This article examines emergency ambulance calls made by lay callers for patients found to be in cardiac arrest when the paramedics arrived. Using conversation analysis, we explored the trajectories of calls in which the caller, before being asked by the call-taker, said why they were calling, that is, calls in which callers pre-empted a reason-for-the-call. Caller pre-emption can be disruptive when call-takers first need to obtain an address and telephone number. Pre-emptions have further implications when call-takers reach the stage when (...) they are required to deliver the scripted turn ‘tell me exactly what happened’. When there has been a pre-emption earlier on, callers tend to treat the scripted turn as a request for more information and may not repeat their reason-for-the-call. This can occasion delays and important information can be lost. We identified an effective alternative strategy used by some call-takers, pre-emption repeat, which callers treat as a request for confirmation. (shrink)

Civility is often treated as an essential virtue in liberal democracies that promise to protect diversity as well as active disagreement in the public sphere. Yet the fear that our tolerant society faces a crisis of incivility is gaining ground. Politicians and public intellectuals call for "more civility" as the solution--but is civility really a virtue? Or is it something more sinister--a covert demand for conformity that silences dissent? Mere Civility sheds light on this tension in contemporary political theory and (...) practice by examining similar appeals to civility in early modern debates about religious toleration. In seventeenth-century England, figures as different as Roger Williams, Thomas Hobbes, and John Locke could agree that some restraint on the wars of words and "persecution of the tongue" between sectarians would be required; and yet, they recognized that the prosecution of incivility was often difficult to distinguish from persecution.--. Literature on consumer ethics tends to focus on issues within the public sphere, such as the environment, and treats other drivers of consumption decisions, such as family, as non-moral concerns. Consequently, an attitude–behaviour gap is viewed as a straightforward failure by consumers to act ethically. We argue that this is based upon a view of consumer behaviour as linear and unproblematic, and an approach to moral reasoning, arising from a stereotypically masculine understanding of morality, which foregrounds abstract principles. By demonstrating (...) the importance of context to consumption decisions and articulating the impact of caring relationships, we highlight how such decisions are both complex and situated. This is particularly evident for decisions involving the needs of others, as occurs in family life. We argue that the incorporation of care ethics provides both theoretical insights and a more complete account of consumer ethics. This is explored empirically through an investigation of the ethical dilemmas arising from consumption decisions made by mothers of young children. Such decisions juxtapose an ethical consumption orientation with care for one’s child. Therefore, what has been previously considered a failure to act ethically may in fact be the outcome of complex decision making, which involves competing ethical considerations. We discuss the implications of our findings for theory and practice and how this approach to consumer ethics could be applied more widely. (shrink)

Ever since Mary Astell was introduced as the “First English Feminist” in 1986, scholars have been perplexed by her dual commitments to natural equality and social, political, and ecclesiastical hierarchy. But any supposed “paradox” in her thought is the product of a modernist conceit that treats equality and hierarchy as antonyms, assuming the former must be prior, normative, and hostile to the latter. Seeing this, two other crucial features of Astell’s thought emerge: her ethics of ascent and her psychology of (...) superiority. These, in turn, illuminate her lifelong fascination with ambition as a feminine virtue, as well as her curious embrace of Machiavelli. Astell’s politics and ethics are thus doubly worthy of recovery, both as the product of a singularly brilliant early modern mind and as a fascinating but forgotten vision of “equality before egalitarianism” that sheds light on the persistent complexities of equality and hierarchy to this day. Assisted reproduction often involves biological contributions by third parties such as egg/sperm donors, mitochondrial DNA donors, and surrogate mothers. However, these arrangements are also characterised by a biological relationship between the child and at least one intending parent. For example, one or both intending parents might use their own eggs/sperm in surrogacy, or an intending mother might conceive using donor sperm or gestate a donor embryo. What happens when this relationship is absent, as in the case of 'double‐donor surrogacy' arrangements (...) (DDS)? Here, a child is conceived using both donor eggs and sperm, carried by a surrogate, and raised by the commissioning parents. In this paper, I critically examine proposals to allow DDS in the United Kingdom, and the intentionalist justification for treating this practice distinctly (morally and legally speaking) from private adoption. I argue that the intentionalist approach cannot plausibly justify such a distinction and that other approaches to moral parenthood are also unlikely to succeed. (shrink)

