In lieu of an abstract, here is a brief excerpt of the content:Philosophies versus Philosophy:In Defense of a Flexible DefinitionRein RaudIt is strange that no one has taken up Carine Defoort's clearly formulated and timely argument about the intercultural tensions in interpreting what philosophy is, although the issue deserves at least a roundtable, if not an international conference.1 I doubt that this is because there is a general consensus that the matter is now settled, and I would therefore like to (...) develop the argument a bit further and offer a few additional factors to consider. It is also obvious that the problem is not limited to the subject of Chinese philosophy alone: all traditions of thought from all over the world, but most notably the Indian, Islamic, and Japanese heritages, are affected by the positions we adopt. As in most debates about the commensurability of cultural traditions, we can find differences when we look for them, and similarities if these are what we would like to see, so the "conflict of sensitivities"2 is also a matter of attitudes. My own position is that regardless of what we prefer to call the practice of deeper thought (and 'philosophy' is a very good name), it would be extremely useful for all of its participants, whatever their origin and upbringing, to find a common denominator for them to be able to exchange ideas and mutually enrich each other on its basis. In what follows I shall try to sketch a perspective from which this could, in my opinion, be theoretically grounded.Four PositionsDefoort has identified four possible points of view on the problem of whether classical Chinese thought could or should be called 'philosophy,' which could be summarized as follows.1. Chinese classical thought is not philosophy, because it does not satisfy the conditions of philosophy as it is generally and traditionally understood in the West.2. Chinese classical thought is philosophy, because it reveals the necessary characteristics if analyzed with the help of an appropriate (Western-type) observation language.3. Chinese classical thought can be called philosophy, but only if we redefine philosophy in order to accommodate some of its nontypical or clearly deviant features.4. Chinese classical thought should not be called philosophy, because philosophy is a culturally closed Western phenomenon with a limited scope and incapable of understanding fundamentally different modes of thought. [End Page 618]If we dismiss the birthright variety of the first position, we could label it essentialist, and its weakest point is certainly that Western philosophy has not remained unchanged during the course of history. It is highly doubtful that Socrates, Diogenes, scholastics, Nietzsche, and Gareth Evans could all be members of the same set if the same strict criteria were applied to all periods and all cultural settings in the same way, and, as Defoort herself stresses,3 the fact that seventeenth-century Jesuits (the basis of whose original worldview, we should think, was much farther from the Chinese than ours) had no problem in calling Chinese thought philosophy should show us that the paradigm shift of the thing called 'philosophy' has occurred in the West and is, so to speak, our problem and not theirs.Now, it is also possible that in some circles even some prominent names in the Western tradition do not qualify as real philosophers, in the sense in which Hegel was treated by Bertrand Russell4 and Heidegger was dismissed by Alfred Ayer5 in their respective histories of philosophy (while they nevertheless felt compelled to include chapters or passages on them). A definition of philosophy that puts forward such criteria that a considerable number of thinkers would have to remain outside its borders should be labeled sectarian-and it can still be met, even if ever less often, in some analytical schools of philosophy. It is my conviction, however, that in our globalizing world any definition that would leave non-European philosophy outside the borders of 'philosophy proper' should be considered sectarian in nature. Replacing logical coherence by effectiveness as criteria of rationality, Gigerenzer et al. show that simple heuristics can outperform comprehensive procedures (e.g., regression analysis) that overload human limited information processing capacity. Although their work casts long overdue doubt on the normative status of the Rational Choice Paradigm, their methodology leaves open its relevance as to how decisions are actually made.

