The present study aims to explore whether the symptoms associated with fibromyalgia are contextually influenced by the area of residence. Furthermore, it is analyzed whether the effect of the acceptance of the disease on the emotional, cognitive and physical symptoms is moderated by the patients’ place of residence. Using a cross-sectional design, a total of 234 women with fibromyalgia were surveyed, of which 55.13% resided in rural areas and 44.87% in urban areas. Self-reported questionnaires were used to assess pain severity, (...) anxiety and depression, functional limitation, physical and mental fatigue and acceptance of the disease. The results show significant differences in acceptance, pain, and physical and mental fatigue between patients from rural and urban areas. The rural area patients presented higher levels of acceptance and pain and lesser levels of physical and mental fatigue compared to the urban area. The moderation analysis add that, only in patients from the rural area, the variables of physical symptoms were significantly and negatively associated with acceptance. This study addresses for the first time the role of the place of residence in suffering from fibromyalgia, suggesting that the rural or urban environment plays a relevant role in the severity and/or management of symptoms in fibromyalgia women. The pandemic outbreak in March 2020 and its associated sanitary regulations and restrictions triggered an abrupt and significant change for society in general and for families’ organization in particular. In Chile, the Santiago Metropolitan District was under a strict lockdown that involved the closure of the entire educational system. From a systemic-family stress perspective, the impact of these changes might have consequences not only for each individual family member, but for the parental dynamic and, consequently, for children’s well-being. This paper (...) presents the results of a follow-up study showing changes in self-reported parental depression and the perceived home organization of mothers and fathers assessed at three different moments: before the pandemic, at the initial outbreak, and after 1 month of strict lockdown. Relevant moderators were explored using linear mixed models to understand the within-subject changes in mothers’ and fathers’ self-reports across the different assessment times. Financial strain, personality traits of self-criticism and dependency, previous parent–child quality interaction, recent major stressful events, and number of children are highlighted as relevant factors that moderate changes in home chaos and parental mental health perception. Significant risks and protective factors are described for fathers and mothers. The use of pre-pandemic measures as baseline levels enabled the identification of personal and family characteristics that were related to better outcomes. The results help increase our understanding of the sanitary regulations’ impacts on the family system and identify vulnerability indicators that should be considered. (shrink)

This article introduces some soft computing methods generally used for data analysis and flow pattern detection in fluid dynamics. These techniques decompose the original flow field as an expansion of modes, which can be either orthogonal in time, or in space or in time and space, or they can simply be selected using some sophisticated statistical techniques. The performance of these methods is tested in the turbulent wake of a wall-mounted square cylinder. This highly complex flow is suitable to show (...) the ability of the aforementioned methods to reduce the degrees of freedom of the original data by only retaining the large scales in the flow. The main result is a reduced-order model of the original flow case, based on a low number of modes. A deep discussion is carried out about how to choose the most computationally efficient method to obtain suitable reduced-order models of the flow. The techniques introduced in this article are data-driven methods that could be applied to model any type of non-linear dynamical system, including numerical and experimental databases. (shrink)

One of the main challenges in medicine is to guarantee an appropriate drug supply according to the real needs of patients. Closed-loop strategies have been widely used to develop automatic solutions based on feedback variables. However, when the variable of interest cannot be directly measured or there is a lack of knowledge behind the process, it turns into a difficult issue to solve. In this research, a novel algorithm to approach this problem is presented. The main objective of this study (...) is to provide a new general algorithm capable of determining the influence of a certain clinical variable in the decision making process for drug supply and then defining an automatic system able to guide the process considering this information. Thus, this new technique will provide a way to validate a given physiological signal as a feedback variable for drug titration. In addition, the result of the algorithm in terms of fuzzy rules and membership functions will define a fuzzy-based decision system for the drug delivery process. The method proposed is based on a Fuzzy Inference System whose structure is obtained through a decision tree algorithm. A four-step methodology is then developed: data collection, preprocessing, Fuzzy Inference System generation, and the validation of results. To test this methodology, the analgesia control scenario was analysed. Specifically, the viability of the Analgesia Nociception Index as a guiding variable for the analgesic process during surgical interventions was studied. Nowadays, batteries play an important role in a lot of different applications like energy storage, electro-mobility, consumer electronic and so on. All the battery types have a common factor that is their complexity, independently of its nature. Usually, the batteries have an electrochemical nature. Several different test are accomplished to check the batteries performance, and commonly, it is predictable how they work depending of their technology. The present research describes the hybrid intelligent system created to accomplish fault detection over a (...) Lithium Iron Phosphate—LiFePO4 power cell type, commonly used in electro-mobility applications. The approach is based on the cell temperatures behaviour for voltage and current specific values. Taken into account the operating range of a real system based on a LiFePO4 cell, a large set of points of operation have been used to achieve the dataset. The different behaviour zones have been obtained by clustering as a first step. Then, different regression techniques have been used over each cluster. Polynomial regression, artificial neural networks and support vector regression were the combined techniques to develop the hybrid intelligent model proposed. The intelligent system gives very good results over the operating range, detecting all the faults tested during the validation. (shrink)

