This paper offers a brief consideration of how narrative, in the form of people’s own stories, potentially figures in health and social care provision as part of the impulse towards patient-centred care. The rise of the epistemological legitimacy of patients’ stories is sketched here. The paper draws upon relevant literature and original writing to consider the ways in which stories can mislead as well as illuminate the process of making individual treatment care plans.

Obstetric Fistula is an abnormal opening between the vagina and rectum resulting from prolonged and obstructed labour. Studies indicate that delays in accessing maternal care and home birth contribute to the development of fistula. Survivors are usually women of low socioeconomic status residing in rural locations. This study explores the birthing experiences of 15 fistula survivors through a narrative inquiry approach at a repair centre in North‐central Nigeria. Using structural violence as a lens, it describes the role of social, political (...) and health systems in the inequitable access to care for women. For women opting for home births, preference for home delivery was mainly due to lack of finances, poor health systems and cultural practices. Rural location inhibited access as women seeking facility delivery faced transfer delays to referral centres when complications developed. Inequitable maternal health services in rural locations in Nigeria are inherently linked to access to health care, and these contribute to the increased incidence of fistulae. Structural intervention is a health policy priority to address poor health systems and achieve universal health coverage to address maternal health issues in Nigeria. (shrink)

Consensus around what constitutes researcher conflicts of interest (COIs) and awareness of their influence on our research are two critical steps in ensuring the integrity of our science. In this research, data were collected from individual scholars via 2 surveys 5 years apart and from journals and associations to examine the level of social consensus and moral awareness among scholars, journals, and associations regarding researcher COIs. Although we observed increases in level of social consensus and moral awareness between 2012 and (...) 2017, results still revealed limited agreement about what relationships constitute a COI and limited awareness about the presence of and the ethical issues surrounding COIs. Although all journals and associations we examined supported COI disclosure, most did not provide researchers with detailed COI-related information, guidance, or disclosure tools. Limited social consensus and moral awareness regarding COIs is problematic because it inhibits the recognition, disclosure, and management of COIs and limits ethical decision making. We need to continue and enhance discussions about COIs and aim to create consensus and awareness where we do not have it with the goal of reducing potential scientific misconduct related to COIs. Mary Midgley, Iris Murdoch, Gertrude Anscombe and Philippa Foot studied together in Oxford during the war, at a time when most of the men had left the university, leaving it to them for themselves. These unique circumstances where decisive for the fact that they all went on to become successful philosophers and were able to develop their own original philosophical theories, opposing the philosophical dogmas of their time, Midgley later wrote. This claim is the point of departure for this article. (...) It has three purposes: First I try to say something about the circumstances that made it possible to develop such alternatives, and second, I try to say something about what they consist in. Three of the United Nations Millennium Development Goals to eradicate poverty are very much inter-related: ‘Promote gender equality and empower women,’ ‘Reduce child mortality,’ and ‘Improve maternal health.’ Although the biblical text has often been used to subordinate and oppress women, it can be a resource to empower women who live and give birth in conditions of grinding poverty. Put in the mouths of pregnant women, the Song of Hannah and Mary’s Magnificat envision a reversal of hierarchies, in which (...) ‘The Lord raises up the poor from the dust and lifts the needy from the ash heap.’ Attention will be paid to the social pre-exilic and Palestinian contexts of exploitation to which these songs speak. In a world with limited resources, allocation of resources to certain individuals and conditions inevitably means fewer resources allocated to other individuals and conditions. Should a patient's personal responsibility be relevant to decisions regarding allocation? In this project we combine the normative and the descriptive, conducting an empirical bioethical examination of how both Norwegian and British doctors think about principles of responsibility in allocating scarce healthcare resources. A large proportion of doctors in both countries supported including responsibility for illness in (...) prioritization decisions. This finding was more prominent in zero‐sum scenarios where allocation to one patient means that another patient is denied treatment. There was most support for incorporating prospective responsibility (through patient contracts), and low support for integrating responsibility into co‐payments (i.e. through requiring responsible patients to pay part of the costs of treatment). Finally, some behaviours were considered more appropriate grounds for deprioritization (smoking, alcohol, drug use)—potentially because of the certainty of impact and direct link to ill health. In zero‐sum situations, prognosis also influenced prioritization (but did not outweigh responsibility). Ethical implications are discussed. We argue that the role that responsibility constructs appear to play in doctors' decisions indicates a needs for more nuanced—and clear—policy. Such policy should account for the distinctions we draw between responsibility‐sensitive and prognostic justifications for deprioritization. (shrink)