Semantic paradoxes like the liar are notorious challenges to truth theories. A paradox can be phrased with minimal resources and minimal assumptions. It is not surprising, then, that the liar is also a challenge to minimalism about truth. Horwich (1990) deals swiftly with the paradox, after discriminating between other strategies for avoiding it without compromising minimalism. He dismisses the denial of classical logic, the denial that the concept of truth can coherently be applied to propositions, and the denial that the (...) liar sentence expresses a proposition, but he endorses the denial that the liar is an acceptable instance of the equivalence schema (E). This paper has two main parts. It first shows that Horwich’s preferred denial is also problematic. As Simmons (1999), Beall and Armour-Garb (2003), and Asay (2015) argued, the solution is ad hoc, faces a possible loss of expressibility, and is ultimately unstable. Finally, the paper explores a different combination of possibilities for minimalism: treating the truth-predicate as context-dependent, rejecting the notion that the liar expresses a proposition, and reinterpreting negation in some contexts as metalinguistic denial. The paper argues that these are preferable options, but signposts possible dangers ahead. (shrink)

Semyon Frank opposed the Neo-Kantian School and admitted the real existence of the objects of cognition. He treated ontologism as essential to the entire movement of Russian religious philosophy. For Frank, one can only know about something thanks to the absolute, which exists prior to the knowing subject. Ontologism, affirming the priority of being over cognition, has a great significance not only for metaphysics and epistemology, but also for the philosophy of religion. In particular, Frank taught that the most privileged (...) mode of cognition of God is intuition, an immediate experience of God or faith. Intuition is at the heart of the ontological proof, which can be found in St. Anselm, Descartes, and Frank himself. Frank dedicated a number of articles to this topic: “K istorii ontologicheskogo dokazatel’stva”, “Ontologicheskoe dokazatel’stvo bytiya Boga”, as well as texts recently discovered at the Bakhmeteff Archive, including “Dokazatel’stvo bytiya Boga”. In this way, Russian ontologism leads to a new interpretation of the traditional ontological proof, one which acknowledges the existence of God, not on the basis of arguments resting on His definition, but on the basis of the intuitive recognition of His being. A number of countries and states prohibit surrogacy except in cases of “medical necessity” or for those with specific medical conditions. Healthcare providers in some countries have similar policies restricting the provision of clinical assistance in surrogacy. This paper argues that surrogacy is never medically necessary in any ordinary understanding of this term. The author aims to show first that surrogacy per se is a socio-legal intervention and not a medical one and, second, that the intervention in question does not (...) treat, prevent, or mitigate any actual or potential harm to health. Legal regulations and healthcare-provider policies of this kind therefore codify a fiction—one which both obscures the socio-legal motivations for surrogacy and inhibits critical examination of those motivations while mobilizing normative connotations of appeals to medical need. The persisting distinction, in law and in moral discourse, between “social” and “medical” surrogacy, is unjustified. (shrink)

The article presents Fr. Georgy Florovsky’s conception of a neo-patristic synthesis (developed by other orthodox theologians) and discusses some polemical aspects of this project. The neo-patristic synthesis is an approach to development of the patristic thought in the contemporary world and application it to the different branches of knowledge, including science. According to Florovsky science has an imperfect character, because to know the empirical world is possible only from the theological point of view. This position is shared by Alexei Nesteruk (...) who claims that the patristic solutions can be treated as a model for the issues of relationship between science and religion. In this way a postulate of the methodological independency of science is breached. We show that this position is also is not accordance with the patristic thought because the Fathers of the Church were very open towards to the intellectual heritage of their time. Although they stressed that secular knowledge is inferior to theology, nevertheless this statement should be taken in the broad historical context of the defense of Christianity. Eastern Christian attitude to science is very sensitive for the transcendence and takes into account the anthropological dimension. In the conclusion we admit that these aspects of the patristic approach can be useful for the reflection about relationship between science and religion, under the condition that they will not break the principle of the methodological naturalism. As a result of the coronavirus (COVID‐19) pandemic, health professionals are faced with situations they have not previously encountered and are being forced to make difficult ethical decisions. As the first group to experience challenges of caring for patients with coronavirus, Chinese nurses endure heartbreak and face stressful moral dilemmas. In this opinion piece, we examine three related critical questions: Whether society has the right to require health professionals to risk their lives caring for patients; whether health professionals have the (...) right to refuse to care for patients during the coronavirus pandemic; and what obligations there are to protect health professionals? Value of care, community expectations, legal obligations, professional and codes of practice may compel health professionals to put themselves at risks in emergency situations. The bioethical principles of autonomy, justice, beneficence and non‐maleficence, as well as public health ethics, guide nurses to justify their decisions as to whether they are entitled to refuse to treat COVID‐19 patients during the pandemic. We hope that the open discussion would support the international society in addressing similar ethical challenges in their respective situations during this public health crisis. (shrink)

