High-dose chemotherapy and autologous stem cell transplantation is used to treat some advanced malignancies. It is a traumatic procedure, with a high complication rate and significant mortality. ASCT patients and their carers draw on many sources of information as they seek to understand the procedure and its consequences. Some seek information from beyond orthodox medicine. Alternative beliefs and practices may conflict with conventional understanding of the theory and practice of ASCT, and ‘contested understandings’ might interfere with patient adherence to the (...) strict and demanding protocols required for successful ASCT.The present study, conducted in Sydney, Australia, examines narrative-style interviews with 10 sequentially recruited ASCT patients and nine of their carers conducted at the time of transplant and three months later. Transcripts were read for instances of mention of alternative advice, and for instances of contested understanding of information relevant to the transplant.Patients and carer pairs expressed closely concordant views about alternative advice. Five pairs were consulting alternative practitioners. Contested understanding was expressed in four domains—understandings of the transplant itself and its underlying theory, of the relationship between the components of the ‘transplant’, of the nature and role of stem cells, and of beliefs about bodily function and life-style. Contested understandings of the transplant treatment were expressed as predominantly personal interpretations of orthodox informationPatients and carers seemed to recognise that alternative and conventional systems were discordant, yet they were able to separate the two, and adhere to each practice without prejudicing their medical treatment. A single case of late, post-transplant repudiation of Western medicine is discussed to emphasise some of the possible determinants of dissonance when it does occur. (shrink)

BackgroundWhen conducting research with Indigenous populations consent should be sought from both individual participants and the local community. We aimed to search and summarise the literature about methods for seeking consent for research with Indigenous populations.MethodsA systematic literature search was conducted for articles that describe or evaluate the process of seeking informed consent for research with Indigenous participants. Guidelines for ethical research and for seeking consent with Indigenous people are also included in our review.ResultsOf 1447 articles found 1391 were excluded (...) ; 56 were relevant and included. Articles were categorised into original research that evaluated the consent process or publications detailing the process of seeking consent and guidelines for ethical research. Guidelines were categorised into international ; national and state/regional/local guidelines. In five studies based in Australia, Canada and The United States of America the consent process with Indigenous people was objectively evaluated. In 13 other studies interpreters, voice recording, videos, pictures, flipcharts and “plain language” forms were used to assist in seeking consent but these processes were not evaluated. Some Indigenous organisations provide examples of community-designed resources for seeking consent and describe methods of community engagement, but none are evaluated. International, national and local ethical guidelines stress the importance of upholding Indigenous values but fail to specify methods for engaging communities or obtaining individual consent. In the ‘Grey literature’ concerns about the consent process are identified but no solutions are offered.ConclusionConsultation with Indigenous communities is needed to determine how consent should be sought from the community and the individual, and how to evaluate this process. Researchers are required to seek consent from Indigenous communities prior to conducting research but there is inadequate information about how Indigenous people understand and become fully engaged with this consent process. Few studies evaluate the preference or understanding of the consent process for research with Indigenous populations. Lack of informed consent can impact on research findings. The Picture Talk Project was initiated with senior Aboriginal leaders of the Fitzroy Valley community situated in the far north of Western Australia. Aboriginal people (...) were interviewed about their understanding and experiences of research and consent processes. Transcripts were analysed using NVivo10 software with an integrated method of inductive and deductive coding and based in grounded theory. Local Aboriginal interpreters validated coding. Major themes were defined and supporting quotes sourced. Interviews with Aboriginal leaders were facilitated by a local Aboriginal Community Navigator who could interpret if necessary and provide cultural guidance. Participants were from all four major local language groups of the Fitzroy Valley; aged 31 years and above; and half were male. Themes emerging from these discussions included Research—finding knowledge; Being respectful of Aboriginal people, Working on country, and Being flexible with time; Working together with good communication; Reciprocity—two-way learning; and Reaching consent. The project revealed how much more there is to be learned about how research with remote Aboriginal communities should be conducted such that it is both culturally respectful and, importantly, meaningful for participants. The consent and community engagement process for research with Indigenous communities is rarely evaluated. Research protocols are not always collaborative, inclusive or culturally respectful. If participants do not trust or understand the research, selection bias may occur in recruitment, affecting study results potentially denying participants the opportunity to provide more knowledge and greater understanding about their community. Poorly informed consent can also harm the individual participant and the community as a whole. Invited by local Aboriginal community leaders of the Fitzroy (...) Valley, the Kimberley, Western Australia, The Picture Talk project explores the consent process for research. Focus groups of Aboriginal community members were conducted to establish preferences for methods of seeking individual consent. Transcripts were analysed through NVivo10 Qualitative software using grounded theory with inductive and deductive coding. Themes were synthesised with quotes highlighted. Focus groups with Aboriginal community members were facilitated by a Community Navigator as a cultural guide and interpreter and a researcher. Participants were recruited from all main language groups of the Fitzroy Valley – Gooniyandi, Walmajarri, Wangkatjungka, Bunuba and Nikinya. Participants were aged ≥18 years, with 5 female groups and one male group. Themes identified include: Reputation and trust is essential; The Community Navigator is key; Pictures give the words meaning – milli milli versus Pictures; Achieving consensus in circles; Signing for consent; and Research is needed in the Valley. Aboriginal communities of the Fitzroy Valley recommend that researchers collaborate with local leaders, develop trust and foster a good reputation in the community prior to research. Local Aboriginal researchers should be employed to provide cultural guidance throughout the research process and interpret local languages especially for elders. Pictures are preferred to written text to explain research information and most prefer to sign for consent. The Fitzroy Valley welcomes research when collaborative and for the benefit of the community. Future research could include exploring how to support young people, promote health screening and improve understanding of medical knowledge. (shrink)

