As contribution to a recent debate (James, 1998; Murphy etal., 1997, 1998) the proportion of twins following ovulation induction (OI) or assisted conception (AC) in 1994 in Oxfordshire and West Berkshire was estimated, and by extrapolation the natural twinning rate in England and Wales was judged to have maintained a plateau phase since the 1970s. Similar figures for 1995 and 1996 from the same study, and hence a more stable local estimate, are now provided. The proportions, as before, were estimated (...) from women's responses to a questionnaire within a case[hyphen]control study, with ascertainment from general practitioners' records or hospital case[hyphen]notes for non[hyphen]responders or for those excluded from the study originally. In 1994, 1995 and 1996 the proportion of twins following OI/AC was overall 27% (24%, 30% and 27% respectively). Restriction to the 87% locally resident made no difference. The national crude twinning rate for those years was overall 13·3 per 1000 maternities (12·8, 13·6 and 13·4 respectively). (shrink)

This Handbook documents the main trends in current research between logic and language, including its broader influence in computer science, linguistic theory and cognitive science. The history of the combined study of Logic and Linguistics goes back a long way, at least to the work of the scholastic philosophers in the Middle Ages. At the beginning of this century, the subject was revitalized through the pioneering efforts of Gottlob Frege, Bertrand Russell, and Polish philosophical logicians such as Kazimierz Ajdukiewicz. Around (...) 1970, the landmark achievements of Richard Montague established a junction between state-of-the-art mathematical logic and generative linguistic theory. Over the subsequent decades, this enterprise of Montague Grammar has flourished and diversified into a number of research programs with empirical and theoretical substance. This appears to be the first Handbook to bring logic-language interface to the fore. Both aspects of the interaction between logic and language are demonstrated in the book i.e. firstly, how logical systems are designed and modified in response to linguistic needs and secondly, how mathematical theory arises in this process and how it affects subsequent linguistic theory. The Handbook presents concise, impartial accounts of the topics covered. Where possible, an author and a commentator have cooperated to ensure the proper breadth and technical content of the papers. Context Physicians are regularly confronted with research that is funded or presented by industry. Objective To assess whether physicians discount for conflicts of interest when weighing evidence for prescribing a new drug. Design and setting Participants were presented with an abstract from a single clinical trial finding positive results for a fictitious new drug. Physicians were randomly assigned one version of a hypothetical scenario, which varied on conflict of interest: ‘presenter conflict’, ‘researcher conflict’ and ‘no conflict’. Participants 515 randomly selected (...) Fellows in the American College of Obstetricians and Gynecologists' Collaborative Ambulatory Research Network; 253 surveys (49%) were returned. Main object measures The self-reported likelihood that physicians would prescribe the new drug as a first-line therapy. Results Physicians do not significantly discount for conflicts of interest in their self-reported likelihood of prescribing the new drug after reading the single abstract and scenario. However, when asked explicitly to compare conflict and no conflict, 69% report that they would discount for researcher conflict and 57% report that they would discount for presenter conflict. When asked to guess how favourable the results of this study were towards the new drug, compared with the other trials published so far, their perceptions were not significantly influenced by conflict of interest information. Conclusion While physicians believe that they should discount the value of information from conflicted sources, they did not do so in the absence of a direct comparison between two studies. This brings into question the effectiveness of merely disclosing the funding sources of published studies. (shrink)

Background: Issues concerning legislation and regulation with respect to the role of nurses in euthanasia and physician-assisted suicide gave the Minister for Health reason to commission a study of the role of nurses in medical end-of-life decisions in hospitals, home care and nursing homes.Aim: This paper reports the findings of a study of the role of nurses in euthanasia and physician-assisted suicide, conducted as part of a study of the role of nurses in medical end-of-life decisions. The findings for hospitals, (...) home care and nursing homes are described and compared.Method: A questionnaire was sent to 1509 nurses, employed in 73 hospitals, 55 home care organisations and 63 nursing homes. 1179 responses were suitable for analysis. The questionnaire was pilot-tested among 106 nurses, with a response rate of 85%.Results: In 37.0% of cases, the nurse was the first person with whom patients discussed their request for euthanasia or physician-assisted suicide. Consultation between physicians and nurses during the decision-making process took place quite often in hospitals and nursing homes and less frequently in home care situations . In some cases , nurses administered the euthanatics.Conclusions: The results show substantial differences between the intramural sector and the extramural sector , which are probably linked to the organisational structure of the institutions. Consultation between physicians and nurses during the decision-making process needs improvement, particularly in home care. Some nurses had administered euthanatics, although this task is by law exclusively reserved to physicians. (shrink)

This paper is an attempt to determine the exact nature and significance of Hayek's own particular brand of ?individualism.?; In particular, it attempts to show that Hayek's MI is grossly misinterpreted when it is viewed as being merely another instance of the atomistic or analytic individualism characteristic of much modern thinking. For many years Professor Sandbach, Emeritus Professor of Classics at Cambridge, lectured on the Stoics. His book—reflecting a contemporary interest in Stoicism—is most welcome, even if it is not the long and comprehensive undertaking his friends were hoping for. Even so it is deceptively short and simple, containing vast erudition and a masterly touch for evaluating sources. Sandbach begins with the life of Zeno and his influences, to put Stoicism in perspective, goes on to treat the "system," and ends with (...) the great personalities of the earlier and later Stoa. The plan is excellent and avoids both the misconceptions of systematization and the disunity of purely biographical approaches. While critical of basic illogicalities and defects in the system, he sympathetically illuminates the context of a statement and the nature of Stoic exaggeration, e.g., Chrysippus on eating a parent’s corpse. Some of Sandbach’s observations can be noted here: the system can be understood and explained without recourse to Semitic origin; the Stoics knew nothing more than Aristotle’s now lost exoteric works; correct action was also concerned with morally indifferent acts, and later Stoics valued a good act performed by the ordinary man; a divine sign was not necessary to commit suicide, nor did Zeno receive such ; the Stoics did not want to abolish passion or emotion, as such, but mental disturbance. Finally, with regard to the great personalities, Sandbach considers both Seneca, and Marcus Aurelius as rather unorthodox Stoics. (shrink)

In his preface to Hobbes: Morals and Politics, Raphael tells us his intention is to provide "a genuine introduction for the student starting from scratch." That ought not deter others more familiar with Hobbes from reading the book. Raphael’s analysis is well-informed, sometimes insightful, and certainly worth reading. In addition to its intention to serve as an introduction, the book is also a vehicle for a rather elaborate interpretation of Hobbes’s theory of obligation.
