The multiobjective optimization on the basis of ratio analysis method captures diverse features such as the criteria and alternatives of appraising a multiple criteria decision-making problem. At the same time, the multiple criteria problem includes a set of decision makers with diverse expertise and preferences. In fact, the literature lists numerous approaches to aid in this problematic task of choosing the best alternative. Nevertheless, in the MCDM field, there is a challenge regarding (...) intangible information which is commonly involved in multiple criteria decision-making problem; hence, it is substantial in order to advance beyond the research related to this field. Thus, the objective of this paper is to present a fused method between multiobjective optimization on the basis of ratio analysis and Pythagorean fuzzy sets for the choice of an alternative. Besides, multiobjective optimization on the basis of ratio analysis is utilized to choose the best alternatives. Finally, two decision-making problems are applied to illustrate the feasibility and practicality of the proposed method. (shrink)

In this paper, we define correlation coefficient measure between any two rough neutrosophic sets. We also prove some of its basic properties.. We develop a new multiple attribute group decision making method based on the proposed correlation coefficient measure. An illustrative example of medical diagnosis is solved to demonstrate the applicability and effecriveness of the proposed method.

Incorporation of the behavioral issues of the decision maker (DM) is among the aspects that each Multicriteria Decision Making (MCDM) method implicitly or explicitly takes into account. As postulated by regret theory, the feelings of regret and rejoice are among the behavioral issues associated with the entire decision making process. Within the context of MCDM, the DM may feel regret, when the chosen alternative is compared with another one having at least one better criterion value. (...) PROMETHEE II is a widely known MCDM method that makes no explicit incorporation of regret attitude of the DM. In this paper, we elaborate on the applicability of regret theory to MCDM context. In particular, we investigate the findings of regret theory and explore the parallel between regret theory and PROMETHEE II method. Relying on the concepts of regret theory, we demonstrate how a decision that is made using a PROMETHEE II based outranking method conforms to the regret attitude of the DM. (shrink)

Decisions related to animal welfare standards depend on farmer’s multiple goals and values and are constrained by a wide range of external and internal forces. The aim of this paper is twofold, i.e., to develop a theoretical framework for farmers’ AW decisions that incorporates farmers’ goals, use and non-use values and to present an approach to empirically implement the theoretical framework. The farmer as a head of the farm household makes choices regarding production to maximize the utility of the (...) household. The overall utility of the farmer is determined by his multiple objectives. For the analysis of multi-objective problems, the multiple criteria decision - making paradigm provides an appropriate theoretical framework. However, theories from the field of social-psychology are needed to facilitate the identification of all relevant aspects in the decision making. The practical use of the conceptual framework is demonstrated using a simple numerical application of a multi-objective programming model. Two workshops were devoted to examining the scientific consistency and the practical usefulness of the approach. Implementing this approach will increase knowledge of the main factors and barriers that determine farmers’ decisions with regard to AW standards. This knowledge is relevant during the development of new AW concepts that aims to supply products that comply with above-legal AW standards for middle-market segments. (shrink)

In this paper, I will explore the relationship between ideal and nonideal theories and bioethics. I explore the challenges we face in their implementation in the real world. I argue that there is no sole theory that can guide the multiplicity of real world cases and policies. If we consider there are a variety of possibilities, we should ask several questions: When should we use NITs and when should we opt for ITs? Are there appropriate criteria for making (...) these decisions? Which are the valid uses or justifications for NITs? Is there a way to guide the decision-making process in real world contexts? This paper will try to answer the above questions. In the first part of this paper, I make some distinctions to present the theoretical arena and debate, and I propose five criteria that can help our decision making process. In the second part, I introduce two cases focused on reproductive rights-related problems in order to apply the decision-making process developed in the first part. The first one examines conscientious objection in the case of legal abortion in Argentina and the second one analyzes the case of research with pregnant women in countries where abortion is illegal. In one case, I argue that we ought to follow NIT, while in the other we should follow IT. (shrink)