Adaptive management is commonly identified as a way to address situations where ecological and social uncertainty exists. Two discourses are common: a focus on experimentation, and a focus on collaboration. The roles of experimental and collaborative adaptive management in contemporary practice are reviewed to identify tools for bridging the discourses. Examples include broadening the scope of contributions during the buy-in and goal-setting stages, using conceptual models and decision support tools to include stakeholders in model development, experimentation using indicators of concern (...) to stakeholders, an experimental focus that reflects the level of statistical confidence required by management, and the engagement of stakeholders in data interpretation so that those affected by management outcomes can learn and adapt accordingly. In this context, a framework of questions that managers can use to reflect on both ecological and social uncertainties as they relate to individual management contexts is proposed. (shrink)

:In this essay I attempt to vindicate the “asymmetry thesis,” according to which ownership of one’s own body is intrinsically different from ownership of other objects, and the view that self-ownership, as libertarians normally understand the concept, enjoys a special “fact-insensitive” status as a fundamental right. In particular, I argue in favor of the following claims. First, the right of self-ownership is most plausibly understood as based on the more fundamental notion of respect for persons, where the concept of a (...) person is in turn understood, along the lines set out by P. F. Strawson and P. M. S. Hacker, as referring to an entire biological organism with a certain set of mental and corporeal characteristics. If we restrict our attention to human persons, we can say on this basis that there is a special moral status attaching to the entire human body, and to no more than the human body. Second, self-ownership is not, as critics have sometimes supposed, based on a more fundamental right to equal freedom or autonomy. Criticisms of self-ownership as insufficiently justified on the basis of such rights are therefore off target. Rather, equal freedom and self-ownership are each based directly on the more fundamental notion of respect for persons. For left-libertarians, the asymmetry thesis serves to give priority to self-ownership when delineating a set of original property rights, given that there are many alternative ways of realizing equal freedom not all of which involve fully respecting people’s property rights in themselves. In Time, Narrative, and History, David Carr argues against the narrativist claim that our lived experience does not possess the formal attributes of a story; this conclusion can be reinforced from a semiotic perspective. Our experience is mediated through temporal signs that are used again in the construction of stories. Since signs are social entities from the start, this approach avoids a problem of individualism specific to phenomenology, one which Carr takes care to resolve. A semiotic framework is also explicit (...) about a theme Carr handles implicitly: the status of representation. Representation is internal to signification, mediating our experience not just retrospectively but prospectively in the planning and execution of action. A model is presented in which the temporal organization of experience and action is formally coordinated with the temporal organization of narrative. The model is then applied to a historical event: John Batman's attempt in 1835 to purchase land from some Aboriginal tribes around what is now Melbourne. The meaning of this event is not located only in historical writing about it but in the semiotic constitution of the event itself. Changes of meaning emerge from relations between events as new events--in this case the Australian High Court's Mabo decision--align with old ones. Finally, a number of contrasts are indicated between this model and proposals made by Arthur C. Danto, Hayden White, F. R. Ankersmit, Fernand Braudel, and Paul Ricoeur. (shrink)

This is an introduction to the volume The Epistemology of Group Disagreement (Routledge, forthcoming), (eds.) F. Broncano-Berrocal and J.A. Carter.

