Most donated organs in the United States come from brain dead donors, while a small percentage come from patients who die in “controlled,” or expected, circumstances, typically after the family or surrogate makes a decision to withdraw life support. The number of organs available for transplant could be substantially if donations were permitted in “uncontrolled” circumstances–that is, from people who die unexpectedly, often outside the hospital. According to projections from the Institute of Medicine, establishing programs permitting “uncontrolled donation after circulatory (...) determination of death,” or uDCDD, throughout the United States has the potential to provide 22,000 more donation opportunities annually. In contrast, U.S. controlled donation after circulatory determination of death, or cDCDD, cases have increased progressively over the past decade from 87 to 848 donors, but currently account for only 10.6 percent of all deceased donors. Following the IOM recommendations, several projects exploring the feasibility of uDCDD were funded by the federal government, including a grant from the Health Resources and Services Administration that supported a pilot project in New York City in which the authors of this article participated.A key feature of our protocol, and indeed of many uDCDD protocols, is the initiation of preservation methods such as chest compressions and extracorporeal membrane oxygenation shortly after death in order to perfuse and preserve the donor's organs. Critics of uDCDD argue that the means of determining death deviates from generally ascribed principles. They assert that reinstituting circulation in order to preserve organs has the effect of “undoing” the prior determination of death. The result is that cDCDD is widely accepted and practiced routinely even though it only marginally increases the number of organs available for transplantation, and uDCDD is widely considered unacceptable despite being ethically embraced and proven to significantly increase organ donation opportunities in other countries. This article explores the evolution of this counterintuitive state of affairs and calls for a policy that, in line with the IOM report, allows for both cDCDD and uDCDD protocols. (shrink)

This paper argues that professionals who make boundary-related decisions should be guided by relationship-based practice. In our roles as service users and professionals, drawing from our lived experiences of professional relationships, we argue we need to move away from distance-based practice. This includes understanding the boundary stories and narratives that exist for all of us – including the people we support, other professionals, as well as the organisations and systems within which we work. When we are dealing with professional boundary (...) issues, we should centre relationship-building skills that are central to many other aspects of our work. Skills that foster relationships at all levels – between professionals, service users, and services – need to be revalued. Our final recommendation is to create, develop, and foster safer spaces within and outside of organisations, as well as inter-professionally, for the discussion and exploration of boundary-related issues and practice. We are interested in hearing from those with experiences of being marginalised by boundaries so that they can inform a reshaping of our collective ideas around boundary related practices. To foster relationship-based practices in organisations, we have outlined several recommendations here; however, we recognize that these do not go far enough, and that collective action is needed to inform systemic change. Two decades of research on euthanasia in the Netherlands have resulted into clear insights in the frequency and characteristics of euthanasia and other medical end-of-life decisions in the Netherlands. These empirical studies have contributed to the quality of the public debate, and to the regulating and public control of euthanasia and physician-assisted suicide. No slippery slope seems to have occurred. Physicians seem to adhere to the criteria for due care in the large majority of cases. Further, it has been shown (...) that the majority of physicians think that the euthanasia Act has improved their legal certainty and contributes to the carefulness of life-terminating acts. In 2005, eighty percent of the euthanasia cases were reported to the review committees. Thus, the transparency envisaged by the Act still does not extend to all cases. Unreported cases almost all involve the use of opioids, and are not considered to be euthanasia by physicians. More education and debate is needed to disentangle in these situations which acts should be regarded as euthanasia and which should not. Medical end-of-life decision-making is a crucial part of end-of-life care. It should therefore be given continuous attention in health care policy and medical training. Systematic periodic research is crucial for enhancing our understanding of end-of-life care in modern medicine, in which the pursuit of a good quality of dying is nowadays widely recognized as an important goal, in addition to the traditional goals such as curing diseases and prolonging life. Charles Dickens began one of his many great works of literature with this seemingly paradoxical, self-contradictory statement. Reflecting on a jury verdict in Northern California in June of 2001, in the context of what has transpired during the decade of the 1990s with regard to the care of dying patients, observations in the genre of Dickens come readily to mind. In 1991, two of the most compelling books on the subject of pain, medicine, and society were published: Eric Cassell's (...) The Nature of Suffering and the Goals of Medicine and David Morris's The Culture of Pain. Both works unflinchingly recognized and explored the implications of the vast clinical literature documenting the failure of modern medicine to respond compassionately and effectively to what had by then become an epidemic of undertreated pain. Book reviewed in this article: The Willowbrook Wars: A Decade of Struggle for Social Justice. By David J. Rothman and Sheila M. Rothman.

