The research aims to study the role of strategic and operational planning as approach for crises management in UNRWA - Gaza Strip field- Palestine. Several descriptive analytical methods were used for this purpose and a survey as a tool for data collection. Community size was (881), and the study sample was stratified random (268). The overall findings of the current study show that strategic and operational planning is performed in UNRWA. The results of static analysis show that (...) there are a relation between strategic and operational planning and crises management. In spite this relation existence, it need more improvement and expanding. Also there are shortcomings in the way that organization manages the crises before and after they occur. A crisis management is only practicing during the crisis. The study suggests that UNRWA must invest in more resources to enhance strategic and operational palming and improve other methods to face potential crises in the future. (shrink)

본 연구의 일차적인 목적은 주희의 스승으로 알려진 이연평이 제시했던 정좌법을 살펴보고, 여기에서 도덕교육적 활용 가능성과 방안을 도출하는 것이다. 그리고 궁극적으로는 유교 명상법이 지니고 있는 현대적 가치를 드러내려는 것이다.BR 이 목적을 위해 본 연구에서는 먼저 연평 사상의 계보와 특이점 및 그가 제안했던 정좌의 목적과 방법 등에 대해 언급하였다. 연평의 사상은 도남학풍을 계승하고 있지만, 사색과 궁리를 중시한다는 점에서 도남학풍과는 일정 이상의 차이점을 보여준다. 그는 정좌를 통해 ‘쇄락’과 ‘융석’의 상태를 지향하였는데, 이 상태에서는 마음에 의심이나 걸림이 없으며, 일상의 행위 역시 조리에 맞고 자연스럽다. 연평은 (...) 이러한 정좌의 방법으로, 전일한 자세로 막혀 있는 곳에 집중하기, 잘못된 습관과 나쁜 기운 제거하기, 감정의 소용돌이를 가라앉힌 순간 환하게 드러나는 마음의 본원처를 궁구하고 함양하기 등을 제시하였다.BR 이어서 본 연구에서는 도덕과 교육과정과 관련 교과서들에서 명상이 어떻게 다루어지고 있는지 살펴보고, 연평 정좌법의 도덕교육적 활용 가능성을 논의하였으며, 그 활용 방안에 대해서도 고찰하였다. 도덕과 교육에서는 『2009(2011) 도덕과 교육과정』을 통해 성찰의 의미를 재규정하고 교수 · 학습 방법의 일환으로 명상을 다루기 시작하였는데, 성찰의 의미나 명상을 활용한 수업 방법은 여전히 불명료하다. 이 같은 상황에서 연평이 제시한 유교 명상법으로서의 정좌는 목적의 ‘명확성’과 적용의 ‘수월성’이라는 측면에서 주목할 만하며, 편한 자세 취하기→집중→궁구→자각→지수→함양의 6단계로 교육방법화 할 수 있다. In an Israeli qualitative study with adolescent cancer survivors and parents who had considered fertility preservation, practically all participants could not recall any discussions with healthcare providers about plans for cryopreserved biological materials in the case of death. This finding is surprising given recent court struggles in Israel over the posthumous use of cryopreserved sperm. In interviews with these adolescent survivors and their parents, intended future use of cryopreserved biological materials is directed for affected individuals’ reproductive purposes later (...) in life, with hardly any consideration of others' use of these materials for posthumous reproduction. To avoid future ethical and legal quagmires, healthcare professionals should have frank discussions with adolescents and their parents about what to do with such materials in the case of death. This paper discusses the socio-cultural and legal Israeli context and considers the ethical implications of using minors’ biological materials for posthumous assisted reproduction in Israel. Individuals vary in the way they navigate through space. Some take novel shortcuts, while others rely on known routes to find their way around. We wondered how and why there is so much variation in the population. To address this, we first compared the trajectories of 368 human subjects navigating a virtual maze with simulated trajectories. The simulated trajectories were generated by strategy-based path planning algorithms from robotics. Based on the similarities between human trajectories and different strategy-based simulated trajectories, we (...) found that there is a variation in the type of strategy individuals apply to navigate space, as well as variation within individuals on a trial-by-trial basis. Moreover, we observed variation within a trial when subjects occasionally switched the navigation strategies halfway through a trajectory. In these cases, subjects started with a route strategy, in which they followed a familiar path, and then switched to a survey strategy, in which they took shortcuts by considering the layout of the environment. Then we simulated a second set of trajectories using five different but comparable artificial maps. These trajectories produced the similar pattern of strategy variation within and between trials. Furthermore, we varied the relative cost, that is, the assumed mental effort or required timesteps to choose a learned route over alternative paths. When the learned route was relatively costly, the simulated agents tended to take shortcuts. Conversely, when the learned route was less costly, the simulated agents showed preference toward a route strategy. We suggest that cost or assumed mental effort may be the reason why in previous studies, subjects used survey knowledge when instructed to take the shortest path. We suggest that this variation we observe in humans may be beneficial for robotic swarms or collections of autonomous agents during information gathering. (shrink)

