While 90 percent of participants in a 2005 Gallup poll indicated that they would donate an organ if asked, only 40 percent of Americans have registered to do so, according to 2012 data from Donate Life America; likely even fewer have shared their donation wishes with loved ones. Undoubtedly, the single biggest reason for the discrepancy between the number of potential transplants and the number actually performed is our failure to talk with loved ones about our wishes regarding organ donation. Fifty years ago this summer, the Uniform Anatomical Gift Act was adopted by the National Conference of Commissioners on Uniform State Laws and approved by the American Bar Association. The UAGA has provided a sound and stable legal platform on which to base an effective nationwide organ donation system. The cardinal principles of altruism, autonomy, and public trust are still important. At a time when confidence and trust in our government and many private institutions has declined, maintaining trust and confidence (...) in our health care system and its commitment to “first, do no harm” has never been more important. Any policies that override these core ethical principles could cause irreparable damage to the public's faith in our transplant system. While progress has been made to increase organ registration and the number of organs transplanted, much more must be done to realize the potential of life‐saving therapy without jeopardizing ethical principles. Ethics is the emphasis of our first-year Introduction to Clinical Medicine-1 course. Introduction to Clinical Medicine-1 uses problem-based learning to involve groups of seven to nine students and two facilitators in realistic clinical cases. The cases emphasize ethics, but also include human behaviour, basic science, clinical medicine, and prevention learning issues. Three cases use written vignettes, while the other three cases feature standardized patients. Groups meet twice for each case. In session one, students read the case introduction, obtain data from (...) the written case or standardized patient, identify the case's ethical problems, formulate learning issues, discuss ways to resolve the moral conflicts, and assign research responsibilities. In session two, students discuss their assigned learning issues and specify and justify clinical actions to address the case's ethical dilemmas. Following three cases, groups write an essay discussing what they learned and describing how they would approach and resolve the case's learning issues. (shrink)

Robert Burton's The Anatomy of Melancholy is one of the last great works of English prose to have remained unedited. The present volume inaugurates an authoritative edition of the work, which is being prepared by scholars on both sides of the Atlantic. It will be followed by two further volumes of text with textual apparatus, and two volumes of commentary. Burton concentrated a lifetime of inquiry into the Anatomy, describing and analysing melancholy and its causes - devoting especial attention to (...) love and religion - and recording possible cures. Primarily a scholarly study of morbid psychology, it is also a compendium of curious facts and anecdotes, and combines seriousness of purpose with a marked satirical vein. First published in 1621, it was a great success: four more editions were published in Burton's lifetime, in each of which new material was added, and a sixth, containing his final revisions, was published in in 1651, eleven years after his death. The textual complexity and Burton's extraordinary range of reference have hitherto deterred editors: this is the first scholarly edition to appear. Maurice Blondel's later works address the problem of the relationship between the Catholic Church and tradition and modernity. This dissertation situates Blondel's developed position between the analyses of modern philosophy and culture developed in the encyclicals Pascendi Dominicus Gregis and Fides et Ratio. Modernism in Catholic circles bears implications for philosophy in general, since modernism has its source in modern philosophy and the culture it gives rise to and reinforces. Three key concepts operating in Blondel's later works are the concrete (...) logic, the philosophy of insufficiency, and the option between egoism and charity. The concrete logic is a logic of the moral life, a logic in which opposition is fundamentally on the basis of hexis and steresis instead of the apophasis and kataphasis , equivalent to the inclusion and exclusion of abstract logic. Use of a logic in philosophy is unavoidable, and inadequate philosophical positions are the result of reliance on abstract logic where a concrete logic is both required and secretly relied upon. The philosophy of insufficiency works within the concrete logic. It has a critical function of assessing and uncovering the insufficiency of the claims made by other philosophical systems, and it has a reconstructive function of leading the human subject towards what could allow it to be rendered sufficient, a supernatural