Doctoral programs at U.S. universities play a critical role in the development of human resources both in the United States and abroad. This volume reports the results of an extensive study of U.S. research-doctorate programs in five broad fields: physical sciences and mathematics, engineering, social and behavioral sciences, biological sciences, and the humanities. Research-Doctorate Programs in the United States documents changes that have taken place in the size, structure, and quality of doctoral education since the widely used 1982 editions. This (...) update provides selected information on nearly 4,000 doctoral programs in 41 subdisciplines at 274 doctorate-granting institutions. This volume also reports the results of the National Survey of Graduate Faculty, which polled a sample of faculty for their views on the scholarly quality of program faculty and the effectiveness of doctoral programs in preparing research scholars/scientists. This much-anticipated update of such an essential reference will be useful to education administrators, university faculty, and students seeking authoritative information on doctoral programs. UNTIL RECENT YEARS moral traditions have not been an important topic for moral philosophy. With few exceptions, attention has been directed to the problem of moral justification, to the search for universal criteria for the assessment of moral beliefs or judgments regardless of their traditional provenance. Generally, philosophers aspire to formulate "the view from nowhere." Since the publication of Alasdair MacIntyre's After Virtue there has been a revival of interest in the concept of a living, moral tradition, especially among moral (...) philosophers concerned with the possibility of an ethics of virtue or character as a viable alternative to the ethics of principle or, say, to deontological, utilitarian, or contractarian ethics. We must observe here that in the three decades prior to 1981, some moral philosophers displayed similar concern with tradition, although terms other than "tradition" were used, for example, "forms of life," "ways of life," "moral practices," and "moral community." Rawls's conception of reflective equilibrium is also developed with an eye on the notion of tradition. Indeed, Rawls has been quite explicit about his tradition-oriented approach to moral theory: "What justifies a conception of justice is not its being true to an order antecedent to and given to us, but its congruence with our deeper understanding of ourselves and our aspirations, and our realization that, given our history and the traditions embedded in our public life, it is the most reasonable doctrine.". The work of the French Lithuanian Jewish philosopher, Emmanuel Levinas, describes a perceptive rethinking of the possibility of concrete acts of goodness in the world, a rethinking never more necessary than now, in the wake of the cruel realities of the twentieth century—ten million dead in the First World War, forty million dead in the Second World War, Hiroshima, Nagasaki, the Soviet gulags, the grand slaughter of Mao’s “Great Leap Forward,” the pointless and gory Vietnam War, the Cambodian self-genocide and (...) many an attempted extermination from the face of the earth of entire nations of indigenous peoples. The list of horrors goes on and on—daily holocausts that do not distinguish between the civilian and the soldier, the guilty and the innocent, those deserving of life and those less deserving. The problem of the psychological unconscious has a history of many centuries. Some investigators seek to demonstrate the existence of the unconscious, and others deny it. Fundamental differences exist with respect to the nature of the psychological unconscious even among proponents of that hypothesis. In recent decades interest in the problem of the unconscious has grown substantially in our country. An extensive literature devoted to this subject has appeared in which discussion centers principally on the significance of the psychological unconscious (...) in various spheres of human activity. However, little attention is given in those works to making clear the specifics, the patterns, and the forms in which the unconscious is manifested. Virtually nothing is said about the techniques by which we penetrate the realm of the psychological unconscious, how we achieve knowledge of it, and what the psychological relationship is between the consciousness and the unconscious. This paper investigates the relation between Tocqueville's conceptual framework and the political thought of the French doctrinaires (Guizot, Royer-Collard, Remusat), that has been unduly neglected by political theorists in the English-speaking world. After a brief description of the doctrinaire group, the paper points out similarities and differences between Tocqueville and the doctrinaires with regard to such issues as history, civilization, the French Revolution, the politics of the July Monarchy, centralization and local liberties, and the contrast between aristocratic and democratic societies. The most important section is devoted to democracy as social condition, a theme which appeared in the political writings of the French doctrinaires more than a decade before Tocqueville's Democracy in America. The network approach to psychopathology posits that mental disorders can be conceptualized and studied as causal systems of mutually reinforcing