Background: Transgender health care is a subject of much debate among clinicians, political commentators, and policy-makers. While the World Professional Association of Transgender Health (WPATH) Standards of Care (SOC) establish clinical standards, these standards contain implied ethics but lack explicit focused discussion of ethical considerations in providing care. An ethics chapter in the SOC would enhance clinical guidelines. Aims: We aim to provide a valuable guide for healthcare professionals, and anyone interested in the ethical aspects of clinical support for gender (...) diverse and transgender people of all ages. Recognizing that the WPATH is a global association, we address broad challenges. We offer a reflection on general ethical principles, providing conceptual tools for healthcare providers, patients, and families to navigate the specific challenges they might encounter in transgender health care, in line with WPATH’s worldwide mission and scope. Method: This article employs a descriptive analysis, and our framework of reference is the four principles of biomedical ethics: respect for autonomy, beneficence, nonmaleficence, and justice. Results: The article presents a discussion on the four ethical principles as applied to transgender health care. We address issues such as respect for patient autonomy in decision-making, the role of beneficence and nonmaleficence in clinical interventions, and the importance of justice in equitable treatment and access to care. Some of the ethical concerns we address in this article pertain to the current sociopolitical climate, where there has been increasing legal interference, internationally, for transgender and nonbinary people, particularly youth, seeking medical care. Discussion: We highlight the interplay between ethical principles and clinical practice, underscoring the need for ethical guidance in addressing the diverse challenges faced by healthcare providers and patients in transgender health care. We advocate for continuous refinement of ethical thinking to ensure that transgender health care is not only medically effective but also ethically sound. (shrink)

Two options are ‘incommensurate’ when neither is better than the other, but they are not equally good. Typically, we will say that one option is better in some ways, and the other in others, but neither is better ‘all things considered’. It is tempting to think that incommensurability is vagueness—that it is (perhaps) indeterminate which is better—but this ‘vagueness view’ of incommensurability has not proven popular. I set out the vagueness view and its implications in more detail, and argue that (...) it can explain most of the puzzling features of incommensurability. This argument proceeds without appeal to John Broome’s ‘collapsing principle’. (shrink)

John Broome has argued that value incommensurability is vagueness, by appeal to a controversial about comparative indeterminacy. I offer a new counterexample to the collapsing principle. That principle allows us to derive an outright contradiction from the claim that some object is a borderline case of some predicate. But if there are no borderline cases, then the principle is empty. The collapsing principle is either false or empty.

Can Bradley's Regress be solved by positing relational tropes as truth-makers? No, no more than Russell's paradox can be solved by positing Fregean extensions. To call a trope relational is to pack into its essence the relating function it is supposed to perform but without explaining what Bradley's Regress calls into question, viz. the capacity of relations to relate. This problem has been masked from view by the (questionable) assumption that the only genuine ontological problems that can be intelligibly raised (...) are those that can be answered by providing a schedule of truthmakers. I argue that the Ruth Chang’s Chaining Argument for her parity view of value incomparability trades illicitly on the vagueness of the predicate ‘is comparable with’. Chang is alert to this danger and argues that the predicate is not vague, but this defense does not succeed. The Chaining Argument also faces a dilemma. The predicate is either vague or precise. If it is vague, then the argument is most plausibly a sorites. If it is precise, then the argument is either (...) question begging or dialectically ineffective. I argue that no chaining-type argument can succeed. (shrink)

