Ethics, in the sense of a recognized branch of inquiry, reputedly began with Socrates and the Sophists, at least for the western world. Ethics, understood as a set of moral standards, traditionalized by maxims and admonitions, has existed in human cultures from so early a time that it would be hazardous indeed to conjecture the date of its probable origin. The "Hopi Ethics" which Mr. Brandt has studied is obviously that of this second sense, whereas his own study, at least (...) insofar as it is a "theoretical analysis" and goes beyond mere description, is "ethics" in the first sense. That the Hopi Indians were somewhat astonished and perplexed by Mr. Brandt's line of inquiry is indicated by some of their quoted remarks, such as their mention of the "hard questions" being put to them, and their amazement at the suggestion of the possibility that "there is no right answer to ethical questions." One gathers, thus, that while the problem of ethical relativism was of high concern to Mr. Brandt, such an idea was nigh unthinkable to the Hopi. (shrink)

Like much of modern scholarship, the study of social problems today is usually conducted in isolation from the truths of faith. Yet Catholics understand that the truths of science and the truths of faith are not in opposition but in harmony. This paper uses the Catholic concept of transcendent human dignity to integrate the scientific analysis of social problems with the Church’s understanding of man. This integral approach places the social scientist on a firm footing from which to identify the (...) principal social problems of our day and to clarify the appropriate solutions, which would guard the dignity of the human person and facilitate his true flourishing. The clinical clerkships in medical school are the first formal opportunity for trainees to apply bioethics concepts to clinical encounters. These clerkships are also typically trainees’ first sustained exposure to the “reality” of working in clinical teams and the full force of the challenges and ethical tensions of clinical care. We have developed a specialized, embedded ethics curriculum for Vanderbilt University medical students during their second year to address the unique experience of trainees’ first exposure to clinical care. Our embedded (...) curriculum is centered around core “ethics competencies” specific to the clerkship: for Medicine, advanced planning and end-of-life discussions; for Surgery, informed consent; for Pediatrics, the patient-family-provider triad; for Obstetrics and Gynecology, women’s autonomy, unborn child’s interests, and partner’s rights; and for Neurology/Psychiatry, decision-making capacity. In this paper, we present the rationale for these competencies, how we integrated them into the clerkships, and how we assessed these competencies. We also review the additional ethical issues that have been identified by rotating students in each clerkship and discuss our strategies for continued evolution of our ethics curriculum. (shrink)

This study investigated the relationship between concussions and medication adherence among 247 adults experiencing homelessness in Oklahoma City, Oklahoma, who were prescribed medication for a psychiatric disorder. Participants were asked whether they had “ever experienced a blow to the head that caused a concussion,” and medication adherence was measured by asking participants whether they had taken their psychiatric medication yesterday. The data were analyzed using univariate and multivariable logistic regressions. Results showed that more than half of the sample had a (...) concussion history, and homeless adults with a concussion history had higher odds of non-adherence to psychiatric medications compared with those who reported no concussion history [OR = 2.13 ]. Findings suggest that medication non-adherence is associated with incurred concussions. Raising awareness among service providers of the relationship between traumatic brain injury and medication adherence may increase efforts to improve adherence in this underserved population. More than twenty-five years after the first signs of potential harm, the US remains locked in the grip of an opioid epidemic, with more Americans dying from overdoses than ever before.1 Diversion of prescription opioids plays an important role in opioid-related harms. Much of the scientific and public health focus on diversion has been on end-users, given how commonly non-medical prescription opioid use occurs, as well as the proportion of individuals who report that their source of non-medical opioids was friends (...) or family. However, diversion of opioids, as well as their rampant oversupply, can be discerned higher up the supply chain, including among wholesalers, pharmacies and rogue prescribers whose behavior may trigger well-described “flags” warranting further evaluation and action. The proposed solution to the problem of skepticism regarding the meaning of a linguistic expression is based on the idea of language and speech activity, limited to the act of use. Such a truncated and one-sided version of the analysis of language presents a picture where the only subject capable of understanding the meanings of words is the speech agent himself. Although this approach (...) partially allows us to resolve the skeptical problem, it arises again when we pay attention not to the agent of speech, but to the recipient to whom speech activity is directed. The late Benjamin L. Whorf proposed "that the linguistic system of each language is not merely a reproducing instrument for voicing ideas but rather is itself the shaper of ideas, the program and guide for the individual's mental activity, for his analysis of impressions, for his synthesis of his mental stock in trade". Whorf went further, maintaining that each language conceals a hidden metaphysics, and that this metaphysics has a profound influence on normal behavior patterns. Whorf was a disciple of (...) the late Edward Sapir, who earlier but more reservedly suggested the thesis of linguistic influence on thought and action. Whorf, in his enthusiasm, tended toward the extreme view that thought worlds rest on language to such an extent that they defy ultimate translation from one language to another. Even an individual equally versed in two languages could only sense the presence of the basic linguistic difference, but could never verbalize it completely in either language. Such a thesis is called "the linguistic relativity principle" by Franklin Fearing. Joseph Greenberg, on the other hand, though shying away from "absolutes" of translatable meaning, sees the end of scientific linguistics unless some common experience is assumed to be expressible between languages. The ablest solution to this problem appears to be the one proposed by Stanley Newman in a technique of translation through "multiple equivalents". R. W. Sharples provides a new edition, with introduction and commentary in English, of the Greek text. The Mantissa is a collection of short discussions, transmitted as a supplement to the treatise On the Soul by the Aristotelian commentator Alexander of Aphrodisias (c.200 AD).The collection includes discussion of a range of topics, among them the nature of soul and intellect, theories of how seeing takes place, issues in ethics, and the nature of fate. The text is based upon a (...) new collation of the principal manuscript, the ninth century Venetus Marcianus graecus 258, and the apparatus corrects Bruns' misreportings of the principal manuscript and of the others that he used. Account has also been taken of the medieval Arabic and Latin versions of some of the sections which circulated independently, notably On Intellect which had a substantial influence on medieval philosophy. The introduction is chiefly concerned with the manuscripts and the relation between them. The commentary is based on the notes to the editor's English translation of the work (London: Duckworth and Ithaca: Cornell University Press, 2004); however, the commentary also takes into account more recent work on the collection by various scholars. Changes in healthcare financing increasingly rely upon patient cost-sharing to control escalating healthcare expenditures. These changes raise new challenges for physicians that are different from those that arose either under managed care or traditional indemnity insurance. Historically, there have been two distinct bases for arguing that physicians should not consider costs in their clinical decisions—an “aspirational ethic” that exhorts physicians to treat all patients the same regardless of their ability to pay, and an “agency ethic” that calls on physicians to (...) be trustworthy advisors to their patients. In the setting of greater patient cost-sharing, physicians' aspiration and agency roles increasingly conflict. Satisfactorily navigating the new terrain of consumer-driven healthcare requires physicians to consider these two roles and how they can best be reconciled so as to maximize quality of care while respecting the heterogeneity of patients' financial resources and willingness to pay. The growth of managed care was accompanied by concern about the impact that changes in health care organization would have on the doctor-patient relationship. We now are in a “post-managed care era,” where some of these changes in health care delivery have come to pass while others have not. A re-examination of the DPR in this setting suggests some surprising results. Rather than posing a new and unprecedented threat, managed care was simply the most recent of numerous strains on the (...) DPR that have occurred throughout the century. These strains are a constant, inevitable consequence of the varying needs and concerns of patient and physicians as they seek to balance their desires for a certain type of DPR with their simultaneous desire for other aspects of care such as lower costs, greater technological sophistication, and improved outcomes. Christianity has become the religion of choice for a growing number of diasporic Chinese. This qualitative study examines the conversion narratives of first-generation Chinese converts to evangelical Protestant Christianity, exploring in particular the spiritual experiences that encouraged or confirmed their resolve to become Christians. Utilizing the method of Interpretative Phenomenological Analysis, twenty English-speaking participants were interviewed, mostly immigrants to the United States or the United Kingdom from China, including Hong Kong, or from Taiwan. The converts to Christianity in this study (...) were in search of an intellectually plausible faith, and an overarching framework of meaning, purpose, and fullness for their lives, and were motivated as well by longings for community and belonging. Participants reported being profoundly “touched”—inexplicably moved—by the love extended to them by other Chinese Christians, and being transformed in wide-ranging and durable ways as a result. In their view, such love was qualitatively different from, and superior to, the more transactional relational dynamics with which they had been familiar. They inferred that it was “otherworldly,” and necessarily emanated from a real and unconditionally benevolent divine source. The study thus illuminates the spiritual dynamics that persist in Chinese Christian conversions, even in a secular age. (shrink)

