Recently, concerns have been raised about the phenomenon of 'overdiagnosis', the diagnosis of a condition that is not causing harm, and will not come to cause harm. Along with practical, ethical, and scientific questions, overdiagnosis raises questions about our concept of disease. In this paper, we analyse overdiagnosis as an epistemic problem and show how it challenges many existing accounts of disease. In particular, it raises questions about conceptual links drawn between disease and dysfunction, harm, and risk. We argue that (...) "disease" should be considered a vague concept with a nonclassical structure. On this view, overdiagnosed cases are 'borderline' cases of disease, falling in the zone between cases that are clearly disease, and cases that are clearly not disease. We then develop a précising definition of disease designed to provide practical help in preventing and limiting overdiagnosis. We argue that for this purpose, we can define disease as dysfunction that has a significant risk of causing severe harm to the patient. (shrink)

In this essay I explore the economic, social, environmental and cultural changes taking place in Bolsena, Italy, where agricultural livelihoods have rapidly diminished in the last two decades. I examine how gender dynamics have shifted with the changing values and livelihoods of Bolsena through three women’s narratives detailing their gendered experiences of environment, community and self. I reflect on these changes with Sabrina, who is engaged in a feminist community-based organization; Anna, who is running an alternative wine bar; and Isabella, (...) a jeweler, who is engaged in ecofeminist practices. My analysis is based on concepts developed by feminist political ecology: specifically, the theory of rooted networks from Dianne Rocheleau, Donna Haraway’s concept of naturecultures (and the work of J. K. Gibson-Graham on new economic imaginaries emerging from the politics of place. I aim to think with, reflect upon and provoke from the “otherwise”, taking into account the lived relations entwining nature and gender. My article looks at the interconnections of gender, environment and livelihoods, attentive to the daily needs, embodied interactions and labours of these three women as part of a reappropriation, reconstruction and reinvention of Bolsena’s lifeworld. By listening to the stories of their everyday lives and struggles, I show the dynamic potential of the politics of place and the efforts to build diverse economies and more ethical economic and ecological relationships based on gender-aware subjectivities and values. On these occasions, he would often attempt to clarify or refine the major concepts presented in that treatise. Nevertheless, even today many aspects of Perelman's work remain enigmatic. In outline, The New Rhetoric itself conveys a sense of structural and theoretical clarity, but for many readers this first impression evaporates when they confront the complexity and detail of the work's exposition. In the preface to their new study of Perelman, Alan Gross and Ray Dearin observe:The New Rhetoric is difficult to read, a task made even more difficult for American audiences because virtually all its examples and illustrations are from a literature in a foreign language.... Moreover, Perelman's later attempts to achieve clarity, in The Realm of Rhetoric, and numerous shorter articles, are equally puzzling. Consequently, some of Perelman's best ideas are virtually buried in a very long work, and therefore largely lost even to the most careful readers. In addition, on such central issues as the relationship between rhetoric and philosophy Perelman remains maddeningly obscure. Discussions of surface pattern in Islamic art resonate within broader tensions about the role of figural representation in communicating meaning. The question of whether geometric pattern communicates—whether it functions as a language without a code—reflects broader tensions about the relationship between secular and spiritual communication. Poised between discussions of modernism and Islam, the attribution of linguistic capacity to geometry serves as a measure for the possibility of abstracting pure reason from the religious roots of representationalism. This paper explores this question (...) in four parts. First, it examines the articulation of geometric meaning in Islamic discourses. It then explores European discussions of geometry between language and ornament poised between a search for universal reason and universal spiritualism, both in modernist painting and in modern exhibitions. The final section the expressive capacity of artistic responses to historical Islamic isometric geometries in the adaptive reuse of historical Iranian arts as contemporary abstraction in the sculptures of Monir Shahroudy Farmanfarmaian and in the popular transformation prints of M. C. Escher. It argues that the frequent recognition of intrinsic meaning in geometry, expressed in both premodern Islamic and modern secular interpretations, undermines art historical expectations of theorization as a necessary intermediary for communication. This paper reconstructs the deficiencies of formal democracies to explain the internal injustices of the modern state, the self‐righteous swaggering foreign policy of Western powers, and the dangerously over‐simplified, polar logic characterizing the war rhetoric of the modern era. In a brief tour through the non‐liberal tradition of democratic thought, drawing connections between the tragic mythological origins of Western understandings of self and world, the paper attempts to demonstrate that a failure to find alternate, healthier means of value‐creation has caused (...) Westerners, in their constructive identity work, to adhere themselves to their systems with a ritualized, ‘religious’ fervour. Legitimacy in the world becomes, in the final analysis, a simple matter of might. This paper considers the use of myth in the Platonic dialogues. It seeks to demonstrate that Plato takesup the task of rewriting the old myths, not in order to clarify the real truth about ancient tales, but to make thosetales serve higher—ethical—ends. Thus Plato makes a valiant effort to replace the old "truths" in order to displaceand overcome ethically dangerous assumptions in the old tales. But I shall demonstrate that, despite the changesin mythical content, the old tropes endure in the (...) new form and the dangerous elements of myth persist. Theelements of myth that I consider to be most dangerous are the motifs of "fallenness" and the irredeemably tragic dimension of earthly existence . These mythologems endure despite the new imagery that seeks to overcome them and they continue to breed a "nostalgia" of loss and tragic origins. This is the archaic infection that I see as the spawning ground of ritualistic patterns of human behaviour that are essentially violent. The infection is sublimated but carried alongin new contextual forms in Platonic myth, concealed but dynamically present. The persistence of these dangerous elements, in my reading, signal the failure of the Platonic project of purification—a failure most clearly evidenced in the tragic character of Eros and in the impotence of the philosopher in the city—indeed in Socrates' being ever atopos with regard to the city and missing even from the lofty perches reserved for the philosopher and the lover . There is no place forSocrates—for the truly just and good man—in the real cities down under the heavens. No place for him in theupper world of lofty contemplations. Without him, wealth and power and honour will ever rule the human scene and justice will be the ideal of simple fools.1That is the essential tragedy of the human condition as it re-emerges in the Platonic corpus. Dualists say plausible things about our mental concepts: there is a way of thinking of pain, in terms of how it feels, which is independent of causal role. Physicalists make attractive ontological claims: the world is wholly physical. The attraction of a posteriori physicalism is that it has seemed to do both: to agree with the dualist about our mental concepts, whilst retaining a physicalist ontology. In this paper I argue that, in fact, a posteriori physicalism departs from the dualist's (...) intuitive picture of our phenomenal concepts in just as radical a manner as more traditional forms of physicalism. Whereas the physicalism of David Lewis and David Armstrong is counterintuitive in holding that our only way of thinking about pain is in terms of its causal role, the physicalism of David Papineau and Brian Loar departs from common sense in holding that our phenomenal concept of pain is opaque: thinking of pain in terms of how it feels reveals nothing of what it is for something to feel pain. The arguments of David Chalmers and Frank Jackson against a posteriori physicalism involve general claims about all concepts. In contrast, my argument makes a claim only about phenomenal concepts: phenomenal concepts are not opaque. (shrink)