The focus here will be on the tension between architecture’s symbolic role and its function as a space to house and present art. ‘Symbolic’ refers both to a building as an aesthetic or sculptural form and secondly to its role in expressing civic identity. ‘Function’ refers to the intended purpose or practical use apart from its role as a form of art. As an art form, it serves important symbolic purposes; its practical purposes are linked to serving individual and community (...) functions requiring the delineation of space. In the present context of museum architecture, certain museum buildings are more likely to be seen as a sculptural object than as functioning buildings. The reasons for this development derive in part from unresolved issues pertaining to the respective roles of symbolic and practicalfunction as is seen in the analysis of architecture provided by G. W. F. Hegel, Rudolf Arnheim and Nelson Goodman. The vocabularies of contemporary architects such Frank Gehry and Santiago Calatrava do not follow the abstract geometrical patterns of Le Corbusier or Louis Kahn who envisioned a universal vocabulary of architectural forms derived from industrial technical forms that underscored Modernist conventions in architecture.By looking at this issue in the contexts provided by the theoretical discussions of Hegel, Arnheim and Goodman, it is possible to see more clearly the importance of examining with a critical eye the relative place of symbolism and function in museum architecture, and to question whether current museum practice has gone astray in allowing the sculptural symbolism to become the dominant element. When either its symbolic or its practical aspects are out of balance the result is sure to be unsatisfactory architecture. If the past is a reliable guide, it works best when the symbolic (sculptural) and the practical in architecture are worked out in harmony with each other. (shrink)

Crucially, both examples avoid all six of the potential confounds that Firestone & Scholl (F&S) raised as arguments against previous studies claiming to demonstrate the influence of top-down effects on perception.

