This is the English (and extended version) of an interview originally published in Estonian in October 2018. In the interview, Simpson summarizes a particular way of defending the practice of no-platforming. The varying appeal of different defences of the practice in different socio-historical contexts (i.e. the UK/US versus a post-Soviet country such as Estonia) is discussed also.

The definitions of disease proffered by philosophers and medical actors typically require that a state of ill health be linked to some known bodily dysfunction before it is classified as a disease. I argue that such definitions of disease are not fully implementable in current medical discourse and practice. Adhering to the definitions would require that medical actors keep close track of the current state of knowledge on the causes and mechanisms of particular illnesses. Yet, unaddressed problems in medical terminology (...) can make this difficult to do. I show that unrecognized misuse of “heterogeneous,” “biomarker,” and other important health terms—which I call index terms—can misrepresent the current empirical evidence on illness pathophysiology, such that unvalidated illness constructs become mistaken for diseases. Thus, implementing common definitions of disease would require closing this “loophole” in medical discourse. I offer a simple rule that, if followed, could help do just that. (shrink)

The biopsychosocial model (BPSM) is increasingly influential in medical research and practice. Several philosophers and scholars of health have criticized the BPSM for lacking meaningful scientific content. This article extends those critiques by showing how the BPSM’s epistemic weaknesses have led to certain problems in medical discourse. Despite its lack of content, many researchers have mistaken the BPSM for a scientific model with explanatory power. This misapprehension has placed researchers in an implicit bind. There is an expectation that applications of (...) the BPSM will deliver insights about disease; yet the model offers no tools for producing valid (or probabilistically true) knowledge claims. I argue that many researchers have, unwittingly, responded to this predicament by developing certain patterns of specious argumentation I call “wayward BPSM discourse.” The arguments of wayward discourse share a common form: They appear to deliver insights about disease gleaned through applications of the BPSM; on closer inspection, however, we find that the putative conclusions presented are actually assertions resting on question-begging arguments, appeals to authority, and conceptual errors. Through several case studies of BPSM articles and literatures, this article describes wayward discourse and its effects. Wayward discourse has introduced into medicine forms of conceptual instability that threaten to undermine various lines of research. It has also created a potentially potent vector of medicalization. Fixing these problems will likely require reimposing conceptual rigor on BPSM discourse. (shrink)