The present study tested the existence of a cognitive schema that guides people's evaluations of the likelihood that observed problem-solving processes will succeed. The hypothesised schema consisted of attributes that were found to distinguish between retrospective case reports of successful and unsuccessful real world problem solving (Lipshitz & Bar Ilan, 1996). Participants were asked to evaluate the likelihood of success of identical cases of problem solving that differed in the presence or absence of diagnosis, the selection of appropriate or inappropriate (...) solutions, and the pairing of diagnosis with appropriate or non-appropriate solutions. Consistent with the proposition, diagnosis affected perceived likelihood of success, albeit only when solution quality was held constant, and appropriate diagnosis with a compatible solution produced higher perceived likelihood of success than appropriate diagnosis with incompatible solutions. In addition, results showed that solution quality played a significant role, and that compatibility with a six-phase rational model of problem solving played no role in judging likelihood of success. This paper presents an integrative framework for analyzing decisions. Three generic modes of decision making are identified: consequential choice , matching and reassessment . The three generic modes are compared on a common set of attributes, and the conditions for their proper use are outlined. Two case analyses illustrate how the framework can be used to analyze specific decisions from multiple perspectives.

Considerable attention in bioethics has been devoted to moral expertise and its implications for handling applied moral problems. The existence and nature of moral expertise has been a contested topic, and particularly, whether philosophers are moral experts. In this study, we put the question of philosophers’ moral expertise in a wider context, utilizing a novel and global study among 4,087 philosophers from 96 countries. We find that despite the skepticism in recent literature, the vast majority of philosophers do believe in (...) moral expertise and in the contribution of philosophical training and experience to its acquisition. Yet, they still differ on what philosophers’ moral expertise consists of. While they widely accept that philosophers possess superior analytic abilities regarding moral matters, they diverge on whether they also possess improved ability to judge moral problems. Nonetheless, most philosophers in our sample believe that philosophers possess an improved ability to both analyze and judge moral problems and that they commonly see these two capacities as going hand in hand. We also point at significant associations between personal and professional attributes and philosophers’ beliefs, such as age, working in the field of moral philosophy, public involvement, and association with the analytic tradition. It is a commonplace that Frege thought ordinary language to be seriously defective. Yet his remarks about ordinary language are not always unflattering. Comparing the relation between his formal language and ordinary language to the relation between the microscope and the eye, Frege remarked: ‘[the eye], because of the range of its applicability and because of the ease with which it can adapt itself to the most varied circumstances, has a great superiority over the microscope’. The point, of course, is (...) that, for Frege, the deficiencies of ordinary language arise in connection with the scientific endeavour: ordinary language is not an acceptable medium in which to pursue truth. As he goes on to observe: ‘… viewed as an optical instrument [the eye] reveals many imperfections … as soon as scientific purposes place strong requirements upon sharpness of resolution, the eye proves to be inadequate. Ted Shotter's founding of the London Medical Group 50 years ago in 1963 had several far reaching implications for medical ethics, as other papers in this issue indicate. Most significant for the joint authors of this short paper was his founding of the quarterly Journal of Medical Ethics in 1975, with Alastair Campbell as its first editor-in-chief. In 1980 Raanan Gillon began his 20-year editorship . Gillon was succeeded in 2001 by Julian Savulescu, followed by John Harris and Soren (...) Holm in 2004, with Julian Savulescu starting his second and current term in 2011. In 2000 an additional special edition of the JME, Medical Humanities , was published, under the founding joint editorship of Martyn Evans and David Greaves. In 2003 Jane Macnaughton succeeded David Greaves as joint editor. Deborah Kirklin, under whose auspices MH became an independent journal, took over in 2008, and she was succeeded in 2013 by Sue Eckstein. This short paper offers reminiscences and reflections from the two journals’ various editors.From the start the JME was committed to clearly expressed reasoned discussion of ethical issues arising from or related to medical practice and research. In particular, both Edward Shotter and Alastair Campbell, each a cleric , were at pains to make clear that the JME was not a religious journal and that it had no sort of partisan axe to grind.Campbell's appointment as founding editor was something of a surprise, as the original intention had been to appoint a medical doctor, who could be expected to know medical practice from the inside. However, in 1972 Campbell, a Joint Secretary of the Edinburgh Medical Group, had published Moral dilemmas in medicine. …. (shrink)