The paper treats Gernot Böhme’s project of new aesthetics as a signal of the advent of postmodern cosmology with the status of a philosophical theory. The main task of this study is to use some categories of Gernot Böhme’s aesthetics in discussing the processes of aestheticization of reality not only in terms of changes that occur between the human environment in the spatial but also in temporal dimension. The starting point is the analysis of aesthetic sense experience with a cognitive (...) character. This experience is analyzed by Böhme within three main fields of aesthetic interest: nature, design, and art. The processes of aestheticizing reality blur the boundaries between the natural and the artificial to such an extent that the essence of aesthetic cognition should be investigated by analyzing nature together with design. Böhme’s categories of philosophy of nature can be utilized creatively to analyze aesthetic experience of the past. The phenomenon of the ephemeral and the concept of atmospheres take the rhythms of nature into account but it contrasts them with the human conception of time. The postmodern attitude towards time suggests a different theoretical approach that can be treated as the expansion of Böhme’s aesthetics, which is more focused on analyzing changes in space. The research analyzes the teledentistry service applied in health services during COVID-19, studying the quality of the teledentistry service and the level of satisfaction of patients treated with this care model. A sample of 341 patients was worked on, using the SERVQUAL model. The results showed a statistically significant relationship between quality of service in teledentistry and patient satisfaction (p<0.05), where 34.9% patients perceived low quality of service and dissatisfaction, while 33.7% were pleased with the care. We conclude that, given (...) the dental health difficulties, the population of the Peruvian central jungle is dissatisfied with the teledentistry service. (shrink)

Type 2 diabetes has been considered a public health threat due to its growing prevalence, particularly in the older population. It is important to know the effects of psychosocial stress and its potential consequences for some basic cognitive processes that are important in daily life. Currently, there is very little information about how people with T2D face acute psychosocial stressors, and even less about how their response affects working memory, which is essential for their functionality and independence. Our aim was (...) to characterize the response to an acute laboratory psychosocial stressor and its effects on WM in older people with T2D. Fifty participants with T2D from 52 to 77 years old were randomly assigned to a stress or control condition. Mood and physiological responses to tasks were measured. In addition, participants completed a WM test before and after the stress or control task. Our results showed that the TSST elicited higher negative affect and greater C and sAA responses than the control task. No significant differences in WM were observed depending on the exposure to stress or the control task. Finally, participants who showed higher C and sAA responses to the stressor had lower WM performance. Our results indicate that medically treated older adults with T2D show clear, typical mood and physiological responses to an acute psychosocial stressor. Finally, the lack of acute psychosocial stress effects on WM suggests that it could be related to aging and not to this disease, at least when T2D is adequately treated. (shrink)

The importance of patients taking an active role in their healthcare is recognized internationally, to improve safety and effectiveness in practice. There is still, however, some ambiguity about the conceptualization of that patient role; it is referred to interchangeably in the literature as engagement, involvement, and participation. The aim of this discussion paper is to examine and conceptualize the concepts of patient engagement, involvement, and participation within healthcare, particularly nursing. The concepts were found to have semantic differences and similarities, although, (...) from a nursing perspective, they can be summoned to illustrate the establishment of a mutual partnership between a patient and a nurse. The individualization of such processes requires the joint effort of engagement, involvement, or participation, represented by interactive actions of both the patient (asking questions, telling/speaking up, knowledge acquisition, learning, and decision‐making) and the nurse (recognizing, responding, information sharing, teaching, and collaborating). Suggesting that the concepts can be used interchangeably comes with some caution, requiring that nurses embrace patients playing a role in their health and healthcare. Further research and practice development should focus on how patients and nurses receive and respond to each other to establish patient engagement, involvement, and participation. (shrink)

Ethical issues related the responsible conduct of research involve questions concerning the rights and obligations of investigators to propose, design, implement, and publish research. When a principal investigator transfers institutions during a grant cycle, financial and recognition issues need to be addressed to preserve all parties’ obligations and best interests in a mutually beneficial way. Although grants often transfer with the PI, sometimes they do not. Maintaining a grant at an institution after the PI leaves does not negate the grantee (...) institution’s obligation to recognize the PI’s original ideas, contributions, and potential rights to some forms of expression and compensation. Issues include maintaining a role for the PI in determining how to take credit for, share and publish results that involve his or her original ideas. Ascribing proper credit can become a thorny issue. This paper provides a framework for addressing situations and disagreements that may occur when a new PI continues the work after the original PI transfers. Included are suggestions for proactively developing institutional mechanisms that address such issues. Considerations include how to develop solutions that comply with the responsible conduct of research, equitably resolve claims regarding reporting of results, and avoid the possibility of plagiarism.
