There is limited guidance on how to assess the ethical acceptability of research risks that extend beyond research participants to third parties (or “research bystanders”). Community or stakeholder engagement has been proposed as one way to address potential harms to community members, including bystanders. Despite widespread agreement on the importance of community engagement in biomedical research, this umbrella term includes many different goals and approaches, agreement on which is ethically required or recommended for a particular context. We analyse the case (...) of a potential Zika virus human challenge trial to assess whether and how community engagement can help promote the ethical acceptability of research posing risks to bystanders. We conclude that, in addition to having intrinsic value, community engagement can improve the identification of bystander risks, effective approaches to minimizing them, and transparency about bystander risks for host communities. Objectives To conduct an independent evaluation of the first phase of the Health Foundation’s Safer Patients Initiative (SPI), and to identify the net additional effect of SPI and any differences in changes in participating and non-participating NHS hospitals. Design Mixed method evaluation involving five substudies, before and after design. Setting NHS hospitals in the United Kingdom. Participants Four hospitals (one in each country in the UK) participating in the first phase of the SPI (SPI1); 18 control hospitals. Intervention The SPI1 (...) was a compound (multi-component) organisational intervention delivered over 18 months that focused on improving the reliability of specific frontline care processes in designated clinical specialties and promoting organisational and cultural change. Results Senior staff members were knowledgeable and enthusiastic about SPI1. There was a small (0.08 points on a 5 point scale) but significant (P<0.01) effect in favour of the SPI1 hospitals in one of 11 dimensions of the staff questionnaire (organisational climate). Qualitative evidence showed only modest penetration of SPI1 at medical ward level. Although SPI1 was designed to engage staff from the bottom up, it did not usually feel like this to those working on the wards, and questions about legitimacy of some aspects of SPI1 were raised. Of the five components to identify patients at risk of deterioration—monitoring of vital signs (14 items); routine tests (three items); evidence based standards specific to certain diseases (three items); prescribing errors (multiple items from the British National Formulary); and medical history taking (11 items)—there was little net difference between control and SPI1 hospitals, except in relation to quality of monitoring of acute medical patients, which improved on average over time across all hospitals. Recording of respiratory rate increased to a greater degree in SPI1 than in control hospitals; in the second six hours after admission recording increased from 40% (93) to 69% (165) in control hospitals and from 37% (141) to 78% (296) in SPI1 hospitals (odds ratio for “difference in difference” 2.1, 99% confidence interval 1.0 to 4.3; P=0.008). Use of a formal scoring system for patients with pneumonia also increased over time (from 2% (102) to 23% (111) in control hospitals and from 2% (170) to 9% (189) in SPI1 hospitals), which favoured controls and was not significant (0.3, 0.02 to 3.4; P=0.173). There were no improvements in the proportion of prescription errors and no effects that could be attributed to SPI1 in non-targeted generic areas (such as enhanced safety culture). On some measures, the lack of effect could be because compliance was already high at baseline (such as use of steroids in over 85% of cases where indicated), but even when there was more room for improvement (such as in quality of medical history taking), there was no significant additional net effect of SPI1. There were no changes over time or between control and SPI1 hospitals in errors or rates of adverse events in patients in medical wards. Mortality increased from 11% (27) to 16% (39) among controls and decreased from 17% (63) to 13% (49) among SPI1 hospitals, but the risk adjusted difference was not significant (0.5, 0.2 to 1.4; P=0.085). Poor care was a contributing factor in four of the 178 deaths identified by review of case notes. The survey of patients showed no significant differences apart from an increase in perception of cleanliness in favour of SPI1 hospitals. Conclusions The introduction of SPI1 was associated with improvements in one of the types of clinical process studied (monitoring of vital signs) and one measure of staff perceptions of organisational climate. There was no additional effect of SPI1 on other targeted issues nor on other measures of generic organisational strengthening. (shrink)

On June 16, 1966, the New England Journal of Medicine published “Ethics and Clinical Research” by Henry K. Beecher. Beecher’s account of 22 examples of unethical contemporary clinical research shook up the medical profession and helped pave the way for U.S. federal regulation of research involving human subjects. Five decades later, in this issue of Perspectives in Biology and Medicine, we pay tribute to the lasting significance of this whistle-blowing article and to the remarkable contributions of Henry Beecher. We study the connection between Schnorr triviality and genericity. We show that while no 2-generic is Turing equivalent to a Schnorr trivial and no 1-generic is tt-equivalent to a Schnorr trivial, there is a 1-generic that is Turing equivalent to a Schnorr trivial. However, every such 1-generic must be high. As a corollary, we prove that not all K-trivials are Schnorr trivial. We also use these techniques to extend a previous result and show that the bases of cones of Schnorr (...) trivial Turing degrees are precisely those whose jumps are at least 0". (shrink)

