BackgroundHuman failure and a lack of effective communication are the main reasons for preventable adverse events, compromising patient safety in obstetrics. In order to improve safety, team and communication interventions have been implemented but lack feasibility in obstetric care. Psychological models such as the health action process approach might help to improve interventions.MethodsIn a cross-sectional online survey with N = 129 healthcare workers and a paper-pencil survey with N = 137 obstetric healthcare workers at two obstetric university hospitals, associations of (...) social-cognitive variables were tested in a path analysis and a multiple regression. Preliminary results informed a communication training for all obstetric healthcare workers. A repeated-measures MANOVA was used to compare pre- and post-intervention data.ResultsSocial-cognitive variables were associated according to model suggestions except for planning in the first study. Triggers of adverse events were associated with communication behavior, action self-efficacy and planning, as well as barriers to effective communication. The intervention was rated positively. Afterward, fewer triggers were reported and coping self-efficacy increased. There were group differences regarding hospital, experience, and time.DiscussionThe health action process approach was examined in the context of safe communication in obstetrics and can be used to inform interventions. A theory-based, short training was feasible and acceptable. Perceived patient safety improved but communication behavior did not. Future research should aim to test a more comprehensive psychological communication intervention in a thorough RCT design. (shrink)

Purpose: This study explores social networkers' interest in and attitudes toward personal genome testing (PGT), focusing on expectations related to the clinical integration of PGT results. Methods: An online survey of 1,087 social networking users was conducted to assess 1) use and interest in PGT; 2) attitudes toward PGT companies and test results; and 3) expectations for the clinical integration of PGT. Descriptive statistics were calculated to summarize respondents' characteristics and responses. Results: Six percent of respondents have used PGT, 64% (...) would consider using PGT, and 30% would not use PGT. Of those who would consider using PGT, 74% report they would use it to gain knowledge about disease in their family. 34% of all respondents consider the information obtained from PGT to be a medical diagnosis. 78% of those who would consider PGT would ask their physician for help interpreting test results, and 61% of all respondents believe physicians have a professional obligation to help individuals interpret PGT results. Conclusion: Respondents express interest in using PGT services, primarily for purposes related to their medical care and expect physicians to help interpret PGT results. Physicians should therefore be prepared for patient demands for information and counsel on the basis of PGT results. (shrink)

Food democracy is a concept with growing influence in food research. Food democracy deals with how actors may regain democratic control over the food system enabling its sustainable transformation. Following multi-level perspective framework's connotations, food democracy research has so far mainly focused on the niche level of the food system. An integrative approach that includes the perspectives of both the regime and the niche is still missing. This study addresses this research gap and proposes a new conceptual framework for food (...) democracy that includes actors from the niche and the regime level. Furthermore, we apply our conceptual framework to the urban food system of Vienna to explore the deeper meaning and practice of food democracy. Finally, we have conducted semi-structured interviews with actors at niche level and regime level within Vienna’s urban food system. Findings from this research broaden the perspective on food democracy and illustrate actors’ contributions at niche and regime level such as promoting organic food, re-localizing food provision, and procuring environmentally sustainable public food. Barriers to food democracy were also identified, e.g.: actors’ self-enhancement values, market-orientation, and capitalist alignment or lack of transparency. We conclude that actors at the niche and, to some extent, at the regime level may contribute to a process of on-going changes that fosters a transformation of established structures within the food system. In Aquinas's account of the beatific vision, human beings are joined to God in a never-ending act of contemplation of the divine essence: a state which utterly fulfills the human drive for knowledge and satisfies every desire of the human heart. In this paper, I argue that this state represents less a fulfillment of human nature, however, than a transcendence of that nature. Furthermore, what’s transcended is not incidental on a metaphysical, epistemological, or moral level.

