Family Medicine first formally confronted systems thinking with the adoption of the biopsychosocial model for understanding disease in a holistic manner; this is a description of a natural system. More recently, Family Medicine has been consciously engaged in developing itself as a system for delivering health care, an artificial system. We make this new system available to all people, whether sick or well, offering to manage not just their diseases, but their lives. However, a major difference between natural and artificial (...) systems is that natural systems have smooth feedback loops to ensures homeostasis, whereas artificial systems do not. They can only adjust themselves by disruptive adaptive and renewal cycles. Further, Family Medicine aspires to base its creation of this artificial system on the key principles of Family Medicine - continuity, comprehensiveness, and compassionate care in the context of family and community. However, there are another set of principles driving the modern Family Medicine system of healthcare, principles that function like the hidden curriculum of medical education. These principles - risk thinking, health as a commodity, and individual responsibility - are part of a deeper current within healthcare called by some biomedicalization. On closer analysis, many of these characteristics are intimately tied with liberal economies that must grow. Yet this economic system is an artificial system that lacks feedback. It is imperative that we in Family Medicine reexamine our commitment to developing artificial systems. (shrink)

One of the clearest examples of a technological system, in the sense that Ellul discussed it, is contemporary biomedical health care. The foundation of technological systems is technique: efficient methods for achieving isolated goals. However, the goal of health care should be to achieve health in the full sense of wholeness. Traditional healing systems addressed heath in this sense, but biomedicine cannot; attempting to use the techniques of traditional systems without the accompanying culture ruptures those systems. Using the contemporary epidemic (...) of chronic noncommunicable diseases as an example, this article attempts to exposes the limitations of a technical approach, as well as the limitations of the methodology of risk analysis. Bellʼs 1964 theorem causes a severe problem for the notion that correlations require explanation, encapsulated in Reichenbachʼs principle of common cause. Despite being a hallmark of scientific thought, dropping the principle has been widely regarded as much less bitter medicine than the perceived alternative—dropping relativistic causality. Recently, however, some authors have proposed that modified forms of Reichenbachʼs principle could be maintained even with relativistic causality. Here we break down Reichenbachʼs principle into two independent assumptions—the principle of common cause proper and (...) factorization of probabilities. We show how Bellʼs theorem can be derived from these two assumptions plus relativistic causality and the law of total probability for actual events, and we review proposals to drop each of these assumptions in light of the theorem. In particular, we show that the non-commutative common causes of Hofer-Szabó and Vecsernyés fail to have an analogue of the notion that the common causes can explain the observed correlations. Moreover, we show that their definition can be satisfied trivially by any quantum product state for any quantum correlations. We also discuss how the conditional states approach of Leifer and Spekkens fares in this regard. (shrink)

