Research studies demonstrate wide variation in how physicians diagnose and treat patients with similar medical conditions and suggest that at least some of the variation reflects inefficiencies and unnecessary medical costs. Health care researchers are actively examining ways to reduce variations in practice through standardization of medicine to reduce the cost of treatment and ensure the quality of outcomes. The most widely accepted form of this standardization is Evidence Based Best Practices. Furthermore, financial health care providers such as hospitals and (...) managed care organizations are investigating methods to tie resource usage to medical protocols in their efforts to monitor and control health care costs. Such proposals are contentious because they report on physicians' medical practice behaviors and such reports could potentially be used to influence their clinical behaviors. The intent of this exploratory study was to examine physicians' perceptions about linking a standard costing system to EBBP guidelines. The authors interviewed nine practicing physicians asking each physician to respond to the question, 'As a physician working in a hospital environment, what are your reactions to and concerns with combining standard costing techniques with EBBP?' The interviews were in-depth and free form in nature. The physicians' responses were recorded and analyzed using Grounded Theory Methodology. Using this methodology the field data was categorized into two major themes. The most important theme centered on ethics and the second theme was concerned with the implementation and use of a standard cost system in regard to EBBP. If physicians' worries about ethical dilemmas and implementation issues are not resolved, then it is likely that doctors would be unwilling to participate in any efforts to develop or use a standard cost-reporting system in medicine. While this study was exploratory in nature, it should provide future guidance to accountants, health care researchers and health care providers about physicians' issues with the use of standard costing methods in medicine. We are the funny organisms that make and follow rules. To understand us, one must understand what is it to institute and follow a rule, to perform correctly or in error. This question is more important than it might at first seem for linguistic meaning is constituted by rules that govern uses of expressions. For example, the fact that 'squid' is correctly applied to squid and incorrectly applied to cuttlefish is part of what makes 'squid' mean what it does. Philosophers (...) sometimes argue further that all representations are rule-governed so that a theory of representation must explain the possibility of misrepresentation. ;The rule-following problem makes an explanation of correctness surprisingly difficult. Philosophers have also not been clear on just what is the problem with rules. I provide a statement of the features of rules and the problems they drive. I then show that some well-known accounts of rule following---Kripke's, Wright's, Blackburn's---are inadequate. My formulation also shows that some favored theories of content---causal, evolutionary, interpretivist---are inadequate. ;Positively, I argue that an essentially social and irreducibly normative organization---a practice---solves the rule-following problem. Therefore, only participants in social practices can follow rules and thence traffic in linguistic meaning. Still, an account of the correctness of a linguistic performance is not yet an explanation of its representational content. I argue that linguistic content is constituted by correctness facts but also by the representational purport of expressions. I explain an expression's purport to represent naturalistically in terms of syndromes of psychological and behavioral symptoms. A theory of linguistic representation, then, must join the non-natural social facts that constitute correctness with the natural individualistic facts that constitute representational purport. ;Such a hybrid theory reveals that thought and language are not as intimately connected as philosophers assume. Whereas linguistic content constitutively partakes of the social organization that sets humans apart from other creatures, thought content involves only refinements of the psycho-behavioral syndromes that we share with most vertebrates. This quantitative study of 3557 high school teachers from 44 states assesses the implications of the social studies teacher-coach. The study compares social studies teacher-coaches and non-coaches in terms of teacher demographics and school contexts, disciplines taught, and instructional preferences. Substantial differences between coaches and non-coaches were found across gender, community type, and teaching experiences. Teacher-coaches disproportionately taught general classes such as government, Economics, and Geography, while non-coaches are more likely to teach Advanced Placement courses. Finally, self-report data measuring teacher's (...) instructional preferences indicates that teacher-coaches were more likely to indicate implementation of lectures, textbooks, worksheets, and videos, while non-coaches were more likely to indicate preference for examining photographs, artifacts, or primary materials. There are decision problems in which rational deliberation fails to result in choosing a pure act. This phenomenon is known as decision instability and has been discussed in the literature on causal decision theory. In this paper we investigate another type of instability, called structural instability in dynamical systems theory. Structural instability indicates that certain qualitative features of the process of rational deliberation are under-determined in a decision situation. We illustrate some of the issues arising from structural instability with a (...) recent argument against causal decision theory proposed by Caspar Hare and Brian Hedden. Art and philosophy are the modes (...) of practice that disrupt and disorganize our habitual ways of doing things, everything from dancing to language, to philosophy and art themselves. They are the reflection at a distance of what we do. “In this sense, then, art is disruptive. Always: everywhere.” From this basis, he tackles a wide range of phenomena from dancing, seeing, writing, and so forth, to introduce a distinction between these as everyday phenomena and their function as art. This delimitation is not material, nor intrinsic to any of the analyzed phenomena; rather, it depends on each practice’s ability to disrupt, disorganize, and force upon us the question of what dancing, seeing, writing is at all. Noë provides an example: At an art exhibition one is confronted with a painting that is ungraspable, foreign, meaningless. But, through a mediation (pointers from a friend, new knowledge of the era, analyses of the style, and so forth) the painting starts becoming significant; it starts to make sense. The viewer is stopped in her tracks, and a painting that was before closed is now opened up to a new perception, a new way of seeing.But Noë extends this aesthetic domain to experience and perception. In perception, we are actively engaged in achieving the things around us, achieving the world. And, in art, this habitual way of attainment is obstructed, for art presents us with new ways of seeing, listening, feeling, and so forth. Noë thus challenges common (especially neuro-aesthetic) views on perception and experience in favor of a more phenomenological approach by which he accentuates the active engagement in the world by which perception and experience come about. And this aesthetic domain, broadly understood, is the locus of showing how we, through these disruptive practices, create new ways of doing, perceiving, understanding, and, even further, create ourselves. We “all operate in a space of significance held open by art.” In other words, it is through a reassessment of the aesthetic that Noë shows how art and philosophy make us what we are. For what we are, for Noë, is this ability of disruption, development, and creation of ourselves. This is what makes us human. Human nature is therefore essentially cultural, a disruption of habit and new avenues of development, but one that is rooted in our body as a horizon of possibilities. Even though he does not conclude on this phylogenetic question, Noë does seem to suggest that the species’s ability to generate art (exemplified by early cave paintings) and philosophy are foundational [End Page 724] to its development or, more strongly, necessary to it being what it is. He thus proposes an alternative to the traditional schism of the nature/nurture and natural/cultural debate, which takes this wide view of aesthetics as its central term.One might raise the question whether the text would have benefited from more thorough analyses of the thinkers who are at play, sometimes at the margin, sometimes at the foundation of the disruption Noë’s text attempts, notably: Merleau-Ponty, with his extensive analyses of habit and style and their place in our life engaging behavior; Derrida’s deconstruction of phonocentrism, which attacks written language’s subordination to the spoken and asks the fundamental question of the ontology of language as such; or perhaps most present by its absence, Nietzsche’s grandiose thought of the human being and life itself as an aesthetic phenomenon. For Nietzsche thereby incorporated both ontology and critique in the sphere of aesthetics in his revaluation of all values. This, quite like Noë, allowed Nietzsche to set forth the human being as a creature of its own creation. In this article I assess the Invariance Principle, which states that only quantities that are invariant under the symmetries of our theories are physically real. I argue, contrary to current orthodoxy, that the variance of a quantity under a theory’s symmetries is not a sufficient basis for interpreting that theory as being uncommitted to the reality of that quantity. Rather, I argue, the variance of a quantity under symmetries only ever serves as a motivation to refrain from any commitment to (...) the quantity in question. (shrink)