Student performance and its evaluation remain a serious challenge for education systems. Frequently, the recording and processing of students’ scores in a specific curriculum have several flaws for various reasons. In this context, the absence of data from some of the student scores undermines the efficiency of any future analysis carried out in order to reach conclusions. When this is the case, missing data imputation algorithms are needed. These algorithms are capable of substituting, with a high level of accuracy, the (...) missing data for predicted values. This research presents the hybridization of an algorithm previously proposed by the authors called adaptive assignation algorithm, with a well-known technique called multivariate imputation by chained equations. This work focuses on the application of machine learning techniques to assist the clinicians in the administration of analgesic drug during general anaesthesia. Specifically, the main objective is to propose the basis of an intelligent system capable of making decisions to guide the opioid dose changes based on a new nociception monitor, the analgesia nociception index. Clinical data were obtained from 15 patients undergoing cholecystectomy surgery. By means of an off-line study, machine learning techniques were applied to analyse the possible (...) relationship between the analgesic dose changes performed by the physician due to the hemodynamic activity of the patients and the evolution of the ANI. After training different classifiers and testing the results under cross validation, a preliminary relationship between the evolution of ANI and the dosage of remifentanil was found. These results evidence the potential of the ANI as a promising index to guide the infusion of analgesia. (shrink)

Automatic control of physiological variables is one of the most active areas in biomedical engineering. This paper is centered in the prediction of the analgesic variables evolution in patients undergoing surgery. The proposal is based on the use of hybrid intelligent modelling methods. The study considers the Analgesia Nociception Index to assess the pain in the patient and remifentanil as intravenous analgesic. The model proposed is able to make a one-step-ahead prediction of the remifentanil dose corresponding to the current state (...) of the patient. The input information is the previous remifentanil dose, the ANI variable and the electromyogram signal. Modelling techniques used are Artificial Neural Networks and Support Vector machines for Regression combined with clustering methods. Both training and validation were done with a real dataset from different patients. Results obtained show the potential of this methodology to calculate the drug dose corresponding to a given analgesic state of the patient. (shrink)

In this study, a hybrid model based on intelligent techniques is developed to predict the active power generated in a bioclimatic house by a low power wind turbine. Contrary to other researches that predict the generated power taking into account the speed and the direction of the wind, the model developed in this paper only uses the speed of the wind, measured mainly in a weather station from the government meteorological agency (MeteoGalicia). The wind speed is measured at different heights, (...) against the usual measurements in others researches, which uses the wind speed and the direction measured in a weather station on the wind turbine nacelle. The prediction is performed 30 minutes ahead, what ensures that the Building Management System knows the energy generated by the low power wind turbine 30 minutes before, and it can adapt the consumption of different equipment in the house to optimize the power use. The main objective is to allow the Building Management System to optimize the uses of energy, taking into account the predicted amount of energy that will be produced and the energy consumed in the house. The developed model uses a hybrid topology with four clusters to improve the prediction, achieving an error lower than 6.5% for Mean Absolute Error measured in a final test. To perform this test, part of the original dataset was isolated from the beginning of the training process to check the model with a dataset that is not used before, simulating the model as it is receiving new data. Closed-loop administration of propofol for the control of hypnosis in anesthesia has evidenced an outperformance when comparing it with manual administration in terms of drug consumption and post-operative recovery of patients. Unlike other systems, the success of this strategy lies on the availability of a feedback variable capable of quantifying the current hypnotic state of the patient. However, the appearance of anomalies during the anesthetic process may result in inaccurate actions of the automatic controller. These anomalies may come from the (...) monitors, the syringe pumps, the actions of the surgeon or even from alterations in patients. This could produce adverse side effects that can affect the patient postoperative and reduce the safety of the patient in the operating room. Then, the use of anomaly detection techniques plays a significant role to avoid this undesirable situations. This work assesses different one-class intelligent techniques to detect anomalies in patients undergoing general anesthesia. Due to the difficulty of obtaining real data from anomaly situations, artificial outliers are generated to check the performance of each classifier. The final model presents successful performance. Los argumentos tradicionales contrarios a la disponibilidad de la propia vida conservan alguna fuerza: en algunas sociedades, el intento de suicidio se considera todavía un delito y la colaboración —o complicidad— sigue siendo igualmente una conducta delictiva. No obstante, en los últimos años se ha llevado a cabo un creciente reconocimiento del llamado “derecho a una muerte digna”. De todos modos, ese derecho soporta ciertas condiciones que lo convierten, a mi entender, más en un eslogan pegadizo y engañoso que en (...) un derecho claramente consolidado y reconocido. En el presente artículo intentaré explicar diversos argumentos a favor y en contra de la libre disponibilidad de la propia vida formulados a lo largo de la historia del pensamiento occidental. Posteriormente, trataré de demostrar que la libre disposición de la propia vida encuentra demasiadas restricciones en España y que en algunas de ellas subyacen principios extraseculares impropios de una sociedad democrática, pluricultural y secular como la nuestra. Requiring family authorization for apnea testing subtracts health professionals control over death determination, a procedure that has traditionally been considered a matter of clinical expertise alone. In this commentary, we first provide evidence showing that health professionals’ (HPs) disposition to act on death determination without family’s prior consent could be much lower than that referred to by Berkowitz and Garrett (2020). We hypothesize that HPs may have reservations about their own expertise as regards death, and may thus hesitate to impose (...) their views on patients’ families. We then address the theoretical question of clinical expertise in death determination by distinguishing judgments about facts (e.g., the presence or absence of spontaneous breathing) from interpretations given of these facts (i.e., their meaning for the vital status of an individual). We argue that, while clinicians may claim some expert authority on the former, they hold no particular authority on the latter. (shrink)

