PurposeThe unique constraints to everyday life brought about by the COVID-19 pandemic have been suggested to negatively impact those with pre-existing mental health issues such as eating disorders. While individuals with eating disorders or disordered eating behaviors likely represent a vulnerable group to the COVID-19 pandemic, the impact of the pandemic is yet to be fully established.MethodsWe systematically examined the impact of the COVID-19 pandemic on eating disorders and disordered eating behaviors. We searched electronic databases MEDLINE, PsycINFO, CINAHL, and EMBASE (...) for literature published until October 2021. Eligible studies were required to report on individuals with or without a diagnosed eating disorder or disordered eating behaviors who were exposed to the COVID-19 pandemic.FindingsSeventy-two studies met eligibility criteria with the majority reporting an increase in eating disorder or disordered eating behaviors associated with the COVID-19 pandemic. Specifically, it appears children and adolescents and individuals with a diagnosed eating disorder may present vulnerable groups to the impacts of the COVID-19 pandemic.DiscussionThis mixed systematic review provides a timely insight into COVID-19 eating disorder literature and will assist in understanding possible future long-term impacts of the pandemic on eating disorder behaviors. It appears that the role of stress in the development and maintenance of eating disorders may have been intensified to cope with the uncertainty of the COVID-19 pandemic. This is an extremely readable and interesting contribution. The author takes the reader through the Patient Self-Determination Act in some depth, but doesn't make any attempt to look at its provisions in real detail. This cannot be taken as an omission, as the purpose of the book is to explore the principles underpinning the legislation and to explore its ethical content. “Frontier” is included in the general category of “limit” (limes: a road bordering a field). But what is at the origin of limit, frontier? An authority, a power that can exercise “the social function of ritual and social significance of the line, the limit whose ritual legitimizes passage, transgression” (Bourdieu, 1982, p. 121). The limit, a traced line, sets up an order that is not only spatial but temporal, since it not only separates a “this side” from a “that side” (...) but also a “before” and an “after”. This dual nature of the limit is at work in the myth of the founding of Rome. Any limit, any frontier, is intentional: it proceeds from a will; it is never arbitrary. Suppose q is some proposition, and let P(q) = v0 (1) be the proposition that the probability of q is v0.1 How can one know that (1) is true? One cannot know it for sure, for all that may be asserted is a further probabilistic statement like P(P(q) = v0) = v1, (2) which states that the probability that (1) is true is v1. But the claim (2) is also subject to some further statement of an even higher probability: P(P(P(q) (...) = v0) = v1) = v2, (3) and so on. Thus, an infinite regress emerges of probabilities of probabilities, and the question arises as to whether this regress is vicious or harmless. Radical probabilists would like to claim that it is harmless, but Nicholas Rescher (2010), in his scholarly and very stimulating Infinite Regress: The Theory and History of Varieties of Change, argues that it is vicious. He believes that an infinite hierarchy of probabilities makes it impossible to know anything about the probability of the original proposition q: unless some claims are going to be categorically validated and not just adjudged probabilistically, the radically probabilistic epistemology envisioned here is going to be beyond the prospect of implementation. . . . If you can indeed be certain of nothing, then how can you be sure of your probability assessments. If all you ever have is a nonterminatingly regressive claim of the format . . . the probability is .9 that (the probability is .9 that (the probability of q is .9)) then in the face of such a regress, you would know effectively nothing about the condition of q. After all, without a categorically established factual basis of some sort, there is no way of assessing probabilities. But if these requisites themselves are never categorical but only probabilistic, then we are propelled into a vitiating regress of presuppositions. (shrink)

Today's industrial society is having an encounter with ecology: in April, 1976 the French government presented the National Assembly with documents on the dumping and burning of waste in the sea, as well as on the protection of nature. Electoral campaigns, discussions and demonstrations are centered about the theme of pollution and environment. In the last century the accumulation of waste had already become a problem : “ One of the most important duties of industry is to find a useful (...) employment for waste,” wrote P. L. Simmons in 1875. The advice was heeded if we may believe what J. Gottmann wrote on the matter nearly a century later: “ If our era is to be defined by its most important raw material and one that is truly proper to it, as is done for the Bronze Age or the Iron Age, we may no longer speak of a Steel or Petroleum Age, nor of an Atomic Age (which may come in the future) but of the Garbage Age.” The extraordinary demographic expansion of humanity that has gone from a population of five million in the Paleolithic Age to four billion today gives rise to a monstrous abundance of refuse resulting as much from the natural mechanisms of living as from industrial activity (factory waste) or from the present attitude haunted by the idea of obsolescence (the throw-away society of Alvin Toffler). The