BackgroundAlthough the Life-Sustaining Treatment (LST) Decision Act was enforced in 2018 in Korea, data on whether it is well established in actual clinical settings are limited. Hospital-acquired pneumonia (HAP) is a common nosocomial infection with high mortality. However, there are limited data on the end-of-life (EOL) decision of patients with HAP. Therefore, we aimed to examine clinical characteristics and outcomes according to the EOL decision for patients with HAP.MethodsThis multicenter study enrolled patients with HAP at 16 referral hospitals retrospectively from (...) January to December 2019. EOL decisions included do-not-resuscitate (DNR), withholding of LST, and withdrawal of LST. Descriptive and Kaplan–Meier curve analyses for survival were performed.ResultsOf 1,131 patients with HAP, 283 deceased patients with EOL decisions (105 cases of DNR, 108 cases of withholding of LST, and 70 cases of withdrawal of LST) were analyzed. The median age was 74 (IQR 63–81) years. The prevalence of solid malignant tumors was high (32.4% vs. 46.3% vs. 54.3%, P = 0.011), and the ICU admission rate was lower (42.9% vs. 35.2% vs. 24.3%, P = 0.042) in the withdrawal group. The prevalence of multidrug-resistant pathogens, impaired consciousness, and cough was significantly lower in the withdrawal group. Kaplan–Meier curve analysis revealed that 30-day and 60-day survival rates were higher in the withdrawal group than in the DNR and withholding groups (log-rank P = 0.021 and 0.018). The survival of the withdrawal group was markedly decreased after 40 days; thus, the withdrawal decision was made around this time. Among patients aged below 80 years, the rates of EOL decisions were not different (P = 0.430); however, mong patients aged over 80 years, the rate of withdrawal was significantly lower than that of DNR and withholding (P = 0.001).ConclusionsAfter the LST Decision Act was enforced in Korea, a DNR order was still common in EOL decisions. Baseline characteristics and outcomes were similar between the DNR and withholding groups; however, differences were observed in the withdrawal group. Withdrawal decisions seemed to be made at the late stage of dying. Therefore, advance care planning for patients with HAP is needed. (shrink)

Time distortion in individuals with social anxiety has been defined as the seemingly slower passage of time in social situations and is related to both arousal and valence. Consequently, adaptive behavior is disrupted and interpersonal situations avoided. We explored the effects of valence and arousal on time distortion in individuals with social anxiety. Participants were assigned to two groups, High Anxiety (HA) and Low Anxiety (LA), presented with four types of facial expression stimuli (positive-high arousal, positive-low arousal, negative-high arousal, and (...) negative-low arousal), and asked to estimate the duration of stimulus presentation. Results indicated that, relative to other stimuli, the HA and LA groups perceived longer presentation for high-arousal negative and low-arousal positive stimuli, respectively. These findings suggest that anxious individuals’ time distortion was more severe in situations that evoked high arousal and involved negative emotion. (shrink)

By drawing new distinctions labelled “appeal” and “response” to replace traditional rhetorical modes of written discourse, the essay sketches a new perspective about what philosophers are doing rhetorically in “doing philosophy.” To think of philosophers as simply engaged in argument is an oversimplification and a distortion of what philosophers do. Crucial to doing philosophy are four activities: (1) definition and redefinition of problems and issues that form both the focus of the canonical historical literature of philosophy and what goes on (...) in contemporary philosophy as a discipline, (2) the development and use of formal languages and technical vocabularies to abbreviate and label complexity and to disambiguate and precise distinctions necessary to deal with problems and issues, (3) the development and exploration of argumentative appeals for acceptance or refutation of answers to questions raised by the philosophical problematic, and (4) the development and exploration of explanatory responses to questions raised by the problematic. In so far as these four activities are driven and sanctioned by the current, self-defining philosophical problematic, contemporary philosophy as a body of knowledge is historical, cumulative, and marked by progress, and the doing of philosophy is fundamentally the making of written appeals and responses about its problematic. From the perspective of a ‘weak’ interpretation I argue that Hirst’s later position differs little from his earlier one in the light both of the relation between the forms of knowledge and social practices, and of the rationalistic