symptoms. This approach, first posited in 2008, has grown substantially over the past decade and is now a full-fledged area of psychiatric research. In this article, we provide an overview and critical analysis of 363 articles produced in the first decade of this research program, with a focus on key theoretical, methodological, and empirical contributions. In addition, we turn (...) our attention to the next decade of the network approach and propose critical avenues for future research in each of these domains. We argue that this program of research will be best served by working toward two overarching aims: (a) the identification of robust empirical phenomena and (b) the development of formal theories that can explain those phenomena. We recommend specific steps forward within this broad framework and argue that these steps are necessary if the network approach is to develop into a progressive program of research capable of producing a cumulative body of knowledge about how specific mental disorders operate as causal systems. In recent decades, attempts to create and argue a new ontology of existence that could provide a robust alternative to the mainstream physicalist metaphysics have been made in science and philosophy. A new book by Bernardo Kastrup, a well-known specialist in the field of philosophy of mind and neuroscience of consciousness, offers the author's conceptually clear and rigorous formulation of the philosophical system. The author proves that appearance and reality in ontology are fundamentally experiential. A universal phenomenal consciousness is the (...) sole ontological primitive, which patterns of excitation constitute existence, and a human being is dissociated mental complexes of this universal consciousness, surrounded like islands by the ocean of its mentation. Kastrup’s idea of the World has an idealist ontological underpinning. The author contrasts his vision of the world with the existing ideas of the world perception, such as physicalism, microexperientialism and cosmopsychism. The quality of the argument, which is given in the book, suggests the revival of the philosophy of idealism as the next paradigm, according to which a form of universal mind will be viewed as nature's sole fundamental entity. Patients in a vegetative state/ unresponsive wakefulness syndrome (VS/UWS) pose ethical dilemmas to those involved. Many conflicts occur between professionals and families of these patients. In the Netherlands physicians are supposed to withdraw life sustaining treatment once recovery is not to be expected. Yet these patients have shown to survive sometimes for decades. The role of the families is thought to be important. The aim of this study was to make an inventory of the professional perspective on conflicts in long-term (...) care of patients in VS/UWS. A qualitative study of transcripts on 2 Moral Deliberations (MD’s) in 2 cases of patients in VS/UWS in long-term care facilities. Six themes emerged: 1) Vision on VS/UWS; 2) Treatment and care plan; 3) Impact on relationships; 4) Feelings/attitude; 5) Communication; 6) Organizational aspects. These themes are related to professionals and to what families had expressed to the professionals. We found conflicts as well as contradictory feelings and thoughts to be a general feature in 4 of these themes, both in professionals and families. Conflicts were found in several actors: within families concerning all 6 themes, in nurse teams concerning the theme treatment and care plan, and between physicians concerning all 6 themes. Different visions, different expectations and hope on recovery, deviating goals and contradictory feelings/thoughts in families and professionals can lead to conflicts over a patient with VS/UWS. Key factors to prevent or solve such conflicts are a carefully established diagnosis, clarity upon visions, uniformity in treatment goals and plans, an open and empathic communication, expertise and understanding the importance of contradictory feelings/thoughts. Management should bridge conflicts and support their staff, by developing expertise, by creating stability and by facilitating medical ethical discourses. Shared compassion for the patient might be a key to gain trust and bridge the differences from non-shared to shared decision making. (shrink)

The aim of this analysis was to examine the effects on stature in two nationally representative samples of Polish 19-year-old conscripts of maternal and paternal education level, and of degree of urbanization, before and after the economic transition of 1990. Data were from two national surveys of 19-year-old Polish conscripts: 27,236 in 1986 and 28,151 in 2001. In addition to taking height measurements, each subject was asked about the socioeconomic background of their families, including paternal and maternal education, and the (...) name of the locality of residence. The net effect of each of these social factors on stature was determined using four-factor analysis of variance. The secular trend towards increased stature of Polish conscripts has slowed down from a rate 2·1 cm per decade across the period 1965–1986 to 1·5 cm per decade between 1986 and 2001. In both cohorts, mean statures increase with increasing size of locality of residence, paternal education and maternal education. The effect of each of these three social factors on conscript height is highly significant in both cohorts. However, the