BackgroundThe preferable position of Deep Brain Stimulation electrodes is proposed to be located in the dorsolateral subthalamic nucleus to improve general motor performance. The optimal DBS electrode localization for the post-operative improvement of balance and gait is unknown.MethodsIn this single-center, retrospective analyses, 66 Parkinson’s disease patients were assessed pre- and post-operatively by using MDS-UPDRS, freezing of gait score, Giladi’s gait and falls questionnaire and Berg balance scale. The clinical outcome was related to the DBS electrode coordinates in x, y, z (...) plane as revealed by image-based reconstruction. Binomial generalized linear mixed models with fixed-effect variables electrode asymmetry, parkinsonian subtype, medication, age class and clinical DBS induced changes were analyzed.ResultsSubthalamic nucleus-deep brain stimulation improved all motor, balance and FoG scores in MED OFF condition, however there were heterogeneous results in MED ON condition. DBS electrode reconstructed coordinates impacted the responsiveness of axial symptoms. FoG and balance responders showed slightly more medially located STN electrode coordinates and less medio-lateral asymmetry of the electrode reconstructed coordinates across hemispheres compared to non-responders.ConclusionDeep brain stimulation electrode reconstructed coordinates, particularly electrode asymmetry on the medio-lateral axis affected the post-operative responsiveness of balance and FoG symptoms in PD patients. Why do feminist philosophers read Kant? Because of his misogyny and his disdain for the body, Barbara Herman has described Kant as the modern moral philosopher whom feminists find most objectionable. But that unhappy status alone would not justify a separate entry on Kant in this volume. Immanuel Kant is the figure in modern philosophy who most clearly articulates the Enlightenment program that reason is the vehicle for humanity's progress towards emancipation from unjust authority, a program that epitomizes the self‐understanding (...) of the Western culture of modernity. Kant's paradigm of objectivity, which formalizes the universal and necessary conditions for knowledge, provides a philosophical justification for the view that cognitive and moral judgments must be disinterested and impartial. This assumption about the impartiality of knowledge is deeply entrenched in academic disciplines. The quest for objectivity undergirds prevailing methodologies in the natural and social sciences as well as in philosophy. The assumption that knowledge is impartial informs the practices of daily life as well – e.g., in how one evaluates newspaper reporting or jury decisions. Feminist philosophers seek to come to terms with these features of modernity. They debate whether Enlightenment conceptions of progress and rationality offer tools for women's emancipation and empowerment, or whether this philosophical inheritance itself has contributed to the historical subordination of women in Western society. In this essay, Robin May Schott criticizes leading proponents of just war theory and introduces the notion of justifiable but illegitimate violence. Instead of legitimating some wars as just, it is better to acknowledge that both the situation of war and moral judgments about war are ambiguous. Schott raises the questions: What are alternative narratives of war? And what are alternative narratives to war? This essay examines Claudia Card's notion of misplaced gratitude, which she explores in one of her last papers, “Gratitude to the Decent Rescuer.” Whereas typically philosophers have been interested in the problems of the failures to honor obligations of gratitude, Card is more interested in the opposite fault of misplaced gratitude. Her interest reflects her social indignation and her fundamental commitment to opposing oppression, exploitation, and injustice in all its forms. The phenomenon of misplaced gratitude becomes visible from this perspective, (...) where one catches sight of what oppression does to people. The essay looks at the question, What does Card's analysis of misplaced gratitude tell us about her own philosophical methods and contributions? It discusses her engagement with both care ethics and Beauvoir's phenomenology of oppression to clarify the centrality of misplaced gratitude for Card's work in developing an ethics of oppression. The phenomenon of misplaced gratitude becomes visible from this perspective, where one catches sight of what oppression does to people. Only then can one see that if, for example, a woman is grateful for not being battered by her husband, her gratitude is misplaced because it is based on socially normative beliefs and practices that in themselves undermine the possibility of justice and moral integrity. One of Card's achievements was to sustain a passionate engagement with issues of oppression coupled with close philosophical scrutiny. Care ethics risks including too much—as in the risk of caring for abusers. In this article I discuss Claudia Card's treatment of war rape in relation to her discussion of the victim's moral power of forgiveness. I argue that her analysis of the victim's power to withhold forgiveness overlooks the paradoxical structure of witnessing, which implies that there is an ungraspable dimension of atrocity. In relation to this ungraspable element, the proposal that victims of atrocity have the power to either offer or withhold forgiveness may have little relevance.