Many parents are hesitant about, or face motivational barriers to, vaccinating their children. In this paper, we propose a type of vaccination policy that could be implemented either in addition to coercive vaccination or as an alternative to it in order to increase paediatric vaccination uptake in a non-coercive way. We propose the use of vaccination nudges that exploit the very same decision biases that often undermine vaccination uptake. In particular, we propose a policy under which children would be vaccinated (...) at school or day-care by default, without requiring parental authorization, but with parents retaining the right to opt their children out of vaccination. We show that such a policy is likely to be effective, at least in cases in which non-vaccination is due to practical obstacles, rather than to strong beliefs about vaccines, ethically acceptable and less controversial than some alternatives because it is not coercive and affects individual autonomy only in a morally unproblematic way, and likely to receive support from the UK public, on the basis of original empirical research we have conducted on the lay public. Philosophy is one of the least inclusive disciplines in the humanities and this situation is changing only very slowly. In this article I consider how one of the women of the Wartime Quartet, Iris Murdoch, can help to challenge this situation. Taking my cue from feminist and philosophical practices, I focus on Murdoch's experience of being a woman and a philosopher and on the role experience plays in her philosophical writing. I argue that her thinking is best characterised with the (...) notion of common sense orsensus communis. This term recognises her understanding of philosophy as based in experience and as a shared effort ‘to make sense of our life’, as Mary Midgley puts it. (shrink)

Hannah Arendt's essays about the 1957 crisis over efforts of a group of youth, the “Little Rock Nine,” to desegregate a high school in Little Rock, Arkansas, reveal a tension in her vision of the “public.” In this article Aaron Schutz and Marie Sandy look closely at the experiences of the youth desegregating the school, especially those of Elizabeth Eckford, drawing upon them to trace a continuum of forms of public engagement in Arendt's work. This ranges from arenas (...) of “deliberative friendship,” where unique individuals collaborate on common efforts, to a more conflictual “public stage,” where groups act in solidarity to change aspects of the public world. While Arendt famously asserted in her essay “The Crisis in Education” that political capacities should not be taught in schools, it makes more sense to see this argument as focused on what she sometimes called the conflictual “public stage,” reflecting the experience of the Little Rock Nine. In contrast, Schutz and Sandy argue that Arendt's own work implies that “deliberative friendship,” as described in her essay “Philosophy and Politics” and elsewhere, should be part of everyday practices in classrooms and schools. How can we critique political theory when all we have to use are its own conceptual tools? As Hannah Arendt observed, it can only be done through leaps, inversions, and the turning of concepts upside-down. But this twisting operation must be done in order to turn those who philosophize back to the hard work of real life change. In Turning Operations, renowned theorist Mary G. ABSTRACT: The concept of the “banality of evil,” put forward by Hannah Arendt to describe the psychological profile of the Nazi criminal in Eichmann in Jerusalem, is intimately tied to her reading of Plato. In Arendt’s examination of the question of evil, she found some support in Kant’s philosophy. However, the problem of guilt under Nazism ultimately goes back to an inability to think. The two-in-one, a concept which describes the activity of thinking, is based on Plato’s dialogues. An (...) examination of these philosophical sources contributes to the understanding of the possibilities and the limits of Arendt’s reflection on evil. RÉSUMÉ : Le concept de banalité du mal, proposé par Hannah Arendt pour décrire le profil psychologique du criminel nazi dans Eichmann in Jerusalem, est intimement lié à ses lectures de Platon. Pour aborder la question du mal, elle a découvert des appuis chez Kant, mais le problème de la culpabilité sous le régime nazi renvoie ultimement au défaut de penser. Ce volume, issu d'un colloque international tenu à Paris en 2012, a voulu les penser ensemble pour la première fois et étudier sur (...) quel fond historique et intellectuel cette double spécificité a été possible. Quelle dette chacun d'entre eux a-t-il pu contracter à l'égard de ce maître commun et quelle distance ont-ils pu prendre (ou ne pas prendre) par rapport à lui après la Seconde Guerre mondiale? A recent legal case involving an ambiguous diagnosis in a woman with a severe disorder of consciousness raises pressing questions about treatment withdrawal in a time when much of what experts know about disorders of consciousness is undergoing revision and refinement. How much should diagnostic certainty about consciousness matter? For the judge who refused to allow withdrawal of artificial nutrition and hydration, it was dispositive. Rather than relying on substituted judgment or best interests to determine treatment decisions, he ruled that (...) withdrawal was categorically prohibited, even as he concluded that Hannah Capes was more likely than not in a permanent vegetative state. In many jurisdictions, his decision would likely be consistent with existing law. Evolving technological advances have demonstrated that biologically distinct diagnoses incorporated into state laws may be difficult to establish even under ideal conditions. We offer the Capes case for purposes of examining the consequences of enshrined legal distinctions between permanent vegetative state and other severe disorders of consciousness. Insistence on proof of the permanent absence of consciousness before treatment withdrawal is allowed fails to respect the rights of persons with disorders of consciousness. Even the well-established rights to treatment withdrawal for those in a permanent vegetative state may be in jeopardy if reform is not undertaken. (shrink)

