome, endometriosis-associated pain, and cancer-related pain. Two chapters provide narrative descriptions of pain, recounted and interpreted by people with pain. Language is important to understanding the meaning of pain since it is the primary tool human beings use to manipulate meaning. As discussed in the book, linguistic meaning may hold clues to understanding some pain-related experiences, including the stigmatisation of people with pain, the dynamics of patient-clinician communication, and other issues, such as relationships between pain, public policy and the law, and attempts to develop a taxonomy of pain that is meaningful for patients. Clinical implications are described in each chapter. This book is intended for people with pain, their family members or caregivers, clinicians, researchers, advocates, and policy makers. (shrink)

In opposition to Aristotle, for whom practical reason differs from theoretical reason solely in the end toward which it directs its theoretical knowledge, Kant means by practical reason the capacity to determine an act unconditionally, that is, freely. Since the Kantian doctrine of determinants is based on a conception of mutual relation between practical reason and volition, this study traces the development, progressive clarification, and precise result of his thought from the Grundlegung zur Metaphysik der Sitten to the Metaphysik der (...) Sitten.... Identification of practical reason with the will in the notion of autonomy guides research on the objective determining ground.... "This is vintage Krell—he is as always, a reader in the best sense of the word...." —Dennis J. Schmidt "Krell is a strong and often eloquent writer.

This passage has occasioned, if not proelia, at least divergent interpretations, not to mention instances of tergiversation. In 1910 Cyril Bailey modelled his first rendering of the lines closely on that of H. A. J. Munro: ‘But unless the breast is cleared, what battles and dangers must enter into us in our own despite’; ‘but unless the heart is cleansed, what battles and dangers must then find their way into us in our own despite’. But a reprint of Bailey's translation (...) in 1921 produced the following change: ‘…what battles and perils must we then enter into despite our will’. What changed Bailey's mind? The explanation appeared years later in Bailey's great edition and commentary of 1947. In his note on the passage , he cites objections to Munro's interpretation that had been raised by C. N. Cole and W. R. Hardie. In this paper I will examine these objections and present my own interpretation of the passage, which amounts to a qualified vindication of Munro. (shrink)

The paper contrasts Hegel's often quite negative treatment of Eastern thought with the generally more positive treatment by W. F. J. Schelling. Especially significant for Schelling's treatment are the exceptional importance of female deities in Eastern myths and systems of thought and the positive role played by storytelling and mythology in the East.

In a discussion-note in Mind, Father P. M. Farrell, O.P., gave an account, in what he admitted to be an embarrassingly brief compass, of the Thomist doctrine concerning evil. There is one sentence in this discussion which at first glance appears paradoxical. Father Farrell has been arguing that a universe containing ‘corruptible good’ as well as incorruptible is better than one containing ‘incorruptible good’ only. He continues: ‘If, however, they are to manifest this corruptible good, they must be (...) corruptible and they must sometimes corrupt.’ The final words, despite Father Farrell's italics, strike one as expressing, not a self-evident truth, but a non sequitur. The fact that I am capable of committing murder does not entail that I will at some time commit it. It is not immediately obvious that a similar entailment holds in the case of corruption and corruptibility. (shrink)

This article examines J. G. Farrell’s depictions of colonial medicine as a means of analysing the historical reception of the further past and argues that the end-of-Empire context of the 1970s in which Farrell was writing informed his reappraisal of Imperial authority with particular regard to the limits of medical knowledge and treatment. The article illustrates how in The Siege of Krishnapur (1973), Farrell repeatedly sought to challenge the authority of medical and colonial history by making direct (...) use of period material in the construction of his fictional narrative; by using these sources with deliberate critical intent, Farrell directly engages with the received historical narrative of colonial India, that the British presence brought progress and development, particularly in matters relating to medicine and health. To support these assertions the paper examines how Farrell employed primary sources and period medical practices such as the nineteenth-century debate between miasma and waterborne Cholera transmission and the popularity of phrenology within his novels in order to cast doubt over and interrogate the British right to rule. Overall the paper will argue that Farrell’s critique of colonial medical practices, apparently based on science and reason, was shaped by the political context of the 1970s and used to question the wider moral position of Empire throughout his fiction.
