IntroductionDeep brain stimulation is an effective treatment for advanced Parkinson’s disease with the targeting bilateral subthalamic nucleus or globus pallidus internus. So far, detailed studies on the efficacy of unilateral STN-DBS for motor symptoms have been reported, but few studies have been conducted on unilateral GPi-DBS.Materials and MethodsSeventeen patients with Parkinson’s disease who underwent unilateral GPi-DBS were selected. We conducted comparison analyses between scores obtained 6–42 months pre- and postoperatively using the following measurement tools: the Movement Disorder Society Unified Parkinson’s (...) Disease Rating Scale part III, the Hoehn and Yahr stage, the presence/absence of dyskinesia, Mini-Mental State Examination, Frontal Assessment Battery, Geriatric Depression Scale, levodopa equivalent dose, and cerebral blood flow by single photon emission computed tomography. Patient backgrounds were compared between four cohorts with favorable and unfavorable postoperative outcome.ResultsSignificant improvement was observed postoperatively in the following: total MDS-UPDRS Part III scores during the off period, contralateral scores, ipsilateral scores, and axial scores. Similarly, the Hoehn and Yahr stages during the off period, and GDS also showed significant decrease. In contrast, LED, MMSE, and FAB remained unchanged while the number of patients who scored positive for dyskinesia decreased by 40%. Abnormal cerebral blood flow preoperatively seen in the cerebral cortex had normalized in the total score-based good responder cohort. In the ipsilateral score-based good responder cohort, cerebral blood flow increased in the contralateral frontal lobe including in the premotor cortex, contralateral to the DBS. Compared to the poor responders, postoperative good responders demonstrated significantly higher preoperative MMSE scores.DiscussionUnilateral GPi-DBS therapy was effective in improving contralateral, ipsilateral, and axial motor symptoms of patients with advanced PD; in particular, it was found to be especially beneficial in PwPD whose cognitive function was unimpaired; the treatment efficacy rivaled that of bilateral counterparts up till at least 6 months postoperatively. Finally, normalization of preoperative abnormalities in cerebral blood flow and increased cerebral blood flow in the contralateral frontal lobe indicated the beneficial potential of this therapy on ipsilateral motor symptoms. While changes in response to the different stages of the pandemic remain unknown, this study investigated the longitudinal impact of the COVID-19 pandemic on depressive symptoms in Japanese university students and identified factors associated with new onset of depression and suicidal ideation. Two surveys were conducted at one university in Akita, Japan, during the first COVID-19 outbreak period and 1 year later. Moderate depressive symptoms were defined as a Patient Health Questionnaire-9 score ≥ 10 and suicide-related ideation score ≥ 1 (...) on question 9 of the questionnaire. Among 985 students who completed surveys in T1 and T2, participants with moderate depressive symptoms and suicide-related ideation increased from 11 to 17% and from 5.8 to 11.8%, respectively. Among 872 students at risk after excluding those with moderate depressive symptoms at T1, 103 students developed moderate depressive symptoms at T2. Among the 928 students at risk, after excluding those who had suicidal ideation at T1, 79 developed suicidal ideation. Multivariate logistic modeling revealed financial insecurity and academic performance as risk factors, while having someone to consult about worries was a coping factor for depressive symptoms and suicidal ideation. Our findings demonstrated that socioenvironmental factors may determine depressive symptoms of university students. (shrink)

This study attempts to explore the possible motivations, both obvious and problematic, behind the ritual suicide (seppuku) committed by the Japanese writer in the name of the Emperor at the Eastern Headquarters of Japan’s Self-Defense Forces in 1970. History does not seem to be a coherent or intelligible process, as man’s struggle for nourishment is most often replaced by thymos, the desire for others to recognize his value or the value system of the ideals or noble purposes he is ready (...) to sacrifice for, ignoring the basic instinct of self-conservation. Yukio Mishima was extremely pessimistic about pragmatic and materialistic contemporary Japan. History brought along increasing consumerism, thus disturbing the harmony of traditional Japanese spirituality. The technological ability to improve human