character of Hirst’s conception of (...) social practices in their connection with education. In the present paper we shall see that the different ways of understanding mind between Confucianism and Enlightenment in the 18th century. In this study each of these two different traditions is regarded as the East Asian context of mind study or as the Western European context of mind study. This idea comes from a kind of constructivism and constructive realism. The former, which comes from ideas of Lev Vygotsky, stresses that human mind is constructed on its cultural context. The (...) latter insisted by Fridriech Wallner approves that every truth constructed on its cultural context can be an absolute truth depending on its cultural context. Due to these two viewpoints, we can see that the truth about human mind is not singular but plural and it should be an important theme of discussionhow to communicate between different understandings of mind and cultural context. Therefore three steps to approach the difference between Asian and European should be discussed in the following way: first, I will explain Confucian ways of understanding mind. It is Confucian cultural context as a social being that used to be the basis of cultural construction of East Asian’s mind. There were three types of Confucianism in 18th century Korea; second I will explain Western European way of understanding mind and the philosophy of Enlightenment as the basic context of cultural construction of mind; and third, I will conclude by comparing these two different ways of understanding mind and mind construction and by looking for a feasible way to communicate and understand each other. The metaphorical understanding of historical movement as spiral is due to the symbolism of the spiral. Spiral is the geometric pattern to depict a self-accumulative growth of energy or life force. For Ham, history neither reiterates “the eternal return” to the primal archetype nor generates “the unilateral straight move of teleology. If history is a living move, it should follow the basic principle of life evolution as all the living experiences the gradual and yet creative advance by long accumulative changes. (...) There are several factors for Ham Sok Hon to establish the idea of the spiral history. First, he studied the Bible and newly experienced the ‘not-yet-being ontology’ in the Abrahamic religion, a view of a religious utopianism toward the future. It is not the view of “the eternal return” ofMircea Eliade but the view of historical reality that urges a life-formation in expectation of the novel emergence of the new. Second, among various streams of East Asian thoughts, Ham was greatly influenced from the neo-Confucian idea of the nature of mind and Hua-yen Buddhism, that is, ‘one is many; many is one’. Finally, Ham’s spiral history is different from Hegel’s phenomenology of spirit in that he denies the historical perspectives of heroism or classism and instead advocates the perspective of ssi-al, a perspective that ssi‐al bears all burden of historical suffering and opens a new chapter by overcoming the historical suffering. This article proposes a geostatistical solution for area-to-point spatial prediction (downscaling) taking into account boundary effects. Such effects are often poorly considered in downscaling, even though they often have significant impact on the results. The geostatistical approach proposed in this article considers two types of boundary conditions (BC), that is, a Dirichlet-type condition and a Neumann-type condition, while satisfying several critical issues in downscaling: the coherence of predictions, the explicit consideration of support differences, and the assessment of uncertainty regarding the (...) point predictions. An updating algorithm is used to reduce the computational cost of area-to-point prediction under a given BC. In a case study, area-to-point prediction under a Dirichlet-type BC and a Neumann-type BC is illustrated using simulated data, and the resulting predictions and error variances are compared with those obtained without considering such conditions. Those who see Confucianism as a premodern imperial ideology or a traditional religion have no problem characterizing its social ideal as inherently hierarchical, as this is fairly typical of such systems of thought. Despite contemporary Confucianism’s aspirations to be a world philosophy, there is an ethnocentric strand within the Confucian tradition, most glaringly exemplified in Han Yu’s attacks on Buddhism. This paper re-assesses Confucian ethnocentrism in the context of contrary practices that indicate a more pragmatic attitude among Confucians toward cross-cultural interactions. It argues that while the ethnocentric tendency serves as constant reminder of the need for vigilance, and recognition of the difficulties of