The study aims to analyze the impact of top management support for strategic planning on crisis management in UNRWA-Gaza Strip field in Palestine. Several descriptive analytical methods were used for this purpose, and a survey as a tool for data collection. Community size was (881), and the study sample was stratified random (268). The overall findings of the current study show that top management provides needed HR for strategic planning but with no financial support. Also there are (...) shortcomings in the way that organization manages the crises before and after they occur. A crisis management is only practicing during the crisis. The study suggest that top management must provide the financial support for strategic planning, periodic meetings to prepare how to deal with potential crisis in the future, establishing a specialized team and provide them with all sources needed. This text intends to show a kind of aprproach and introduction to Deleuze's work. Based on the problem of the non-existence of consensus in the world of life, we feel the need to start thinking and formulating the concept of virtual multiplicity. Then we present a kind of guide to do so. The situation calculus is a logical formalism that has been extensively developed for planning. We apply the formalism in a complex multi-agent domain, modelled on the game of Clue. We find that the situation calculus, with suitable extensions, supplies a unified representation of (1) the interaction protocol, or structure of the game, (2) the dynamics of the knowledge and common knowledge of the agents, and (3) principles of strategic planning.

With the spread of the COVID-19 pandemic worldwide, university teachers are coping with and adjusting to online teaching platforms. In this concurrent mixed-methods study, 10 science and technology universities as the research sites were first chosen, and educational planning in these sites during the pandemic was examined; then, eight selected teacher participants in these sites were interviewed to report how their beliefs and practices changed during the pandemic echoing the examined educational planning. The results show that educational planning and (...) policies assisted teachers in accommodating the new demands and changes during the pandemic; teachers' beliefs and practices generally echoed the educational planning and policies, with certain tensions still existing. The discussion part of the study is centered around emergency remote teaching and planning, tensions between teacher beliefs and practices, and the shift from emergency remote teaching to regular, sustainable online schooling. The study provides administrators and teacher educators with insights on how emergency remote teaching can be planned and implemented during an unprecedented time. (shrink)

The end-state comfort effect refers to the consistent tendency of healthy adults to end their movements in a comfortable end posture. In children with and without developmental coordination disorder, the results of studies focusing on ESC planning have been inconclusive, which is likely to be due to differences in task constraints. The present pilot study focused on the question whether children with and without DCD were able to change their planning strategy and were more likely to plan for ESC (...) when demanded by complex object manipulations at the end of a task. To this end, we examined ESC planning in 18 children with and without DCD using the previously used sword-task and the newly developed hammer-task. In the sword-task, children had to insert a sword in a wooden block, which could be relatively easily completed with an uncomfortable end-posture. In the hammer-task, children had to strike down a nail in a wooden pounding bench, which required additional force and speed demands, making it relatively difficult to complete the movement with an uncomfortable end-posture. In line with our hypothesis, the results demonstrated that children with and without DCD were more likely to plan for ESC on the hammer-task compared with the sword-task. Thus, while children with and without DCD show inconsistent ESC planning on many previously used tasks, the present pilot study shows that many of them are able to take into account the end-state of their movements if demanded by task constraints. The UN Convention of the Rights of the Child (CRC) in the 1990s becomes a landmark and has now been ratified by every other country of the world. Bring hope for a better world—for all children was highlighted as one of the most far-sighted human right instrument in the convention. Yet access to basic livelihood needs such as clean drinking water, primary health services, primary education, recreation and other urban infrastructure becomes a nightmare for millions of children and youth in (...) the developing world and particularly in metropolitan cities in India. The Indian children are living in varying residential environment, comprised of residential colonies of rich, high-raised flats of middle and low-income groups and the economically weaker sections living in slums and squatter colonies. The conventional method of planning residential neighbourhood lacks in indentifying the varying needs of children while building urban infrastructure. As a result there is a large gap in getting access to basic livetihood needs for children of rich and poor. The basic question becomes, is the urban child and youth population just as a numerical variable in planning infrastructure needs in metropolitan cities? This article attempts to analyse the situation in some of the selected residential areas in Delhi. (shrink)