Citizens can be meaningfully involved in multiple phases of the urban planning process from decision-making to implementation via a dedicated online platform through which they can interact with planners and decision-makers. In historical contexts, local people are essential resources for decision-makers seeking critical local information needed for effective planning and intervention—including what those citizens recall from the past about the area’s social values and the built environment and what they imagine and hope for their neighborhood’s (...) future. This public knowledge, collected through storytelling and mapping, can play a fundamental role in shaping the framework for neighborhood rehabilitation plans. As a foundation for designing an online visual questionnaire to collect local knowledge and understand the views and expectations of local people, this study focuses on theoretical ideas to define a framework for an online visual questionnaire tool. The tool presented provides a common visual language that people can easily understand and use whatever their social class, education, or background. It also has the benefit of enabling citizens and planners to interact directly regardless of their geographical location. To proposing a framework for the online visual questionnaire, the first step was to review some existing visual tools and methods, including analog used for community engagement and digital used in urban planning to encourage and support public participation. Through an analysis of these methods, some important variables and criteria were identified that further our understanding of citizens’ perceptions of their neighborhoods. The next step, as part of a larger study, will be examining a historical public space, including a bazaar, hammam, and cistern—located in the historical context of Yazd— a central city in Iran registered as a cultural heritage site by UNESCO— as a case study with the larger goal of identifying some specific. On the basis of the combination of single-valued neutrosophic sets and hesitant fuzzy sets, this article proposes a single-valued neutrosophic hesitant fuzzy set as a further generalization of the concepts of fuzzy set, intuitionistic fuzzy set, single-valued neutrosophic set, hesitant fuzzy set, and dual hesitant fuzzy set. Then, we introduce the basic operational relations and cosine measure function of SVNHFSs. Also, we develop a single-valued neutrosophic hesitant fuzzy weighted averaging operator and a single-valued neutrosophic hesitant fuzzy weighted geometric operator and (...) investigate their properties. Furthermore, a multiple-attribute decision-making method is established on the basis of the SVNHFWA and SVNHFWG operators and the cosine measure under a single-valued neutrosophic hesitant fuzzy environment. In decision-making involving multiple criteria or attributes, a decision maker first identifies all relevant evaluative attributes in making decisions. Then, a dominance principle is often invoked whenever applicable: whenever an option x is better than an option y in terms of some attribute and no worse than y in terms of any other attributes, x is judged to be better than y. If, however, this dominance principle is not applicable, then the decision maker (...) determines the relative importance between the identified evaluative attributes, consults with contextual features of the options under consideration, and makes a decision. It is shown that the combination of these principles runs into problems in the presence of rationality properties, such as transitivity, and a weak continuity requirement on decisions. This paper proposes several concepts of efficient solutions for multicriteria decision problems under uncertainty. We show how alternative notions of efficiency may be grounded on different decision ‘contexts’, depending on what is known about the Decision Maker's (DM) preference structure and probabilistic anticipations. We define efficient sets arising naturally from polar decision contexts. Many decisions involve multiple stages of choices and events, and these decisions can be represented graphically as decision trees. Optimal decision strategies for decision trees are commonly determined by a backward induction analysis that demands adherence to three fundamental consistency principles: dynamic, consequential, and strategic. Previous research found that decision-makers tend to exhibit violations of dynamic and strategic consistency at rates significantly higher than choice inconsistency across various levels of potential reward. The current research extends (...) these findings under new conditions; specifically, it explores the extent to which these principles are violated as a function of the planning horizon length of the decision tree. In this paper, we propose a new hybrid neutrosophic multi criteria decision making (MCDM) framework that employs a collection of neutrosophic analytical network process (ANP), and order preference by similarity to ideal solution (TOPSIS) under bipolar neutrosophic numbers. The MCDM framework is applied for chief executive officer (CEO) selection in a case study at the Elsewedy Electric Group, Egypt. The proposed approach allows us to assemble individual evaluations of the decision makers and therefore perform accurate personnel (...) selection. As a generalization of both single-valued neutrosophic element and hesitant fuzzy element, single-valued neutrosophic hesitant fuzzy element is an efficient tool for describing uncertain and imprecise information. Thus, it is of great significance to deal with single-valued neutrosophic hesitant fuzzy information for many practical problems. In this paper, we study the aggregation of SVNHFEs based on some normalized operations from geometric viewpoint. Firstly, two normalized operations are defined for processing SVNHFEs. Then, a series of normalized aggregation operators which fulfill some (...) basic conditions of a valid aggregation operator are proposed. Additionally, a decision-making method is developed for resolving multiattribute decision-making problems based on the proposed operators. This book synthesizes the game-theoretic modeling of decision-making processes and an ancient moral requirement, called the Golden Rule of ethics (GR). This rule states, "Behave to others as you would like them to behave to you." The GR is one of the oldest, most widespread and specific moral requirements that appear in Christianity, Islam, Judaism, Buddhism, and Confucianism. The book constructs and justifies mathematical models of dynamic socio-economic processes and phenomena that reveal the mechanism of the GR and (...) are based on the concept of Berge equilibrium. The GR can be naturally used for resolving or balancing conflicts, and its "altruistic character" obviously excludes wars, blood-letting, and armed clashes. The previous book of the authors, The Berge Equilibrium: A Game-Theoretic Framework for the Golden Rule of Ethics, covers the static case of the GR. In this book, the dynamic case of the GR is investigated using the altruistic concept of Berge equilibrium and three factors as follows: a) a modification of N.N. Krasovskii's mathematical formalization of differential positional games (DPGs), in view of the counterexamples given by A.I. Subbotin and A.F. Kononenko; b) the method of guiding control, proposed by N.N. Krasovskii; c) the Germeier convolution of the payoff functions of different players. Additionally, the book features exercises, problems, and solution tips collected together in Appendix 1, as well as new approaches to conflict resolution as presented in Appendices 2-4. In today’s society, it becomes increasingly important to assess which non-human and non-verbal beings possess consciousness. This review article aims to delineate criteria for consciousness especially in animals, while also taking into account intelligent artifacts. First, we circumscribe what we mean with “consciousness” and describe key features of subjective experience: qualitative richness, situatedness, intentionality and interpretation, integration and the combination of dynamic and stabilizing properties. We argue that consciousness has a biological function, which is to present the subject with (...) a multimodal, situational survey of the surrounding world and body, subserving complex decision-making and goal-directed behavior. This survey reflects the brain’s capacity for internal modeling of external events underlying changes in sensory state. Next, we follow an inside-out approach: how can the features of conscious experience, correlating to mechanisms inside the brain, be logically coupled to externally observable properties? Instead of proposing criteria that would each define a “hard” threshold for consciousness, we outline six indicators: goal-directed behavior and model-based learning; anatomic and physiological substrates for generating integrative multimodal representations; psychometrics and meta-cognition; episodic memory; susceptibility to illusions and multistable perception; and specific visuospatial behaviors. Rather than emphasizing a particular indicator as being decisive, we propose that the consistency amongst these indicators can serve to assess consciousness in particular species. The integration of scores on the various indicators yields an overall, graded criterion for consciousness, somewhat comparable to the Glasgow Coma Scale for unresponsive patients. When considering theoretically derived measures of consciousness, it is argued that their validity should not be assessed on the basis of a single quantifiable measure, but requires cross-examination across multiple pieces of evidence, including the indicators proposed here. Current intelligent machines, including deep learning neural networks and agile robots, are not indicated to be conscious yet. Instead of assessing machine consciousness by a brief Turing-type of test, evidence for it may gradually accumulate when we study machines ethologically and across time, considering multiple behaviors that require flexibility, improvisation, spontaneous problem-solving and the situational conspectus typically associated with conscious experience. The interval neutrosophic uncertain linguistic variables can easily express the indeterminate and inconsistent information in real world, and TOPSIS is a very effective decision making method more and more extensive applications. In this paper, we will extend the TOPSIS method to deal with the interval neutrosophic uncertain linguistic information, and propose an extended TOPSIS method to solve the multiple attribute decision making problems in which the attribute value takes the form of the interval neutrosophic uncertain (...) linguistic variables and attribute weight is unknown. Firstly, the operational rules and properties for the interval neutrosophic variables are introduced. Then the distance between two interval neutrosophic uncertain linguistic variables is proposed and the attribute weight is calculated by the maximizing deviation method, and the closeness coefficients to the ideal solution for each alternatives. Finally, an illustrative example is given to illustrate the decision making steps and the effectiveness of the proposed method. (shrink)

