Worries about long-term effects have led to a proposal to link treatment data with cancer registers. How should one deal with informed consent for such a study? We designed a qualitative study with semi-structured telephone interviews. From 1200 women who were to be followed-up in cancer registers, we randomly selected 22 women. Major themes were a wish to be involved and a positive (...) attitude to the proposed register research. The women did not express worry after reading the study protocol, but did convey considerable frustration that this research had not been initiated earlier. Active consent was not seen as crucial. We found strong interest in a high participation rate and a concern over missing data. The selection of information and consent or the decision to go ahead without consent in register follow-up is a delicate balancing act. Study participants wish to be contacted, but acknowledge the primary goal of answering important questions. Our study provides support for safeguarding privacy in epidemiological linkage studies and in follow-up of medical treatment without losing the scientific value by requesting for informed consent. The result of many years of painstaking scholarship, the new volume will replace the now nearly 100 year old original edition, and is destined to become just as long-lasting a classic.

It is often argued that human emotions, and the cognitions that accompany them, involve refinements of, and extensions to, more basic functionality shared with other species. Such refinements may rely on common or on distinct processes and representations. Multi-level theories of cognition and affect make distinctions between qualitatively different types of representations often dealing with bodily, affective and cognitive attributes of self-related meanings. This paper will adopt a particular multi-level perspective on mental architecture and show how a mechanism of subsystem (...) differentiation could have allowed an evolutionarily “old” role for emotion in the control of action to have altered into one more closely coupled to meaning systems. We conclude by outlining some illustrative consequences of our analysis that might usefully be addressed in research in comparative psychology, cognitive archaeology, and in laboratory research on memory for emotional material. To design effective strategies to improve ethics consultation (EC) practices, it is important to understand the views of ethics practitioners. Previous U.S. studies of ethics practitioners have overrepresented the views of academic bioethicists. To help inform EC improvement efforts, we surveyed a random stratified sample of U.S. hospitals, examining ethics practitioners’ opinions on EC in general, on their own EC service, on strategies to improve EC, and on ASBH practice standards. Respondents across all categories of hospitals had very positive perceptions (...) of their own ethics consultation service (ECS) and few concerns about quality. Our findings suggest that the ethics-related needs of small, rural, non-teaching hospitals may be very different from those of academic medical centers, and therefore, different approaches to addressing ethical issues might be warranted. BackgroundTo help inform the development of more accessible, acceptable, and effective ethics consultation (EC) training programs, we conducted an EC training needs assessment, exploring ethics practitioners’ opinions on: the relative importance of various EC practitioner competencies; the potential market for EC training (that is, how many individuals would benefit and how much individuals and hospitals would be willing to pay); and the preferred content, format, and characteristics of EC training.MethodsAs part of a multipart study, we surveyed “best informants” who self-identified (...) as the person most actively involved in EC or healthcare ethics in a random sample of 600 U.S. general hospitals, stratified for bed size.ResultsThe competency that was ranked most important for a lead or solo ethics consultant was knowledge of ethics, while common sense was ranked least important. The median estimated number of individuals at each hospital who would benefit from EC training was six at the basic level, three at the advanced level, and two for EC management training. In 19.1 percent of hospitals, respondents thought their hospital would not be willing to pay anything for EC training within the next two years. Respondents thought potential trainees would be likely to participate in EC training on multiple different topics. Opinions varied widely on preferred formats. Most respondents thought it very important to be able to interact with instructors and with other trainees, practice EC skills, receive a certificate for completing EC training, and complete EC training during work hours.ConclusionsThese findings provide U.S. population data that may be useful to healthcare educators and bioethics leaders in their efforts to develop EC training programs and products that match trainees’ preferences and needs. This study examined three main issues among 364 primary school children: (1) self?reported levels of perceived safety in classroom and playground, and relationship with teacher, (2) associations between perceived safety in the two contexts and peer reported levels of being bullied, and (3) if relationship with teacher moderated the associations between peer reported levels of being bullied and perceived safety in classroom and playground. Data were collected in individual and small group interviews. Overall, while most participants reported positive relationships with (...) their class teacher, and felt safe in their classroom and in the playground, a substantial minority did not. The correlations between level of being bullied and perceived safety in classroom and playground were significant but of modest size. Relationship with teacher did moderate the correlation for perceived safety in the classroom, but did not do so for perceived safety in the playground. No significant age or sex differences were obtained. The theoretical and practical implications of these findings were discussed. (shrink)