This paper argues that Charlie Gard’s parents should have been the decision-makers about their son’s best interests and that determination of Charlie’s best interests depended on a moral decision about which horn of a profound moral dilemma to choose. Charlie’s parents chose one horn of that moral dilemma and the courts, like Charlie Gard’s doctors, chose the other horn. Contrary to the first UK court’s assertion, supported by all the higher courts that considered it, that its judgement was ‘objective’, this (...) paper argues that the judgement was not and could not be ‘objective’ in the sense of objectively correct but was instead a value judgement based on the judge’s choice of one horn of the moral dilemma. While that horn was morally justified so too was the horn chosen by the parents. The court could and should have avoided depriving the parents of their normal moral and legal right and responsibility to decide on their child’s best interests. Instead, this paper argues that the court should have acknowledged the lawfulness of both horns of the moral dilemma and added to its judgement that Charlie Gard’s doctors were not legally obliged to provide treatment that they believed to be against their patient’s best interests the additional judgement that Charlie’s parents could lawfully transfer his care to other doctors prepared to offer the infant a trial of the experimental treatment requested by his parents. This paper argues that the central issue in the abortion debate has not changed since 1967 when the English parliament enacted the Abortion Act. That central issue concerns the moral status of the human fetus. The debate here is not, it is argued, primarily a moral debate, but rather a metaphysical debate and/or a theological debate—though one with massive moral implications. It concerns the nature and attributes that an entity requires to have “full moral standing” or “moral inviolability” including a (...) “right to life”. It concerns the question when, in its development from newly fertilised ovum to unequivocally mature, autonomous morally inviolable person does a human being acquire that nature and those attributes, and thus a “right to life”. The paper briefly reviews standard answers to these questions, outlining some problems associated with each. Finally there is a brief discussion of one way in which the abortion debate has changed since 1967—notably in the increasingly vociferous claim, especially from disability rights sectors, that abortion on grounds of fetal abnormality implies contempt for and rejection of disabled people—a claim that is rebutted. Justice, one of the four Beauchamp and Childress prima facie basic principles of biomedical ethics, is explored in two excellent papers in the current issue of the journal. The papers stem from a British Medical Association essay competition on justice and fairness in medical practice and policy. Although the competition was open to all comers, of the 235 entries both the winning paper by Alistair Wardrope1 and the highly commended runner-up by Zoe Fritz and Caitríona Cox2 were written by practising (...) doctors—a welcome indication of the growing importance being accorded to philosophical reflection about medical practice and practices within medicine itself. Both papers are thoroughly thought provoking and represent two very different approaches to the topic. Each deserves a careful read. The competition was a component of a BMA 2019/2020 ‘Presidential project’ on fairness and justice and asked candidates to ‘use ethical reasoning and theory to tackle challenging, practical, contemporary, problems in health care and help provide a solution based on an explained and defended sense of fairness/justice’. In this guest editorial I’d like to explain why, in 2018 on becoming president-elect of the BMA, I chose the theme of justice and fairness in medical ethics for my 2019–2020 Presidential project—and why in a world of massive and ever-increasing and remediable health inequalities biomedical ethics requires greater international and interdisciplinary efforts to try to reach agreement on the need to achieve greater ‘health justice’ and to reach agreement on what that commitment actually means and on what in practice it requires. First, some background. As president I was offered the wonderful opportunity to pursue, with the organisation’s formidable assistance, a ‘project’ consistent with the BMA’s interests and values. The chapter contributes to the debate on Dōgen’s theory of time by discussing key concepts of the Shōbōgenzō’s “Uji” fascicle in a broader context, comparing them with other cases of usage in the entire work, their provenance in the tradition of Zen thought, as well as with their possible translational equivalents and their connotations in the Western tradition. A central claim for which I argue is that a presentist reading of the fascicle (as well as other related passages in Dōgen’s (...) work) leaves the fewest of the cryptic passages unexplained. It does this while blending seamlessly with Dōgen’s other claims about reality, language, our ways to understand them, and praxis.I begin, however, by touching on the much-discussed issue of Dōgen’s relationship with philosophy, arguing that it is incorrect to exclude non-Western traditions from the domain of philosophy because of particular features that certain canonically recognised Western thinkers and texts also possess. The issue is important, because it legitimates a register of reading of texts such as Dōgen’s in a way that places