Lloyd (2009) contends that climate models are confirmed by various instances of fit between their output and observational data. The present paper argues that what these instances of fit might confirm are not climate models themselves, but rather hypotheses about the adequacy of climate models for particular purposes. This required shift in thinking—from confirming climate models to confirming their adequacy-for-purpose—may sound trivial, but it is shown to complicate the evaluation of climate models considerably, both in principle and in practice.

Whether in characterizing Catharine MacKinnon's theory of gender as itself pornographic or in identifying liberalism as unable to make good on its promises, Wendy Brown pursues a central question: how does a sense of woundedness become the basis for a sense of identity? Brown argues that efforts to outlaw hate speech and pornography powerfully legitimize the state: such apparently well-intentioned attempts harm victims further by portraying them as so helpless as to be in continuing need of governmental protection. "Whether (...) one is dealing with the state, the Mafia, parents, pimps, police, or husbands," writes Brown, "the heavy price of institutionalized protection is always a measure of dependence and agreement to abide by the protector's rules." True democracy, she insists, requires sharing power, not regulation by it; freedom, not protection.Refusing any facile identification with one political position or another, Brown applies her argument to a panoply of topics, from the basis of litigiousness in political life to the appearance on the academic Left of themes of revenge and a thwarted will to power. These and other provocations in contemporary political thought and political life provide an occasion for rethinking the value of several of the last two centuries' most compelling theoretical critiques of modern political life, including the positions of Nietzsche, Marx, Weber, and Foucault. Concern for human vulnerability seems to be at the heart of bioethical inquiry, but the concept of vulnerability is under-theorized in the bioethical literature. The aim of this article is to show why bioethics needs an adequately theorized and nuanced conception of vulnerability. We first review approaches to vulnerability in research ethics and public health ethics, and show that the bioethical literature associates vulnerability with risk of harm and exploitation, and limited capacity for autonomy. We identify some of the challenges (...) emerging from this literature: in particular, how to reconcile universal human vulnerability with a context-sensitive analysis of specific kinds and sources of vulnerability; and how to reconcile obligations to protect vulnerable persons with obligations to respect autonomy. We then briefly survey some of the theoretical resources available within the philosophical literature to address these challenges, and to assist in understanding the conceptual connections between vulnerability and related concepts such as harm, exploitation, needs, and autonomy. We also sketch out a taxonomy of sources and kinds of vulnerability. Finally, we consider the implications for policy evaluation of making vulnerability an explicit and central focus of bioethics. Our investigation is in the form of a broad survey motivating a research agenda rather than a detailed analysis. (shrink)