Competition for land is increasing as demand for multiple land uses and ecosystem services rises. Land regulation of the principles of landscape ecology is necessary to develop more sustainable approaches to land use planning. The research evaluated the present land patterns and determined best practices for its regulation of Dongwang Township in Quyang County, located in the Taihang Mountain area of Hebei Province, China. The research used the landscape ecology theory to construct an index system for landscape pattern analysis based (...) on the GIS and Fragstats 3.3 software. In this study, we examined the specific reasons that landscape ecology is superior to traditional methods in land consolidation planning and design, which is conducive to the comprehensive development of land ecological benefits. Landscape ecological planning can effectively reduce landscape fragmentation and improve intensive management. The result found that the descending order of the Shannon index was current landscape, landscape ecological planning, and traditional planning. Landscape ecological planning could protect the natural diversity than traditional planning. Landscape ecological planning enables the creation of long corridors, with higher densities and connectivity and lower average corridor widths than traditional planning. Besides, it can improve ecological service function values in the study area to varying degrees, thus discouraging residents from limiting themselves to grain production. This research has great potential to improve the visibility of ecosystem services in local land use planning and, thus, to improve the ecological functioning of future landscapes. (shrink)

The aim of this study was to determine men’s perceptions about family planning and how they participate or wish to participate in family planning activities in Mpigi District, central Uganda. Four focus group discussions were conducted with married men and with family planning providers from both the government and private sector. In addition, seven key informants were interviewed using a semi-structured interview guide. The results indicate that men have limited knowledge about family planning, that family planning services do not (...) adequately meet the needs of men, and that spousal communication about family planning issues is generally poor. However, almost all men approved of modern family planning and expressed great interest in participating. The positive change of the beliefs and attitudes of men towards family planning in the past years has not been recognized by family planning programme managers, since available services are not in line with current public attitudes. A more couple-oriented approach to family planning is needed. Measures could include, for example, recruiting males as family planning providers, offering more family planning counselling for couples, and promoting female-oriented methods with men and vice versa. Currently, there are two bottleneck problems in the research of college students’ entrepreneurial intention and entrepreneurial behavior: lack of comprehensive and systematic theoretical framework and empirical analysis to reveal the role path that affects entrepreneurial intention, and most studies ignore the gap between entrepreneurial intention and behavior. Based on the literature review, this study adopted the Theory of Planned Behavior as the theoretical framework introduced entrepreneurial situational factors and entrepreneurial implementation intention, and constructed a two-step extended entrepreneurial intention–behavior model. The structural equation was constructed using AMOS24.0 to empirically analyze the antecedent variables of college students’ entrepreneurial intention and the factors influencing entrepreneurial behavior. The empirical results showed that expected material possessions, expected social reputation, expected self-evaluation, mission and responsibility, and career development are the antecedent variables of entrepreneurial attitude. Support from families and friends, college teachers’ views, and the role models are antecedent variables of entrepreneurial subjective norms. Professional ability, entrepreneurial ability, entrepreneurial experiences, and personality traits are the antecedent variables of entrepreneurial perceived behavior control. In the formation stage of college students’ entrepreneurial intention, attitude, subjective norms, perceived behavior control, and entrepreneurial situational factors have significant impacts on the formation of college students’ entrepreneurial intention, while entrepreneurial intention, perceived behavior control, and entrepreneurial situational factors have significant impacts on the transformation phase of entrepreneurial behavior. Entrepreneurial implementation intention plays an intervening role between entrepreneurial intention and behavior of college students. (shrink)