In this paper I will examine the history of the first three, of a sequence of five, experiments performed by the Mann-O'Neill collaboration at the Princeton-Pennsylvania Accelerator. The experiments were conducted over a period of four years and measured aspects of K+ meson decay. Each of the experiments was done with essentially the same basic apparatus, with modifications for each of the specific measurements. We will see the increasing expertise of the experimenters as the experiments progressed. In 1934, Enrico Fermi proposed a theory of beta decay. Although it was supported by existing experimental evidence, a more detailed examination revealed discrepancies. Emil Konopinski and George Uhlenbeck proposed an alternative theory that better fit the results and was accepted by the physics community. It was later realized that both the experimental results and the experiment-theory comparison were incorrect. When both problems were corrected, in part by the work of Konopinski and Uhlenbeck themselves, Fermi’s theory was supported. As Konopinski (...) publicly stated in a 1943 review article, “Thus, the evidence of the spectra, which has previously comprised the sole support for the K-U theory, now definitely fails to support it”. A real is Martin-Löf (Schnorr) random if it does not belong to any effectively presented null ${\Sigma^0_1}$ (recursive) class of reals. Although these randomness notions are very closely related, the set of Turing degrees containing reals that are K-trivial has very different properties from the set of Turing degrees that are Schnorr trivial. Nies proved in (Adv Math 197(1):274–305, 2005) that all K-trivial reals are low. In this paper, we prove that if ${{\bf h'} \geq_T {\bf 0''}}$ , then h (...) contains a Schnorr trivial real. Since this concept appears to separate computational complexity from computational strength, it suggests that Schnorr trivial reals should be considered in a structure other than the Turing degrees. To describe a book as “long-awaited” is a reviewer's cliché. In the case of this book, however, it is a gross understatement. Materials for the new edition of Malalas were initially assembled by K. Weierholt. In the mid–1970s, after Weierholt's death, the project was taken over by Hans Thurn. By the time of Thurn's own death in 1993 the edition was almost ready, but still a further seven years, and the assistance of several other scholars, were required before Ioannis Malalae (...) Chronographia finally appeared. In the interim we had the curious phenomenon of the “Australian Malalas” — an English translation and apparatus based on a far fuller set of testimonia than the edition on which it was principally based (Dindorf's Bonn edition of 1831, shorn of that edition's misreadings of the main witness, MS Baroccianus Graecus 182 in the Bodleian Library). Industries of production and scientific research rely on the use of nonhuman animals and plants, remaking environments, populations, and even genetic information to suit human designs. This issue of _Social Text_ considers the radical implications of questioning the exceptional status of humans among the planet’s species. Responding to growing interest in animal studies and posthumanism, the contributors draw on racial, feminist, queer, postcolonial, and disability theories to probe the diversity of human relationships with other forms of biosocial life. “Interspecies” queries (...) the politics of traditional species taxonomy and examines the ways humans use the material characteristics of other species to pursue their economic, political, and social aims. This collection goes beyond companionate species to examine less charismatic life forms: viruses, vermin, transgenic pigs, and commodified plants. Bringing together prominent scholars and artists from a range of fields, the issue examines the histories of species collection and display. In the context of current public health challenges, including the swine flu epidemic and the scarcity of donor organs, the contributors explore the limits of transgressing species boundaries that arise when human bodies contain other species, such as viruses or transplanted organs from genetically customized pigs. “Interspecies” analyzes the use of nonhuman species in the biopolitics of warfare and torture and examines how interspecies relationships shape conditions of colonialism, imprisonment, and violence. Bu değişiklikler genellikle olumlu olsa da bazı tarih yazımı tartışmaları bilim tarihi metinlerine yeterince yansımamıştır. Bu yazıda Rosalind Franklin örneği üzerinden kadın bilimcilerin bilim tarihinde nasıl ele alındığını tartıştım. Kadınlara bilim tarihinde hak ettikleri yeri verme çabamız sırasında bilim tarihçileri olarak nelere dikkat etmemiz gerektiğini ele aldım. Özellikle vurguladığım noktalar bilimsel araştırma biçimlerinin doğru yansıtılması, kurumsal ayrımcılıkların üstünün örtülmesinden kaçınılması ve ele alınan figürün bilime katkısının (...) açıkça yazılması oldu. Yazıda Franklin hakkındaki literatüre bir katkı sunmayı amaçladım ancak daha da önemlisi Franklin başta olmak üzere kadın bilimciler hakkındaki metinlerde sık yapılan hatalara işaret etmek istedim. -/- Following the realization that history cannot be written objectively, there have been some transformations in the historiography of science. Although these changes are generally positive, some historiography debates have not been adequately reflected in historical texts on science. In this article, I discussed how women scientists are represented in the history of science through the case of Rosalind Franklin. In our effort to give women the place they deserve in the history of science, I have discussed what should we pay attention to as historians of science. My particular emphasis was on reflecting the structure of scientific research correctly, avoiding covering institutional discrimination and clearly writing the contribution of the figure discussed to science. I intend to make a contribution to the literature on Franklin, but more importantly, I tried to point out some common mistakes in texts about women scientists, especially Franklin. (shrink)
