Ovarian salvage from a patient with brain death is not available and will not preserve viable ova for future reproduction. Previous interest in assisted reproductive technology is only the first step in this process, which requires careful assessment of maternal risks and potential for recurrent genetic disease.

Background: International developments suggest that providing clinical ethics services to help clinicians negotiate ethical issues that arise in clinical practice is beneficial and reflects best practice in promoting high ethical standards and patient-centered care. The aim of this study was to explore the needs and experiences of clinical staff members to inform the development of future clinical ethics support. Methods: Health professionals at a large regional health service completed an online survey containing questions about the frequency of ethical and legal (...) issues encountered in clinical practice, the type of situations that gave rise to the ethical and/or legal uncertainty or concern, how clinicians currently address these issues, and what support would be welcome. Results: The survey was completed by 369 staff members, including 61% with more than 10 years in the profession and 51% in nursing/midwifery. Two-thirds (66%) indicated they often considered ethical implications of their clinical decisions, and half (49%) often considered the legal implications. More than half (58%) were often/occasionally concerned about the ethically right thing to do. Patient requests for borderline treatment (47%), staff disagreements about patient care (48%), and patients declining recommended treatment (54%) were the most frequent reasons given for ethical or legal uncertainty. Sixty-nine percent of respondents indicated that the way their department addressed ethical issues could be improved and 85% agreed that there is a need for more discussion on ethical issues. The majority (82%) of respondents had encountered ethically challenging clinical situations where additional support would have helped. Common approaches to issues were discussion with colleagues/supervisor (91.1%), discussion at handover/group forums (50.8%), and consultation with guidelines/protocols (60.9%). Conclusions: A majority of clinical staff members surveyed have encountered ethically/legally challenging cases daily for which they have often sought additional advice. This study indicates that many clinical staff members would welcome some form of additional clinical ethics support including advice and education. (shrink)

The association of determinism with the coming of experimental science in the modern world was an unnecessary carry-over of certain aspects of the medieval belief that the aim of knowledge is to grasp or contemplate “eternal truth.” If we take a science such as chemistry or physics as typical instead of astronomy, it would seem that control of actual concrete transformations is the acid experimental test rather than prediction of “inevitable” future events. The latter surely has the smell of pre-scientific (...) prophecy. If so, and if we choose to base our interpretation of science and our philosophy of nature on this consideration, we can avoid determinism without in any way limiting the efficacy or potential universality of science. At the same time we avoid the traditional metaphysical or theological notion of “free will.”. (shrink)

Yet the weight which Bohr himself attaches to Einstein's insistence may indicate that if the latter's difficulty could be understood, then, through the cooperation of understanding physicists, another great accomplishment of unification might be obtained which would be as fruitful for the future as either (...) or both the general theory of relativity and Bohr's statistical theory have been to the present. In part one of this paper we shall elucidate an operational concept of logical simplicity as it seems to function in natural science. Although this is a tentative and exploratory formulation, its novelty, relative to the great European philosophical tradition, seems to require a preliminary warning about misunderstandings.In Part Two we shall apply the formulation to social inquiry and make a positive suggestion for further development of the proposal.

In view of the importance of the philosophical method argued in Mr. C. I. Lewis's “Experience and Meaning,” I wish to call attention to ambiguities which may have an important bearing on one of his conclusions. The method for which he argues is a certain empirical test of meaningfulness. It is his ‘positivistic’ inference from this method which I wish to challenge. To do so I shall present four points: A summary of his empirical test of meaningfulness; A ‘non-positivistic’ hypothesis (...) which Mr. Lewis rejects; An analysis of this hypothesis in view of his test; The theory of universals implied in my analysis. The argument of this paper is intended merely as a reassertion of John Dewey's naturalistic thesis that value and existence cannot be separated; and that the traditional concept of science which separates them is vicious. But the writer is not at all sure that he has not perverted the ideas of Dewey beyond recognition. If so, he hopes he has perverted them away from truth, but he fears otherwise. For there is not much hope of “reforming” science; at least, not (...) of cutting and stretching it to fit the Procrustian bed herein prescribed. Chronic diseases represent the major illness burden of developed nations. A chronic disease databank system consists of parallel longitudinal data sets from diverse locations describing the courses of thousands of patients with chronic illness over many years. Illustrated by ARAMIS (The American Rheumatism Association Medical Information System), such data resources facilitate analysis of long term health outcomes and the factors associated with particular outcomes. A model for clinical investigation of contemporary disease is presented, based on the overwhelming prevalence of chronic (...) illness, the variability, complexity, and uniqueness of the individual patient course, the difficulties of traditional univariate reductionist approaches, and the time span required for study. In this model, data are systematically accrued and continually analyzed, and the data collected are gradually modified based upon evolving anticipation of future needs. The strategies underlying the development of ARAMIS are described, investigational results summarized, and future directions outlined. The “determination to catch the world's multiplicity and variety in a single net appears to be an incurable bias of the learned mind.” The extremes to which a philosopher will go to make his net hold water should be nothing short of absurd to the literal minded who neglect “the climate of opinion” which shapes the very logic of the systems or who fail to sympathize with the deep underlying human needs which call forth the great philosophies. Most thinkers in (...) this anthropological day make their bow to the fact of cultural conditioning and to the fact of personal and social surds in the thought of the past. We make our bow, and then proceed to forget that we are human individuals living in an historical period with its own unique strains and symphonies and discords: if only today's system can be shown to be scientific, it must then be beyond dispute. One debate in the experimental exploration of everyday judgments about free will is whether most people are compatibilists or incompatibilists. Some recent research suggests that many people who have incompatibilist intuitions are making a mistake; as such, they do not have genuine incompatibilist intuitions. Another worry is whether most people appropriately understand determinism or confuse it with similar, but different, notions such as fatalism. In five studies we demonstrate people distinguish determinism from fatalism. While people overall make this distinction, a (...) large percentage of people still judge that a person who is fated to perform an action is both free and morally responsible for that action. Those who thought that one freely performs and is morally responsible for a fated action had much stronger compatibilist judgments.