What holds together the various fields, which - considered together - are supposed to constitute the general intellectual discipline that people now call cognitive science? Some theorists identify the common subject matter as the mind, but scientists have not been able to agree on any single, satisfactory answer to the question of what the mind is. This book argues that all cognitive sciences are not equal, and that rather only neurophysiology and cultural psychology are suited to account for the mind's (...) ontology. (shrink)

Human rights defenders (HRDs) fight for various human rights and address concerns related to corruption, employment, the environment, and other issues. They also challenge powerful state and private stakeholders and seek justice for human rights abuses. Therefore, HRDs are increasingly becoming targets of violent attacks and abuse with the aim of silencing them. This article begins by providing a brief definition of HRDs and then proceeds to outline the risks associated with their work in defending human rights. It also identifies (...) the perpetrators responsible for these violations. The article categorises the types of abuses against HRDs into two main categories, with a particular focus on the widespread tactic of using the legal system to target and silence defenders in Europe, which is also emerging globally. It introduces a taxonomy of various types of violations through the legal system. By categorising the types of violations against HRDs and establishing a taxonomy to aid in identifying these tactics, the article seeks to deepen understanding and awareness of the varied abuses experienced by HRDs, as well as their deviation from human rights standards, providing a valuable resource for academics, practitioners, and defenders. Humanitarian healthcare work presents a range of ethical challenges for expatriate healthcare professionals, including tragic choices requiring the selection of a least-worst option. In this paper we examine a particular set of tragic choices related to the prioritization of care and allocation of scarce resources between individuals in situations of widespread and urgent health needs. Drawing on qualitative interviews with clinicians, we examine the nature of these choices. We offer recommendations to clinical teams and aid organizations for preparing and supporting (...) frontline clinicians in their efforts to determine the least-worst option, and in their responsibility for making such choices. Seed rematriation is a rising movement within greater efforts to improve seed and food sovereignty for Native American communities in the United States. As a feminized reframing of repatriation, rematriation seeks to heal Indigenous relationships with food, seeds, and landscapes. Since first contact, Native agricultural practices have been systematically targeted by colonization, resulting in the diminished biodiversity of cultural gardening systems. Of this vast wealth, many varieties exist today solely under the stewardship of non-Native institutions. Seed rematriation is therefore the (...) process and movement by which Native nations reclaim their cultural seed heritages. Once seeds are returned to the hands and soils of their home communities, Indigenous Nations can reestablish healthy, diverse, and sustainable seed and foodways for generations to come. This article explores the history of the term rematriation within Indigenous sovereignty scholarship as well as its evolving interpretations and applications. Considering how the seed rematriation movement has been shaped by several seed keepers in the Midwest reveals the cultural understandings and significances that underpin this work across many aspects of Indigenous lifeways. The resulting discussion from ethnographic material demonstrates why seed reclamation and seed sovereignty movements in the Midwest uphold Native nationhood through both resurgence and refusal. The Indigenous processes of recognizing and reclaiming seeds work beyond recovering agricultural knowledge to also mend severed kin relationships, rejuvenate cultural knowledge, and reestablish authority over Indigenous food systems. As digital media becomes more central to the lives of adolescents, it also becomes increasingly relevant for their sexual communication. Sexting as an important image-based digital medium provides opportunities for self-determined digital communication, but also carries specific risks for boundary violations. Accordingly, sexting is understood either as an everyday, or as risky and deviant behavior among adolescents. In the affectedness of boundary violations gender plays an important role. However, it is still unclear to what extent digital sexual communication restores stereotypical (...) gender roles and restrictive sexuality norms or, alternatively, enables new spaces of possibility. In this sense, current research points to a desideratum regarding adolescents’ orientations toward sexting as a practice between spaces of possibility and boundary violations. This paper discusses the possibilities, but also the risks, of intimate digital communication among adolescents. The main question is, how adolescents themselves perceive sexting practices and how they position themselves between both spaces for possibility and for the exchange of unwanted sexual content. For this purpose, orientations toward normalities and gender of students are reconstructed. To answer these questions, twelve single-sex, group discussions were carried out with students aged 16 and 17 at five different secondary schools in northern Germany. A total of 20 boys and 22 girls took part. The group discussions were structured by a narrative generating guideline. The analysis draws its methodology from the Documentary Method, regarding implicit and explicit forms of knowledge and discourse. It results in a typology of three types with different orientations. The study shows, that most of the students consider sexting to be a risky practice; only one type shows normality in the use of sexting. At the same time, some of the young people are interested in experimenting with image-based intimate digital communication. Further, gender differences in use and affectedness are also documented. In this way, orientations toward gender stereotypes “favor” both the attribution of responsibility to girls, and overlook the responsibility of students who perpetrated the boundary violation. The orientations of adolescents should be taken more into account in research as well as in educational programs for the prevention of sexual violence. (shrink)