Although previous research has shown that there exist individual and cross-linguistic differences in planning strategies during language production, little is known about how such individual differences might vary depending on which language a speaker is planning. The present series of studies examines individual differences in planning strategies exhibited by speakers of American English, French, and German. Participants were asked to describe images on a computer monitor while their eye movements were monitored. In addition, we measured participants' working memory capacity and (...) speed of processing. The results indicate that in the present study, English and German were planned less incrementally prior to speech onset compared to French, which was planned more incrementally. Crucially, speed of processing predicted the scope of planning for French speakers, but not for English or German speakers. These results suggest that the different planning strategies that are invoked by syntactic choices available in different languages are associated with the tendency for speakers to rely on different cognitive support systems as they plan sentences. This paper raises two problems for plan-expressivism concerning normative judgments about non-corealizable actions: actions which cannot both be performed. First, plan-expressivists associate normative judgment with an attitude which satisfies a corealizability constraint, but this constraint is (in the interpersonal case) unwarranted, and (in the intrapersonal case) warranted only at the price of a contentious normative premise. Ayars (2022) holds that the pair of judgments ‘A should φ’ and ‘B should ψ’ is coherent only if one believes that A can φ (...) while B ψ’s. But this is false. Both Gibbard (2003) and Ayars hold that the pair of judgments ‘A should φ’ and ‘A should ψ’ is coherent only if one believes that A can φ and ψ. But this assumes possibilism. Second, the paper demonstrates, cases involving interpersonal non-corealizability prompt judgments about what multiple agents should do which – contra Gibbard – are not plausibly associated with any planning subject. Enterprise Resource Planning systems are used by universities to handle the academic services and business processes while providing enhanced experience and services to students. This study begins with a background review of ERPs in higher education institutions, the impact on the business processes through optimization and the importance of critical success factors for easier implementation. Secondly, Academic Planning, a core part of the student module of ERPs for higher education, is analyzed in this paper from the prism of data (...) integration, business process workflow, and process optimization. The issues that arise with development of a module are addressed through a case study at SEE-University. The data and business process workflows are based on an actual study by real implementation at this institution. The findings from this study will serve other universities who are in the process of implementation of an ERP to ease their development process and improve the efficiency of the services provided. Main contribution of this study is that it reduces the gap in literature and practice for issues and solutions that arise with the development of a new system, especially in higher education institutions, which in turn are very scarce in nature. Although advance care planning can lead to more patient-centered care, the communication around it can be challenging in acute care hospitals, where saving a life or shortening hospitalization is important priorities. Our qualitative study in an acute care hospital in Japan revealed when specifically physicians and nurses start communication to facilitate ACP. Seven physicians and 19 nurses responded to an interview request, explaining when ACP communication was initiated with 32 patients aged 65 or older. Our qualitative approach employed descriptive (...) analysis to identify major themes, which included “initiation by patients” and “initiation by healthcare professionals.” In the latter case, seven specific triggers were identified: when the patients’ medical condition changed in terms of symptom relief, when the patients’ medical condition changed in terms of prognostic prediction, when serious events occurred, when a choice of treatment was presented, when the location for end-of-life care was chosen, when the patients’ cognitive function deteriorated, and when serious events settled down. Within this group of healthcare professionals, physicians were more focused on changes in their patients’ medical condition, whereas nurses focused more on their patients’ desire for a long-term perspective. Nurses encouraged patients to consider ACP themselves, which developed into an approach to respect patients’ autonomy. In acute care hospitals, it appeared to be desirable to have an early discussion