The reliability of clinical practice guidelines has been disputed because guideline panel members are often burdened with financial conflicts of interest. Current recommendations for COI regulation advise not only detailed declaration but also active management of conflicts. To continuously assess COI declaration and management in German guidelines we established the public database LeitlinienWatch. We analyzed all German guidelines at the highest methodological level that included recommendations for pharmacological therapy according to five criteria: declaration and assessment of COI, composition of the (...) guideline development group, independence of the coordinators and lead authors, imposed abstentions because of COI and public external review. Each criterion was assessed using predefined outcome categories. Most guidelines contained a detailed declaration of COI. However, none of the guidelines provided full transparency of COI assessment results. The guideline group was composed of a majority of participants with COI in 55% of the guidelines, no guideline was free of participants with COI. Only 9% of guidelines had coordinators and lead authors without any financial COI. Most guidelines did not provide a rule for abstentions for participants with COI. In 21% of guidelines there was a rule, but abstentions were either not practiced or not documented, whereas in 7% partial abstentions and in 2% complete abstentions were documented. Two thirds of the guideline drafts were not externally reviewed via a public website. COI are usually documented in detail in German guidelines of the highest methodological level. However, considerable improvement is needed regarding active management of COI, including recruitment of independent experts for guideline projects, abstention from voting for participants with COI and external review of the guideline draft. We assume that the publicly available ratings on GuidelineWatch will improve the handling of conflicts of interest in guideline development. (shrink)

Waking hallucinations suggest mechanisms of dream initiation and maintenance. Visual association cortex activation, yielding poorly attended-to, visually ambiguous dream environments, suggests conditions favoring hallucinosis. Attentional and visual systems, coactivated during sleep, may generate imagery that is inserted into virtual environments. Internally consistent dreaming may evolve from successive, contextually evoked images. Fluctuating arousal and context-evoked imagery may help explain dream features.

Nielsen's concept of “covert REM sleep” accounts for more of the complexity in sleep psychophysiology than its conceptual predecessors such as the tonic-phasic model. With new neuroimaging findings, such concepts lead to more precise sleep psychophysiology including both traditional polysomnographic signs and neuronal activity in greater proximity to the actual point sources and distributed networks which generate dreaming. Activity in the prefrontal cortex may distinguish the meta-awareness experienced during lucid dreams from its absence in normal dreams. To examine a possible relationship between dream lucidity and prefrontal task performance, we carried out a prospective study in 28 high school students. Participants performed the Wisconsin Card Sort and Iowa Gambling tasks, then for 1 week kept dream journals and reported sleep quality and lucidity-related dream characteristics. Participants who exhibited a greater degree of lucidity performed significantly better on the task (...) that engages the ventromedial prefrontal cortex, but degree of lucidity achieved did not distinguish performance on the task that engages the dorsolateral prefrontal cortex, nor did it distinguish self-reported sleep quality or baseline characteristics. The association between performance on the Iowa Gambling Task and lucidity suggests a connection between lucid dreaming and ventromedial prefrontal function. (shrink)

Preterm birth is defined as delivery occurring before 37 weeks gestation.1 Infants born prematurely have increased risks of morbidity and mortality throughout life, especially during the first year. These risks increase as the gestational age at birth decreases.2 Additionally, there are significant racial and ethnic differences in preterm birth rates. In 2022, the rate of preterm birth among non-Hispanic black women was approximately 50% higher than that observed in non-Hispanic white women.1 The outcomes for these infants are also disparate–preterm birth (...) and low birth weight are the second-leading causes of infant mortality (deaths before 1 year of age). Notably, among preterm neonates, infants born to non-Hispanic black women have a mortality rate more than double that of infants born to non-Hispanic white mothers.3 Importantly, there is a small but significant group of neonates that can be classified as severely premature (those born <28 weeks). Infants born before 28 weeks have varied and uncertain outcomes. While this group accounts for only 0.4% of preterm births, it also accounts for 40% of deaths among preterm neonates.4 As noted by Romanis and Adkins, the advent of artificial amnion and placenta technology (AAPT) holds promise for its potential to address the physiologic complications and mortality risks of extreme prematurity by introducing amniotic substitutes that mimic in vivo placental oxygenation and substrate delivery. The chapter presents Bergson’s conception of philosophy as a way of life, as a thinking that seeks to make contact with the creativity of life as a whole. This endeavor to alter our vision of the world, and ultimately, our action and sense of being in the world, seeks to operate a “conversion of attention.” For Bergson, such a conversion is tied in with what he calls the “true empiricism” that allows us to experience and think change as that which (...) makes up living reality as a whole. Bergson conceptualizes this move beyond the human in terms of sympathy, a term employed both descriptively, to develop the notion of a sympathetic whole of life in which philosophy as a way of life resituates the self, and prescriptively, as urging us to overcome our estrangement from “the ocean of life” to which we owe our existence. This effort of sympathy takes the form of a spiritual exercise. Not limited to mere contemplation of the world, it transforms the manner in which we perceive the reality of duration and thus opens the path for a different way of living. (shrink)