effect of maternal education has increased substantially while that of size of locality of residence and paternal education diminished between 1986 and 2001. These results imply that the influence of parental education on child growth cannot be due solely to a relationship between education and income, but is also perhaps a reflection of household financial management which benefits child health and growth by better educated parents, regardless of level of income. In addition they suggest that, irrespective of whether there are one or two breadwinners in the family, it is the mother, more so than the father, who is principally responsible for the extent to which such management best favours child health and growth. The asymmetry between the importance of maternal as against paternal education for child growth, clearly seen in the 1986 cohort, became more accentuated in 2001, after the abrupt transition from a command to a free-market economy in the early 1990s. This article endeavours to present an overview rather than a complete inventory of LGBTQ studies undertaken in France, in French, mainly in the social and human sciences, in particular in history, but also in law, psychology and psychiatry. A number of explanatory hypotheses will also be advanced to account for why France has lagged behind in this particular area, as well as more broadly in feminist, gender and sexuality studies.

Over the past two decades, work relations have changed dramatically. New phenomena like “gig-economy” or “crowd work” not only constitute precarious working conditions but also contradict with our social esteem of work resulting from the social theories of the classical economy of the eighteenth and nineteenth centuries. The central focus of classical economists on building an educated and disciplined workforce provided not only the base for the upcoming industrial society but also resulted in a work-based society where “being employed” became (...) the precondition for social security and social participation. It is the aim of this contribution to show how our positive attitudes towards work, established by the political economic theories of the eighteenth and nineteenth centuries, are jeopardized by the social changes in post-industrialized societies, due to the effects of globalized economies, digitalization and changed industrial relations. This has also far-reaching consequences for managerial theories based on conceptions like meaningful work or discussions about social responsibilities vis-à-vis employees as primary stakeholder groups. In the contemporary period of history, so deeply contradictory and in many respects conflict ridden, eschatological themes, predictions of universal catastrophe and the ecological or nuclear destruction of mankind, and other motifs of the end of human history have become unprecedentedly widespread and greatly attractive. Despite the huge stores of scientific knowledge accumulated in the past few decades, many people are being drawn more and more into the stream of mystical monstrous visions and are beginning to connect the end of (...) the second millennium with implausible, obsessive apocalyptic presentiments with regard to the world as a whole as well as to our own Russian history. Soothsayers have appeared who have begun to link the phenomena of disintegration and decline and the destruction of political, ethical, and aesthetic criteria being manifested in our country in one way or another directly with the end of Russian history, with the final act of the Russian historical drama. In the sphere of concrete theoretical and philosophical-sociological knowledge, notions of the limit of historical progress, that history has fundamentally, in its essentials, run its course and that from now on the future will bring nothing essentially new, are being put forward and defended. In 2007, ten world-renowned neuroscientists proposed “A Decade of the Mind Initiative.” The contention was that, despite the successes of the Decade of the Brain, “a fundamental understanding of how the brain gives rise to the mind [was] still lacking” (2007, 1321). The primary aims of the decade of the mind were “to build on the progress of the recent Decade of the Brain (1990-99)” by focusing on “four broad but intertwined areas” of research, including: healing (...) and protecting, understanding, enriching, and modeling the mind. These four aims were to be the result of “transdisciplinary and multiagency” research spanning “across disparate fields, such as cognitive science, medicine, neuroscience, psychology, mathematics, engineering, and computer science.” The proposal for a decade of the mind prompted many questions (See Spitzer 2008). In this chapter, I address three of them: (1) How do proponents of this new decade conceive of the mind? (2) Why should a decade be devoted to understanding it? (3) What should this decade look like? The United States remains the only global superpower, but domestic division linked to partisan politics and the “culture wars” opposing secular liberals to religious conservatives is undermining the strength upon which U.S. supremacy ultimately rests. At the time of his death in 2009, he had plans to bring together a number of his most significant papers. This is the first of three volumes to realize those plans. Drawing on three decades of work, it contains previously uncollected articles that have shaped many