There exists a common view that for theories related by a ‘duality’, dual models typically may be taken ab initio to represent the same physical state of affairs, i.e. to correspond to the same possible world. We question this view, by drawing a parallel with the distinction between ‘interpretational’ and ‘motivational’ approaches to symmetries.

The conceptual pair of own-world and common world constitutes an archaic pair, originally introduced by Heraclitus. More than two millennia after its introduction, Binswanger picked up this conceptual pair in the attempt to understand existence and mental disorder. Ever since, this conceptual pair has been part of the conceptualization of schizophrenia in phenomenological psychopathology. However, the concepts of ídios kósmos and koinós kósmos have seldomly been elaborated and expanded upon, and certain unclarities rest within the literature. This paper resolves some (...) of these unclarities and contributes to a theory of anthropological proportions, which furthers our understanding of ídios kósmos and koinós kósmos. The paper begins with an exploration of the theoretical underpinnings of this conceptual pair, i.e., it discusses the concept of world, what is understood by common world, and what it means to live in an own-world. This theoretical exploration segues into an in-depth analysis of the alterations of experiential life in schizophrenia, which can be illuminated via this conceptual pair. It is further argued that this conceptual pair contributes to an understanding of the puzzling phenomenon of double bookkeeping. Finally, the paper concludes in a discussion of clinical implications of this conceptual pair, stressing the importance of a mindfulness of the alterations to the ídios kósmos and koinós kósmos in schizophrenia. Quantum mechanical measurements on a physical system are represented by observables - Hermitian operators on the state space of the observed system. It is an important question whether all observables may be realized, in principle, as measurements on a physical system. Dirac’s influential text ( [1], page 37) makes the following assertion on the question: The question now presents itself – Can every observable be measured? The answer theoretically is yes. Post-conflict economies often prove daunting for foreign investors. Many of these nations are reaching out to diasporans, emigrants, and their descendants living abroad, for much-needed foreign investment capital. Little is known about why diasporans invest in their countries of origin. Recent scholarly inquiry regarding investment decision making has suggested that non-pecuniary, psychological concerns often motivate investment decisions. We develop a conceptual model identifying three types of investment return expectations — financial, emotional, and those related to social status — that may (...) drive diaspora investment interest. We contend that the psychological realities and social-status structure of the diaspora experience generate greater salience for non-pecuniary investment motivations, particularly when the origin country emerges from a period of conflict. We outline an agenda for future research and discuss the implications of our model for policies seeking to promote peace utilizing diaspora investment. (shrink)