When Katniss first arrives in the Capitol, she is both amazed and repulsed by the dramatic body- modifications and frivolous lives of its citizens. “What do they do all day, these people in the Capitol,” she wonders, “besides decorating their bodies and waiting around for a new shipment of tributes to roll in and die for their entertainment?” In this paper, I argue that the more time and energy the Capitol citizens focus on body-modification and their social lives, the more (...) self-focused they become and the less likely they are to notice or care about political injustices that don’t directly affect them. A further examination of how the frivolity of the citizens is actually used by the Capitol to strengthen its power also provides insight into what seems most troubling about the lives of the citizens not just of the Capitol but of District 13 as well—namely, their lack of self-directed significance. Theoretical models for physician-patient communication in clinical practice are described in literature, but none of them seems adequate for solving the communication problem in clinical practice that emerges in case of factitious disorder. Theoretical models generally imply open communication and respect for the autonomy of the patient. In factitious disorder, the physician is confronted by lies and (self)destructive behaviour of the patient, who in one way or another tries to involve the physician in this behaviour. It is no longer controversial (...) that the physician should communicate his consideration of a factitious disorder without insistence that the patient accepts this diagnosis. However, the balance between patient autonomy and open communication on the one hand, and the preservation of the patient's health, physician integrity and of a constructive physician-patient relationship on the other is easily disrupted. In this article, an epistemological model is described to facilitate a positive outcome of confrontation in treatment of factitious disorder. Analysing the problem in terms of systems theory will help the physician to assess what information is appropriate to use in which phase of the patient's treatment, while preserving the physician-patient relationship. In this book, Erneling and Johnson identify two problems with defining this discipline. First, some theorists identify the common subject matter as the mind, but scientists and philosophers have not been able to agree on any single, satisfactory answer to the question of what the mind is. Second, those who speculate about the general characteristics that belong to cognitive science (...) tend to assume that all the particular fields falling under the rubric--psychology, linguistics, biology, and son on--are of roughly equal value in their ability to shed light on the nature of mind. This book argues that all the cognitive science disciplines are not equally able to provide answers to ontological questions about the mind, but rather that only neurophysiology and cultural psychology are suited to answer these questions. However, since the cultural account of mind has long been ignored in favor of the neurophysiological account, Erneling and Johnson bring together contributions that focus especially on different versions of the cultural account of the mind. The scholarship of Ulf Hannerz is characterized by its extraordinary breadth and visionary nature. He has contributed to the understanding of urban life and transnational networks, and the role of media, paradoxes of identity and new forms of community, suggesting to see culture in terms of flows rather than as bounded entities. Contributions honor Hannerz' legacy by addressing theoretical, epistemological, ethical and methodological challenges facing anthropological inquiry on topics from cultural diversity policies in Europe to transnational networks in Yemen, and (...) from pottery and literature to multinational corporations. Orthorexia is a condition in which the subject becomes obsessed with identifying and maintaining the ideal diet, rigidly avoiding foods perceived as unhealthy or harmful. In this paper, I examine widespread cultural factors that provide particularly fertile ground for the development of orthorexia, drawing out social and historical connections between religion and orthorexia (which literally means “righteous eating”), and also addressing how ambiguities in the concept of “health” make it particularly prone to take on quasi-religious significance. I argue that what (...) makes this sort of disordered eating destructive to both men and women is ultimately a common urge to transcend rather than to embrace the realities of embodiment. In sum, I believe that orthorexia is best understood as a manifestation of age-old anxieties about human nitude and mortality—anxieties which current dominant sociocultural forces prime us to experi- ence and express in unhealthy attitudes toward healthy eating. This paper analyses various approaches to the concept of a ‘safety culture’ in terms of their epistemological assumptions regarding the nature of learning. As a result of this analysis, the study proposes a relational-interpretive framework for the promotion of safety in health care, which is based on relational theories and the philosophy of conceptual pragmatism as this can be used to integrate the various strands of current safety research. In particular, the approach based on a relational-interpretive perspective can bridge the (...) apparent dualist gap that exists between the rational objectivist perspective and the relativist perspective on the role of learning in developing a safety culture. According to the relational-interpretive perspective of safety management that is proposed here, organizational members need to give continuous attention to the accepted organizational norms and values, which shape the safety culture. A case study from a health care safety project in Sweden is utilized to illustrate the ideas advanced in this paper. (shrink)

