Major depression (MD) is a highly heterogeneous diagnostic category. Diverse symptoms such as sad mood, anhedonia, and fatigue are routinely added to an unweighted sum-score, and cutoffs are used to distinguish between depressed participants and healthy controls. Researchers then investigate outcome variables like MD risk factors, biomarkers, and treatment response in such samples. These practices presuppose that (1) depression is a discrete condition, and that (2) symptoms are interchangeable indicators of this latent disorder. Here I review these two assumptions, elucidate (...) their historical roots, show how deeply engrained they are in psychological and psychiatric research, and document that they contrast with evidence. Depression is not a consistent syndrome with clearly demarcated boundaries, and depression symptoms are not interchangeable indicators of an underlying disorder. Current research practices lump individuals with very different problems into one category, which has contributed to the remarkably slow progress in key research domains such as the development of efficacious antidepressants or the identification of biomarkers for depression. The recently proposed network framework offers an alternative to the problematic assumptions. MD is not understood as a distinct condition, but as heterogeneous symptom cluster that substantially overlaps with other syndromes such as anxiety disorders. MD is not framed as an underlying disease with a number of equivalent indicators, but as a network of symptoms that have direct causal influence on each other: insomnia can cause fatigue which then triggers concentration and psychomotor problems. This approach offers new opportunities for constructing an empirically based classification system and has broad implications for future research. (shrink)

In the present article, we consider the question of the primary elements in Plato’s Timaeus, the components of the whole universe reduced, by an extraordinarily elegant construction, to two right triangles. But how does he reconcile such a model with the infinite diversity of the universe? A large part of this study is devoted to Cornford’s explanation in his commentary of the Timaeus and its shortcomings, in order to finally propose a revised one, which we think to be entirely consistent (...) with Plato’s text. This analysis is an essential step for the understanding of the connection between the sensible world and the mathematical principles that underlies Timaeus’ cosmological account. (shrink)

Both patients and clinicians frequently report problems around communicating and assessing pain. Patients express dissatisfaction with their doctors and doctors often find exchanges with chronic pain patients difficult and frustrating. This chapter thus asks how we could improve pain communication and thereby enhance outcomes for chronic pain patients. We argue that improving matters will require a better appreciation of the complex meaning of pain terms and of the variability and flexibility in how individuals think about pain. We start by examining (...) the various accounts of the meaning of pain terms that have been suggested within philosophy and suggest that, while each of the accounts captures something important about our use of pain terms, none is completely satisfactory. We propose that pain terms should be viewed as communicating complex meanings, which may change across different communicative contexts, and this in turn suggests that we should view our ordinary thought about pain as similarly complex. We then sketch what a view taking seriously this variability in meaning and thought might look like, which we call the “polyeidic” view. According to this view, individuals tacitly occupy divergent stances across a range of different dimensions of pain, with one agent, for instance, thinking of pain in a much more “bodycentric” kind of way, while another thinks of pain in a much more "mindcentric” way. The polyeidic view attempts to expand the multidimensionality recognised in, e.g., biopsychosocial models in two directions: first, it holds that the standard triumvirate— dividing sensory/cognitive/affective factors— needs to be enriched in order to capture important distinctions within the social and psychological dimensions. Second, the polyeidic view attempts to explain (at least in part) why modulation of experience by these social and psychological factors is possible in the first place. It does so by arguing that because the folk concept of pain is complex, different weightings of the different parts of the concept can modulate pain experience in a variety of ways. Finally, we argue that adopting a polyeidic approach to the meaning of pain would have a range of measurable clinical outcomes. (shrink)

Objective: To examine current research payment practices and to inform development of clearer guidelines for researchers and ethics committees.Design: Exploratory email based questionnaire study of current research participant reimbursement practices. A diverse sample of organisations and individuals were targeted.Setting: Australia.Participants: Contacts in 84 key research organisations and select electronic listservers across Australia. A total of 100 completed questionnaires were received with representations from a variety of research areas .Main measurements: Open-ended and fixed alternative questions about type of research agency; type (...) of research; type of population under study; whether payment is standard; amounts and mechanisms of payment; factors taken into account when deciding on payment practices; and whether payment policies exist.Results: Reimbursement practice is highly variable. Where it occurs it is largely monetary and is for time and out-of-pocket expenses. Ethics committees were reported to be often involved in decision making around reimbursement.Conclusions: Research subject payment practices vary in Australia. Researchers who do provide payments to research participants generally do so without written policy and procedures. Ethics committees have an important role in developing guidelines in this area. Specific guidelines are needed considering existing local policies and procedures; payment models and their application in diverse settings; case study examples of types and levels of reimbursement; applied definitions of incentive and inducement; and the rationale for diverse payment practices in different settings. In this paper, we study the so-called ‘Mathematical part’ of Plato’s Theaetetus. Its subject concerns the incommensurability of certain magnitudes, in modern terms the question of the rationality or irrationality of the square roots of integers. As the most ancient text on the subject, and on Greek mathematics and mathematicians as well, its historical importance is enormous. Its interpretation presents a certain degree of difficulty because of the intertwined fields that play a role in it : philosophy, history and mathematics. (...) But conversely, correctly understood, it provides clues on both the question of the origins of the irrationals in Greek mathematics and some points concerning Plato’s thought. Taking into account the historical context and the philosophical background generally forgotten in mathematical analyses, we get a new interpretation of this text which, far from being a tribute to some mathematicians, is a radical criticism of their ways of thinking. And the mathematical lesson, far from being a tribute to some future mathematical achievements, ends on an aporia, in accordance with the whole dialogue. (shrink)