From Realism to "Realicism" is a unique critical study of Peirce's metaphysics, and his repeated insistence on the realism of the medieval schoolman as the key to understanding his own system. By tracing the problem of universals beginning with its Greek roots, Rosa Maria Perez-Teran Mayorga provides the necessary yet underrepresented background of moderate realism and Peirce's eventual revision of metaphysics.

In Ancient Greece, the figure of the hero was identified as a demigod, possessed of altruistic and virtuous deeds. When Pierre de Coubertin reinstated the Olympic Games, the athlete was personified as a modern hero. Its antithesis, the anti-hero, has more virtue that defects, no evil but he does not care on the means to achieve his goals. In the eyes of everyone involved in sports competition, these characters captivate and at the same time, create conflicts of ethics and aesthetics. (...) The purpose of this paper is to perform an ethical reflection linked to principles that contribute for the human growth and accomplishment, as well as the aesthetic on the perception of the sensitive, reverberated by sensations and feelings emerging from athletes. Connecting the ethic with the aesthetic spheres, we could have in the sports a phenomenon walking toward a common point between moral and aesthetic, between the good and the beauty. (shrink)

This article captures and critiques a recurring and prominent political argument against animal welfare improvements in Switzerland which we term the “ranking argument”. This states that Swiss animal welfare law ranks among the strictest in the world, therefore no improvements are called for. This argument was advanced three times by Swiss government authorities in 2022 alone, but also in a case dating back to 1984, to advise the electorate on popular initiatives aiming at animal welfare improvements. We argue that, while (...) the argument commits a fallacy of relative privation and is ethically dubious, it can be deployed to great effect by agents opposed to norm change in animal welfare regulation. We conclude with some thoughts on how the ranking argument can and should be challenged in public discourse. (shrink)

El manual escolar sigue siendo un recurso didáctico clave en la enseñanza de la historia. En Educación Primaria y Secundaria pervive su uso, a pesar de las numerosas reformas educativas llevadas a cabo desde hace siglo y medio. Este trabajo pretende analizar su vigencia, estructura y cambios a lo largo del tiempo. Los cada vez más numerosos estudios se han escrito desde una perspectiva española o portuguesa, así que interesa conocer la evolución paralela y establecer comparaciones. Se analizan las escuelas (...) historiográficas y pedagógicas que han influido en la evolución de los currículos educativos. Además, se reflexiona sobre los contenidos históricos de los manuales, especialmente las narrativas nacionales. (shrink)

En el artículo se recogen los relatos que las fuentes árabes, sobre todo andalusíes, proporcionan sobre las primeras fundaciones religiosas de al-Andalus. Las tradiciones de la Conquista, en este aspecto, muestran similitudes con el modelo de islamización puesto en práctica previamente en otros ámbitos del Mediterráneo islámico y con la historiografía árabe de los Futuh. Por razones de prestigio religioso y de legitimidad política, los autores insisten en aspectos como la orientación de las primeras mezquitas, atribuyéndolas a Musà o a (...) los tabi¿un, o en la destrucción de iglesias para levantar mezquitas sobre ellas. En estas tradiciones encontramos, sin embargo, escasos datos sobre las características arquitectónicas de estos primeros oratorios andalusíes por lo que es imprescindible la información aportada hasta ahora por la arqueología. El caso de la mezquita de Córdoba muestra la importancia de analizar en conjunto ambas aportaciones. (shrink)

We show that both Rado's Conjecture and strong Chang's Conjecture imply that there are no special ℵ2-Aronszajn trees if the Continuum Hypothesis fails. We give similar result for trees of higher heights and we also investigate the influence of Rado's Conjecture on square sequences.