A 1999–2000 national study of U.S. hospitals raised concerns about ethics consultation (EC) practices and catalyzed improvement efforts. To assess how practices have changed since 2000, we administered a 105-item survey to “best informants” in a stratified random sample of 600 U.S. general hospitals. This primary article details the methods for the entire study, then focuses on the 16 items from the prior study. Compared with 2000, the estimated number of case consultations performed annually rose by 94% to 68,000. The (...) median number of consults per hospital was unchanged at 3, but more than doubled for hospitals with 400+ beds. The level of education of EC practitioners was unchanged, while the percentage of hospitals formally evaluating their ECS decreased from 28.0% to 19.1%. The gap between large, teaching hospitals and small, nonteaching hospitals widened since the prior study. We suggest targeting future improvement efforts to hospitals where needs are not being met by current approaches to EC. (shrink)

BackgroundWhile previous research has examined various aspects of ethics consultation (EC) in U.S. hospitals, certain EC practices have never been systematically studied.MethodsTo address this gap, we surveyed a random stratified sample of 600 hospitals about aspects of EC that had not been previously explored.ResultsNew findings include: in 26.0% of hospitals, the EC service performs EC for more than one hospital; 72.4% of hospitals performed at least one non-case consultation; in 56% of hospitals, ECs are never requested by patients or families; (...) 59.0% of case consultations involve conflict; the usual practice is to visit the patient in all formal EC cases in 32.5% of hospitals; 56.6% of hospitals do not include a formal meeting in most EC cases; 61.1% of hospitals do not routinely assess ethics consultants’ competencies; and 31.6% of hospitals belong to a bioethics network. We estimate the total number of non-case consultations performed in U.S. hospitals to be approximately one half the number of case consultations; we estimate the total number of ECs performed in U.S. hospitals, including both case and non-case consultations, to be just over 100,000 per year.ConclusionsThese findings expand our current understanding of EC in U.S. hospitals, and raise several concerns that suggest a need for further research. Within this submission the authors share their experiences as a blended research team with Aboriginal community and mainstream academic researchers. The team has collaborated since 2004 on several externally funded research projects. Initially, the team engaged in research through mainstream methodologies. In the process, the community co-researchers and participants were silenced through mainstream cultural practices that were unfamiliar and meaningless in Wikwemikong culture. More recently, the team has employed a community conceived de-colonizing methodology, developed from within Wikwemikong Unceded Indian Reserve. (...) Within this submission, the authors will highlight their initial cultural missteps, followed by more recently utilized culturally relevant approaches. It is proposed that what might be ethically sound research with mainstream participants and among mainstream researchers can silence and subvert practices among those from marginalized groups/cultures. COVID-19 caused nearly every college and university in the United States to rapidly shift to remote learning during the spring 2020 semester. While this impacted all students to different degrees, students with disabilities faced new challenges related to their mental health, the accessibility of their instruction, the receipt of accommodations, and their interactions with faculty and student support personnel. Literature is emerging that describes the experiences of SWD during the spring 2020 semester and the swift change to remote learning. However, (...) little is currently known about what followed for these students. The present study builds from a prior investigation of SWD during the spring 2020 semester and examines student experiences and perceptions during the 2020–2021 academic year. Eighty-eight SWD from colleges across the United States completed an instrument that contained a mix of demographic, yes/no, Likert scale and open-ended items. Responses revealed most items related to accessing services and instruction showed no improvement from the spring 2020 semester, and that items related to mental health, motivation to learn, and connections with peers were perceived as worse than in spring 2020. Open-ended responses revealed similar themes, with some students describing no improvements, and others noting that accessibility service offices and faculty provided enhanced methods of communication and support. H´ector-Neri Casta˜neda-Calder´on (December 13, 1924–September 7, 1991) was born in San Vicente Zacapa, Guatemala. He attended the Normal School for Boys in Guatemala City, later called the Military Normal School for Boys, from which he was expelled for refusing to fight a bully; the dramatic story, worthy of being filmed, is told in the “De Re” section of his autobiography, “Self-Profile” (1986). He then attended a normal school in Costa Rica, followed by studies in philosophy at the University of San (...) Carlos, Guatemala. He won a scholarship to the University of Minnesota, where he received his B.A. (1950), M.A. (1952), and Ph.D. (1954), all in philosophy. His dissertation, “The Logical Structure of Moral Reasoning”, was written under the direction of Wilfrid Sellars. He returned to teach in Guatemala, and then received a scholarship to study at Oxford University (1955–1956), after which he took a sabbatical-replacement position in philosophy at Duke University (1956). His first full-time academic appointment was at Wayne State University (1957– 1969), where he founded the philosophy journal Noˆus (1967, a counter-offer made to him by Wayne State to encourage him to stay there rather than to take the chairmanship of philosophy at the University of Pennsylvania). In 1969, he moved (along with several of his Wayne colleagues) to Indiana University, where he eventually became the Mahlon Powell Professor of Philosophy and, later, its first Dean of Latino Affairs (1978–1981). He remained at Indiana until his death. He was also a visiting professor of philosophy at the University of Texas at Austin (1962–1963) and a fellow at the Center for Advanced Study in the Behavioral Sciences (1981–1982). He received grants and fellowships from the Guggenheim Foundation (1967–1968), the T. Andrew Mellon Foundation, the National Endowment for the Humanities, and the National Science Foundation. He was elected President of the American Philosophical Association Central Division (1979– 1980), named to the American Academy of Arts and Sciences (1990), and received the Presidential Medal of Honor from the Government of Guatemala (1991). Casta˜neda’s philosophical interests spanned virtually the entire spectrum of philosophy, and his theories form a highly interconnected whole.. (shrink)