In a certain sense, trust really cannot be created at all, because trust lies outside that which we can decide. But trust itself contains some creative element in the sense that trust allows our feeling for the new to gain a place in our experience of the world, including our experience of our own future and the future of others as promising. In that sense, there is an intrinsic element of generation or creation in trust. This circularity between trust and (...) creation points to a virtuous circle of dependency or interdependency between people that communicate. In order to describe this connection between maintaining an open mind towards the new and trust, trust is seen as threefold. It is an attitude of trust in oneself or self-confidence; it is trust in the people one knows and shares experiences with; and it is a more generalized trust understood as an affinity for new things – that is, confidence in meeting whatever the unknown brings on. Looking at both Hannah Arendt's and K.E. Løgstrup’s phenomenological analyses of trust, we see trust as the experience that one is not a sovereign being, but a being that is given over – or as Løgstrup puts it: surrendered - a self-surrender that entails a risk of becoming self-exposure if one’s trust is not accepted. The core of trust is that a person leaves something up to the other, and if that something falls on barren soil, then it withers and dies. But when we are seen and heard, not only as individuals, but as persons with opinions and actions, we achieve visibility, not only to others, we also become visible to ourselves, and experience the world as real in a manner we cannot access by any other means. Løgstrup even speaks of “a trust in life itself, in the ongoing renewal of life” when we are met by others in dialogue. The strength in Løgstrup’s and Arendt’s phenomenological analyses of trust lies in their ability to point out the mutual interdependency between self-surrender and trust, and between appearance and community. Appearance is the ability to be counted on as a person who can stand behind one's actions, and a person who can give promises to others. Community, in this context, both connects and divides us as people with different opinions and perspectives of the same thing. This makes the promise the ultimate deed a person can perform with words, namely, creating relationships with other people in mutuality and respect. One’s ability and, especially, one’s will to be bound to something and to allow oneself to be bound by the promise – these constitute the creativity of trust. Making a promise both creates and confirms the common space within which people count on each other and trust each other. (shrink)

Hannah James makes a persuasive case for the use of donated bodies and body parts in surgical training, enabling high fidelity training, improved competency of surgeons and reduced risk of harm to patients from trainees ‘learning on the job’.1 She also identifies some pertinent ethical questions that arise from this practice that should be considered by training organisations, regulatory authorities and the trainees themselves. Many countries throughout the world have regulated programmes, governed by strict ethical principles, for donating bodies, (...) usually to academic institutions for the purposes of medical education.2 In the UK the Human Tissue Authority sets out guiding principles for institutions licensed to handle human tissue including donation of bodies for anatomical examination, education and research; consent, dignity, quality, and honesty and openness.3 The Nuffield Council on Bioethics, in its 2011 report, Human Bodies: donation for medicine and research, identified a number of relevant ethical values including autonomy, altruism, justice, dignity, reciprocity, maximising welfare, and honesty and respect.4 While this report did not focus specifically on donation of human tissue for education and training the principles identified are equally relevant in this context. The title of Hannah Arendt's lecture notes for a course given in 1970 at the New School of Social Research could raise unfounded expectations. Recent work in political philosophy by Rawls and Dworkin, among others, has made it clear that a careful study of Kant's political philosophy, including his formidable Rechtslehre, is long overdue. Arendt, however, is not concerned with the political philosophy Kant wrote, which she finds "rather boring," but with the one "he did not write" and of (...) which he may not have been aware. Following Hans Saner, whose book she greatly admires, she assumes that Kant's interest in constitutional law dates from a time when his mental faculties were deteriorating. What she proposes to expound is the political philosophy he might have written had he become interested in the subject earlier. (shrink)