order. This supernatural order imposes a requirement for the human subject to make an option either for egoism of for charity. Modernism represents one modality of the option for egoism, and the option for charity can lead not only to the supernatural order but also to recognition of the continued importance of the Catholic tradition in supporting and keeping intelligible the option for charity. In this paper, we redefine the term luxus consumption to mean food waste and overconsumption leading to storage of body fat, health problems, and excess resource utilization. We develop estimates of the prevalence of luxus consumption and its environmental consequences using US food supply, agricultural, and environmental data and using procedures modeled after energetics analysis and ecological footprint analysis. Between 1983 and 2000, US food availability (food consumption including waste) increased by 18% or 600 kcal (2.51 MJ) per person. This (...) luxus consumption required 0.36 hectares (ha) of land and fishing area per capita, 100.6 million ha for the US population, and 3.1% of total US energy consumption. Luxus consumption increased more for particular foods, such as high fructose corn syrup (HFCS), 22% of which was used in carbonated beverages. As an example, the luxus consumption of sweetened soda, 31.8 l per capita, used 0.8% of the US corn crop (230,555 ha of land); 33.6 million kg of nitrogen fertilizer; 175,000 kg of Atrazine herbicide; 34 million kg of nitrogen fertilizer; 2.44 trillion kcal (10.2 PJ) for production inputs and post-harvest handling; and led to 4.9 million metric tons of soil erosion. Diet soft drink luxus consumption was 43.9 l/capita. Assuming half of US soft drink luxus consumption was bottled in plastic, the energy cost for plastics would have been 2.49 trillion kcal (10.4 PJ) in 2000. Total HFCS availability above baseline in 2000 required 4.6 times the resources used for soft drinks alone. This analysis suggests the utility and applicability of the concept of luxus consumption to environmental analysis and for estimating the effects of excess food utilization. (shrink)

The lack of clarity in the “vice–mental disorder relationship” reflects centuries old struggles to create clear demarcations between “mad” and “bad.” Sadler points (...) out that the DSMs have been and are both the products of historical, cultural, and social forces and the purveyors of defining boundaries that shape contemporary psychiatric and judicial practices. Sadler does American psychiatry a great service by resurrecting a discussion that was quite rich in the nineteenth century, but generally dormant in the twentieth. I thought I could best use this commentary by bringing to the attention the readers of Philosophy, Psychiatry & Psychology the scope of the debate amongst Sadler’s nineteenth-century predecessors.Sadler’s PredecessorsPhilippe Pinel, who presented the first clear modern description of the form of mental illness subsequently named moral insanity appeared well aware, in the early years of the nineteenth century, of the difficulties there would be accepting the concept of a limited or partial moral mania:There are certain rare examples of a peculiar turbulent and embittered character, who otherwise show no evidence of disturbed reason. Ought they to be included under the heading of “mania without delirium?” At any rate the authorities have preferred to have them admitted to asylums rather than let them mix with delinquents in houses of correction.(As cited by Walk 1954, 821)James Cowles Prichard, a British physician, is credited with “inventing” the term moral insanity, which he defined as “a morbid perversion of the feelings, affections, and active powers, without any illusion or erroneous conviction impressed upon the understanding” (Prichard 1837, 20; 1842). [End Page 19]Isaac Ray, the first American to specifically address these matters (in the first edition of his Treatise on The Medical Jurisprudence of Insanity (1838), defined “partial moral mania,” indicating that, “In this form of insanity, the derangement is confined to one or a few of the affective faculties, the rest of the moral and intellectual constitution preserving its ordinary integrity” (p. 186). Ray highlighted those partial moral mania he considered to “have the most important legal relations” (p. 187): “an irresistible propensity to steal,” “an inordinate propensity to lying,” “erotic mania,” “a morbid propensity to incendiarism,” and “homicidal insanity” or “homicidal monomania” (pp. 187–234). Ray cogently described what we would categorize today as an impulse disorder: “Amid the rapid and tumultuous succession of feelings that rush into his mind, the reflective powers are paralyzed, and his movements are solely the result of a blind, automatic impulse with which the reason has as little to do, as with the movements of a newborn infant” (p. 261). The story of the end of the Philosophical Society of Edinburgh in 1783, is linked with that of the founding of the Society of Antiquaries of Scotland and the