crossing cultural boundaries, there are nevertheless resources within Confucianism for (...) constructing an ethics of communication that is urgently needed to deal with the moral problems of cultural pluralism. The paper analyses the role of various common Confucian virtues such as ren, yi, li, zhi in communication, and argues that a virtue of flexibility is implicit in Confucius’s insistence of bugu and could contribute significantly to a Confucian ethics of communicative virtues. We argue that in contemporary business organizations, in which fundamental purpose is construed to be increased value—especially in ‘participative’ organizations, in which non–hierarchal interaction (for example, work teams) is the norm; and in ‘adaptive’ organizations, in which unpredictable change is the rule—a process of values covenanting will be much more valueable than just espoused values or even values covenants. We propose such a process model for organizational values covenanting and argue that such covenanting reflects an anthropomorphism of the human character (...) development process, validated in terms of the theory of viable systems. SignificanceElectrical burns can cause severe damage to the nervous system, resulting in autonomic dysfunction with reduced cerebral perfusion. However, few studies have investigated these consequences.AimTo elucidate changes in prefrontal cerebral hemodynamics using functional near-infrared spectroscopy (fNIRS) during the head-up tilt table test (HUT) for patients with electrical burns.ApproachWe recruited 17 patients with acute electrical burns within 1 week after their accidents and 10 healthy volunteers. The NIRS parameters acquired using an fNIRS device attached to the forehead were analyzed in five (...) distinct HUT phases.ResultsBased on their HUT response patterns, patients with electrical burns were classified into the group with abnormal HUT results (APG, n = 4) or normal HUT results (NPG, n = 13) and compared with the healthy control (HC, n = 10) participants. We found trends in hemodynamic changes during the HUT that distinguished HC, NPG, and APG. Reduced cerebral perfusion and decreased blood oxygenation during the HUT were found in both the NPG and APG groups. Patients with electrical burns had autonomic dysfunction compared to the HC participants.ConclusionsUsing fNIRS, we observed that acute-stage electrical burn injuries could affect cerebral perfusion. New York and London: Routledge, 2003. Pp. 1,020.Encyclopedias can be useful in teaching philosophy when students need some reading that sets out the basics on a key topic in a few pages without their getting lost in too many complex details or being misled by oversimplifications. They can also be edifying for the philosophy teacher (...) and scholar—providing reliable, up-to-date information on topics that lie in their field but that are not part of their working knowledge. For these reasons, those working in Chinese philosophy will welcome the publication of the Encyclopedia of Chinese Philosophy. The 187 entries cover a variety of subject areas such as "Moral Philosophy" and "Philosophy of language"; Confucianism in different periods and countries, Daoism, Mohism, Legalism, the school of names, Buddhism, and Marxism in China; philosophical trends in post-Mao China, Taiwan, and overseas; key concepts such as dao (the way), fa (law, standards), liyi fenshu (principle and manifestations), and quanli (rights); texts such as the Huainanzi and the Wenzi; and thinkers from Laozi and Confucius to Hu Shih and Mou Zongsan.Instead of more numerous short entries, this encyclopedia has opted for fewer entries of greater length—the Preface tells us that entries on important thinkers and philosophical topics typically range from five thousand to nine thousand words in length, and a quick check reveals that even the shortest entry is at least two pages long (more than one thousand words). This format allows authors to explore the philosophical issues at some depth, and expound even complex arguments in detail. The connections between different concepts, thinkers, texts, philosophical theories, and development that stretch across different periods and territories can be delineated more clearly in a single entry. There is a danger of duplication with this format, as some texts and concepts may be covered in several articles; for example, most of "Confucianism: Constructs of Classical Thought" is on what the author identifies as six key texts of Classical Confucianism: the Analects, Mencius, Xunzi, Yizhuan, Zhongyong (Doctrine of the mean), and Daxue (Great Learning). The sections on Mencius and Xunzi (pp. 66-67) merely introduce a small part of what is discussed in much greater depth and detail in the entries on these two thinkers (pp. 440-448 and 821-829). The Zhongyong and Daxue also have their own entries, and "Confucianism: Confucius" (p. 61) contains an entire section on the Analects.As it turns out, the duplication is less than expected. Pei-jung Fu, author of "Confucianism: Constructs of Classical Thought," implicitly questions Xunzi's Confucian pedigree, beginning his discussion with "Xunzi professed to be an advocate of Confucianism" (p. 67). Such doubt is clearly not shared by A. S. Cua, who authored [End Page 111] the entry on "Xunzi," introducing the thinker as "an important exponent of classical Confucianism" (p. 821). Cua also does not share Fu's view that "the real issue between Mencius and Xunzi lies in their concepts of heaven (tian)" (p. 67). Fu and the author of the entry on the Zhongyong translate key terms in the texts differently: Fu discusses "five ethical codes" and "three comprehensive virtues" (p. 68) while Yanming An calls them "five universal paths" and "three universal virtues" (p. 890). Such differences can be either thought provoking or confusing (especially when unaccompanied by Chinese characters or even pinyin), depending on the reader. The sections on the Analects in "Confucianism: Confucius" and "Confucianism: Constructs of Classical Thought" have hardly anything in common; the first examines it as an account of how Confucius lives as an exemplar remembered by his students, while the latter discusses its content, focusing on ren and li, as well as on Confucius' religious concerns. While concepts such as ren and li are frequently discussed, the different ways in which authors employ them and the different characteristics, contexts, and philosophical stakes they pay attention to enrich one's understanding of the many dimensions of these concepts even as similarities in diverse entries reinforce certain aspects and perspectives.One comes across disagreements to various degrees on common or related topics among different entries. Assessments of an action done intentionally, as we might expect, influence judgments of moral responsibility. What we don't expect is the converse--judgments of moral responsibility influencing assessments of whether an action was done intentionally. Yet this is precisely how people decide, according to Knobe (2003, 2004) and Mendlow (2004) and Nadelhoffer (2004a). I evaluate whether the studies actually support this biasing effect. I argue that the studies are at best inconclusive and that even if they demonstrated that people fall under (...) the biasing effect, such tendencies ought to have no bearing upon philosophical analyses of the concept of intentional action. One of the driving questions in philosophy of mind is whether a person can be understood in purely physical terms. In this presentation, I wish to continue the project initiated by Donald Davidson, whose subtle position on this question has left many more perplexed than enlightened. The main reason for this perplexity is Davidson’s rather obscure pronouncements about the normativity of intentionality and its role in supporting psychophysical anomalism – the claim that there are no laws bridging our intentional states (...) with states of our brain. Insofar as Davidson’s thesis is an ontological one – about the existence of laws or otherwise modally significant connections between the mental and the physical – I think his critics are correct: Davidson has not provided us with a successful argument for psychophysical anomalism. There is, however, a different argument, also based upon considerations about the normativity of intentionality that lead to an equally important conclusion. The conclusion is not ontological but rather epistemic: if thoughts do indeed display normativity, it is hard to understand how they would arise out of mere mechanical occurrences in the brain. To borrow a well-worn phrase, there is an “explanatory gap” between the mental and the physical. Originally coined to capture the epistemic darkness we confront in our attempt to understand phenomenal experiences in purely physical terms, the idea has yet to be explored in the area of contentful mental states or intentionality in general. My argument shall be this: considerations about the normativity of intentionality demonstrate that there is an explanatory gap between the intentional and the physical. In fact, if there were laws of the kind Davidson denies, then the world be more mysterious than if no such laws existed. The presence of an explanatory gap explains why this is so. (shrink)

Non-reductive physicalism is committed to two theses: first, that mental properties are ontologically autonomous, and second, that physicalism is true. Jaegwon Kim has argued that this view is unstable – to honor one thesis, one must abandon the other. In this paper, I present an account of property realization that addresses Kim’s criticism and that explains how the two theses are indeed comfortably compatible.