Critical reasoning (CR) when confronted with contradictory information from multiple sources is a crucial ability in a knowledge-based society and digital world. Using information without critically reflecting on the content and its quality may lead to the acceptance of information based on unwarranted claims. Previous personal beliefs are assumed to play a decisive role when it comes to critically differentiating between assertions and claims and warranted knowledge and facts. The role of generic epistemic beliefs on critical stance and attitude (...) in reflectively dealing with information is well researched. Relatively few studies have been conducted on the influence of domain-specific beliefs, i.e. beliefs in relation to specific content encountered in a piece of information or task, on the reasoning process, and on how these beliefs may affect the information-processing and decision-making processes. This study focuses on students’ task- and topic-related beliefs that may influence their reasoning when dealing with multiple and partly contradictory sources of information. To validly assess CR among university students, we used a newly developed computer-based performance assessment in which the students were confronted with an authentic task which contains theoretically defined psychological stimuli for measuring CR. To investigate the particular role of task- and topic-related beliefs on CR, a purposeful sample of 30 university students took part in a performance assessment and then were interviewed immediately afterwards. In the semi-structured cognitive interviews, the participants’ task-related beliefs were assessed. Based on qualitative analyses of the interview transcripts, three distinct profiles of decision-making among students have been identified. More specifically, the different types of students’ beliefs and attitudes derived from the cognitive interview data suggest their influence on information processing, reasoning approaches and decision-making. The results indicated that the students’ beliefs had an influence on their selection, critical evaluation and use of information as well as on their reasoning processes and final decisions. (shrink)