Predictive coding models of brain processing propose that top-down cortical signals promote efficient neural signaling by carrying predictions about incoming sensory information. These “priors” serve to constrain bottom-up signal propagation where prediction errors are carried via feedforward mechanisms. Depression, traditionally viewed as a disorder characterized by negative cognitive biases, is associated with disrupted reward prediction error encoding and signaling. Accumulating evidence also suggests that depression is characterized by impaired local and long-range prediction signaling across multiple sensory domains. This review highlights (...) the electrophysiological and neuroimaging evidence for disrupted predictive processing in depression. The discussion is framed around the manner in which disrupted generative predictions about the sensorium could lead to depressive symptomatology, including anhedonia and negative bias. In particular, the review focuses on studies of sensory deviance detection and reward processing, highlighting research evidence for both disrupted generative predictions and prediction error signaling in depression. The role of the monoaminergic and glutamatergic systems in predictive coding processes is also discussed. This review provides a novel framework for understanding depression using predictive coding principles and establishes a foundational roadmap for potential future research. (shrink)

The COVID-19 pandemic poses a health threat that has dominated media coverage. However, not much is known about individual media use to acquire knowledge about COVID-19. To address this open research question, this study investigated how the perceived threat is linked to media use and how media use is associated with perceived and actual knowledge about COVID-19. In a German online survey conducted between April 16 and April 27, 2020, N = 952 participants provided information on their perceived threat and (...) media use to inform themselves about COVID-19. In this process, they indicated how well they were informed about COVID-19 and subsequently completed a COVID-19 knowledge test. Results indicated that individuals who felt more threatened by COVID-19 used media more often to inform themselves but focused on fewer different media channels. Further, frequent media use was associated with higher perceived knowledge, but not with higher actual knowledge about COVID-19, reflecting an illusion of knowledge. Additionally, using fewer media channels was linked to higher perceived and actual knowledge. Finally, explorative analyses on the use of different media channels revealed that an illusion of knowledge emerged for using social media, public television, and newspapers. Potential explanations for the findings and implications for future research are discussed. (shrink)

Der Sammelband geht den Fragen nach, was eigentlich studierendenzentrierte Lehre ist, welche konkreten Herausforderungen bei der Umsetzung einer studierendenzentrierten Lehre auftreten und wie eine solche Lehre angesichts dieser Herausforderungen umgesetzt werden kann. Aber auch: Welche Hürden haben sich bisher als unüberwindbar erwiesen, und wie können Lehrende damit umgehen?