where patients could understand the significance ACP, which would matter even after their discharge from the hospital. Do AI programs just make it quicker and easier for humans to do what they can do already, or can the range of do-able things be extended? This paper suggests that cognitively-oriented technology can make it possible for humans to construct and carry out mental operations, which were previously impossible. Probable constraints upon possible human mental operations are identified and the impact of cognitive technology upon them is evaluated. It is argued that information technology functions as a cognitive prosthetic enhancing (...) human intelligence and planning capabilities. Boundaries and constraints which Kant, Whorf, and many post-modernist theorists have seen as intrinsic to human cognition now cease to apply. (shrink)

Humans routinely make inferences about both the contents and the workings of other minds based on observed actions. People consider what others want or know, but also how intelligent, rational, or attentive they might be. Here, we introduce a new methodology for quantitatively studying the mechanisms people use to attribute intelligence to others based on their behavior. We focus on two key judgments previously proposed in the literature: judgments based on observed outcomes (you're smart if you won the game) and (...) judgments based on evaluating the quality of an agent's planning that led to their outcomes (you're smart if you made the right choice, even if you didn't succeed). We present a novel task, the maze search task (MST), in which participants rate the intelligence of agents searching a maze for a hidden goal. We model outcome‐based attributions based on the observed utility of the agent upon achieving a goal, with higher utilities indicating higher intelligence, and model planning‐based attributions by measuring the proximity of the observed actions to an ideal planner, such that agents who produce closer approximations of optimal plans are seen as more intelligent. We examine human attributions of intelligence in three experiments that use MST and find that participants used both outcome and planning as indicators of intelligence. However, observing the outcome was not necessary, and participants still made planning‐based attributions of intelligence when the outcome was not observed. We also found that the weights individuals placed on plans and on outcome correlated with an individual's ability to engage in cognitive reflection. Our results suggest that people attribute intelligence based on plans given sufficient context and cognitive resources and rely on the outcome when computational resources or context are limited. The “Freedom Agenda” of President George W. Bush for the Middle East assumed that the liberation of Iraq from the dictatorship of Saddam Hussein and the start of political change would trigger the process of democratization of the entire region. Encouraged by financial and economic support, Arab countries should have been willing to implement political and educational support, which would lead to the creation of civil society and grassroots political changes initiated by society itself. A number of mistakes made by (...) the Bush administration in Iraq has not only caused the mission of the democratization of Iraq to be a failure, but also influenced the situation that today Iraq is closer to being a failed state than a democracy. The Covid-19 pandemic has been analysed and discussed from many disciplinary perspectives. An aspect that still needs critical exploration is the role—that is, the modes and forms—of regulatory interventions during the pan- demic. It is interesting to note in this regard that, in many studies, regulatory measures are labelled “non-pharma- ceutical interventions”, as if they do not have any specificity on their own and only represent a theoretically residual category. The main aim of this article is instead to focus on (...) the distinctive features of normative measures as such. As regards the article’s focus, it centres on the normative interventions in the first period of the pandemic—that is, 2020—with particular reference to Italy. We have chosen to focus on this period because the most extensive and severe restrictions introduced to combat Covid-19 were established at that time. And we have chosen to con- sider Italy because it was the first country, after China, to be hard hit by the virus and to react in a draconian manner. As regards the methodology, the article is based, in general and primarily, on an extensive interdisciplinary literature review. With reference to the Italian case, the study is additionally based on: first-hand data collection and analysis (especially concerning Italian normative measures mainly issued at the national level: their type, frequency, target, hierarchical relationships); second-hand data and analysis (for instance as regards the effectiveness of Italy’s and other countries’ regulatory measures). In discussing the Italian response to the Covid-19 pandemic, certain weaknesses have been identified; and possible lessons have been highlighted, in terms of both “planning” (i.e. the necessity to better pre-define concrete and circumscribed sets of actions) and of “meta-planning” (i.e. the necessity of better planning the way in which public authorities could and should plan and act). Although this article is mainly based on the Italian situation, what we can learn from this case is largely generalizable. (shrink)