The temptation to use prospective observational studies (POS) instead of conducting difficult trials (RCTs) has always existed, but with the advent of powerful computers and large databases, it can become almost irresistible. We examine the potential consequences, were this to occur, by comparing two hypothetical studies of a new treatment: one RCT, and one POS. The POS inevitably submits more patients to inferior research methodology. In RCTs, patients are clearly informed of the research context, and 1:1 randomized allocation between experimental (...) and validated treatment balances risks for each patient. In POS, for each patient, the risks of receiving inferior treatment are impossible to estimate. The research context and the uncertainty are down-played, and patients and clinicians are at risk of becoming passive research subjects in studies performed from an outsider’s view, which potentially has extraneous objectives, and is conducted without their explicit, autonomous, and voluntary involvement and consent. But we must reluctantly concede that Pool’s treatment barely glimmers where we might expect to find light-hearted joy in the passion-resurrection and eschatological promise that he does acknowledge is at the heart of the Christian gospel.In his effort to follow methodological strictures consonant with his Chicago schooling with Langdon Gilkey and others, Pool is at pains to keep his academic back to the wall. So the first part of this two-part volume simply rehearses the enormous plurality of “Christian attestations” to the theme of divine suffering (and patripassianism). Attestations of every stripe rub shoulders here and seem to acquire almost equal footing. Yet by calling his topic a “symbol,” Pool does not mean to question the believed ontological actuality of “divine suffering.” In his view, this suffering is not just a symbol but describes for Christian faith an actuality that corners into the meaning and purpose of our very existence. So, as it turns out, he offers us much more than a history-of-religions compilation of diverse “attestations”; he is intent on reworking a tenable systematic theology and theological ethic as these intersect with his theme.Pool’s criteria for ranking some “attestations” over others remain obscure, and we might think him to be arguing rationalistically from personal preferences, until finally (217 pages into his text) he slips in what he calls “an additional epistemological interlude” that divulges what apparently has been an underlying hermeneutic basis all along: his own Messianic secret, as it were, a Christological-revelational criterion and final norm based on the apostolic tradition.Pool’s proclivity for rational analysis may become self-deceptive. At points he does include disclaimers, explaining that his “analyses” of living realities are only mental abstractions describing aspects of personal wholes that in fact cannot be split apart. Yet he forgets himself here, I think, and attempts to subject both divine and human personhood to scholastic “analysis.” For example, his descriptive categories of different “human capacities to love” (along the lines of Nygren’s three loves) become so reified in his “analysis” that he begins to refer to them as if they were schizoid entities that modify, block, and limit each other in our psyche and even in God’s (as if we could strap down the mystery of holistic love and dissect it). Nevertheless, theologians may find Pool’s analytic endeavors quite challenging.This book is definitely not for your introductory ethics course. Pool’s language is labored and presupposes a wide command of the field. His treatment is original, in some respects idiosyncratic, and intentionally revisionist. Written in a neoscholastic vein, this study will give you far more about God’s suffering than you thought anyone ever wanted or needed to know. Yet it promises you a substantial theological workout, offering something provocative and worth considering on nearly every page. Its greatest strength is its perceptive bibliographical annotations, both in the text and in the wealth of footnotes that compose between a third and half of the book. Even though there do seem to be moves towards interdisciplinary rapprochement in most of the essays collected here, I am not sure that the editor's goal of "breaking down the absoluteness of the relativist/antirealist positions of the literary camp and the objectivist/realist positions of the scientific one" is accomplished. Analytically inclined philosophers may well find it eye-opening to discover first hand what Levine details in his excellent essay: that the literary theorists proceed by assuming anti-realism to be the received position (...) and a commonsense starting position. Although there is talk about the problem of theoretical entities, only Livingston seems to be primarily concerned with this notion, treating the aesthetic object as a theoretical entity because of the radically differing interpretations typically proposed for a literary work. Most of the literary theorists' contributions seem to be concerned with the problems engendered by representative realism. Thus the early placement of Churchland's clear description of connectionism is apt. Rorty's observations in his short comment on Scholes, that contextual or holistic accounts of meaning such as those of Davidson and Derrida actually undercut anything interesting to be made of the connection between reality and representation, seem devastating to the presuppositions of the literary theorists. Significant associations have been found between specific human leukocyte antigen (HLA) alleles and organ transplant rejection, autoimmune disease development, and the response to infection. Traditional searches for disease associations have conventionally measured risk associated with the presence of individual HLA alleles. However, given the high level of HLA polymorphism, the pattern of amino acid variability, and the fact that most of the HLA variation occurs at functionally important sites, it may be that a combination of variable amino acid sites shared (...) by several alleles (shared epitopes) are better descriptors of the actual causative genetic variants. Here we describe a novel approach to genetic association analysis in which genes/proteins are broken down into smaller sequence features and then variant types defined for each feature, allowing for independent analysis of disease association with each sequence feature variant type. We have used this approach to analyze a cohort of systemic sclerosis patients and show that a sequence feature composed of specific amino acid residues in peptide binding pockets 4 and 7 of HLA-DRB1 explains much of the molecular determinant of risk for systemic sclerosis. Research has demonstrated that people readily pay more attention to negative than to positive and/or neutral stimuli. However, evidence from recent studies indicated that such an attention bias to negative information is not obligatory but sensitive to various factors. Two experiments using intergroup evaluative tasks (Study 1: a gender-related groups evaluative task and Study 2: a minimal-related groups evaluative task) was conducted to determine whether motivation to strive for a positive social identity - a part of one's self-concept - drives (...) attention toward affective stimuli. Using the P1 component of event-related brain potentials (ERPs) as a neural index of attention, we confirmed that attention bias toward negative stimuli is not mandatory but it can depend on a motivational focus on affective outcomes. Results showed that social identity-based motivation is likely to bias attention toward affectively incongruent information. Thereby, early onset processes - reflected by the P1 component - appeared susceptible to top-down attentional influences induced by the individual's motivation to strive for a positive social identity. The subjective time of an instrumental action is shifted towards its outcome. This temporal binding effect is partially retrospective, i.e., occurs upon outcome perception. Retrospective binding is thought to reflect post-hoc inference on agency based on sensory evidence of the action – outcome association. However, many previous binding paradigms cannot exclude the possibility that retrospective binding results from bottom-up interference of sensory outcome processing with action awareness and is functionally unrelated to the processing of the action – outcome association. Here, (...) we keep bottom-up interference constant and use a contextual manipulation instead. We demonstrate a shift of subjective action time by its outcome in a context of variable outcome timing. Crucially, this shift is absent when there is no such variability. Thus, retrospective action binding reflects a context-dependent, model-based phenomenon. Such top-down re-construction of action awareness seems to bias agency attribution when outcome predictability is low. (shrink)