Institutional suggestions for how to rethink democracy in response to changing state responsibilities and capabilities have been numerous and often mutually incompatible. This suggests that conceptual unclarity still reigns concerning how the normative ideal of democracy as collective self-determination, i.e. ?rule by the people?, might best be brought to bear in a transnational and global context. The aim in this paper is twofold. First, it analyses some consequences of the tendency to smudge the distinction between democratic theory and moral theories (...) of legitimacy and justice. Second, it develops a conceptual framework that distinguishes between necessary conditions, aspects and aims of democracy. On this basis it specifies three objectives of democracy, some of which may also hold for multilevel governance. It is argued that there are in principle at least three reasons to value democratic institutions: they are intrinsically justified to the extent that they distribute fair shares of political influence over decision-making; they are instrumentally justified to the extent that they secure several of our other best interests, one of which is our interest in non-domination; and finally, they are also instrumentally justified insofar as they secure the just distribution of other goods. The aim of this framework is not to develop a specific theory of multilevel governance but to point at important distinctions to be made and normative criteria to be specified. The intention is to take the debate forward by noting some of the issues that any satisfactory account must address. The framework lays out the grounds for analysing the institutional challenges facing legitimate multilevel governance through what is speculatively called ?multiple citizenship?, understood in explorative terms, opening the door for the manifold roles that citizens could and ought to play in multilevel governance, not only as democratic agents, but also as agents of democracy and agents of justice. The problem of merging multiple sources information is central in many information processing areas such as databases integrating problems, multiple criteria decision making, expert opinion pooling, etc. Recently, several approaches have been proposed to merge classical propositional bases, or sets of (non-prioritized) goals. These approaches are in general semantically defined. Like in belief revision, they use priorities, generally based on Dalal's distance, for merging the classical bases and return a new classical base as a result. (...) An immediate consequence of the generation of a classical base is the impossibility of iterating the fusion process in a coherent way w.r.t. priorities since the underlying ordering is lost. This paper presents a general approach for fusing prioritized bases, both semantically and syntactically, when priorities are represented in the possibilistic logic framework. Different classes of merging operators are considered including conjunctive, disjunctive, reinforcement and adaptive operators. We show that the approaches which have been recently proposed for merging classical propositional bases can be embedded in this setting. The result is then a prioritized base, and hence the process can be coherently iterated. Moreover, we also provide a syntactic counterpart for the fusion of classical bases. (shrink)