In the dissertation I examine the split between cognition and eros in Kant's notion of objectivity, which has become paradigmatic for modern theories about knowledge. I argue that the split between cognition, on the one hand, and feelings and desires, on the other, does not capture the necessary conditions of knowledge, as Kant claims, but involves a suppression of erotic factors of existence. ;The split between pure knowledge and sensual existence in Kant's thought reflects an ascetic tradition inherited from both (...) Greek and Christian sources, which views the body, sexuality, and in particular women's sexuality as a source of pollution. According to this tradition, since thought must be divested of the pollution of sensuous existence, women's sexuality precludes them from rational activity. Consequently, the philosophical commitment to purity has justified the exclusion of women from the practice of knowledge. ;The particular form of asceticism which is evident in Kant's treatment of sensibility of objective knowledge, of morality, and of aesthetic judgement, reflects the reified nature of relations in an emerging capitalist economy. The suppression of the immediate, sensual qualities in both the subject and object of knowledge, in Kant's system, corresponds to the suppression of the immediate, qualitative features of the subject and object in the process of commodity production. Both persons and things become reduced to a formal abstract equivalence. Kant's notion of objectivity makes normative this objectification of relations between persons and things. ;Thus, the paradigm of objective knowledge is not only damaging to the thinker, who must detach himself from the emotional and sensual facets of existence. It serves as an ideology which has justified the exclusion of women from the pursuit of knowledge, and which more generally legitimates the distorted human relations generated by the world of commodity production. ;By considering Kant's commitment to pure knowledge in the context of the genealogy of the concept of purity, the themes of asceticism and fetishism emerged as mutually illuminating. Implicit in the ascetic denial of sensuality is a dialectic which leads to an objectification of persons and things. Moreover, the fetishism of commodities involves a detachment of erotic interests from persons which results in an obsessive interest in objects. (shrink)

Standard models of adolescent risk taking posit that the cognitive abilities of adolescents and adults are equivalent, and that increases in risk taking that occur during adolescence are the result of socio emotional differences in impulsivity, sensation seeking, and lack of self-control. Fuzzy-trace theory incorporates these socio emotional differences. However, it predicts that there are also cognitive differences between adolescents and adults, specifically that there are developmental increases in gist-based intuition that reflects understanding. Gist understanding, as opposed to verbatim-based analysis, (...) generally has been hypothesized to have a protective effect on risk taking in adolescence. Gist understanding is also an essential element of informed consent regarding risks in medical decision- making. Evidence thus supports the argument that adolescents’ status as mature minors should be treated as an exception rather than a presumption, because accuracy in verbatim analysis is not mature gist understanding. Use of the exception should be accompanied by medical experts’ input on the bottom-line gist of risks involved in treatment. (shrink)

One can love and not forgive or out of love decide not to forgive. Or one can forgive but not love, or choose to forgive but not love the ones forgiven. Love and forgiveness follow parallel and largely independent paths, a truth we fail to acknowledge when we pressure others to both love and forgive. Individuals in conflict, sparring social and ethnic groups, warring religious communities, and insecure nations often do not need to pursue love and forgiveness to achieve peace (...) of mind and heart. They need to remain attentive to the needs of others, an alertness that prompts either love or forgiveness to respond. By reorienting our perception of these enduring phenomena, the contributors to this volume inspire new applications for love and forgiveness in an increasingly globalized and no longer quite secular world. With contributions by the renowned French philosophers Jacques Derrida and Jean-Luc Marion, the poet Haleh Liza Gafori, and scholars of religion, psychoanalysis, Islamic and political philosophy, and the Bible and literature, this anthology reconstructs the historical and conceptual lineage of love and forgiveness and their fraught relationship over time. This study investigated the relationship between dream emotion and dream character identification. Thirty-five subjects provided 320 dream reports and answers to questions on characters that appeared in their dreams. We found that emotions are almost always evoked by our dream characters and that they are often used as a basis for identifying them. We found that affection and joy were commonly associated with known characters and were used to identify them even when these emotional attributes were inconsistent with those of (...) the waking state. These findings are consistent with the finding that the dorsolateral prefrontal cortex, associated with short-term memory, is less active in the dreaming compared to the wake brain, while the paleocortical and subcortical limbic areas are more active. The findings are also consistent with the suggestion that these limbic areas have minimal input from the dorsolateral prefrontal cortex in the dreaming brain. (shrink)

This article argues for the reassessment of Indrabhuti’s Jnanasiddhi in the light of newly discovered primary witnesses and previously unnoticed relations to other works. Presumably composed in the late eighth to early ninth century CE, it belongs to the earlier phase of Tantric Buddhism, a transitional period between the predominance of the Yoga- and Mahayogatantras to the later Yoginitantras, and relates to a variety of important scriptures of this formation. While this article briefly discusses the origins, authorship, contents and context (...) of the Jnanasiddhi, including a brief overview of its modern scholarship, it also highlights its intertextual relations with and notable influences upon other tantric works of major importance. The Jnanasiddhi stands out as a tantric scripture of paramount importance, and we attempt to demonstrate the necessity of its reassessment with recourse to hitherto neglected but nonetheless vital sources that have only recently come to light. Science is atypical because it is cultivated with communal participation from throughout the world. This global formation has evolved recently. It originates in the institutionalization of a cosmopolitan tradition in Europe. The cosmopolitan orientation and the perceived usefulness of the European tradition promoted its adoption and institutionalization in the non-Western civilizations. A global institutional frame, including a global science policy regime, sustains communal participation in world science. Participation is described in terms of individual, national, and global communalformations.