them in dialogue with ideas that they have historically had no connection with. And it also enables us to use them in contexts for which they have no historical relevance — something that treating them solely as subject matter for the history of ideas would not let us do. Lying is an important moral phenomenon that most people are affected by on a daily basis—be it in personal relationships, in political debates, or in the form of fake news. Nevertheless, surprisingly little is known about what actually constitutes a lie. According to the traditional definition of lying, a person lies if they explicitly express something they believe to be false. Consequently, it is often assumed that people cannot lie by more indirectly communicating believed‐false claims, for instance by merely conversationally (...) implicating them. In this paper, we subject this claim to an empirical test. In a preregistered study of 300 participants, we investigate how people judge cases of implicit deceptions that would usually be excluded by the traditional definition of lying (i.e., conversational implicatures, presuppositions, and nonverbal actions). Our results show that people do in fact consider it possible to lie by indirect means, suggesting that people have a broader concept of lying than is usually assumed. Moreover, our findings indicate that lie judgments are closely tied to the extent to which agents are perceived as having committed themselves to the believed‐false claims they have communicated. We discuss the implications of our results for the traditional definition of lying and propose a new commitment‐based definition of lying that can account for the findings of our experiment. Reality exists independently of human observers, but does the same apply to its structure? Realist ontologies usually assume so: according to them, the world consists of objects, these have properties and enter into relations with each other, more or less as we are accustomed to think of them. Against this view, Rein Raud develops a radical process ontology that does not credit any vantage point, any scale or speed of being, any range of cognitive faculties with the privilege to judge (...) how the world ‘really’ is. In his view, what we think of as objects are recast as fields of constitutive tensions, cross-sections of processes, never in complete balance but always striving for it and always reconfiguring themselves accordingly. The human self is also understood as a fluctuating field, not limited to the mind but distributed all over the body and reaching out into its environment, with different constituents of the process constantly vying for control. The need for such a process philosophy has often been voiced, but rarely has there been an effort to develop it in a systematic and rigourous manner that leads to original accounts of identity, continuity, time, change, causality, agency and other topics. Throughout his new book, Raud engages with an unusually broad range of philosophical schools and debates, from New Materialism and Object-Oriented Ontology to both phenomenological and analytical philosophy of mind, from feminist philosophy of science to neurophilosophy and social ontology. Being in Flux will be of interest to students and scholars in philosophy and the humanities generally and to anyone interested in current debates about realism, materialism and ontology. (shrink)

In one of his essays on art, Dutch author and essayist Joost Zwagerman reflects on the work of South African artist Marlene Dumas. Zwagerman addresses in particular Dumas' My Mother Before She Became My Mother, painted 3 years after her mother died. In his reflections, Zwagerman proposes an interpretation of Dumas' work. He suggests that Dumas, in her art, does not accept the omnipotence of death. Maybe against better judgement, but Dumas keeps creating images that not only illustrate the desire (...) for meaning but also embody this desire. The image and the desire for meaning merge in Dumas' paintings. The painting itself becomes an autonomous 'desire machine', according to Zwagerman. In this article, a theological reading of Zwagerman's own posthumously published volume of poetry, 'Wakend over God', is presented, with a specific interest in art, death and God. The sacramental hermeneutics of Richard Kearney and the theopoetics of John Caputo are brought into the conversation to elicit the dimensions of faith and religion in Zwagerman's own 'desire machine'. This paper, based on a talk given at a conference on compassion in health care held at the Royal Society of Medicine in November 2012, argues that the ethical requirement for humanity in health care is obvious and needs little ethical analysis – the problem is to get the results of ethical reflection, ordinary humanity and everyday common sense, into everyday behaviour. The author offers some suggestions that might help to achieve this aim and bring back the human face of (...) health and social care. These suggestions concern organisational structural changes (including `humanity objectives' in appraisal and reward schemes); individual attitudes (including self assessment of their own humanity in their work by all health and social care workers – `does my own practice manifest a human face?'); and a possible research agenda (and a concomitant effort to remind all health care research funders that `humanity is an integral component of medical, health and care research'. And the author proposes a standing high level `humanity task force' to implement and oversee Health Education England's recent `humanity mandate'. (shrink)