He argues that French opera moved away from the politics of the absolute monarchy in which it originated to address Enlightenment concerns with sensibility (...) and feeling. The book combines close readings of significant seventeenth-century and eighteenth-century operatic works, circumstantial writings and theoretical works on theatre and opera, together with a measure of reception history. It is well known that Husserl wanted his philosophy to be " presuppositionless." The idea of a presuppositionless philosophy tends to arouse immediate objections, yet it is an idea which is far from clear. In this paper, I would like to clarify what Husserl meant by " presuppositionless philosophy." In particular, I want to show the relationship of presuppositionlessness to Husserl's ideal of a self-justifying science, and to offer this relationship as the context for interpreting the epoche or "suspension" of (...) the so-called natural attitude. This procedure has at least two advantages: it gives a functional interpretation of phenomenological method, and it shows why, for Husserl, phenomenology must become transcendental in order to be philosophical. Before undertaking this, it will be useful to review some of the received opinions about Husserl's idea of presuppositionlessness, progressing from the least to the most radical suggestions. Concern for global warming, climate change and pollution has grown in recent years, with countries across the world facing natural disasters on unprecedented scales. The communication of environmental protection is therefore a necessary area of enquiry, especially from a Conceptual Metaphor Theory perspective. The present article explores (1) how the themes of global warming, climate change, pollution and activism are conceptualized in a corpus of 51 noncommercial advertisements, (2) the interaction of metonymy with metaphor, (3) the distribution across verbal and (...) visual modes of metaphoric source and target domains, and (4) how value is evoked. Findings show that half of the corpus frames environmental themes through source domains such as weapons, predators and natural disasters. The other half triggers incongruous mappings, such as between concrete entities, and relies on metonymic processes of inference to access the main rhetorical message. Among the most frequent metonymies, CAUSE-EFFECT and CATEGORY FOR SALIENT PROPERTY highlight the negative effects of the represented phenomena. Multimodality usually occurs within source and/or target domains rather than across the metaphoric mapping. Most of the campaigns project mixed value, where a negative evaluation of a theme is accompanied by a positive message, inviting the audience to take action. (shrink)

This study examined the influence of integrative motivation, instrumental motivation and second language (L2) proficiency on socio-cultural/academic adaptation in a sample of two groups of international students studying Chinese in China. Results revealed that the non-Asian student group reported higher levels of integrative motivation, socio-cultural adaptation and Chinese language proficiency than the Asian student group, who reported a higher level of instrumental motivation. No significant difference was found in academic adaptation between the two groups. Moreover, the relationships between study variables (...) are the same across Asian and non-Asian student groups. Further evidence from this research indicated that socio-cultural adaptation was influenced more by integrative motivation and less by L2 proficiency. Integrative motivation was found to be the only significant predictor of academic adaptation. Implications for future research are discussed. (shrink)

I take our question to be this: given that causal laws are universal statements, can they be said to be about, to apply to, to hold for, individual things? -/- Peter Downing: Mr. Alexander maintains that there are 'irreducibly singular' causal statements, (...) and that from this it follows that causal laws need not be purely general. The view that there are irreducibly singular causal statements appears to commit Alexander to rejecting two theses: (i) that singular causal statements are analysable into statements containing specific generalizations, [...] (ii) That singular causal statements are analysable into statements containing incompletely specified generalizations [...]. Thesis (i) is, I think, clearly false. [...] Thesis (ii), however, is not easily refuted. (shrink)

