We investigated whether, in the emergency department, educational video-assisted informed consent is superior to the conventional consent process, to inform trauma patients undergoing surgery about the procedure, benefits, risks, alternatives, and postoperative care. We conducted a prospective randomized controlled trial, with superiority study design. All trauma patients scheduled to receive trauma-related debridement surgery in the ED of Kaohsiung Medical University Hospital were included. Patients were assigned to one of two education protocols. Participants in the intervention group watched an educational video (...) illustrating informed consent information, whereas those in the control group read an informed consent document. The primary outcome was knowledge scores and the secondary outcome was assessment of patient satisfaction. A multivariable regression model, with predefined covariates, was used to analyze differences in knowledge scores and patient satisfaction levels between the groups. A total of 142 patients were enrolled, with 70 and 72 assigned to the intervention and control groups, respectively. Mean knowledge scores were higher in the intervention ) than in the control group. By multivariate analysis, the intervention group had significantly greater differences in knowledge scores. Age, injury severity score, and baseline knowledge score significantly affected the differences in knowledge scores. Significant improvements were observed in patients’ perception of statements addressing comprehension of the information provided, helpfulness of the supplied information for decision making, and satisfaction with the informed consent process. Multivariate analysis showed significant correlations between video education and patient satisfaction. Both the educational approach and severity of injury may have an impact on patient understanding during the informed consent process in an emergency environment. Video-assisted informed consent may improve the understanding of surgery and satisfaction with the informed consent process for trauma patients in the ED. Institutions should develop structured methods and other strategies to better inform trauma patients, facilitate treatment decisions, and improve patient satisfaction. Obtaining adequate informed consent from trauma patients is challenging and time-consuming. Healthcare providers must communicate complicated medical information to enable patients to make informed decisions. This study aimed to explore the challenges of obtaining valid consent and methods of improving the quality of the informed consent process for surgical procedures in trauma patients. We conducted a systematic review of relevant English-language full-text original articles retrieved from PubMed that had experimental or observational study design and involved adult trauma patients. Studies involving (...) informed consent in clinical or research trials were excluded. Titles and abstracts of searched articles were reviewed and relevant data were extracted with a structured form. Results were synthesized with a narrative approach. A total of 2044 articles were identified in the initial search. Only eight studies were included in the review for narrative synthesis. Six studies involved orthopedic surgeries, one involved nasal bone surgeries, and one involved trauma-related limb debridement. Only one study was conducted in an emergency department. Information recall was poor for trauma patients. Risk recall and comprehension were greater when written or video information was provided than when information was provided only verbally. Patient satisfaction was also greater when both written and verbal information were provided than when verbal information alone was provided; patients who received video information were more satisfied than patients who received written or verbal information. Many articles have been published on the subject of informed consent, but very few of these have focused on trauma patients. More empirical evidence is needed to support the success of informed consent for trauma patients in the emergency department, especially within the necessarily very limited time frame. To improve the informed consent process for trauma patients, developing a structured and standardized informed consent process may be necessary and achievable; its effectiveness would require evaluation. Adequately educating and training healthcare providers to deliver structured, comprehensive information to trauma patients is crucial. Institutions should give top priority to ensuring patient-centered health care and improved quality of care for trauma patients. (shrink)

The resulting consensus was used to develop the video content and questions for measuring the understanding of the informed consent for debridement surgery in limb trauma patients. The expert panel included experienced patients. The participants in this pilot study were enrolled as a convenience sample of adult trauma patients scheduled to receive surgery. The modified Delphi technique comprised three rounds over a 4-month period. The items given higher scores by the experts in several categories were chosen for the subsequent rounds until consensus was reached. The experts reached a consensus on each item after the three-round process. The final knowledge measure comprising 10 questions was developed and validated. Thirty eligible trauma patients presenting to the Emergency Department were approached and completed the questionnaires in this pilot study. The participants exhibited significantly higher mean knowledge and satisfaction scores after watching the educational video than before watching the video. Our process is promising for developing procedure-specific informed consent and audiovisual aids in medical and surgical specialties. The educational video was developed using a scientific method that integrated the opinions of different stakeholders, particularly patients. This video is a useful tool for improving the knowledge and satisfaction of trauma patients in the ED. The modified Delphi technique is an effective method for collecting experts’ opinions and reaching a consensus on the content of educational materials for informed consent. In this article, we offer a critical realist conception of curriculum that aims to cultivate critical thinking (CT) and liberate students from egocentric rationality. We first examine egocentric rationality as a problem emerging from the technicist paradigm of cultivating CT in higher education, exemplified by issues arising from the pedagogical activity of debate. We then examine existing approaches to cultivating CT, focusing on the extent to which their goals and conceptions of CT could liberate students from egocentric rationality. Drawing on (...) Roy Bhaskar’s stratified conception of being and principle of immanent critique, we offer a critical realist conception of curriculum, explaining its definition of CT as the capacity for self-critique. We shall illustrate, in the context of an undergraduate debate course, how such capacity can be developed in order to liberate students from egocentric rationality. Curriculum is thus a critical praxis, a created space where student’s CT develops in a way that is intrinsically entwined with their being and becoming. This article extends the philosophical basis for integrating CT with curriculum and offers pedagogical implications. (shrink)