In recent decades, the discipline of bioethics has grown rapidly, as has the practice of ethical consultation. Interestingly, this new recognition of the relevance of moral philosophy to our daily life has been accompanied by skepticism among philosophers regarding the existence of moral expertise or the benefits of philosophical training. In his recent article in Bioethics, William R. Smith suggested that this skepticism is rooted in philosophers' belief that moral expertise is inconsistent with liberal–democratic values, when in fact they are (...) compatible. In this paper, we provide a unique opportunity to empirically examine Smith's observation by utilizing and extending global data on philosophers' beliefs about moral expertise, involving 4087 philosophers from 96 countries. Our findings support Smith's theoretical observation and show that societal levels of support for liberal–democratic values are associated with greater skepticism about moral expertise. We suggest that these findings might be explained by the cognitive process of motivated reasoning and an invalid inference of “is” from “ought.” Consequently, the potential tension between moral expertise and liberal–democratic values is invalidly used for rejecting the existence of moral expertise, while its main and valid implication is for how moral expertise should be applied in liberal–democratic settings. This article argues for a new way to interpret Dōgen's theory of time, reading the notion of uji as momentary existence, and shows that many notorious difficulties usually associated with the theory can be overcome with this approach, which is also more compatible with some fundamental assumptions of Buddhist philosophy (the non-durational existence of dharmas, the arbitrariness of linguistic designations and the concepts they point to, the absence of self-nature in beings, etc.). It is also shown how this reading leads (...) to an innovative treatment of the concept of selfhood, viewing the self as the active openness of an existent to the surrounding world, with which it is able to identify through a mutual relation with other existents within the existential moment. This argument is supported by an alternative translation in the "momentary mode" of those extracts of the fascicle that introduce or elaborate on Dōgen's key concepts. (shrink)

In it, Japanese poetry is characterized as follows:Narrow in its scope and resources, it is chiefly remarkable for its limitations-for what it has not, rather than what it has.... Indeed, narrative poems of any kind are short and very few, the only ones which I (...) have met with being two or three ballads of a sentimental cast. Didactic, philosophical, political and satirical poems are also conspicuously absent. The Japanese muse does not meddle with such subjects, and it is doubtful whether, if it did, the native Pegasus possesses sufficient staying power for them to be dealt with adequately.([1899] 1977, p. 24)Nevertheless, Aston took it for granted that Japan had a literature and that one part of it was poetry, even if it did not quite live up to his expectations. Timothy Reiss, writing a century later, was not so generous: he argued that literature in the proper (i.e., his) sense of the word appeared in Europe in the seventeenth century "as part of a social solution to a crisis viewed explicitly at the time as at once logico-linguistic and sociopolitical" (1992, p. 5). As was fashionable at the time, Reiss' voluminous study also set out to prove that, once the crisis had been resolved, literature would also cease to exist.These two approaches can certainly be seen in the treatment of non-Western philosophical traditions as well. We might say that Aston's assessment of Japanese literature ("ingenious and inventive, but hardly capable of high intellectual achievement" [1899] 1977, p. 4) could perhaps be compared to the views of Christian missionaries and other early mediators of non-Western traditions, while the position of Reiss is similar to the position of those contemporary Western scholars who would like to reserve the right to define philosophy as narrowly as possible. In my comment earlier in this issue I tried to argue that neither of these positions, nor their derivates, is really satisfactory in the contemporary intellectual environment.In her thorough response, Carine Defoort has clarified most issues that provoked me in her initial article on this topic, particularly in emphasizing that her "family model" of philosophy is meant to be descriptive and not prescriptive. Nevertheless, I do find it a bit too pessimistic even as a description of the present situation, and I am also inclined to disagree with her certainty that "a world philosophy has not arisen and is not on the rise." Of course, a lot remains to be done in this area, but there is a clear tendency to the contrary, although this is perhaps due more to the contribution of Japanese rather than Chinese thinkers. A brief internet search reveals a considerable number of scholars who occupy professorial positions in philosophy (not Asian studies) departments at various universities around the world who list, for example, the Kyoto school philosophers or Dōgen in their research interests. Defoort as well does not have too high hopes of the Chinese tradition entering the field on a [End Page 661] more permanent setting because of the complexity of the language; on the other hand, not so many Western