Advertising by health care institutions has increased steadily in recent years. While direct-to-consumer prescription drug advertising is subject to unique oversight by the Federal Drug Administration, advertisements for health care services are regulated by the Federal Trade Commission and treated no differently from advertisements for consumer goods. In this article, we argue that decisions about pursuing health care services are distinguished by informational asymmetries, high stakes, and patient vulnerabilities, grounding fiduciary responsibilities on the part of health care providers and health (...) care institutions. Using examples, we illustrate how common advertising techniques may mislead patients and compromise fiduciary relationships, thereby posing ethical risks to patients, providers, health care institutions, and society. We conclude by proposing that these risks justify new standards for advertising when considered as part of the moral obligation of health care institutions and suggest that mechanisms currently in place to regulate advertising for prescription pharmaceuticals should be applied to advertising for health care services more broadly. By exploring these themes through a series of roundtable discussions with invited guests, we attempted to imbue the word "sustainability" with new meaning and reclaim it from corporate optimization rhetoric.Situated Solar Relations: Rethinking Scale for the Renewable Energy Agechallenged the sitelessness of large-scale initiatives by asking what might happen if situatedsolar relations were to become the primary protagonist of research, production, and outreach. Across three roundtables, an interactive workshop, and a showcase of some of our functional prototypes, the symposium addressed creative practices as a mode of future building. Roundtable discussions approached the question of how to build a just solar future through multiple angles: art, literature, and aesthetics; ludic interactions; and social, political, and economic transformations. Rather than taking for granted the principle of "scaling up" that requires sustainability to be replicable and expandable into universal and precise sets of standardized models, we argue instead for the necessity of having (non)scalable, situated, and evolving relationships with the sun. 3In other words, we turn to process rather than product, hope rather than despair, and careful research rather than extractive data practices.Relations of scale are at the heart of these dichotomies. Max Liboiron and Josh Lepawsky have recently argued that power lies in the way things appear to be "true, natural and good," which is determined and reproduced on a homogeneous scale by specific interest groups. 4Intervening against extractive, energy-intensive, and polluting practices therefore requires multiple contextualized and situated knowledges of the processes at play, which in turn requires an understanding of the scales within which energy and waste-defining relationships occur. 5They write:Problems arise when one situated way of knowing disavows its context, becomes dominant to the exclusion of other ways of knowing, or thinks it accounts for all forms of discards. The effect of such monolithic approaches to knowledge can be termed "scalar mismatch," where one instance is taken to be the whole phenomenon, or where one perspective is assumed to work in all cases. 6Scale here is about the specific processes and relationships that matter in a given context. Dominical categories are categories in which the notions of partial morphisms and their domains become explicit, with the latter being endomorphisms rather than subobjects of their sources. These categories form the basis for a novel abstract formulation of recursion theory, to which the present paper is devoted. The abstractness has of course its usual concomitant advantage of generality: it is interesting to see that many of the fundamental results of recursion theory remain valid in contexts far removed from their classic (...) manifestations. A principal reason for introducing this new formulation is to achieve an algebraization of the generalized incompleteness theorem, by providing a category-theoretic development of the concepts and tools of elementary recursion theory that are inherent in demonstrating the theorem.Dominical recursion theory avoids the commitment to sets and partial functions which is characteristic of other formulations, and thus allows for an intrinsic recursion theory within such structures as polyadic algebras. In crafting our paper on addressing the ethical challenges in HIV prevention research with people who inject drugs,1 we had hoped to stimulate further discussion and deliberation about the topic. We are pleased that three commentaries on our paper have begun this process.2 3 4 The commentaries rightly bring up important issues relating to community engagement and problems in translating research into practice in the fraught environments in which PWID face multiple risks. These risks include acquisition of HIV as well (...) as criminalisation, stigma and lack of access to needed healthcare, prevention and social services. We take this opportunity to respond to the excellent points raised by the commentators. All of the commentaries support our emphasis on robust community engagement with PWID and other stakeholders in designing and conducting HIV prevention research, but urge us to go farther. Wolfe highlights the difficulty of even engaging with community members in oppressive settings, where authorities severely restrict civil liberties of PWID so that even discussing issues related to drug use and enforcement may place individuals at risk. To overcome such limitations, he appropriately suggests interviewing confidentially those who have previously been detained in closed settings as part of the community engagement process. Similarly, Wolfe observes that critical issues can be overlooked with a narrow focus on study procedures if contextual factors before, during or after a study are ignored. For example, he cites the risk of overdose for study participants who have been abstinent during a study and subsequently resume injecting when the study concludes. Despite recent advances in HIV prevention and treatment, high HIV incidence persists among people who inject drugs. Difficult legal and political environments and lack of services for PWID likely contribute to high HIV incidence. Some advocates question whether any HIV prevention research is ethically justified in settings where healthcare system fails to provide basic services to PWID and where implementation of research findings is fraught with political barriers. Ethical challenges in research with PWID include concern about whether research evidence will (...) be translated into practice; concerns that research might exacerbate background risks; and ethical challenges regarding the standard of HIV prevention in research. While these questions arise in other research settings, for research with PWID, these questions are especially controversial. This paper analyses four ethical questions in determining whether research could be ethically acceptable: Can researchers ensure that research does not add to the burden of social harms and poor health experienced by PWID? Should research be conducted in settings where it is uncertain whether research findings will be translated into practice? When best practices in prevention and care are not locally available, what standard of care and prevention is ethically appropriate? Does the conduct of research in settings with oppressive policies constitute complicity? We outline specific criteria to address these four ethical challenges. We also urge researchers to join the call to action for policy change to provide proven safe and effective HIV prevention and harm reduction interventions for PWID around the world. (shrink)

Little research has explored the possible effects of government institutions in emerging economies on ethical reviews of multinational research. We conducted semi-structured, in-depth telephone interviews with 15 researchers, Research Ethics Committees personnel, and a government agency member involved in multinational HIV Prevention Trials Network research in emerging economies. Ministries of Health or other government agencies often play pivotal roles as facilitators or barriers in the research ethics approval process. Government agency RECs reviewing protocols may face particular challenges, as they can (...) lack resources, be poorly organized, have inconsistent review processes and limited expertise, and use differing definitions of national interests, including upholding national reputation and avoiding potential exploitation and stigma of the country's population. The MOH/governmental review body may be affected by power dynamics and politics in study reviews; may consider issues both related and unrelated to research ethics as understood elsewhere; and may prioritize particular diseases, treatments, or interventions over other topics/types of research. Poor communication and deeply-rooted tensions may exist between sponsor and host countries, impeding optimal interactions and reviews. Investigators must understand and plan for the potential effects of governmental agencies on multinational collaborative research, including preserving adequate time for agency review, and contacting these agencies beforehand to address issues that may arise. Better understanding of these issues can aid and advance appropriate global scientific collaboration. (shrink)

