Analyzing a dream series of elderly, healthy persons obtained from weekly telephone interviews, the internal consistency of a series of 20 dreams and retests after 4 or 22 weeks, respectively, had been computed. The findings indicate that dream recall and dream length are quite stable, but dream characteristics such as bizarreness and emotional tone underlie large intraindividual fluctuations. In order to obtain (...) reliable measures for these variables which will be important for correlational studies, including waking-life trait measures, one has to obtain as many dreams as possible (about 20) in a very short time period. Advance care directives for health care have been promoted as a way to improve end-of-life decision making. These documents allow a patient to state, in advance of incapacity, the types of medical treatments they would like to receive, to name a surrogate to make those decisions, or to do both. Although studies have shown that both physicians and patients generally have positive attitudes about the use of these documents, relatively few individuals have actually completed one.What underlies this discrepancy between attitudes (...) and behavior with regard to advance care directives? One obvious explanation is lack of access. Emanuel et al. estimated that approximately 90 percent of the population desire an advance care directive, and they pointed to access as the major barrier. Yet interventions that increase accessibility have typically failed to yield more than a 20 percent completion rate. Thus, it appears that access is not the sole determinant of advance care directive completion. (shrink)

OBJECTIVE: To report and analyse the pattern of end-of-life decision making for terminal Chinese cancer patients. DESIGN: Retrospective descriptive study. SETTING: A cancer clinical trials unit in a large teaching hospital. PATIENTS: From April 1992 to August 1997, 177 consecutive deaths of cancer clinical trial patients were studied. MAIN MEASUREMENT: Basic demographic data, patient status at the time of signing a DNR consent, or at the moment of returning home to die are documented, and circumstances surrounding these events evaluated. RESULTS: (...) DNR orders were written for 64.4% of patients. Patients in pain (odds ratio 0.45, 95% CI 0.22-0.89), especially if requiring opioid analgesia (odds ratio 0.40, 95% CI 0.21-0.77), were factors associated with a higher probability of such an order. Thirty-five patients were taken home to die, a more likely occurrence if the patient was over 75 years (odds ratio 0.12, 95% CI 0.04-0.34), had children (odds ratio 0.14, 95% CI 0.02-0.79), had Taiwanese as a first language (odds ratio 6.74, 95% CI 3.04-14.93), or was unable to intake orally (odds ratio 2.73, 95% CI 1.26-5.92). CPR was performed in 30 patients, none survived to discharge. CONCLUSIONS: DNR orders are instituted in a large proportion of dying Chinese cancer patients in a cancer centre, however, the order is seldom signed by the patient personally. This study also illustrates that as many as 20% of dying patients are taken home to die, in accordance with local custom. (shrink)

The rapid and dynamic nature of digital transformation challenges companies that wish to develop and deploy novel digital technologies. Like other actors faced with this transformation, companies need to find robust ways to ethically guide their innovations and business decisions. Digital ethics has recently featured in a plethora of both practical corporate guidelines and compilations of high-level principles, but there remains a gap concerning the development of sound ethical guidance in specific business contexts. As a multinational science and technology company (...) faced with a broad range of digital ventures and associated ethical challenges, Merck KGaA has laid the foundations for bridging this gap by developing a Code of Digital Ethics (CoDE) tailored for this context. Following a comprehensive analysis of existing digital ethics guidelines, we used a reconstructive social research approach to identify 20 relevant principles and derive a code designed as a multi-purpose tool. Versatility was prioritised by defining non-prescriptive guidelines that are open to different perspectives and thus well-suited for operationalisation for varied business purposes. We also chose a clear nested structure that highlights the relationships between five core and fifteen subsidiary principles as well as the different levels of reference—data and algorithmic systems—to which they apply. The CoDE will serve Merck KGaA and its new Digital Ethics Advisory Panel to guide ethical reflection, evaluation and decision-making across the full spectrum of digital developments encountered and undertaken by the company whilst also offering an opportunity to increase transparency for external partners, and thus trust. In much the same way that genomic technologies are changing the complexion of biomedical research, the issues they generate are changing the agenda of IRBs and research ethics. Many of the biggest challenges facing traditional