philosophers are proficient in Greek, either, but this is not perceived to be a major obstacle in their work. Moreover, although articles and books on Japanese thinkers sometimes appear that are on the naive side, relying on translations as they do, this situation does not necessarily have to stay the way it is.Nevertheless, Defoort is most certainly right in that the opposite number of philosophy departments, where Asian traditions are not considered to be proper philosophy, is, for the time being, much bigger. And some of them are in the most respectable institutions. However, when thinkers such as Jacques Derrida deny the status of philosophy to Chinese thought, one should remember that in many of these respectable institutions the label of "philosophy" is denied to the work of Derrida himself as well. This paper examines six cross-sector partnerships in South Africa and Zambia. These partnerships were part of a research study undertaken between 2003 and 2005 and were selected because of their potential to contribute to poverty reduction in their respective countries. This paper examines the context in which the partnerships were established, their governance and accountability mechanisms and the engagement and participation of the partners and the intended beneficiaries in the partnerships. We argue that a partnership approach which has proven successful (...) in one context can be used as a valuable learning resource. However, a partnership's work, which includes all aspects of the partnership and its activities, cannot necessarily be transferred directly to another partnership without a thorough and locally informed analysis of the context in which it is implemented. In addition, we suggest that it is difficult to assess whether the good intentions behind partnerships were translated into real benefits for target groups as effective monitoring and evaluation procedures were not in place in the partnerships studied. Similarly, the absence of regularised governance and accountability systems in partnerships made it difficult to support partner and beneficiary participation and engagement. We conclude that there is a need to move beyond a 'one-size-fits-all' approach to partnerships and that partnership replication should focus more strongly on the transfer of learning about partnership processes instead of simply copying partnership activities. In this paper, the elaboration of the concept of practical realist philosophy of science which began in the author's previous papers is continued. It is argued that practical realism is opposed to standard scientific realism, on the one hand, and antirealism, on the other. Standard scientific realism is challengeable due to its abstract character, as being isolated from practice. It is based on a metaphysical-ontological presupposition which raises the problem of the God's Eye point of view (as it was called (...) by Hilary Putnam). Joseph Rouse's conception of science as practice, Sami Pihlström's pragmatic realism, and even Ilkka Niiniluoto's critical scientific realism are interpreted as practical realist conceptions. Pihlström suggests that the contemporary scientific realist should be prepared to accept the pragmatically naturalized Kantian transcendental perspective on realism. It is argued, however, that this realistically naturalized Kantianism can be nothing more than practical realism, as originated by Karl Marx. (shrink)

The philosophical analysis of chemistry has advanced at such a pace during the last dozen years that the existence of philosophy of chemistry as an autonomous discipline cannot be doubted any more. The present paper will attempt to analyse the experience of philosophy of chemistry at the, so to say, meta-level. Philosophers of chemistry have especially stressed that all sciences need not be similar to physics. They have tried to argue for chemistry as its own type of science and for (...) a pluralistic understanding of science in general. However, when stressing the specific character of chemistry, philosophers do not always analyse the question ‘What is science?’ theoretically. It is obvious that a ‘monistic’ understanding of science should not be based simply on physics as the epitome of science, regarding it as a historical accident that physics has obtained this status. The author’s point is that the philosophical and methodological image of science should not be chosen arbitrarily; instead, it should be theoretically elaborated as an idealization (theoretical model) substantiated on the historical practice of science. It is argued that although physics has, in a sense, justifiably obtained the status of a paradigm of science, chemistry, which is not simply a physical science, but a discipline with a dual character, is also relevant for elaborating a theoretical model of science. The theoretical model of science is a good tool for examining various issues in philosophy of chemistry as well as in philosophy of science or science studies generally. (shrink)

This commentary briefly argues that the four prima facie principles of beneficence, non-maleficence, respect for autonomy and justice enable a clinician (and anybody else) to make ethical sense of the author's proposed reliance on professional guidance and rules, on law, on professional integrity and on best interests, and to subject them all to ethical analysis and criticism based on widely acceptable basic prima facie moral obligations; and also to confront new situations in the light of those acceptable principles.