History is familiar with great scientific traditions which have been substantial, effective, cumulative and progressive.* At the level of great eras of civilization, extensive and not episodic phenomena, very ancient Chinese science, Greek science and Arab science are objects of investigation for historical erudition, but also for the scientific historian and the philosopher of sciences. Many of the elements of these systems were the source of “modern science”, as it is called, or are integral parts ol’ this system of knowledge (...) and practice which has been constructed over a little more than three centuries. Diachronically, the “scientific revolution” of the classical age in Europe does not signify a total break with what preceded it: Chinese astronomy, Arab algebra and the hospital organization of the Islamic world were used, even if the ideology of scientists when they wrote their own history, particularly from the 19th century onward, tends to make of modern science a purely European phenomenon. This article articulates our everyday notion of cooperation. First, I topple an orthodoxy of shared agency theory by arguing that shared intentions to J are neither necessary nor sufficient for J to be cooperative. I refute the necessity claim by providing examples of shared intention-free cooperation (in institutional contexts and beyond). I refute the sufficiency claim by observing that coercion and exploitation need not preclude shared intentions but do preclude cooperation. These arguments, in turn, lead to my positive proposal. People (...) cooperate, I argue, just in case their activities are coordinated in ways that do not undermine any participant’s agency. (shrink)

Pour rendre compte de la première démonstration d’existence d’une grandeur irrationnelle, les historiens des sciences et les commentateurs d’Aristote se réfèrent aux textes sur l’incommensurabilité de la diagonale qui se trouvent dans les Premiers Analytiques, les plus anciens sur la question. Les preuves usuelles proposées dérivent d’un même modèle qui se trouve à la fin du livre X des Éléments d’Euclide. Le problème est que ses conclusions, passant par la représentation des fractions comme rapport de deux entiers premiers entre eux, (...) i.e. la proposition VII.22 des Éléments, ne correspondent pas aux écrits aristotéli­ciens. Dans cet article, nous proposons une nouvelle démonstration, conforme aux textes des Analytiques, fondés sur des résultats très anciens de la théorie du pair et de l’impair. Ne passant pas par la proposition VII.22, ni par aucune autre propriété établie par l’absurde, cette irrationalité apparaît comme le premier résultat que l’on ne pouvait établir par une autre méthode. L’importance de ce résultat, révélant un nouveau domaine mathématique, celui des grandeurs irrationnelles, rend compte de la centralité que cette forme de raisonnement acquiert alors, d’abord en mathématique, puis dans tout type de discours rationnel. À partir des conséquences qui suivent de cette nouvelle démonstration, on peut interpréter très simplement la leçon sur les irrationnels du passage mathématique figurant dans le Théétète de Platon (147d-148b), ce que nous ferons dans un article à paraître dans un prochain numéro. With the passage by virtually every state legislature of healthcare proxy laws, the medical profession increasingly can expect to rely on the participation of surrogates in making decisions on behalf of incompetent patients. Several concerns about the legitimacy of proxy decision making have been discussed in the ethical and general medical literature: the lack of concordance between the views of patients and their surrogates have been documented on multiple occasions, and cases of abuse by proxies or potential conflict of interest (...) have been reported. Another dilemma that deserves discussion arises when proxies demand withdrawal of treatment that physicians and nurses regard as essential to the wellbeing of the patient. Artykuł stanowi próbę pokazania zagadnienia społecznej odpowiedzialności przedsiębiorstw z filozoficznej perspektywy obecnej w pismach Waltera Lippmanna. Krótko omówione zostały istotne w tym kontekście elementy biografii autora powiązane z jego twórczością. Następnie przedstawione zostało samo pojęcie społecznej odpowiedzialności i jego ewolucja w okresie, w którym powstawały pisma Lippmanna, z naciskiem na amerykański kontekst kulturowo-historyczny. W kolejnej części pokazane zostay dwa różne ujęcia problematyki charakterystycznej dla opisywanego zagadnienia, które odnaleźć można w pismach autora. Pierwsze odnosi się do postulowanej przez autora teorii ekonomii (...) społecznej, drugie powiązane jest ze specyficznym sposobem definiowania społeczeństwa. Wskazane zostały także źródła potencjalnych inspiracji we współczesnych Lippmannowi teoriach ekonomicznych, filozoficznych i społecznych. Niektóre propozycje autora są typowe dla epoki, inne zdają się nosić znamiona innowacyjności. (shrink)

My topic is authenticity in or perhaps as painting, not the authenticity of paintings; I know next to nothing about the problem of verifying claims of authorship. I am interested in another kind of genuineness and fraudulence, the kind at issue when we say of a person that he or she is false, not genuine, inauthentic, lacks integrity, and, especially when we say he or she is playing to the crowd, playing for effect, or is a poseur. These are not (...) quite moral distinctions (no one has a duty to be authentic), but they are robustly normative appraisals, applicable even when such falseness is not a case of straight hypocrisy but of lack of self-knowledge or of self-deceit. (A person can be quite sincere and not realize the extent of her submission to the other’s expectations and demands.) This sort of appraisal also has a long history in post-Rousseauist reflections on the dangers of uniquely modern forms of social dependence, and they are prominent worries in the modern novel. Last year, the Joint Commission on Accreditation of Healthcare Organizations for the first time included provisions in its Hospital Accreditation Manual requiring institutions to have mechanisms in place to consider ethical issues arising in the care of patients and to educate care givers and patients on bioethical issues. This new requirement is notably vague. There is no indication of what type of mechanisms would be appropriate or how those involved in considering ethical issues should conduct themselves. This vagueness is by (...) no means accidental, but it reflects a recognition that clinical bioethics is still in its formative stages and that there are currently almost as many different mechanisms for dealing with bioethical issues in the healthcare setting as there are institutions that have such mechanisms. The Joint Commission's action, however, also implies that the time for experimentation will soon be over. (shrink)