research ethics today — privacy and confidentiality of research subjects; ownership, control, and sharing of research data; return of results and incidental findings; the relevance of group interests and harms; the scope of informed consent; and the relative importance of the therapeutic misconception — have become (...) important policy issues over the last 20 years because of the ways they have been magnified by genomic research efforts. Research that examines the ethical, legal, and social implications of human genomics research has become a burgeoning international field of scholarship over the last 20 years, thanks in part to its support first by the genome research funding bodies in the U.S. and then by national science agencies in other countries. The impact of COVID-19 on chronic pain in non-infected vulnerable South American subjects is unknown. Healthcare workers are at increased risk for CP. During the pandemic, many HCWs with CP kept working. Knowing how cognition is affected by CP in these subjects is an important subject for work safety. The attention domain has a pivotal role in cognition. Previously, the Continuous Visual Attention Test was applied to detect specific attention deficits in fibromyalgia patients. The present investigation described CP prevalence in (...) non-infected Brazilian HCWs during the pandemic and assessed HCWs’ attentional performance with the aid of the CVAT. This study was carried out at a reference University Hospital in Rio de Janeiro, Brazil. HCWs of both sexes, aged 20 or older, were interviewed from August to December 2020. A 90-second version of the CVAT was performed. The average reaction time to correct responses and the respective intraindividual reaction time variability for correct responses to target was determined. Omission and commission errors were also calculated. Then, for each participant we calculated the Z-scores of the CVAT variables based on the distribution of CVAT performance of 211 healthy subjects. HCWs with Z-scores > 1.64 were classified as significantly impaired. From the 154 selected HCWs, 72 reported CP during the pandemic. Post hoc ANCOVAs showed that the average correct VRT was significantly higher in the CP group than in the non-CP group. The percentage of participants with impaired VRT performance was 30% in the CP group and 16% in the non-CP group. The difference between these two propositions reached significance. As VRT is associated with the sustained-attention subdomain, our data suggest that this subdomain is disrupted in the CP group. (shrink)

We report the identification of a recurrent, 520-kb 16p12.1 microdeletion associated with childhood developmental delay. The microdeletion was detected in 20 of 11,873 cases compared with 2 of 8,540 controls and replicated in a second series of 22 of 9,254 cases compared with 6 of 6,299 controls. Most deletions were inherited, with carrier parents likely to manifest neuropsychiatric phenotypes compared to non-carrier parents. Probands were more likely to carry an additional large copy-number variant when compared to matched controls. The clinical (...) features of individuals with two mutations were distinct from and/or more severe than those of individuals carrying only the co-occurring mutation. Our data support a two-hit model in which the 16p12.1 microdeletion both predisposes to neuropsychiatric phenotypes as a single event and exacerbates neurodevelopmental phenotypes in association with other large deletions or duplications. Analysis of other microdeletions with variable expressivity indicates that this two-hit model might be more generally applicable to neuropsychiatric disease. © 2010 Nature America, Inc. During preparation for early abortion in Norway, an ultrasound examination is usually performed to determine gestation and viability. This article aims to provide a deeper understanding of women’s and health care personnel’s experiences with ultrasound viewing during abortion preparation in the first trimester. Qualitative in-depth interviews with women who had been prepared for early abortion and focus group interviews with HCP from gynaecological units were carried out. A hermeneutic-phenomenological analysis, inspired by van Manen, was chosen. Thirteen women who were pregnant (...) and considering abortion in their first trimester and 20 HCP, namely, 19 registered nurses and one medical doctor, were recruited from gynaecological units at six hospitals. The study was approved by the ethics committee. The essential meaning structure of ‘autonomy under pressure’ consisted of two themes that expressed the different experiences of both the women and the HCP, namely, expectations versus precautions and choice versus protection. The women and HCP expressed different attitudes before the consultation that affected their experiences of the ultrasound examination. While the women had expectations of a clarification based on their choice to either see or not see the ultrasound image, HCP seemed to be more concerned with predetermined rules that they believed would protect the women. Consequently, the