Royal Society of Edinburgh , both of which were given Royal Charters sealed on 6 May 1783. It is a story which has been admirably told by Steven Shapin. He persuasively argued that the P.S.E. was a casualty of bitter quarrels rooted in local Edinburgh politics, in personal animosities and in disputes (...) about the control of cultural property and intellectual leadership. In all this he was surely correct just as he was in finding the principal actors in this controversy to be: David Erskine, 11th Earl of Buchan; the Reverend Dr John Walker, Professor of Natural History in Edinburgh University; Dr William Cullen, Professor of the Theory and Practice of Medicine and Vice-President of the P.S.E.; Mr William Smellie, Printer to the Society of Antiquaries; Henry Home, Lord Kames, S.C.J. and President of the P.S.E.; Sir George Clerk-Maxwell, Vice-President of the P.S.E.; John Robison, Professor of Natural Philosophy and Secretary to the P.S.E.; Edinburgh University's Principal, William Robertson; the Curators of the Advocates Library: Ilay Campbell, Robert Blair, Alexander Abercromby, Alexander Fraser Tytler, Professor of Public Law; Henry Dundas, Lord Advocate and M.P. for Midlothian. In a peripheral way, the Royal Colleges of Physicians and Surgeons were probably also involved; so too were Lord Buchan's brothers, Henry and Thomas Erskine, Foxite Whigs who opposed Dundas politically. Henry Erskine displaced Dundas as Lord Advocate in August 1783. After the change of ministry on 18 December 1783 he was ousted, but became Dean of the Faculty of Advocates in 1785. This target article considers the relation of fluid cognitive functioning to general intelligence. A neurobiological model differentiating working memory/executive function cognitive processes of the prefrontal cortex from aspects of psychometrically defined general intelligence is presented. Work examining the rise in mean intelligence-test performance between normative cohorts, the neuropsychology and neuroscience of cognitive function in typically and atypically developing human populations, and stress, brain development, and corticolimbic connectivity in human and nonhuman animal models is reviewed and found to provide evidence of (...) mechanisms through which early experience affects the development of an aspect of cognition closely related to, but distinct from, general intelligence. Particular emphasis is placed on the role of emotion in fluid cognition and on research indicating fluid cognitive deficits associated with early hippocampal pathology and with dysregulation of the hypothalamic-pituitary-adrenal axis stress-response system. Findings are seen to be consistent with the idea of an independent fluid cognitive construct and to assist with the interpretation of findings from the study of early compensatory education for children facing psychosocial adversity and from behavior genetic research on intelligence. It is concluded that ongoing development of neurobiologically grounded measures of fluid cognitive skills appropriate for young children will play a key role in understanding early mental development and the adaptive success to which it is related, particularly for young children facing social and economic disadvantage. Specifically, in the evaluation of the efficacy of compensatory education efforts such as Head Start and the readiness for school of children from diverse backgrounds, it is important to distinguish fluid cognition from psychometrically defined general intelligence. In this paper it is proposed that important components of moral development and moral judgment rely on two forms of emotional learning: stimulus-reinforcement and response-outcome learning. Data in support of this position will be primarily drawn from work with individuals with the developmental condition of psychopathy as well as fMRI studies with healthy individuals. Individuals with psychopathy show impairment on moral judgment tasks and a pronounced increased risk for instrumental antisocial behavior. It will be argued that these impairments are developmental (...) consequences of impaired stimulus-aversive conditioning on the basis of distress cue reinforcers and response-outcome learning in individuals with this disorder. This paper surveys interdisciplinary research on gratitude that has been conducted since the review paper translated into German in this issue ‘Recent work on the concept of gratitude in philosophy and psychology’, was published in the Journal of Value Inquiry in 2013. We share progress on our subsequent research, and report on key developments in the field. We revisit familiar themes regarding conditions placed on gratitude, the structure and moral value of gratitude, and the pedagogical implications of research on gratitude, (...) addressing the issue of how the virtue of gratitude should be promoted and taught. As befits a collection dedicated to exploring gratitude’s potential ‘shadow’, we consider again the valence of gratitude and whether it is as quintessentially positive as many have assumed. (shrink)