The present study examined the behavioral effects of the first-person possessive pronoun. In each trial, a noun was presented to participants after visual presentation of a possessive pronoun “wo de” or “ta de” , which formed ownership. Half participants were assigned to contextual encoding condition in which they were required to judge whether they liked the item expressed by a noun from the first or third-person perspective. The rest were assigned to perceptual encoding condition in which they were asked to (...) judge what color the noun was. A subsequent recall test was performed. The results showed that there were significant memory and response advantages for nouns in “my” ownership under both conditions. The results were discussed with reference to self-specificity and other effects in the current study. (shrink)

This paper traces Hegel’s conception of consciousness back to Hölderlin and argues accordingly that the structure of judging, which Hegel links to an attitude of knowing, determines the standpoint of consciousness to be overcome through the Phenomenology of Spirit. Taking Hegel’s texts into consideration comprehensively, it is then argued that a shape of consciousness is concerned withadvancing its own nominal definition of the concept “Truth,” which is a concept reflecting a culture’s way to organize its life. The Phenomenology, accordingly, should (...) be understood as an enterprise demonstrating that, though advancing their own definitions, shapes of consciousness nonetheless participate in the development of one single concept of the Truth. In this work we report a simple way to measure the similarity between two nucleotide sequences by using graph theory and information theory. This method reported allows for theoretical comparisons of naturally occurring nucleotide sequences.

This paper introduces the Generalized Argumentation Theory which takes argumentation as a locally rational socio-cultural interaction governed by social norms and carried out through discourse between the members of a socio-cultural community in order to reason things out. Then we bring in the basic structure of generalized argumentation and the localized procedure of Generalized Argumentation Theory for studying the argumentative rules. On the basis of above introduction, we use the localized procedure to analyze a case of political argumentation by reciting (...) poems in ancient China. By doing so, it’s indicated that political argumentation by reciting poems, in essence, takes cultural principle of Li of the Spring and Autumn Period as argumentative rules and uses poems to express ideas, in virtue of which arguments are accepted as locally rational by the politicians at that time. At last, the Little Red Book quotation fight, a similar political activity, is discussed briefly to show the significance that ancient Chinese political argumentation has on contemporary argumentation. Amphibian metamorphosis is a post‐embryonic process that systematically transforms different tissues in a tadpole. Thyroid hormone plays a causative role in this complex process by inducing a cascade of gene regulation. While natural metamorphosis does not occur until endogenous thyroid hormone has been synthesized, tadpoles are competent to respond to exogenous thyroid hormone shortly after hatching. In addition, even though the metamorphic transitions of individual organs are all controlled by thyroid hormone, each occurs at distinct developmental stages. Recent molecular studies (...) suggest that this competence of premetamorphic tadpoles to respond to the hormone and the developmental stage‐dependent regulation of tissue‐specific transformations are determined in part by the levels of thyroid hormone receptors and the concentrations of cellular free thyroid hormone. In addition, at least two genes, encoding a cytosolic thyroid hormone binding protein and a 5‐deiodinase, respectively, are likely to be critical players in regulating cellular free thyroid hormone concentrations. This review discusses how all of these molecuar components coordinate to induce amphibian metamorphosis in a correct spatial and temporal manner. These studies provde us with general clues as to how and why tissues become competent to respond to hormonal signals. (shrink)

The genetic equidistance result shows that different species are approximately equidistant to a simpler outgroup in protein sequence similarity, as first reported by Margoliash in 1963. This result, together with those of Zuckerkandl and Pauling in 1962 inspired the molecular clock and in turn the neutral theory of evolution. Here it is shown that the clock/neutral theory had from the beginning overlooked another characteristic of the equidistance result, the overlap feature, which shows a large number of overlapped mutant amino acid (...) positions where any pair of any three species is different provided that the species concerned differ from one another in complexity as a result of macroevolution. In contrast, when simple organisms of similar complexity and short evolutionary divergence are compared, there are only a small number of overlaps largely consistent with chance or the neutral theory. The full reality of the equidistance result strongly supports the Maximum Genetic Diversity hypothesis, a more complete account of hereditary changes. (shrink)

This article attempts to explore ancient Chinese philosophical thought by analyzing how pioneering Chinese thinkers made judgments and inferences, and compares it to ancient Greek philosophy. It first addresses the starting-point and the object of cognition in Chinese ancient philosophy, then analyses how early thinkers construed definition and proposition, and finally discusses how they made inferences on the basis of definition and proposition. It points out that categorization is an important methodology in ancient Chinese philosophy, and that rectification of names (...) and the doctrine of the mean are key criteria in making judgments.