Animals navigate through three-dimensional environments, but we argue that the way they encode three-dimensional spatial information is shaped by how they use the vertical component of space. We agree with Jeffery et al. that the representation of three-dimensional space in vertebrates is probably bicoded (with separation of the plane of locomotion and its orthogonal axis), but we believe that their suggestion that the vertical axis is stored (that is, not containing distance or direction metrics usable for novel computations) is unlikely, (...) and as yet unsupported. We describe potential experimental protocols that could clarify these differences in opinion empirically. (shrink)

It thus illustrates the potential for clinicians to play a pivotal role in health promoting structural interventions based on harm-reduction goals and public health models. Popular media images of drug use as uniformly damaging, and abstinence as the only acceptable goal of treatment, threaten to distort clinical care away from a basis in evidence, which shows that some ways (...) of using drugs are far more harmful than others and that punitive approaches and insistence on total abstinence as the only goal of treatment often increases the harms of drug use rather than reducing drug use. Therefore the leadership and scientific authority of clinicians who understand the health impact of harm-reduction strategies is needed. Through a review of harm-reduction interventions in Canada, the United Kingdom, the United States, Australia, Switzerland, and the Netherlands, we identify three ways that clinicians have helped to achieve a paradigm shift from punitive approaches to harm-reduction principles in clinical care and in drug policy: through clinical research to provide data establishing the effectiveness and feasibility of harm-reduction approaches, by developing innovative clinical programmes that employ harm reduction, and thereby changing the standard of care to include routine use of these evidence-based approaches in their practices. The Marxist theory of crisis has fallen on hard times. Marx's ‘law of the tendency of the rate of profit to fall’, generally seen, at least in recent times, as the basis of the theory, is now widely rejected by economists who regard themselves as broadly working in his tradition. This state of affairs is in large part a consequence on the larger assault on mounted on the theoretical structure of Capital by self-proclaimed supporters of Piero Sraffa during the 1970s. (...) Analytical Marxism, during its brief efflorescence in the 1980s, took for granted the validity of the Sraffian critique. One of this school's more vulgar advocates published an ‘obituary’ of the TRPF which dismissed Marxists' attachment to the theory as a result of the influence of ‘“extra-scientific” considerations’ on them. The editors of a dictionary of Marxian economics expressed the hope not long ago that the resolution of the debate provoked by the Sraffian critique would be to ‘release Marxian ideas on crises, growth, imperialism, the social and economic evolution of forms of production, and so on, into the mainstream of economics’. Most of us have grown up with faces on television that look back at us, talk to us, even whenwe ignore them. They smile at us, and seem to address us personally. But they cannot seeor hear us, and we may or may not know who they are. Increasingly, in societies wherescreens are prevalent , our encounters with fellow humanbeings are mediated in ways such as this. Has the ubiquitous intervention of screens in ourlives thus made it harder to understand (...) and communicate directly with one another? Or,have screens extended our capacity to empathise and ‘socialise’, bringing us face-to-facewith people and points of view that we otherwise would never have encountered? In thisessay, I examine the idea that cinematic perception enables us to see the social world froma radically different perspective, and that an experience of this perspective may in itself beethical. I focus on the use of ‘direct address’, and discuss two documentaries by Errol Morriswhere the technique of direct address is used in ways that complicate ideas of mediationand empathy: Mr. Death: The Rise and Fall of Fred A. Leuchter, Jr. and The Fog of War:Eleven Lessons from the Life of Robert S. McNamara . I also draw on the philosophyof Emmanuel Levinas, particularly his work on ethics and the face, to analyse the effects ofMorris’s techniques. The essay highlights the importance of responsibility in human communication, and maintains that by reflecting on the viewing situation we are betterpositioned to empathise with Morris’s controversial subjects. Despite the example's influence among philosophers, the argument behind it is riddled with difficulties. Dropping the example allows us to re-version Nozick's argument in a way that makes it far more forceful - and less dependent on people's often divergent intutions (...) about the experience machine. Among other interesting claims made in Robert Reich's 2007 treatise, Supercapitalism, it is asserted in various ways that proponents of corporate social responsibility (CSR) or what I would call 'business ethics' are engaged in relatively unproductive exercises. Their resources would be better used if they undertook the hard work of engagement in democratic political processes leading to legislation that would force corporations to pursue the public interest as well as their own. In this article, I summarize some of Reich's central (...) theses and arguments, show that they are fatally flawed and explain why the institution of morality is essential for business, law and democracy.