The basic relations and functions that mathematicians use to identify mathematical objects fail to settle whether mathematical objects of one kind are identical to or distinct from objects of an apparently different kind, and what, if any, intrinsic properties mathematical objects possess. According to one influential interpretation of mathematical discourse, this is because the objects under study are themselves incomplete; they are positions or akin to positions in patterns or structures. Two versions of this idea are examined. It is argued (...) that the evidence adduced in favor of the incompleteness of mathematical objects underdetermines whether it is the objects themselves or our knowledge of them that is incomplete. Also, holding that mathematical objects are incomplete conflicts with the practice of mathematics. The objection that structuralism is committed to the identity of indiscernibles is evaluated and it is also argued that the identification of objects with positions is metaphysically suspect. Whether a predicate is a referential expression depends upon what reference is conceived to be. Even if it is granted that reference is a relation between words and worldly items, the referents of expressions being the items to which they are so related, this still leaves considerable scope for disagreement about whether predicates refer. One of Frege's great contributions to the philosophy of language was to introduce an especially liberal conception of reference relative to which it is unproblematic to suppose (...) that predicates are referring expressions. According to this liberal conception, each significant expression in a language has its own distinctive semantic role or power, a power to effect the truth-value of the sentences in which it occurs. Fine-tuning in physics and cosmology is often used as evidence that a theory is incomplete. For example, the parameters of the standard model of particle physics are “unnaturally” small, which has driven much of the search for physics beyond the standard model. Of particular interest is the fine-tuning of the universe for life, which suggests that our universe’s ability to create physical life forms is improbable and in need of explanation, perhaps by a multiverse. This claim has been challenged on (...) the grounds that the relevant probability measure cannot be justified because it cannot be normalized, and so small probabilities cannot be inferred. We show how fine-tuning can be formulated within the context of Bayesian theory testing in the physical sciences. The normalizability problem is seen to be a general problem for testing any theory with free parameters, and not a unique problem for fine-tuning. Physical theories in fact avoid such problems in one of two ways. Dimensional parameters are bounded by the Planck scale, avoiding troublesome infinities, and we are not compelled to assume that dimensionless parameters are distributed uniformly, which avoids non-normalizability. The assumption that psychological states and processes are computational in character pervades much of cognitive science, what many call the computational theory of mind. In addition to occupying a central place in cognitive science, the computational theory of mind has also had a second life supporting “individualism”, the view that psychological states should be taxonomized so as to supervene only on the intrinsic, physical properties of individuals. One response to individualism has been to raise the prospect of “wide computational systems”, (...) in which some computational units are instantiated outside the individual. “Wide computationalism” attempts to sever the link between individualism and computational psychology by enlarging the concept of computation. However, in spite of its potential interest to cognitive science, wide computationalism has received little attention in philosophy of mind and cognitive science. This paper aims to revisit the prospect of wide computationalism. It is argued that by appropriating a mechanistic conception of computation wide computationalism can overcome several issues that plague initial formulations. The aim is to show that cognitive science has overlooked an important and viable option in computational psychology. The paper marshals empirical support and responds to possible objections. Discussions of extended cognition have increasingly engaged with the empirical and methodological practices of cognitive science and psychology. One topic that has received increased attention from those interested in the extended mind is music cognition. A number of authors have argued that music not only shapes emotional and cognitive processes, but also that it extends those processes beyond the bodily envelope. The aim of this paper is to evaluate the case for extended music cognition. Two accounts are examined in detail: (...) Joel Krueger’s “musically extended emotional mind” and Tom Cochrane’s “expression and extended cognition.” Each account is evaluated using three “anti-extension” arguments. I argue that Krueger and Cochrane’s accounts offer important steps toward extended music cognition, but that each account remains underdeveloped in various ways. To supplement existing approaches, I propose a complementary extended computational approach to music cognition. The claim is that music cognition forms part of an extended system in virtue of involving computational processes that range across environmental and in-the-head elements. The paper concludes by showing how the ECMC deals with each of the three anti-extension challenges and responds to objections. (shrink)

Sergio Tenenbaum and Diana Raffman contend that ‘vague projects’ motivate radical revisions to orthodox, utility-maximising rational choice theory. Their argument cannot succeed if such projects merely ground instances of the paradox of the sorites, or heap. Tenenbaum and Raffman are not blind to this, and argue that Warren Quinn’s Puzzle of the Self-Torturer does not rest on the sorites. I argue that their argument both fails to generalise to most vague projects, and is ineffective in the case of the Self-Torturer (...) itself. (shrink)

In the UK, healthcare workers are again considering whether to strike, and the moral status of strike action is being publicly debated. Mpho Selemogo argued that we can think productively about the ethical status of healthcare strikes by using the ethical framework often applied to armed conflict (2014). On this view, strikes need to be just, proportionate, likely to succeed, a last resort, pursued by a legitimate organisation and publicly communicated. In this article, I argue for a different approach to (...) the just war comparison. Selemogo adopts a traditional, collectivist conception of just war reasoning but this is not the only view. So-called ‘individualist’ approaches to the morality of war can also be applied to strike action. Taking an individualist perspective complicates the traditional picture of a dispute arising between three well-defined groups of healthcare workers, employers and the innocent subjects of collateral damage: patients and the public. We arrive instead at a more complicated moral picture: some people might be more morally liable to be harmed than others during a strike, or can justly bear increased risks, and some are more obliged to strike than others. I describe this shift of framework before critically examining some of the traditionaljus ad bellumconditions as applied to strikes. (shrink)