outline of a universe submerged in waste matter appears, similar to that evoked by Italo Calvino in Città Invisibili apropos of the town of Leonia: “The refuse of Leonia would gradually invade the world if the interminable garbage heaps were not pressed upon, beyond the last crest, by the refuse of other towns that also push mountains of refuse far away from themselves. Perhaps the entire world beyond the borders of Leonia is covered with craters of trash, each with a continually erupting metropolis in its center. The borders between foreign or enemy towns are thus contaminated bastions in which the detritus of the one and the other serves as mutual support, menacing and mixing together.” The world of Metamorphosis is approaching in which man, transformed into an insect, covered with dust and remains of food, gnaws on the cores and parings of half-rotten vegetables. Science fiction invents a remedy to fit the problem : a planet specialized in storing all the garbage unloaded by an entire galaxy. (shrink)

ObjectivesTo describe reporting of informed consent in pragmatic trials, justifications for waivers of consent and reporting of alternative approaches to standard written consent. To identify factors associated with (1) not reporting and (2) not obtaining consent.MethodsSurvey of primary trial reports, published 2014–2019, identified using an electronic search filter for pragmatic trials implemented in MEDLINE, and registered in ClinicalTrials.gov.ResultsAmong 1988 trials, 132 (6.6%) did not include a statement about participant consent, 1691 (85.0%) reported consent had been obtained, 139 (7.0%) reported a (...) waiver and 26 (1.3%) reported consent for one aspect (eg, data collection) but a waiver for another (eg, intervention). Of the 165 trials reporting a waiver, 76 (46.1%) provided a justification. Few (53, 2.9%) explicitly reported use of alternative approaches to consent. In multivariable logistic regression analyses, lower journal impact factor (p=0.001) and cluster randomisation (p<0.0001) were significantly associated with not reporting on consent, while trial recency, cluster randomisation, higher-income country settings, health services research and explicit labelling as pragmatic were significantly associated with not obtaining consent (all p<0.0001).DiscussionNot obtaining consent seems to be increasing and is associated with the use of cluster randomisation and pragmatic aims, but neither cluster randomisation nor pragmatism are currently accepted justifications for waivers of consent. Rather than considering either standard written informed consent or waivers of consent, researchers and research ethics committees could consider alternative consent approaches that may facilitate the conduct of pragmatic trials while preserving patient autonomy and the public’s trust in research. Although Antoine Arnauld has been best known to contemporary (...) philosophers as the author of the fourth set of objections to Descartes’ Meditations, interest in Arnauld’s own philosophical thought appears to be on the rise within the Anglo-American philosophical community, as indicated by the publication, within the last eight years, of two new translations of his Des vrayes et des fausses idées (a philosophical polemic against Malebranche) and a new translation of the Port-Royal Logic that Arnauld co-authored with Pierre Nicole. This new collection, from an international group of contributors, is thus well-timed. The two main themes that emerge from the collection as a whole serve to indicate the potential importance of Arnauld scholarship to our understanding of seventeenth century philosophy.The first theme that unites most of the papers is that of the diversity of views that existed within the constellation of philosophers who identified in one way or another with the doctrines of René Descartes. Arnauld was, of course, one such Cartesian. Nevertheless, many of the papers take the differences between Descartes and Arnauld as their starting point. Peter Schouls sets up a general contrast between the modernity of Descartes’ thought, as founded on the validity of subjective consciousness, and the tradition-grounded character of Arnauld’s philosophy. Jill Vance Buroker focuses productively on Arnauld’s and Descartes’ opposing views on the nature of assertion and judgment. Alan Nelson uses Arnauld’s challenge to Descartes’ account of the material falsity of ideas as the basis for a controversial but fascinating reassessment of a range of crucial issues in Descartes scholarship: clarity and distinctness, objective reality, and representation, as well as material falsity. Jean-Luc Solère’s contribution (described further below) documents the fact that significant philosophical debates existed, not [End Page 367] just within Cartesianism, but even within the still narrower Jansenist community that agreed in taking Augustine and Descartes as their philosophical models. Bullying often results in negative coping in victims, including an increased consumption of alcohol. Recently, however, an increase in alcohol use has also been reported among perpetrators of bullying. The factors triggering this pattern are still unclear. We investigated the role of empathy in the interaction between bullying and alcohol use in an adolescent sample (IMAGEN) at age 13.97 (±0.53) years (baseline (BL), N = 2165, 50.9% female) and age 16.51 (±0.61) years (follow-up 1 (FU1), N = 1185, 54.9% female). (...) General empathic distress served as a significant moderator of alcohol use in perpetrators (F9, 493 = 17.978, p < 0.01), which was specific for males and FU1. Male perpetrators, who are generally less sensitive to distress, might thus be more vulnerable to alcohol abuse. (shrink)