Ethical problems with stakeholder relationships sometimes occur because organizational members are operating out of narrowly scripted organizational routines that do not include an explicit ethical component. In this article we describe methods of single-loop, double-loop, and triple-loop action learning that aim at helping organizational members develop understandings that incorporate conscious ethical considerations in stakeholder relationships. We describe the extent to which these methods achieve "second-order" change in the direction of ethical stakeholder relationships, give examples of the use of each of (...) the methods, and we consider their strengths and limitations for achieving the desired change. The objective of this study is to test the embeddedness of codes of ethics (ECE) in organizations on aggregated data from three countries, namely Australia, Canada and the United States. The properties of four constructs of ECE are described and tested, including surveillance/training, internal communication, external communication and guidance. The data analysis shows that the model has satisfactory fit, validity and reliability. Furthermore, the results are fairly consistent when tested on each of the three samples (i.e. cross-national validation). This cross-national (...) study makes a contribution beyond previous descriptive or exploratory studies by using confirmatory factor analysis and structural equation modeling. Nevertheless, a number of limitations are raised, all of which provide opportunities for further research in refining, extending and testing the proposed ECE model in other cultural and corporate settings. I suggest a description of the theory that I find applicable to understanding self-knowledge. The theory of its own complexity focuses on the structure of individual thoughts about themselves. Own complexity concerns two features of a person's self-determination: the number of social roles that a person has, and the ability of a person to differentiate among these roles. For example, I would be considered a weak bearer of the idea of ​​my own complexity if I considered myself as the bearer (...) of a relatively small number of roles and I would describe existing roles as similar to those that I carry out. I would have a greater degree of my own complexity if I looked at the number of roles I increased and made more distinctions between them. (shrink)

The objective of this paper is to outline challenges associated with the inclusion of welfare issues in breeding goals for farm animals and to review the currently available methodologies and discuss their potential advantages and limitations to address these challenges. The methodology for weighing production traits with respect to cost efficiency and market prices are well developed and implemented in animal breeding goals. However, these methods are inadequate in terms of assessing proper values of traits with social and ethical values (...) such as animal welfare, because such values are unlikely to be readily available from the product prices and costs in the market. Defining breeding goals that take animal welfare and ethical concerns into account, therefore, requires new approaches. In this paper we suggest a framework and an approach for defining breeding goals, including animal welfare. The definition of breeding goals including values related to animal welfare requires a multidisciplinary approach with a combination of different methods such as profit equations, stated preference techniques, and selection index theory. In addition, a participatory approach involving different stakeholders such as breeding organizations, food authorities, farmers, and animal welfare organizations should be applied. We conclude that even though these methods provide the necessary tools for considering welfare issues in the breeding goal, the practical application of these methods is yet to be achieved. (shrink)

This article focuses on an advertisement campaign run in Danish national newspapers promoting Boeing’s combat aircraft F 18 Super Hornet. The campaign received extensive media attention due to its scale and unconventional methods. On the basis of pragmatic text analysis we describe three features in the advertisements: Genre problems, a controversial depiction of sender and recipient, and problems relating to argumentation. We conclude that the analyzed text is predominantly commercial in intent, although framed as information by a sender position that (...) is partly ambiguous in terms of identity, and the campaign’s main arguments are flawed, since decisive justification is not accessible. Based on the findings, the conclusion suggests that the campaign is best understood as a hybrid between public relations and public affairs. (shrink)

Do continuing education (CE) mandates increase participation in ethics programs and enhance their perceived outcomes? In a study of 5,198 North American psychologists, significant differences were found between mandated and nonmandated psychologists in relation to their participation in ethics programs but not in the perceived outcomes associated with those trainings. Although 64.3% of those psychologists operating under ethics mandates reported completing at least one ethics training within the previous year, only 40.7% of those without such mandates reported doing likewise. Overall, (...) both groups tended to view their ethics training quite favorably in relation to its perceived outcomes, though they differed in relation to their endorsement of CE mandates. Results are discussed in relation to the ongoing development of evidence-based CE and its role in developing critical professional competencies. (shrink)

Although there is a growing recognition that older adults and those with extensive comorbid conditions undergo cancer screening too frequently, there is little information about patients’ perceptions regarding cessation of cancer screening. Information on older adults’ views of screening cessation would be helpful both for clinicians and for those designing interventions to reduce overscreening.

If it is true, as suggested by Sir Michael Marmot and other researchers, that status impacts health and therefore accounts for some of the social gradient in health, then it seems to be the case that it would be possible to bring about more equality in health by equalizing status. The purpose of this article is to analyze this suggestion. First, we suggest a working definition of what status precisely is. Second, following a luck egalitarian approach to distributive justice, we (...) consider whether and to which extent individuals are responsible themselves for their position in a status hierarchy. Third, we consider the contours of a difficult question, namely which political measures are feasible in order to reduce health-affective inequalities in status and fourth, whether or to what extent such measures are legitimate. We argue that on the basis of these considerations, we have at least some prima facie reasons to counter (at least some) status inequalities in order to equalize health. (shrink)