Critical types of infrastructure are provided by the state to maintain the people’s livelihood, ensure economic development, and systematic government operations. Given the development of ever more complicated critical infrastructure systems, increasing importance is being attached to the protection of the components of this infrastructure to reduce the risk of failure. Power facilities are one of the most important kinds of critical infrastructure. Developing an effective risk detection system to identify potential failure modes of power supply equipment is crucial. This (...) study seeks to improve upon prior approaches for risk assessment by proposing a hybrid risk-assessment model using the concepts of failure mode and effect analysis and multiple-criteria decision-making. The proposed model includes a cost-based factor for decision-makers. The subjectivity and uncertainty in FM assessment are adjusted through the rough number method. The original risk priority number can be expanded by including the entropy weights in the risk index. Furthermore, to rank the risk priorities in a rational manner, a modified technique for order preference by similarity to ideal solution is adopted. The applicability and effectiveness of the proposed method were demonstrated by considering an example of a turbine steam engine in a nuclear power plant. (shrink)

In this paper, we introduce the concepts of Pythagorean fuzzy valued neutrosophic set (PFVNS) and Pythagorean fuzzy valued neutrosophic (PFVNV) constructed by considering Pythagorean fuzzy values (PFVs) instead of numbers for the degrees of the truth, the indeterminacy and the falsity, which is a new extension of intuitionistic fuzzy valued neutrosophic set (IFVNS). By means of PFVNSs, the degrees of the truth, the indeterminacy and the falsity can be given in Pythagorean fuzzy environment and more sensitive evaluations are made by (...) a decision maker in decision making problems compared to IFVNSs. In other words, such sets enable a decision maker to evaluate the degrees of the truth, the indeterminacy and the falsity as PFVs to model the uncertainty in the evaluations. (shrink)

TODIM is a well-known multiple-criteria decision-making which considers the bounded rationality of decision makers based on prospect theory. However, in the classical TODIM, the perceived probability weighting function and the difference of the risk attitudes for gains and losses are not consistent with the original idea of PT. Moreover, probabilistic hesitant fuzzy information shows its superiority in handling the situation that the DMs hesitate among several possible values with different possibilities. Hence, a novel TODIM with (...) probabilistic hesitant fuzzy information is proposed in this paper to simulate the perceptions of the DMs in PT. To show the advantages of the proposed method, a novel TODIM is combined with hesitant fuzzy information. Finally, a case study is carried out to demonstrate the feasibility of the proposed method, and a series of comparative analyses and the sensitivity analyses are used to show the stability of the proposed method. Most jurisdictions require that a mental capacity assessment be conducted using a functional model whose definition includes several abilities. In England and Wales and in increasing number of countries, the law requires a person be able to understand, to retain, to use or weigh relevant information and to communicate one’s decision. But interpreting and applying broad and vague criteria, such as the ability ‘to use or weigh’ to a diverse range of presentations is challenging. By examining actual court (...) judgements of capacity, we previously developed a descriptive typology of justifications used in the application of the Mental Capacity Act criteria. We here critically optimise this typology by showing how clear definitions—and thus boundaries—between the criteria can be achieved if the ‘understanding’ criterion is used narrowly and the multiple rationales that fall under the ability to ‘use or weigh’ are specifically enumerated in practice. Such a typology-aided practice, in theory, could make functional capacity assessments more transparent, accountable, reliable and valid. It may also help to create targeted supports for decision making by the vulnerable. We also discuss how the typology could evolve legally and scientifically, and how it lays the groundwork for clinical research on the abilities enumerated by the MCA. No data are available. Link prediction, which aims to forecast potential or missing links in a complex network based on currently observed information, has drawn growing attention from researchers. To date, a host of similarity-based methods have been put forward. Usually, one method harbors the idea that one similarity measure is applicable to various networks, and thus has performance fluctuation on different networks. In this paper, we propose a novel method to solve this issue by regarding link prediction as a multiple-attribute decision- (...) class='Hi'>making problem. In the proposed method, we consider RA, LP, and CAR indices as the multiattribute for node pairs. The technique for order performance by similarity to ideal solution is adopted to aggregate the multiattribute and rank node pairs. The proposed method is not limited to only one similarity measure, but takes separate measures into account, since different networks may have different topological structures. Experimental results on 10 real-world networks manifest that the proposed method is superior in comparison to state-of-the-art methods. (shrink)