basis for dialogue was not optimal, and women’s autonomy was under pressure. Health care personnel are ethically challenged during preabortion ultrasound examinations. Meeting the individual woman’s needs and respecting her autonomy during preparation for abortion requires sensitivity, involvement, and dialogue skills by health personnel. According to the woman’s desire to be informed about the possibility of viewing the image during the abortion preparations, a dialogue that is focused in this direction should arise before the examination. Listening and responding to patients’ stories for over 20 years as an emergency physician has strengthened my appreciation for the many ways that the skills and principles drawn from writing fiction double as necessary clinical skills. The best medicine doesn’t work on the wrong story, and the stories patients tell sometimes feel like first drafts—vital and fragile works-in-progress. Increasingly complex health challenges compounded by social, financial, and psychological burdens make for stories that are difficult to articulate and comprehend. In this (...) essay, I argue that healthcare providers need to think like creative writers and the skills and sensitivities necessary to story construction deserve a vital space in medical education. A thorough understanding of story anatomy and the imaginative flexibility to work stories into open spaces serve as antidotes to the reductive nature of clinical decision making and have implications as patient safety and risk management strategies. The examples that I have selected demonstrate how thinking like a creative writer functions at the bedside, providing tools for clinical excellence and empathy. This approach asks that we re-imagine the importance of story in clinical care: from a vehicle to a diagnosis to its place as a critical destination. (shrink)

Zum Sechstagekrieg und dessen Folgen veröffentlichte der israelische Autor Ephraim Kishon in rascher Folge drei Bücher. Zwei davon sind zum Teil in starker Bearbeitung durch den österreichisch jüdischen Autor Friedrich Torberg auf Deutsch erschienen. Der vorliegende Aufsatz untersucht die Parameter dieses Gemeinschaftswerks zum ersten Mal vergleichend und auf der Basis unveröffentlichter Briefe. Die von Torberg vorgenommenen Veränderungen betreffen besonders Vergleiche mit NS-Deutschland, Verweise auf den Holocaust sowie den ironischen Umgang mit antisemitischen Stereotypen. Des Weiteren wird deutlich, dass sich Kishon in (...) seinen Texten auf die beiden zentralen Strömungen des „jüdischen Humors“ zu Beginn des 20. Jahrhunderts bezieht: die in der deutschsprachigen Rezeption dominant wahrgenommenen,harmlosen‘ Humoresken in der Tradition des osteuropäischen „jüdischen Witzes“ und die von Meyer-Sickendiek (2009) beschriebene westjüdische Tradition des literarischen Sarkasmus. Mit den beiden Bänden zum Sechstagekrieg wurde das deutschsprachige Publikum stärker mit der sarkastischen Seite von Kishons Humor konfrontiert. (shrink)

An incomplete science begets imperfect models. Nevertheless, the target article advocates for jettisoning deep-learning models with some competency in object recognition for toy models evaluated against a checklist of laboratory findings; an approach which evokes Alan Newell's 20 questions critique. We believe their approach risks incoherency and neglects the most basic test; can the model perform its intended task.

In lieu of an abstract, here is a brief excerpt of the content:Hume's Theory of Belief Michael M. Gorman Belief is a key concept in Hume's philosophy, and yet Hume's statements aboutbeliefappear to be hopelesslyinconsistent.1 Various solutions have been offered, from saying that Hume is incorrigibly confused to saying that his theory ofbeliefchanged over the course of his career. This article will focus on the question ofthe nature ofbelief and show that Hume's theory is in fact consistent. In sections 1 (...) and 2, I will separate those passages where Hume discusses the nature of beUeffrom those where he discusses other questions. In sections 3-6, 1 will examine his theory ofthe nature ofbelief. Finally, in section 7, 1 will briefly look at his theories on other questions concerning belief. 1.Hume's statements about belief can be divided into at least nine different categories. Hume says that a beliefis: 1.an idea conceived in a certain manner (e.g., E 49, T 96); 2.that certain manner ofconception itself(e.g., E 49, T 97); 3.an idea that feels a certain way (e.g., E 48, T 103); 4.that certain feeling itself(e.g., E 49, T 624, 629); 5.an idea that has a great influence on the mind (e.g, T 118-20); 6.an act ofmind rendering realities influential on the mind (e.g., E 49, T 629); 7.a lively idea related to an impression (e.g., T 96); 8.a lively manner of conceiving an idea, which manner arises from an impression (e.g., E 50); 9.something that makes ideas forceful and vivacious (e.g., T 101, 627). If one were in a really bad mood, one could probably split Hume's formulations into even more categories.2 2.One solution wouldbe to saythat Hume