BackgroundEverybody has the right to decide whether to receive specific medical treatment or not and to provide their free, prior and informed consent to do so. As dementia progresses, people with Alzheimer’s dementia (PwAD) can lose their capacity to provide informed consent to complex medical treatment. When the capacity to consent is lost, the autonomy of the affected person can only be guaranteed when an interpretable and valid advance directive exists. Advance directives are not yet common in Germany, and their (...) validity is often questionable. Once the dementia diagnosis has been made, it is assumed to be too late to write an advance directive. One approach used to support the completion of advance directives is ‘Respecting Choices’®—an internationally recognised, evidence-based model of Advance Care Planning (ACP), which, until now, has not been evaluated for the target group of PwAD. This study’s aims include (a) to investigate the proportion of valid advance directives in a memory clinic population of persons with suspected AD, (b) to determine the predictors of valid advance directives, and (c) to examine whether the offer of ACP can increase the proportion of valid advance directives in PwAD.MethodWe intend to recruit at leastN = 250 participants from two memory clinics in 50 consecutive weeks. Of these, the first 25 weeks constitute the baseline phase (no offer of ACP), the following 25 weeks constitute the intervention phase (offer of ACP). The existence and validity of an advance directive will be assessed twice (before and after the memory clinic appointment). Moreover, potential predictors of valid advance directives are assessed.DiscussionThe results of this study will enhance the development of consent procedures for advance directives of PwAD based on the ACP/Respecting Choices (R) approach. In a distributed multi-agent system, an idle agent may be available to assist other agents in the system. An agent architecture called intent recognition is proposed in this article to accomplish this with minimal communication. To assist other agents in the system, an agent performing recognition observes the tasks other agents are performing. Unlike the much-studied field of plan recognition, the overall intent of an agent is recognized instead of a specific plan. The observing agent may use capabilities that it (...) has not observed. In this study, the key research question is: What are intent-recognition systems and how can these be used to have agents autonomously assist each other effectively and efficiently? A conceptual framework is proposed to address this question. An implementation of the conceptual framework is tested and evaluated. A set of metrics, including task time and number of communications, is used to compare the performance of plan recognition and intent recognition. The aim of this paper is to clarify under what conditions a philosophical problem arises. I will describe two ways in which we might perceive a question as a problem. First, when we fnd ourselves inclined to believe in propositions that appear incompatible with each other. Second, when we fnd ourselves inclined to believe in propositions that seem incompatible with our desires. I will discuss both of these cases and articulate a didactic strategy – the Clash Strategy – which can (...) be used in order to plan and facilitate dialogic inquiry in the classroom. This strategy suggests introducing stimuli that will help students appreciate the clash that drives the philosophical question to be explored during the dialogue. In summary, this paper aims to ofer insights to all teachers interested in promoting dialogic inquiry, whether philosophical or otherwise, in education. (shrink)

Religiosity is one of the most prominent and extensive social factors influencing one’s behavior; however, there is a lack of research analyzing the religiosity impact on pro-environmental behavior, particularly for women in rural areas. To narrow the research gap, this study established a theoretical research model by incorporating religiosity into the Theory of Planned Behavior to explore factors affecting rural female facilitators’ pro-environmental behavior. The extended Theory of Planned Behavior model was consequently tested by empirical data collected from 110 (...) rural female facilitators in Qom Province in the center of Iran. The results of structural equation modeling indicated that subjective norms and environmental attitude were positively and significantly related to pro-environmental intentions. In addition, pro-environmental intentions and perceived behavioral control were