Community detection is one of the key research directions in complex network studies. We propose a community detection algorithm based on a density peak clustering model and multiple attribute decision-making strategy, TOPSIS. First, the two-dimensional dataset, which is transformed from the network by taking the density and distance as the attributes of nodes, is clustered by using the DBSCAN algorithm, and outliers are determined and taken as the key nodes. Then, the initial community frameworks are formed and (...) expanded by adding the most similar node of the community as its new member. In this process, we use TOPSIS to cohesively integrate four kinds of similarities to calculate an index, and use it as a criterion to select the most similar node. Then, we allocate the nonkey nodes that are not covered in the expanded communities. Finally, some communities are merged to obtain a stable partition in two ways. This paper designs some experiments for the algorithm on some real networks and some synthetic networks, and the proposed method is compared with some popular algorithms. Continuous environmental concerns regarding construction industry have been driving general constructors of mega infrastructure projects to incorporate green contractors. Although conventional multiple attributes decision-making methodologies have provided feasible ways to select contractor, high complexity in scenarios of megaprojects still challenges existing MADM methods in concurrently accommodating three key issues of decision hesitancy, attributes interdependency, and group attitudinal character. To elicit decision-makers’ hesitant fuzzy assessments more objectively and comprehensively, we define an expression tool called interval-valued dual (...) hesitant fuzzy uncertain unbalanced linguistic set and develop aggregation operators through its operations. To exploit attributes interdependency, we establish a synthesized attributes’ weighting model to fuse an attributes interdependency-based weighting vector and an argument-dependent weighting vector, which are, respectively, derived through Decision-Making and Trial Evaluation Laboratory technique and maximizing deviation method. To effectively utilize decision-makers’ group attitudinal characters, we also develop a TOPSIS-based method to rationally transform group ideal attitudes into order-inducing vectors. On the strength of the above methods, an integrated MADM approach is then constructed. Finally, illustrative case study and experiments are conducted to validate our approach. (shrink)

In this paper I proceed on the assumption that moral philosophers can and should contribute to the resolution of perplexing moral problems. The ones considered here relate to decisions concerning the distribution of scarce medical resources as between those in need of treatment. I draw on considerations of egalitarianism and concern for the maximization of the use of scarce resources in the task of satisfying basic human needs (such as for good health). I propose certain principles and offer some supporting (...) suggestions as guidelines or rules for medical decisions with a view to providing a helpful decision procedure for the various persons (doctors, community panels and so on) whose task it is to make the life-or-death decisions involved in allocating certain scarce medical resources. -/- . Background Nurses have an essential role in caring for end-of-life patients. Nevertheless, the nurse’s involvement in the passive euthanasia decision-making process is insufficient and lower than expected. Objectives To explore factors associated with nurses’ intention to be involved in non-treatment decisions (NTD) regarding passive euthanasia decision-making versus their involvement in the palliative care of patients requesting euthanasia, using the Theory of Planned Behavior (TPB) framework. Design A cross-sectional study utilizing a random sample. Participants and research context (...) The study was conducted in one of the largest hospitals in Israel among 125 nurses employed in internal and surgical care wards. Data was collected through face-to-face interviews between March and April 2019. Method A closed structured questionnaire was developed according to TPB instructions. A paired sample t test and two multiple hierarchical regressions were conducted. Variance explained (R2) and the significance of F change were calculated for each regression. The study used the STROBE statement guideline. Ethical considerations The study was approved by the hospital’s Helsinki Committee (#20.11.2017). Findings A paired sample t test revealed that nurses’ involvement in the palliative care of patients requesting passive euthanasia was significantly higher than in NTD regarding euthanasia. Regression analyses revealed that nurses’ position and attitudes explain their intention to be involved in decision-making; attitudes and perceived behavioral control explain nurses’ intention to be involved in the care of patients requesting euthanasia. Conclusions According to the TPB, nurses’ attitudes explained their intention to participate in decision-making regarding passive euthanasia. It is recommended to enhance open discussion of this complex issue to encourage nurses’ willingness to participate in NTD decision-making regarding euthanasia. (shrink)