Trials are, among other things, centrally concerned with determining truth: whether a criminal defendant has in fact culpably committed the act of which they are accused, or whether a civil defendant is in fact responsible for the damages alleged by the plaintiff. But are trials truth-conducive? Assessing the value of trials as truth-seeking endeavors requires that we consider a host of underlying social epistemological questions. This paper provides a new solution to the concept horse paradox. Frege argued no name co-refers with a predicate because no name can be inter-substituted with a predicate. This led Frege to embrace the paradox of the concept horse. But Frege got it wrong because predicates are impurely referring expressions and we shouldn’t expect impurely referring expressions to be intersubstitutable even if they co-refer, because the contexts in which they occur are sensitive to the extra information they carry about their (...) referents. I argue that the separation of conjoined twins in infancy or early childhood is unethical. Cases may be divided into three types: both twins suffer from lethal abnormalities, only one twin has a lethal abnormality, or neither twin does. In the first kind of case, there is no reason to separate, since both twins will die regardless of treatment. In the third kind of case, I argue that separation at an early age is unethical because the twins are likely to (...) achieve an irreplaceably good quality of life—the goods of conjoinment—that separation takes away. Evaluation of this possibility requires maturation past early childhood. Regarding the second type, I point out that with conceivable but unrecorded exceptions, these cases will consistently involve sacrifice separation. I present an argument that sacrifice separation is unethical, but in some cases a moral dilemma may exist in which separation and refraining from separation are both unethical. Perhaps in such cases a decision can be made on non-moral grounds; however, the possibility of such a decision serves not to mitigate but to underscore the fact that the separation is unethical. My conclusion, which applies to all three types of cases, is that it is unethical to separate conjoined twins before their developing personalities give some reliable indication as to whether they desire separation and whether they will achieve those goods of conjoinment. Community gardens are of increasing interest to scholars, policymakers, and community organizations but there has been little systematic study of community garden management at a broad scale. This study complements case study research by revealing shared experiences of community garden management across different contexts. In partnership with the American Community Gardening Association, we developed an online questionnaire. Results from 445 community garden organizations across the US and Canada reveal common themes as well as differences that are particularly significant across different (...) organizational sizes. The findings suggest that organizers see multiple benefits, and respondents confirmed recent expansion of gardening efforts. Analysis then focuses on challenges, which are closely related to garden management. We address garden losses as well as challenges to routine operation. Key challenges included funding, participation, land, and materials. We developed a typology based on organization size, to reveal distinctions between small organizations (serving 1 garden), medium-sized organizations (2–3 gardens), large organizations (4–30 gardens) and very large organizations (31 or more gardens). These categories shed light on different needs for funding, land, material, and participation. Together, this analysis suggests that community gardens can be linked through the work it takes to sustain them rather than specific causes or outcomes. Community gardens can be better integrated into local food systems through analysis of how people involved with this work navigate these shared processes. (shrink)

Sustainability is commonly recognized as an important goal, but there is little agreement on what sustainability is, or what it requires. This paper looks at some common approaches to sustainability, and while acknowledging the ways in which they are useful, points out an important lacuna: that for something to be sustainable, people must be willing to work to sustain it. The paper presents a framework for thinking about and assessing sustainability which highlights people working to sustain. It also briefly discusses (...) Integrated Water Resource Management and the example of the California Water Plan to explore what such a perspective brings that is overlooked in other approaches, and how this approach might be pursued. One of the most significant obstacles to inquiry and deliberation is citizenship education. There are few mechanisms for the development of citizens’ democratic character within most societies, and greater opportunities need to be made to ensure our democracies are epistemically justifiable. The character and quality of citizens’ interactions are a crucial aspect for any democracy; their engagement make a significant difference between a deliberative society and an electoral oligarchy. I contend that through demarchic procedures, citizens are subject to collective learning (...) process in virtue of being part of communal decision-making and in so doing can develop their capacities for deliberation with practice over time. Demarchic bodies can be utilised as communities of inquiry. By viewing democracy as both a learning process and a decision-making mechanism, the quality of deliberation and participation can improve over time as well. (shrink)