found to be significantly associated with pro-environmental behaviors. The results revealed that there was a direct and indirect relationship between religiosity and pro-environmental behaviors via perceived behavioral control. In addition, there was an indirect relationship between religiosity and pro-environmental intentions via subjective norms and environmental attitudes. Therefore, this study revealed that religiosity as social pressure plays an important role in determining pro-environmental intentions and behaviors among rural female facilitators in Iran. Thus, in order to promote pro-environmental behavior, the religiosity aspects should be considered and people should be stimulated to act in a more environmentally friendly mode via religious prism. This study presents the case of a Metropolitan Municipality in the Aegean Region of Turkey, which undertook a series of initiatives to conduct projects on environmental protection and sustainability. This case study was conducted as two separate studies as a part of Horizon 2020-funded ECHOES project under Work Package 6, aiming to gain insight into the collective magnitudes of energy-related choices and behavior. The starting point of the process is marked, in 2015, by the municipality becoming a party (...) to the Covenant of Mayors movement, joining around 8,000 signatories from over 50 countries. In line with European Union’s climate targets and associated energy-related policies, signatories of the Covenant of Mayors aim to decrease carbon emissions by 20% by 2020 and by 40% by 2030. In order to enhance the design and operationalization of policies for achieving these targets, each partner in the Covenant of Mayors is required to develop a Sustainable Energy Action Plan. The SEAP is to be prepared within 2 years of becoming a party to the Covenant of Mayors and involves action plans and projects in order to operationalize the strategies for achieving the associated targets. To this end, this study analyzes the Metropolitan Municipality’s SEAP and its components, which include zero-emission public transportation project, transformation of existing buildings to a more energy-efficient standard and related energy audit studies, a project for decreasing waiting periods in traffic via a Smart Traffic System, a pedestrianization project, and a project for increasing the use of geothermal energy for district heating. This study set out to identify the internal and external factors, as well as bottom-up and top-down mechanisms involved in various phases of the preparation and implementation of the SEAP. The research method was expert interviews, incorporating viewpoints and perceptions of stakeholders from different levels of the municipality. Among the key results are understanding the roles of enthusiasts and frontrunners in such initiative and the importance of top-management and central government support. (shrink)

BackgroundAs part of the research project “End-of-life Communication in Nursing Homes. Patient Preferences and Participation”, we have studied how Advance Care Planning is carried out in eight Norwegian nursing homes. The concept of ACP is a process for improving patient autonomy and communication in the context of progressive illness, anticipated deterioration and end-of-life care. While an individualistic autonomy based attitude is at the fore in most studies on ACP, there is a lack of empirical studies on how family members’ participation (...) and involvement in ACP- conversations may promote nursing home patients’ participation in decisions on future treatment and end-of-life care. Based on empirical data and family ethics perspectives, the purpose of this study is to add insights to the complexity of ACP-conversations and illuminate how a family ethics perspective may improve the quality of the ACP and promote nursing home patients’ participation in advance care planning.MethodsParticipant observations of ACP-conversations in eight nursing homes. The observations were followed by interviews with patients and relatives together on how they experienced being part of the conversation, and expressing their views on future medical treatment, hospitalization and end-of-life issues.ResultsWe found that the way nursing home patients and relatives are connected and related to each other, constitutes an intertwined unit. Further, we found that relatives’ involvement and participation in ACP- conversations is significant to uncover, and give the nursing home staff insight into, what is important in the nursing home patient’s life at the time. The third analytical theme is patients’ and relatives’ shared experiences of the dying and death of others. Drawing on past experiences can be a way of introducing or talking about death.ConclusionsAn individual autonomy approach in advance care planning should be complemented with a family ethics approach. To be open to family ethics when planning for the patient’s future in the nursing home is to be open to diversity and nuances and to the significance of the patient’s former life and experiences.
