Moral distress is a major problem for nurses, other clinicians, and the health system itself. But if properly understood and responded to, it is also a promising guide for healthcare improvement. When individuals experience moral distress or burnout, their reports must be seen as crucial data requiring careful attention to the individuals and to the organization. Distress and burnout will often point to important opportunities for system improvements, which may in turn reduce the experience of distress. For this potential virtuous (...) cycle to happen, individuals must be able to articulate their concerns without fear of retribution, and organizational leaders must be able to listen in an undefensive, improvementoriented manner. While the prevalence of opioid use disorder among pregnant women has multiplied in the United States in the last decade, buprenorphine treatment for peripartum women with OUD has been administered to reduce risks of repeated cycles of craving and withdrawal. However, the maternal behavior and bonding in mothers with OUD may be altered as the underlying maternal behavior neurocircuit is opioid sensitive. In the regulation of rodent maternal behaviors such as licking and grooming, a series of opioid-sensitive brain regions are (...) functionally connected, including the ventral pallidum. In humans, these brain regions, interact with the supplementary motor area to regulate maternal behaviors and are functionally dysregulated by opioids. It is unclear how these brain regions respond to the emotions of their child for mothers receiving BT. In this functional magnetic resonance imaging pilot study in 22 mothers within the first postpartum year, including six mothers receiving BT and 16 non-OUD mothers as a comparison group, we devised a child face mirroring task in fMRI settings to assess maternal responses to pictures of facial expressions of own child and an unknown child in an empathic mirroring condition and a non-mirroring observation condition. In each condition, faces of neutral, ambiguous, distressed, and joyful expressions of each child were repeatedly displayed in a random order. The response of SMA during empathic mirroring vs. non-mirroring of own child was reduced among BT/OUD vs. CG. Within MBN, the left VP, critical for parental sensitivity, had a similar deficit. This study outlines potential mechanisms for investigating the risks of deficits in the neural responses to actual maternal sensitivity and parenting behavior in mothers with OUD, and potential targets for interventions that reduce stress and augment maternal behavior and child outcome. Although leaders in the field of ethics have for many years pointed to the crucial role that organizations play in shaping healthcare ethics, organizational ethics remains a relatively undeveloped area of ethics activity. Clinical ethics committees are an important source of potential expertise, but new skills will be required. Clinical ethics committees seeking to extend their purview to organizational issues will have to respond to three challenges—how to gain sanction and support for addressing controversial and sensitive issues, how to develop (...) an acceptable process, and how to make a difference on the ground. For Husserl excess is a part of any phenomenon. For Heidegger the horizon of the phenomenon is also excessive. Levinas and Marion ask us to think about what exceeds the horizon. I focus on Marion’s fifth kind of saturated (transcendent) phenomenon, revelation. How are we to understand it? Marion says he argues only for the possibility of revelation, but only Jesus could be the revelation for which he argues. The excess of the divine cannot remain merely a metaphysical beyond. It (...) must reveal itself in the world as a possible phenomenon, as Flesh, for there is no excess without flesh, and no flesh without being. Excess is either enfleshed or thingly excess. For the Christian this being-together of flesh and word in God means the same being-together in us: Christian life is fully incarnate life, life as enspirited flesh rather than as dead body. Thus the being-together of flesh and word is “in the accusative”. Christian life is, prior to action, a life of submission. That, however, explicitly puts Christian belief and practice at odds with any unrecuperated, merely metaphysical metaphysics which undercuts not only the Christian dogma of Christ’s incarnation and incarnate resurrection, but also the Christian message that the divine life is found only in the life that bends its knee and seeks to bring about justice: dikaiosunae. In this article, I argue that there is no discrepancy between Kant's Doctrine of Right (The Metaphysics of Morals) (1797), which legally permits lies that do not deprive someone of their rights or property, and his On a Supposed Right to Lie from Love of Humanity (1797), which argues that it would be a crime to lie to a murderer about the whereabouts of the innocent person he is pursuing.

Although Martin Heidegger is undeniably one of the most influential philosophers of the twentieth century, among the philosophers who study his work we find considerable disagreement over what might seem to be basic issues: why is Heidegger important? What did his work do? This volume is an explicit response to these differences, and is unique in bringing together representatives of many different approaches to Heidegger's philosophy. My criticisms of parapsychology are neither based on its subject matter per se, nor simply on a charge of sloppy research, but rather on the whole pattern of theory and research in this domain. The lack of a positive definition of psi, the use of ad hoc principles such as psi-missing and the experimenter psi effect to account for failures to confirm hypotheses, and the failure to produce a single phenomenon that can be replicated by neutral investigators are among the (...) major problems that keep parapsychology outside regular science. In 2012, a U.S. Institute of Medicine report called for a different approach to health care: “Left unchanged, health care will continue to underperform; cause unnecessary harm; and strain national, state, and family budgets.” The answer, they suggested, would be a “continuously learning” health system. Ethicists and researchers urged the creation of “learning health organizations” that would integrate knowledge from patient‐care data to continuously improve the quality of care. Our experience with an ongoing research study on atrial fibrillation—a trial known (...) as IMPACT‐AFib—gave us some insight into one of the challenges that will have to be dealt with in creating these organizations. Although the proposed educational intervention study placed no restrictions on what providers and health plans could do, the oversight team argued that the ethical principle of beneficence did not allow the researchers to be “bystanders” in relation to a control group receiving suboptimal care. In response, the researchers designed a “workaround” that allowed the project to go forward. We believe the experience suggests that what we call “bystander ethics” will create challenges for the kinds of quality improvement research that LHOs are designed to do. (shrink)

Recently, American Psychologist published a review of the evidence for parapsychology that supported the general claims of psi (the umbrella term often used for anomalous or paranormal phenomena). We present an opposing perspective and a broad-based critique of the entire parapsychology enterprise. Our position is straightforward. Claims made by parapsychologists cannot be true. The effects reported can have no ontological status; the data have no existential value. We examine a variety of reasons for this conclusion based on well-understood scientific principles. (...) In the classic English adynaton, “pigs cannot fly.” Hence, data that suggest that they can are necessarily flawed and result from weak methodology or improper data analyses or are Type I errors. So it must be with psi effects. What we find particularly intriguing is that, despite the existential impossibility of psi phenomena and the nearly 150 years of efforts during which there has been, literally, no progress, there are still scientists who continue to embrace the pursuit. Whether we're in the boardroom or the classroom, we spend far too much time and energy looking for the right answer. But the truth is that questions are just as important as answers, often more so. If you ask the wrong question, for instance, you're guaranteed to get the wrong answer. A good question, on the other hand, inspires a good answer and, in the process, invites deeper understanding and more meaningful connections between people.