existence seemed to alter inevitably the moral evolution of contemporary Japanese. Against this background of ruling “costs” and “benefits”, the Japanese writer seems to believe that it is only the thymotic man, the “man of anger”, who can fight for the recognition and salvation of the Japanese soul (yamato damashii). Believed for centuries to be the true art of dying, Yukio Mishima’s seppuku turned from a “beautiful” gesture into one of protest and accusation: the Japanese society had begun the transition from a closed society to an open one, governed by anxiety, in which individuals faced personal decisions. Novelist, playwright, actor, exhibiionist —his novels are rife with homoerotic and violent imagery, while his fanatical and nihilistic philosophy calls for a return to a Samurai ethos. Mishima thus attained infamy in Japan and in the West, as his shocking novels (...) inspired hordes ofyoung Japanese into a cult ofMishima worship, while critics who acknowledge his genius are still nervous about his sadistic homosexuality and his suicide. Mishima's works are suffused with erotic bloodshed and misogyny, and Mishima finally ended his life by ritually disemboweling himself. I will make the case that Mishima was incessantly plagued by torturous mimetic frustrations, and that his final act of spilling his blood and intestines can be interpreted as the result of mimetic hatred against his rivals. This paper aims to analyse how literary fiction deals with two real cases of philistine violence on cultural objects, one artistic and the other scientific. In this way, we will analyse Mishima's novel, The Temple of the Golden Pavilion, which narrates the destruction of one of the “jewels of Kyoto,” as well as Joseph Conrad’s The Secret Agent, which novelised the attack against the meridian of Greenwich. Aside from the revival of standby texts such as Goudsblom's Nihilism and Culture and Rosen's Nihilism, there has been a glut of books in areas (...) as diverse as economics (Ritzer's The Globalization of Nothing), theology (Cunningham's Genealogy of Nihilism), and theory (Clemen's The Romanticism of Contemporary Theory). John Marmysz's contribution, Laughing at Nothing, is a refreshing change inasmuch as he invites us to laugh at nihilism and, in effect, to laugh with him at much of the scholarly work on nihilism. Primarily philosophical in tone, Laughing at Nothing is an academic treatise on nihilism and humor, an exploration of contemporary American culture, and a plea for understanding in a hostile environment.Marmysz is determined, as few others are, to see a positive potential in nihilism. Although he never uncovers a "positive" nihilism, he is willing to accept the idea that nihilism may be a valuable cultural concept. In his introduction, he stresses the fact that most critics of nihilism "underplay the constructive and positive role of nihilism in both individual and collective human history" (p. 10). This book attempts to undermine such trite interpretations of nihilism; evident from his observation that, "too often scholars proceed to draw conclusions about nihilism itself on the basis of philosophies that are, in fact, very poor models" (p. 11). This is an important point, and one to which I will return.All exegeses on nihilism must include a history of the concept to orient the reader within the given intellectual milieu, and Marmysz's is no exception. A4) featuring Hitomi Kanehara, a twenty-year-old author of a novel about (...) her post-bubble generation. Her portrait of a radically changing society in Japan reflects what I learned from two dozen interviews with Japanese friends and colleagues over the course of my visit.The Times article praised Kanehara for the portrait of young people in Japan since the 1990 collapse of Japan's "bubble economy" of the 1970s and 1980s:It is a world of "freeters," young Japanese surviving on part-time jobs and unconcerned with their future; of unsentimental sex and a profound inability to communicate verbally; a world in which a killing is viewed with amorality. The institutions that built postwar Japan—the family, school and companies—are noticeable by their absence. In a nation known for its social cohesion, the characters have no interest in playing a role in society, but only in finding personal satisfaction among themselves. Unlike Japanese in, say, their 30s, the characters in the novel are not disillusioned at Japanese society, since they had few expectations to begin with. "There are many people who don't expect anything from society," Ms. Kanehara says. "That's precisely why they are looking inward or to people [End Page 678] closest to them.