In current society, eating is most definitely a gendered act: that is, what we eat and how we eat it factors in both the construction and the performance of gender. Furthermore, eating is a gendered act with consequences that go far beyond whether one orders a steak or a salad for dinner. In the first half of this paper, I identify the dominant myths surrounding both female and male eating, and I show that those myths contribute in important ways to (...) cultural constructions of male and female appetites more generally speaking. In the second half, I argue that the Christian church should share feminism’s perception of these current cultural myths as fundamentally disordered, and I claim that the Christian traditions of fasting and feasting present us with a concrete means to counter those damaging conceptions and reclaim a healthy attitude toward our hunger. An important devotional genre in the thirteenth and fourteenth centuries, meditations invited their readers to place themselves at the scene of various moments in Christ’s life and encouraged them to have particular emotional responses—joy, sorrow, compassion, and the like—to those imaginative experiences. In its emphasis on feeling, meditation was seen as an activity particularly suited for women and their closer ties with the body. Meditation was also seen as an activity distinct from contemplation, which was portrayed as a “higher,” more (...) intellective pursuit. Yet meditation was intended to increase love to Christ, and love was widely considered to increase knowledge—knowledge of the same sort that contemplation also claimed to yield. Over time, then the widespread popularity of this spiritual exercise opens up space for women’s claims to knowledge to be heard because of (rather than despite) their association with the body. This book presents and deconstructs the existing explanations for the differential career development of qualified men and women. It reframes the problem of discrimination in the workplace as a matter of organizational ethics, social responsibility and compliance with existing equal opportunity laws. Sensitive points are identified where social biases, decision-makers' individual economic interests and shortcomings of organizational incentive policies may lead to discrimination against qualified women. The ideas put forward are empirically tested in an original laboratory experiment that examines personnel (...) selection in the male-dominated field of science and technology. It contrasts the selection of applicants with gendered and gender-blind applications available to subjects under controlled conditions. 30% of participants were high-level decision-makers, which is unprecedented in this field of research. The results, highly relevant for organizational practice, are explained and discussed in detail. (shrink)

An increasing number of Muslim asylum seekers and refugees convert to Christianity in Europe. The conversion motifs in these individuals are unknown. In this study, we applied biographical interviews in 124 converts. There were two dominant patterns: intellectual —intellectual plus experimental motifs, and mystical —mystical plus affectional motifs. Pure experimental and affectional motifs were rare, and there were no revivalist and coercive motifs. Demographic parameters did not predict conversion motifs. We found no evidence for social pressure. These results indicate that (...) finding meaning and consolation in Christian religious teachings and mystical experiences with a high emotional content are the two leading religious conversion motifs. Social anxiety disorders are among the most prevalent mental disorders, with high impact on the life of an affected social system and its individual social system members. We developed a manualized disorder-specific integrative systemic and family therapy for SAD, and evaluated its feasibility in a pilot randomized controlled trial. The ISFT is inspired by Helm Stierlin’s concept of related individuation developed during the early 1980s, which has since continued to be refined. It integrates solution-focused language, social network diagnostics, and genogram (...) work, as well as resource- and problem orientation for both case conceptualization and therapy planning. Post-Milan symptom prescription to fluidize the presented symptoms is one of the core interventions in the ISFT. Theoretically, the IFST is grounded in radical constructivism and “Cybern-Ethics,” multi-directional partiality, and a both/and attitude toward a disorder-specific vs. non-disorder-specific therapy approach. SAD is understood from the viewpoint of social systems theory, especially in adaptation to a socio-psycho-biological explanatory model of social anxiety. In a prospective multicenter, assessor-blind pilot RCT, we included 38 clients with SAD : 18 patients participated in the ISFT, and 20 patients in Cognitive Behavioral Therapy. Within-group, simple-effect intention-to-treat analyses showed significant reduction in social anxiety, while intention-to-treat mixed-design ANOVA demonstrated the advantage of ISFT. Per-protocol analyses supported these results. The remission rate based on blind diagnosticians’ ratings was good to satisfactory ; this has yet to be verified in a subsequent confirmatory RCT. The article will present the ISFT rationale and manual, including a special focus on multi-person settings, and the central findings from our pilot RCT. (shrink)