of the central debates in political philosophy, as well as papers published here for the first time. Over the past decade, Jacques Rancière’s writings have increasingly provoked and inspired political theorists who wish to avoid both the abstraction of so-called normative theories and the philosophical platitudes of so-called postmodernism. Rancière offers a new and unique definition of politics, la politique, as that which opposes, thwarts and interrupts what Rancière calls the police order, la police — a term that encapsulates most of what we normally think of as politics (the actions of bureaucracies, parliaments, and courts). Interpreters (...) have been tempted to read Rancière as proffering a formally pure conception of politics, wherein politics is ultimately separate from and in utter opposition to all police orders. Here I provide a different account of Rancière’s thinking of politics: for Rancière politics goes on within police orders and for this reason he strongly rejects the very idea of a pure politics. Politics is precisely that which could never be pure; politics is an act of impurity, a process that resists purification. In carefully delineating the politique—police relation I show that the terms of Rancière’s political writings are multiple and multiplied. Rancière consistently undermines any effort to render politics pure, and therein lies his potential contribution to contemporary political theory. (shrink)

The author examines the 1945 and 1948 versions of the Davis-Moore functionalist theory of stratification. The analysis explores the basic postulates, assumptions, and logical articulation of the theory and isolates its distinctively functionalist components. This is followed by a historical account of the major criticisms leveled at the theory. The critics have succeeded in showing the fallacious and tautologous character of the functionalist components, thus, in effect, destroying the theory as a theory. Despite this destruction, various portions of the Davis-Moore (...) theory are shown to be usable, to contain valuable insights, and to be capable of further development. (shrink)

The theory and practice of advertising self-regulation have been evolving for decades in pursuit of basic standards for advertising quality. In Spain, this discipline was put into practice in 1995, the year the Association for the Self-Regulation of Commercial Communication was created. This article aims to examine in depth the functioning of the Spanish advertising self-regulation system, with special emphasis on the Advertising Jury, and explore to what extent some of the normative requirements of rigour, independence and participation can be (...) considered to have been met. The paper is based on a case study in which interviews with Autocontrol members, Jury members and consumer associations have particular bearing. The unknown southern continent is perhaps one of the most puzzling aspects of Gerardus Mercator's otherwise strikingly modern cartography. This paper is an attempt to reconsider it in view of Renaissance cosmology and to outline two factors that led Mercator to engage with the mythical terra australis over decades: his socio-professional status as an artisan and the desire to be a philosopher, on the one hand, and the harsh business of mapmaking in the Low Countries on the other. The resulting (...) unknown southern continent was intimately connected to the classical tradition and geocentric cosmology but also to the specific social niche Mercator was trying to establish for himself. This paper is an attempt to reconsider it in view of Renaissance cosmology and to outline two factors that led Mercator to engage with the mythical terra australis over decades: his socio-professional status as an artisan and the desire to be a philosopher, on the one hand, and the harsh business of mapmaking in the Low Countries on the other. The resulting (...) unknown southern continent was intimately connected to the classical tradition and geocentric cosmology but also to the specific social niche Mercator was trying to establish for himself. ernard Baars’ metaphor of the theatre of consciousness has the ability to powerfully evoke and illustrate in a single image many of the functions of the mind and the brain, both conscious and unconscious. The simplicity and sheer appeal of the image of the stage, with conscious events acted out upon it by players moving under the spotlight of attention, makes it easy to visualize, to remember, and to think about. Thus, it is richly evocative, inviting further speculation as to (...) how the elements in front, behind, and on the stage, interact to yield the conscious experiences of daily life. I suspect that the ease and power of this metaphor will go a long way toward making the study of consciousness in psychology not only academically legitimate -- a battle we have just about won -- but something other than mush in the minds of most academic psychologists. I believe that this metaphor will play the role for the cognitive psychology of consciousness that the Atkinson-Shiffrin stage model of memory played for the cognitive psychology of memory some decades back . Like that model, it summarizes most current findings and organizes them in a fashion easily understood by a wide readership. My guess is that it will stimulate a broad range of research and scholarship during the coming years. (shrink)