Pp. xxi + 129.At a time when one hears all too often of the irreconcilable differences between religions, it is a relief and a delight to read the words of someone who has gleaned much from Christianity (as a Jesuit priest) and from Zen Buddhism (as a practitioner whose (...) enlightenment has been duly authenticated). Father Ruben L. F. Habito's challenge has been to harmonize the beliefs of these two distinct traditions, both of which have provided him with quite distinctive transformative experiences. It is a mutual influencing that he presents in very accessible prose in Living Zen, Loving God.While the lectures in this volume are not attempts to reconcile competing doctrines and approaches in detail, they do strive to move the reader back to those originary experiences of spiritual insight that both traditions share. Father Habito seeks to uncover the spiritual springs of Zen and Christianity, letting each speak in its own way, for religion is not about doctrines and dogma; rather, as the Zen philosopher Nishida Kitarō maintained, "religion is an event of the soul."1 Habito seems to be in agreement with such a position. In explicating this experiential territory, he includes chapters on emptiness and fullness, the Heart Sūtra, the four vows of the Bodhisattva, Kuan-yin with a thousand hands, and Zen and Christian spirituality. There are many apt conclusions and comparisons in these pages, such as a Christian adaptation of the Buddhist admonition to slay the Buddha if you meet him-for to meet him is to lose him by fixing his significance and intent as rigid doctrine and by rendering the absolute a "thing" in the first place. Habito writes that "if you meet the Christ, crucify him!" (p. 36). Or again, there is an excellent and simple account of compassion: "an out-stretched hand adjusting a pillow in the middle of the night... the night of emptiness, of unknowing.... And in the middle of this, as my pillow slips off, and my head feels displaced, spontaneously, my hand reaches out to adjust the pillow, and I go back to sleep. That's all. This koan is saying, that's compassion" (p. 76). The self has dropped off, and one now acts spontaneously and effortlessly from the purity and depth of radical interconnectedness.What is impressive about this book is the accuracy and ease with which difficult Zen and Buddhist ideas are expressed. For example, kensho is seeing clearly "the reality of everything as not separate from oneself, of everything as it really is" (p. xiii). This realization of interconnectedness results in social engagement, "the discovery of the real and deep bond that makes that person one with society, with the marketplace, with the whole universe" (p. 22). More precisely, "one sees one's True Self in the 'other,' the 'other' in one's True Self" (p. 22). While the goal of meditation is personal enlightenment, the fruit of enlightenment is the emptying of the ego-self, [End Page 343] and a reconnection with the everyday world, with a concern and passion heretofore unknown.Yet, as a devout Roman Catholic, Habito filters Zen through Christian lenses as often as he filters Christianity through Zen lenses. Creation ex nihilo, he urges, can be understood "as an invitation to that experience of the nothingness that is at the heart of our being, which becomes the fulcrum for experiencing the infinite life of God pulsing through this being, through the universe, from moment to moment" (p. 23). Each and every thing is now to be understood as "a complete and perfect manifestation of the Infinite" (p. 23). But what is less than clear here is the meaning of "manifestation." Exactly what is every thing a manifestation of? The Zen answer must be that the awareness of everything as a manifestation of the "absolute" must be understood nondualistically. Habito provides a powerful chapter on the Heart Sūtra, which instructs us that emptiness is precisely form, and form precisely emptiness. Stephen Carter argues that biblical literalism is predicated on an epistemological position drastically different than that maintained by mainstream scientists inasmuch as it operates on the basis of a “hermeneutic of inerrancy” with respect to the ideas laid out in the Bible. By relying on considerations offered by Charles Taylor and recent sociological studies, I contend that Carter’s thesis is incorrect. The divide between proponents and opponents of biblical literalism is ethical rather than epistemological. Beyond the philosophical implications (...) of my contention, this displays that deliberative engagement between these parties—which depends on shared epistemological norms—is possible in principle. Although much evidence indicates that young infants perceive unitary objects by analyzing patterns of motion, infantsÕ abilities to perceive object unity by analyzing Gestalt properties and by integrating distinct views of an object over time are in dispute. To address these controversies, four experiments investigated adultsÕ and infantsÕ perception of the unity of a center-occluded, moving rod with misaligned visible edges. Both alignment information and depth information affected adultsÕ and infantsÕ perception of object unity in similar ways, and infants perceived (...) object unity by integrating information about object features over time. However, infants perceived a moving, misaligned, three-dimensional object as indeterminate in its connectedness, whereas adults perceived it as connected behind the occluder. These findings indicate that the effectiveness of common motion in specifying unified surfaces across an occluder is reduced by misalignment of edges. Alignment information enhances perception of object unity either by serving directly as information for unity or by optimizing the detectability of motion-carried information for unity. In addition, young infants are able to retain information about edge orientation over short intervals in determining connectedness via a process of spatiotemporal integration. Ethics for Enemies comprises three original philosophical essays on torture, terrorism, and war. F. M. Kamm deploys ethical theory in her challenging new treatments of these most controversial practical issues. First she considers the nature of torture and the various occasions on which it could occur, in order to determine why it might be wrong to torture a wrongdoer held captive, even if this were necessary to save his victims. In the second essay she considers what makes terrorism wrong--whether it (...) is the intention to harm civilians, rather than harm to them being 'collateral damage,' or something else--and whether terrorism is always wrong. The third essay discusses whether having a right reason, in the sense of a right intention, is necessary in order for a war to be just. Scholars have generally followed this injunction and found in history pretty much what they set out to discover. For a long time, the nineteenth century was seen as an age of progress, its own self-image seeming to bear this out. More recently our own concern with decadence has led us to discover similar views in earlier times. After A. E. Carter, 1 Professor Koenraad Swart has given us The Sense o] Decadence in NineteenthJCentury France (The Hague : 1964), the seventh volume to appear in Martinus Nijhoff's International Archives of the History of Ideas. This is a broader chronicle than Carter's. It presents the antecedents of the idea of decadence from antiquity to the French Revolution, its modern forms rooted in a diversity of romantic reactions, its fin-de-si~cle culmination and its (temporary) waning before the brief affirmative revival that preceded World War I. In terms of present preoccupations, this is a timely book. A solid concluding chapter sketches "the foreign perspective," in which French developments appear as part of a broader context. The study provides a mine of information, a convenient companion and counterindicator to Bury's Idea o] Progress. Why, then, is it a dull and unsatisfactory book? In the first place because, having suggested the existence of another dimension to the histoire de grand-papa, the history of ideas cannot be written as, simply, another version of the serial chronicle of events.