Bioethics is a practically oriented discipline that developed to address pressing ethical issues arising from developments in the life sciences. Given this inherent practical bent, some form of advocacy or activism seems inherent to the nature of bioethics. However, there are potential tensions between being a bioethics activist, and academic ideals. In academic bioethics, scholarship involves reflection, rigour and the embrace of complexity and uncertainty. These values of scholarship seem to be in tension with being an activist, which requires pragmatism, (...) simplicity, certainty and, above all, action. In this paper I explore this apparent dichotomy, using the case example of my own involvement in international efforts to end forced organ harvesting from prisoners of conscience in China. I conclude that these tensions can be managed and that academic bioethics requires a willingness to be activist. (shrink)

Individuals have different experiences of work when they are self-employed, when they perform tasks in the gig economy, and when they follow directives from managers. But such differences are not represented in some of the most prominent non-ideal theories of work. These describe workers as a coherent group, with a position in the structure of the liberal capitalist economy. I present an alternative that does better at acknowledging difference, through a description of work and workers that has greater ‘pluralism’ and (...) less ‘unifying coherence’. Some might insist that their ‘unifying’ description has superior empirical plausibility. But if ‘pluralistic’ descriptions are valid rivals to provide an accurate characterization of our current condition, then we should consider whether their use in theory can serve valuable aims. I identify the distinctive and valuable non-ideal aims – epistemic, evaluative, and normative – that can be pursued with ‘pluralistic’ descriptions of work and workers. (shrink)

Tolerance is generally regarded as an unqualified achievement of the modern West. Emerging in early modern Europe to defuse violent religious conflict and reduce persecution, tolerance today is hailed as a key to decreasing conflict across a wide range of other dividing lines-- cultural, racial, ethnic, and sexual. But, as political theorist Wendy Brown argues in Regulating Aversion, tolerance also has dark and troubling undercurrents. Dislike, disapproval, and regulation lurk at the heart of tolerance. To tolerate is not to (...) affirm but to conditionally allow what is unwanted or deviant. And, although presented as an alternative to violence, tolerance can play a part in justifying violence--dramatically so in the war in Iraq and the War on Terror. Wielded, especially since 9/11, as a way of distinguishing a civilized West from a barbaric Islam, tolerance is paradoxically underwriting Western imperialism. Brown's analysis of the history and contemporary life of tolerance reveals it in a startlingly unfamiliar guise. Heavy with norms and consolidating the dominance of the powerful, tolerance sustains the abjection of the tolerated and equates the intolerant with the barbaric. Examining the operation of tolerance in contexts as different as the War on Terror, campaigns for gay rights, and the Los Angeles Museum of Tolerance, Brown traces the operation of tolerance in contemporary struggles over identity, citizenship, and civilization. There has recently been a revival of interest in panpsychism as a theory of consciousness. The hope of the contemporary proponents of panpsychism is that the view enables us to integrate consciousness into our overall theory of reality in a way that avoids the deep difficulties that plague the more conventional options of physicalism on the one hand and dualism on the other. However, panpsychism comes in two forms — strong and weak emergentist — and there are arguments that seem (...) to show that weak emergentist panpsychism faces problems analogous to those of physicalism whilst strong emergentist panpsychism faces problems analogous to those of dualism. In this paper, I will develop a new hybrid of the strong and weak emergentist forms of panpsychism, a view according to which subjects of experience are strongly emergent but their phenomenal properties are weakly emergent. I will argue that this hybrid view manages to avoid the challenges facing both physicalism and dualism, and the analogues of those challenges that seem to undermine standard forms of panpsychism. The first half of this book argues that physicalism cannot account for consciousness, and hence cannot be true. The second half explores and defends Russellian monism, a radical alternative to both physicalism and dualism. The view that emerges combines panpsychism with the view that the universe as a whole is fundamental.

Panpsychism is the view that consciousness is a fundamental and ubiquitous feature of the physical world. This chapter outlines two major arguments for panpsychism, one in terms of its role in solving the hard problem of consciousness, and two the intrinsic nature argument. It also responds to the worry that panpsychism is too counterintuitive to be true.

Low-income women and, disproportionately low-income women of color seeking reproductive and pregnancy care are increasingly subject to what this article terms carceral care – care compromised by its’ proximity to punishment systems. This article identifies the legal and health care practice mechanisms leading to carceral care and proposes solutions designed to stop criminalization at the bedside.