This article explores the development of therapeutic psychological techniques for emotional management as exemplified by Albert Ellis’s development of rational therapy (RT) in the 1950s and 1960s. A precursor to contemporary Cognitive Behavioral Therapy (CBT), rational therapy conceptualized emotion as manageable through scientific self-narration. The article examines how Ellis’s immersion in media techniques and forms accessible to a general audience shaped his focus on a new language for clinical practice inspired by behaviorist principles, and how much this ‘clarified’ language exemplifies (...) the application of ‘high modernist ideology’ to the treatment of the postwar American psychological subject. It has been claimed that we need singular self-knowledge to function properly as rational agents. I argue that this is not strictly true: agents in certain relations could dispense with singular self-knowledge and instead rely on plural self-knowledge. In defending the possibility of this kind of ‘selfless agent’, I thereby defend the possibility of a certain kind of ‘seamless’ collective agency; agency in a group of agents who have no singular self-knowledge, who do not know which member of the group (...) they are. I discuss four specific functions for which singular self-knowledge has been thought indispensable: distinguishing intentional from unintentional actions, connecting non-indexical knowledge with action, reflecting on our own reasoning, and identifying which ultimate practical reasons we have. I argue in each case that by establishing certain relations between agents – relations I label ‘motor vulnerability’, ‘cognitive vulnerability’, ‘evidential unity’ and ‘moral unity’ – we would allow those agents to do everything a rational agent needs to do while relying only on plural, rather than singular, self-knowledge. Finally, I consider the objection that any agents who met the conditions I lay out for selfless agency would thereby cease to qualify as distinct agents, merging into a single agent without agential parts. Against this objection, I argue that we should recognise the possibility of simultaneous agency in whole and parts, and not regard either as disqualifying the other. (shrink)

According to one creation myth, analytic philosophy emerged in Cambridge when Moore and Russell abandoned idealism in favour of naive realism: every word stood for something; it was only after “the Fall,” Russell's discovery of his theory of descriptions, that they realized some complex phrases (“the present King of France”) didn't stand for anything. It has become a commonplace of recent scholarship to object that even before the Fall, Russell acknowledged that such phrases may fail to denote. But we need (...) to go further: even before the Fall, Russell had taken an altogether more discerning approach to the ontology of logic and relations than is usually recognized. (shrink)

In Conservatism and Pragmatism, Seth Vannatta posits and explores several major conceptual and practical affinities between classical pragmatism and conservatism. Characterizing both as essentially methods rather than ideologies, he argues that the two ought to be understood as mutually supportive and corrective, and that they conjointly supply an especially robust set of intellectual resources relevant to contemporary moral, political, and legal concerns. This essay critically examines Vannatta’s marriage of conservatism and pragmatism in the realm of legal theory. It argues that (...) while Vannatta’s work provides a rigorous pragmatist alternative to the familiar legal formalism adopted by many American conservatives, its foundations may be narrower than Vannatta appreciates and the resulting theory may have little necessary connection to the dispositional, methodological conservatism upon which it is purportedly based. Rationale Treatment guidelines recommend a more conservative surgical approach than mastectomy for early stage breast cancer and a stronger emphasis on adjuvant therapy. Registry data at South Australian teaching hospitals have been used to monitor survivals and treatment in relation to these guidelines.Aims and objectives To use registry data to: (1) investigate trends in survival and treatment; and (2) compare treatment with guidelines.Methods Registry data from three teaching hospitals were used to analyse trends in primary courses of treatment of breast (...) cancers during 1977–2003 (n = 4671), using univariate analyses and multiple logistic regression. Disease-specific survivals were analysed using Kaplan–Meier product limit estimates and multivariable Cox proportional hazards regression.Results The 5-year survival was 79.9%, but with a secular increase, reaching 83.6% in 1997–2003. The relative risk of death (95% confidence limits) was 0.74 (0.62, 0.88) for 1997–2003, compared with previous diagnoses, after adjusting for tumour node metastasis stage, grade, age and place of residence. Treatment changes included an increase in conservative surgery (as opposed to mastectomy) from 51.7% in 1977–1990 to 76.8% in 1997–2003 for stage I (P < 0.001) and from 31.1% to 52.2% across these periods for stage II (P < 0.001). Adjuvant radiotherapy also became more common (P < 0.001), with 20.6% of patients receiving this treatment in 1977–1990 compared with 60.7% in 1997–2003. Radiotherapy generally was more prevalent when conservative surgery was provided, although also relatively common in mastectomy patients when tumour diameters exceeded 50 mm or when there were four or more involved nodes. The proportion of patients receiving chemotherapy increased (P < 0.001), from 19.6% in 1977–1990 to 36.9% in 1997–2003, and the proportion having hormone therapy also increased (P < 0.001), from 34.3% to 59.4% between these periods.Conclusions Survivals appear to be increasing and treatment trends are broadly consistent with guideline directions, and the earlier research on which these recommendations were based.