In decision-making based on multiple criteria, situations may arise where agents find their options to be neither better than, worse than nor equal to each other with respect to the relevant criteria. How, if at all, can a justified choice be made between such options? Are the options incomparable? This article explores a hypothetical case that illustrates how such a situation can arise in an environmental context; more specifically, it considers the deliberations of an imagined (...) 'ethics committee' as it struggles to decide whether or not a deep-sea mining project should be allowed to proceed. I argue that the case is best understood as involving options that are comparable in the sense of being 'on a par'. Working out from a discussion of Ruth Chang's 'self-governance' theory of choice in cases of parity, I suggest that, in the environmental context, the idea of choices expressing a moral identity may lead us in the direction of a plausible solution to these particularly challenging cases. In the setting of transplant medicine, decision making needs to take into account the multiple clinical and psychosocial case variables, rather than turn to arbitrary rules that cannot be scientifically supportedThe yearly demand for liver transplants far exceeds the supply of available organs .1 Additionally, alcoholic cirrhosis has been a controversial indication for transplant as these recipients can be viewed as having caused their own illness—an illness that is preventable by abstaining from alcohol . While not categorically (...) denying liver transplantation to those with alcoholic cirrhosis, many hospitals have incorporated a six month alcohol abstinence criterion 2 in an effort to select optimal candidates. The six month rule has two purposes; namely, allowing the liver a chance to recover in the absence of alcohol , and also observation of the patient to verify that he/she remains alcohol free, with the hope of reducing the risk of relapse. Everhart et al,3 determined that 85% of US liver transplant programmes and 43% of third party payers require a defined period of abstinence—for example, three or six months—as part of the waiting list process.Liver transplantation does not cure alcoholism, as evidenced by the fact that roughly 20% of these patients use alcohol following transplant, with one third exhibiting “repetitive or heavy drinking”.4 Arguments abound as to the scientific merit of a six month abstinence criterion in reducing the risk of alcohol relapse following transplant. The Patient Self Determination Act of 1991 brought much needed attention to the importance of advance care planning and surrogate decision-making. The purpose of this law is to ensure that a patient’s preferences for medical care are recognized and promoted, even if the patient loses decision-making capacity. In general, patients are presumed to have DMC. A patient’s DMC may come under question when distortions in thinking and understanding due to illness, delirium, depression or other psychiatric symptoms (...) are identified or suspected. Physicians and other healthcare professionals working in hospital settings where medical illness is frequently comorbid with depression, adjustment disorders, demoralization and suicidal ideation, can expect to encounter ethical tension when medically sick patients who are also depressed or suicidal request do not resuscitate orders. Historically, parents looking for guidance turned to a small cadre of trusted individuals such as grandparents and pediatricians. In the Internet era, this paradigm has shifted. With a few keystrokes, anxious parents have access to a seemingly endless array of opinions from faceless sources with unknown agendas. For some parents, this can cause more uncertainty, and for the parents of a child with a medical condition, navigating this information can be overwhelming. In this modern paradigm, the pediatrician's duty has also (...) become more complex, especially with the shift from paternalism to patient autonomy in medical decision-making. It is within this context that Alan Fleischman's book, Pediatric Ethics: Protecting the Interests of Children, should be examined. A pediatrician and neonatologist, Fleischman has witnessed this evolution over more than five decades of practice. As a clinical and academic bioethicist, he has also witnessed a transformation in the field of bioethics, with shifting perspectives on areas such as research ethics and medical decision-making. Because of his rich experience and deep understanding of the past and present of ethical issues in pediatrics, Fleischman approaches this subject with a perspective that warrants attention and thoughtful consideration. But what exactly does it mean to say that a subject has or lacks the requisite capacity to decide? This question has (...) to do with what is commonly called “decisional capacity”, a central concept in health care law and ethics, and increasingly an independent topic of philosophical inquiry. -/- Decisional capacity can be defined as the ability of subjects to make their own medical decisions. Somewhat similar questions of capacity arise in other contexts, such as capacity to stand trial in a court of law and the ability to make decisions that relate to personal care and finances. However, the history behind the more general legal notions of capacity to stand trial and capacity to manage one’s life is different and operates somewhat differently in law (Roth, Meisel, & Litz 1977; Zapf & Roesch 2005). For the purposes of this discussion the notion of decisional capacity will be limited to medical contexts only; most notably, those where decisions to consent or to refuse treatment or participation in clinical research are concerned.