Of Kant's major works, none has received less attention than The Metaphysics of Morals, particularly its first part, Metaphysische Anfangsgründe der Rechtslehre. From Schopenhauer to Hannah Arendt it has been dismissed as an inferior work, the product of his declining years. In the last decade, however, this neglect has begun to be remedied. The Rechtslehre has become a subject of intense scholarly activity in Germany, and interest in it is spreading to North America. Post–World War II developments concerning citizenship and access as one of the dimensions of citizenship are examined through the prism of noncitizenship and rights, using the drafting of the asylum paragraph of the 1949 Grundgesetz of the Federal Republic of Germany as a specific case study. The aim of this article is to look into the creation of the right to asylum in West Germany, to examine its political history by exploring its development and by searching for its conceptual, political, (...) and rhetorical origins. The article investigates the birth of the unique conceptualization of asylum in the debates of the Parliamentary Council, the constitutional and quasi-parliamentary assembly responsible for the writing of the postwar Basic Law, and examines the political choices, motivations, and compromises behind its creation. To connect the matter of asylum to a wider problematic related to noncitizens and rights, the article benefits from the political philosophy of Hannah Arendt, with reference to her writings on human rights and refugees in the immediate post–World War II period. In the Origins of Totalitarianism, Hannah Arendt makes the unexpected statement that totalitarian violence "is expressed much more frighteningly in the organization of its followers than in the physical liquidation of its opponents." Of course, her intention is not to deny the radical physical violence of totalitarianism but rather to understand the distinctive features of totalitarian terror. In order to fully understand the importance of what Arendt is describing, we should compare this first moment of the analysis with another (...) assertion that seems just as paradoxical and that is also in The Origins of Totalitarianism: noting totalitarianism's contempt for facts and reality, Arendt remarks that the propaganda of totalitarian movements is "invariably as frank as it is mendacious." Totalitarian propaganda does not just lie about the aims and real actions of totalitarian movements or regimes: it also gives itself the organization required to change the real world and make it "true" to its assertions, though they be utterly absurd and utterly monstrous. Through totalitarian organization the natural bonds of solidarity and communication are broken; they are replaced by distrust and informing. The objective is to pervert human plurality into a mass of fragmented individuals, to suppress the common world and substitute it with alienation from the world, from others, and from oneself. From then on, everything is blurred for the outside observer who would still like to distinguish between adherence to the regime out of conviction and submission through terror, organization, and indoctrination. The issue of knowing whether this enthusiasm is forced or sincere loses much of its pertinence. Let us keep this important point in mind when we pass judgment too rapidly on the "fanaticism" of Islamic crowds streaming down the streets of Teheran or any other totalitarian theocracy. (shrink)

The article explores the way American author Cormac McCarthy uses the Gothic genre in his novel The Road as a means to address what has been called “our globalized order,” in particular the way it has turned human beings into consuming or consumed entities. Some dimensions of this globalized order indeed involve the reintroduction of slavery through human trafficking, unprecedented greed and labor capitalism, surveillance and personal data gathering. Hannah Arendt notes in The Origin of Totalitarianism that the disasters (...) of the twentieth century had proved that a globalized order might “produce barbarians from its own midst by forcing millions of people into conditions which, despite all appearances, are the conditions of savages.” The artist’s task is to find the right language and images to address the breaking of the world. French philosopher J. P. Dupuy, for example, has argued that the financial world is a way to contain the violence of competition, placing it into acceptable forms away from primal physical competition. McCarthy’s graphic use of Gothic tropes—including cannibalism, the wild forest, the haunted house, the chase, the conflict between light and darkness, the blurring of boundaries between different categories—creates a shock. The article also addresses the larger question of the impact of globalization on Gothic literature, and the impact of Gothic literature on real world matters as it contributes to and reflects upon and challenges global regimes of economic, social and economic power. Epistemic injustice is defined by Miranda Fricker as injustice done to people specifically in their capacities as knowers. Fricker argues that this injustice can be either testimonial or hermeneutical in character. A hearer commits testimonial injustice against a speaker by assigning unfairly little credibility to the speaker’s testimony. Hermeneutical injustice exists in a society when the society lacks the concepts necessary for members of a group to understand their social experiences. We argue that epistemic injustice is necessary to permit the (...) functioning of race-based chattel slavery and that this necessity is illustrated in slave narratives. The testimonies of slave narratives like those of Frederick Douglass, Olaudah Equiano, Harriet Jacobs, and Mary Prince identify and transform a culture of race-based epistemic hermeneutic and testimonial injustice. Through telling their stories, these agents establish their capacity as knowers and thus resist the epistemic injustice that undergirds the oppressive system of race-based chattel slavery.