ishopelessly confused.3 This is not accurate, however. The first thingtobe done is to notice thatonly the statements belonging to the first four categories are actually answers to the question, What is the nature of beUef? Hume tells us that abeUefis: an ideaconceived in acertainmanner, theverymanner ofconception ofsuch an idea, an idea thatfeels a certain way, thevery feeling of such an idea. To explicate Hume's theory of the nature of Volume XLX Number 1 89 MICHAEL M. GORMAN beUef, I will show that the second formulation is a poor version ofthe first and that the fourth formulation is a poor version ofthe third—in other words, that there are reallyonlytwo theorieshere, notfour. Then I will reconcile these two theories. As for Hume's discussion ofother questions concerning belief, the statementsin the fifth and sixth categories address the question, What does à belief do? Those in the seventh and eighth categories take for granted the statements in the first and second categories respectively and address the further question, What causes beliefs? Those in the ninth category are probably cases of bad writing. All these will be examined in section 7. 3. Let us look at the texts that fit into the first and second categories listed in section 1. These express what can be called Hume's "manner-of-conception theory" (MCT). First, Hume says that a belief is a certain kind ofidea, namely an idea conceived in a certain manner. He describes this manner ofconception by saying that such an idea is conceived with a high degree of force and vivacity, liveliness, and so forth. This analysis rests on Hume's distinction between the content of an idea and the manner in which the idea is conceived. [A]s 'tis certain there is a great difference betwixt the simple conception ofthe existence ofan object, and the beliefofit, and as this difference lies not in the parts or composition of the idea, which we conceive; it follows, that it must lie in the manner, in which we conceive it. (T 94-95) One can believe that something is the case, or one can simply conceive ofits being the case without assenting to it. In both cases, Hume says, the content ofthe idea must be the same. Im Allgemeinen widmet sich das vorliegende Buch dem gemeinsamen Problembereich der zeitgenössischen Theologie und Philosophie, aber den konkreten Anlass der hiesigen Beiträgen geben jedoch die Parallelität in der Erscheinung der Enzyklika Fides et Ratio vom Papst Hl. Johannes Paulus II., und die Heiligsprechung Edith Steins (Mitpatronin Europas) von demselben heiligen Papst. Die Autoren des Bandes reflektieren sich auf die Gemeinsamkeit des Denkens dieser zwei Heiligen des 20. Context: The use of first-person micro-phenomenological interviews and their productive interaction with third-person physiological data is a challenging and pressing issue in order to offer an effective and fruitful application of Varela’s neurophenomenological hypothesis. Problem: We aim at offering a generative method of analysis of first-person micro-phenomenological interviews using third-person physiological data. Our challenge is to describe this generative first-person analysis with the third-person physiological framework rather than put Varela’s hypothesis into practice in a generative way (as we did in (...) another paper. Method: The present contribution is a first pioneering study as far as the exposition of such an interactive generative methodology is concerned. It is also a new issue insofar as it deals with a case study, surprise in depression, that has not been thoroughly dealt with so far, either in philosophy or in psychopathology. Results: We show that the analysis of first-person data is an intrinsic generative one, insofar as new refined categories and multifarious circular micro- and macro-processes were discovered in the very process of analyzing. They provide the initial structural generic third-person description of surprise inherited both from philosophical phenomenological a priori categories and from the experimental startle setting with a refined micro-segmentation of the dynamic of the experience. Implications: Our article could be of interest to neurophenomenologists looking for an effective application and to researchers in quest of a method of analysis of first-person data. The present limitations are due to the still preliminary data-results we need to complete. Constructivist content: The article is directly linked to Varela’s neurophenomenological program and aims at extending and reforming it with a cardio-phenomenological approach. Interactive narratives are inextricable from the way that we understand our encounters with digital technology. This is based upon the way that these encounters are processually formed into a narrative of episodic events, arranged and re-arranged by various levels of agency. After describing past research conducted at the iCinema Research Centre at the University of New South Wales, this paper sets out a framework within which to build a relational theory of interactive narrative formation, outlining future research in the area.
