In contrast to attempts that have been made to measure the clarity of reporting of the methods of clinical trials in journal articles, we report here an attempt to measure the accuracy of methods reporting. We focus in this article on eligibility criteria as a test case for the reporting of clinical trial methods. We examined the reporting of eligibility criteria in the protocol, methods paper (if applicable), journal article, and Clinical Alert for articles appearing in print between January 1988 (...) and September 1994 for which a Clinical Alert had been issued. Eligibility criteria were further classified into five categories in order to examine the content of information loss, if any. On average, 82% of protocol eligibility criteria were reported in methods papers. Journal articles and Clinical Alerts fared somewhat worse: 63% of criteria were reported in journal articles, 19% in Clinical Alerts. In all three categories of medical communication, the reporting of criteria that defined the study disease tended to be complete; reporting of criteria relating to trial precision, patient safety, legal and ethical concerns, and administrative considerations, was not complete. We found that criteria for clinical trial eligibility are frequently under-reported in medical communications. Moreover, some of the criteria omitted are of considerable clinical importance. We suggest that in the design phase of clinical trials, proposed eligibility criteria be scrutinized closely. Those criteria that survive this scrutiny and that have clinical import must be reported upon fully and accurately when communicating trial results. (shrink)

En décembre 2007, Jacqueline Hamesse a fêté son 65ème anniversaire, puis a accédé à l’éméritat en 2008. Experiences of substitute decision-makers with requests for consent to non-therapeutic research participation during the dying process, including to what degree such requests are perceived as burdensome, have not been well described. In this study, we explored the lived experiences of family members who consented to non-therapeutic research participation on behalf of an imminently dying patient. We interviewed 33 family members involved in surrogate research consent decisions for dying patients in intensive care. Non-therapeutic research involved continuous physiological monitoring of dying patients (...) prior to and for 30 min following cessation of circulation. At some study centres participation involved installation of bedside computers. At one centre electroencephalogram monitoring was used with a subset of participants. Aside from additional monitoring, the research protocol did not involve deviations from usual end-of-life care. Thematic analysis of interviews suggests most family members did not perceive this minimal-risk, non-therapeutic study to affect their time with patients during the dying process, nor did they perceive research consent as an additional burden. In our analysis, consenting for participation in perimortem research offered families of the dying an opportunity to affirm the intrinsic value of patients’ lives and contributions. This opportunity may be particularly important for families of patients who consented to organ donation but did not proceed to organ retrieval. Our work supports concerns that traditional models of informed consent fail to account for possible benefits and harms of perimortem research to surviving families. Further research into consent models which integrate patient and family perspectives is needed. All data relevant to the study are included in the article. (shrink)

The de minimis principle states that some risks are so trivial that they can be ignored or treated categorically differently from non-trivial risks. Lundgren and Stefánsson criticize the de minimis principle, arguing that it either has to be applied locally or globally and that problems arise whichever application is chosen. Aven and Seif respond to Lundgren and Stefánsson’s argument and defend the de minimis principle as a “meaningful and useful perspective for handling risk in practice.” The response highlights some aspects (...) of the argument in Lundgren and Stefánsson that needs clarification, which is what we do in this note. يعتبر منهج الشك من أبرز مباحث الفلسفة بمختلف نواحيه، وكما يمكن له أن يكون عملية عشوائية أقرب للسفسطة، فهو قادر على أن يكون عملية منظمة يقوم بها الفرد أملاً في الوصول لمعرفة صحيحة خالية من الشبهة والريب. ويعد الشك المنهجي الذي ينافي الشك المطلق مبحثًا لا غنى لأي مفكر عن معرفته واستعماله للوصول للحقيقة وإدراك اليقين. وإن كان الشك بمعناه العام مذموماً وغير مجدياً في الفلسفة والفكر، ولا يؤدي لمعرفة مفيدة حقيقية، إلا أن نموذج الشك القائم على المنهجية قد شكل (...) المثال الأبرز للأداة المحَّاصة للمعارف والسالكة سبيل الوصول للحقيقة، وتتجلى هذا المنهجية في مضمون الكثير من العلوم والمعارف التي وصل إليها العلماء والفلاسفة. وفي هذا البحث سنتطرق باختصار لمفهوم الشك وأوجهه في الفلسفة، كما سنركز الحديث على الشك المنهجي الذي هو في واقع الأمر رفض وصدّ لأنواع الشك الأخرى. وأملا في فهم هذا المنهج سنتعرف على الإمام أبي حامد الغزالي الذي يعد أحد أهم علماء الحضارة الإسلامية وفلاسفتها في القرن العاشر الميلادي. ويحوز الغزالي مكانة كبيرة في مختلف العلوم والبيئات لما قدمه من تراث ثري في شتى العلوم من الفلسفة والمنطق والتصوف والفقه الإسلامي وغيرها. ويعتبر العالم والفيلسوف المسلم الغزالي من أبرز من تبنى المنهج الشكي ومضى على منواله، إذ نرى تجليات ذلك في كتابه "المنقذ من الضلال" الذي لخص فيه تجربته مع الشك ورحلة معاناته بين المعارف والمسالك راجياً الحقيقة الصحيحة والطريق المستقيم. وبعد خمسة قرون تقريبًا من وفاة الغزالي برز هذا المنهج من جديد على يد الفيلسوف الفرنسي رينيه ديكارت، أحد أعلام الفلسفة الفرنسية وروادها، والذي كان له أيضاً حضور في العلوم الرياضية والطبيعية والطب. وقد تبنى ديكارت هذا المنهج في ظل النهضة الفكرية الأوروبية، في خضم انتشار الشك المطلق لدى الكثيرين من أقرانه. ويتراءى لنا استعمال ديكارت لمنهج الشك المنهجي من خلال كتابه "مقال عن المنهاج" الذي يشبه في أسلوبه وتناوله لهذا الموضوع كتاب المنقذ للغزالي. ويأتي هذا البحث كي يحدد معالم الشك المنهجي لدى كلاً من الغزالي وديكارت، ويتفحص طريقتهما في التعامل مع المعارف وصولاً للحقيقة، كما يتناول موقفهما وآرائهما من المعارف التقليدية والعقلية والحسية، بالإضافة للقواعد المعرفية التي جعلوها الوسيلة الأمثل للوصول للمعرفة الحقيقة. ونعتمد في ذلك على مطالعة كتابيهما المنقذ والمقال وتحليلهما، مستفيدين من تجربتهما ورحلتهما المعرفية في تحديد معالم الشك المنهجي وتطبيقاته لديهما. وفي نهاية ورقتنا نشير إلى إمكانية تأثر ديكارت بالغزالي في هذا السياق، كما نتطرق لأوجه الشبه بين آرائهما.