One of the deepest tensions in Locke’s Essay, a work full of profound and productive conflicts, is one between Locke’s metaphysical tendencies—his inclination to presuppose or even to argue for substantive metaphysical positions—and his devout epistemic modesty, which seems to urge agnosticism about major metaphysical issues. Both tendencies are deeply rooted in the Essay. Locke is a theorist of substance, essence, quality. Yet, his favorite conclusions are epistemically pessimistic, even skeptical; when it comes to questions about how the world is (...) constituted, our understandings cannot penetrate very far. Locke seems torn between metaphysics and modesty, between dogmatism and skepticism. This chapter will consider two specific examples of this sort of tension. The first involves the ontology of body, and the second, the ontology of mind. The conflict concerning bodily natures looks like this: As is well-known, Locke typically describes bodies in the terms of the corpuscularian science of his day, as exemplified especially by the natural philosopher Robert Boyle. Locke’s characterizations of the real essences of bodies are mechanist. He envisions them as corpuscularian textures-- spatial arrangements of particles possessing size. (shrink)

George Berkeley, Bishop of Cloyne, was one of the great philosophers of the early modern period. He was a brilliant critic of his predecessors, particularly Descartes, Malebranche, and Locke. He was a talented metaphysician famous for defending idealism, that is, the view that reality consists exclusively of minds and their ideas. Berkeley's system, while it strikes many as counter intuitive, is strong and flexible enough to counter most objections. His most studied works, the Treatise Concerning the Principles of Human Knowledge (...) Principles, for short) and Three Dialogues between Hylas and Philonous Dialogues), are beautifully written and dense with the sort of arguments that delight contemporary philosophers. He was also a wide ranging thinker with interests in religion (which were fundamental to his philosophical motivations), the psychology of vision, mathematics, physics, morals, economics, and medicine. Although many of Berkeley's first readers greeted him with incomprehension, he influenced both Hume and Kant, and is much read (if little followed) in our own day. (shrink)

In a devastating critique Raymond Tallis exposes the exaggerated claims made for the ability of neuroscience and evolutionary theory to explain human consciousness, behaviour, culture and society. While readily acknowledging the astounding progress neuroscience has made in helping us understand how the brain works, Tallis directs his guns at neuroscience’s dark companion – "Neuromania" as he describes it – the belief that brain activity is not merely a necessary but a sufficient condition for human consciousness and that consequently our (...) everyday behaviour can be entirely understood in neural terms. With the formidable acuity and precision of both clinician and philosopher, Tallis dismantles the idea that "we are our brains", which has given rise to a plethora of neuro-prefixed pseudo-disciplines laying claim to explain everything from art and literature to criminality and religious belief, and shows it to be confused and fallacious, and an abuse of the prestige of science, one that sidesteps a whole range of mind–body problems. The belief that human beings can be understood essentially in biological terms is a serious obstacle, argues Tallis, to clear thinking about what human beings are and what they might become. To explain everyday behaviour in Darwinian terms and to identify human consciousness with the activity of the evolved brain denies human uniqueness, and by minimising the differences between us and our nearest animal kin, misrepresents what we are, offering a grotesquely simplified and degrading account of humanity. We are, shows Tallis, infinitely more interesting and complex than we appear in the mirror of biologism. Combative, fearless and always thought-provoking, _Aping Mankind _is an important book, one that scientists, cultural commentators and policy-makers cannot ignore. (shrink)

Descartes and Boyle were the most influential proponents of strict mechanist accounts of the physical world, accounts which carried with them a distinction between primary and secondary (or sensible) qualities. For both, the distinction is a piece of natural philosophy. Nevertheless the distinction is quite differently articulated, and, especially, differently grounded in the two thinkers. For Descartes, reasoned reflection reveals to us that bodies must consist in mere extension and its modifications, and that sensible qualities as we conceive of them (...) based on sense perception can pertain only to the mind. Just how we are supposed to arrive at this realization is, this essay will argue, a deep puzzle that brings us to the basic assumptions of Descartes' metaphysics. For Boyle, by contrast, while reflection can reveal the unique explanatory status of mechanism, and, thus, the primary/secondary quality distinction, only experience can confirm its truth. Our central focus will be on Descartes, and on the question: How does he intend to remove the sensible qualities from the physical world, how does he strip them from bodies? I will try to show that Descartes has an argument that he takes to show a priori that sensible qualities cannot be attributed to the material world (as foundational qualities, or, as we conceive of them based on sense experience). The argument fails, however, leaving him with at best a partly empirical case for removing the sensible qualities, based on the purported explanatory success of his physics. (shrink)