IN DEFENCE OF MEDICAL COMMITMENT CEREMONIESI confess to an overwhelming astonishment on first reading my friend Bob Veatch's attack on white coat ceremonies. Surely, I had thought, everyone who considered the issue would want doctors to commit themselves to the basic moral goals of medicine and especially that ancient Hippocratic goal of working to benefit the health of their/our patients, and only risking or doing harm with the intention and likely outcome of producing their net health benefit? Surely, too, it's (...) a good idea for the new medical student—or at least the new clinical student about to encounter and interact with real patients—to commit himself or herself to those goals? But as usual Professor Veatch is an incisive and thought-provoking critic and his three criticisms require careful reflection. Those criticisms were: that the oaths or affirmations were not legitimate, especially because the students were unlikely to understand them or their implications; that it is not clear which of the competing candidate oaths or commitments should be chosen and why, and that the “bonding” aspect of the ceremony in which the new student is welcomed as a student member of the medical profession is undesirable in so far as it tends to remove medical students from their own cultures and place them instead in a medical culture “increasingly isolated from the people and cultures from which they have come”.Too ill informed, too young, too coerced to commit?As for the first criticism, I suppose a sensible development of the white coat ceremony would be to send each medical school applicant a copy of the commitment that he or she would be expected to make on joining the medical school so that he or she can study, reflect on, and discuss it with family and friends. In this journal, Dr Daniel Daly, an American bioethicist, uses a principlist approach (respect for autonomy, non-maleficence, beneficence and justice) to argue that intravenous opiate users should not be denied repeat heart valve replacements if these are medically indicated, ‘unless the valve replacement significantly violates another’s autonomy or one or more of the three remaining principles’.1 In brief outline, the paper seeks to use a widely accepted ethical theory—‘principlism’ as developed by Beauchamp and Childress over the last 40 plus years (...) and eight editions of their ground-breaking book Principles of Biomedical Ethics2—to resolve clinical disagreement about the ethics of denying medically indicated life-prolonging treatment to patients who continue or resume intravenous opiate use. The argument Dr Daly's argument in very brief summary is that in the context of contemporary American medical practice, such treatment is ethically justified—perhaps even ethically required—if requested or accepted by an adequately autonomous patient and thus respects the patient’s autonomy, if it is not harmful to the patient, if it is beneficial to the patient, and if it is fair and just in terms of Aristotle’s formal theory of justice according to which equals should be treated equally while unequals should be treated unequally in proportion to the morally relevant inequality or inequalities. Dr Daly focuses his argument around a typical case description where these conditions are met and therefore where, he concludes, repeat heart valve replacements ought to be provided. As Dr Daly notes, principlism ‘is not without its problems; nonetheless it does provide a viable set of principles that are widely held by medical ethicists and inform the work of ethics committees at many secular medical facilities’. We wish to describe and acknowledge the exceptional contributions to medical ethics, both in the UK and internationally, made by Edward Shotter1 who died at home on 3 July 2019. He was founder of the London Medical Group2 3 and instigator of similar student-led medical ethics groups throughout the UK; founder of the Institute of Medical Ethics4 and founder of the Journal of Medical Ethics. Ted Shotter transformed the study of medical ethics in the UK in the interests of patients (...) and professionals alike. In 1963, he established the pioneering ‘Medical Group’ model, an innovative bottom-up method whereby students in the health professions could gain a grounding in ethics that had previously been denied to the profession.5 It was with these Medical Groups that many of the leading figures in contemporary UK medical ethics and law began their careers in the subject including Sir Kenneth Calman, Sir Ian Kennedy, Professor Margaret Brazier OBE, Professor John Harris and Professor Sir Jonathan Montgomery to name but ….