Corrupt behavior presents major challenges for organizations in a wide range of settings. This article embraces a complexity theoretical perspective to elucidate the causal patterns of factors underlying consumers’ unethical judgments. This study examines how causal conditions of four distinct domains combine into configurational causes of unethical judgments of two frequent forms of corrupt consumer behavior: shoplifting and fare dodging. The findings of fuzzy-set Qualitative Comparative Analyses indicate alternative, consistently sufficient “recipes” for the outcomes of interest. This study extends prior (...) work on the topic by offering new insights into the interplay and the interconnected structures of multiple causal factors and by describing configurational causes of consumers’ ethical evaluations of corrupt behaviors. This knowledge may support practitioners and policy makers to develop education and control approaches to thwart corrupt consumer behaviors. (shrink)

“Parting is such sweet sorrow.” Taste metaphors provide a rich vocabulary for describing emotional experience, potentially serving as an adaptive mechanism for conveying abstract emotional concepts using concrete verbal references to our shared experience. We theorized that the popularity of these expressions results from the close association with hedonic valence shared by these two domains of experience. To explore the possibility that this affective quality underlies the semantic similarity of these domains, we used a behavioral “odd-one-out” task in an online (...) sample of 1059 participants in order to examine the semantic similarity of concepts related to emotion, taste, and color, another rich source of sensory metaphors. We found that the semantic similarity of emotion and taste concepts was greater than that of emotion and color concepts. Importantly, the similarity of taste and emotion concepts was strongly related to their similarity in hedonic valence, a relationship which was also significantly greater than that present between color and emotion. These results suggest that the common core of valence between taste and emotion concepts allows us to bridge the conceptual divide between our shared sensory environment and our internal emotional experience. Here we evaluate our current understanding of the function of the nervous system in Hydra, a non‐bilaterian animal which is among the first metazoans that contain neurons. We highlight growing evidence that the nervous system, with its rich repertoire of neuropeptides, is involved in controlling resident beneficial microbes. We also review observations that indicate that microbes affect the animal's behavior by directly interfering with neuronal receptors. These findings provide new insight into the original role of the nervous system, and suggest (...) that it emerged to orchestrate multiple functions including host‐microbiome interactions. The excitement of future research in the Hydra model now relies on uncovering the common rules and principles that govern the interaction between neurons and microbes and the extent to which such laws might apply to other and more complex organisms. Physicians have special professional obligations to respond to medical emergencies. A bioterrorism attack would be a medical emergency. Thus, it seems that physicians would have an obligation to respond to a bioterrorist attack. However, the scope of those obligations, and their limits, are vexed topics. General rules may be comforting but the details and nuances of particular situations will always be relevant.