In three decades of reform China has become a society that is radically different both from what it used to be in Mao's time and from a liberal society. This new China poses especially interesting questions about freedom - interesting not only in the Chinese context but also more generally. Pivotal for my treatment of these questions is a distinction I draw between de facto freedom and the value of freedom, the latter in turn understood in terms of a larger (...) notion of agency, and divided into such distinct dimensions as moral resource, right, and mode of subjection. This distinction allows me to argue that while China has no shortage of de facto freedom, it lacks the value of freedom, especially freedom as a mode of subjection. The lack of freedom as a value has serious moral and political consequences, as manifested in a prolonged moral crisis in post-Mao China and in a certain fragility of social order caused by the pitting of order against freedom. Such consequences suggest a strong need for China to develop the value of freedom. But it is possible and important to do so in a critical spirit, as I argue through a critical analysis of the place of freedom in a liberal society. © 2011 by Duke University Press. (shrink)

Addiction Trajectories is a collection of anthropological essays that brings a refreshingly human perspective to the scientific pursuit of addiction. This book encourages the reader to step back from the details, giving voice to the experiences of the drug user as they grapple to come to terms with their condition and the efforts of the treatment community. At the same time, the book provides insight into the machinations of the treatment community struggling to understand the scope of their task and (...) to respond to the shifting intellectual landscapes of licit and illicit drugs. The addiction trajectories framework is constructed around three themes. Theme one concerns the changing categories and concepts of addiction through time and across disciplinary boundaries. Theme two represents changes in therapeutic concepts of addiction across cultures and organisations. Theme three represents the experiences of the addicted individual. [Review] Liz P.Y. Chee. Mao’s Bestiary: Medicinal Animals and Modern China. Duke University Press, 2021. 288 pp. The COVID-19 pandemic has secured its place as a 21st century global public health disaster. It has killed more than 6.2 million and infected close to 500 million people worldwide. Acknowledging Wuhan’s wildlife market as the ground zero of the pandemic and the devastation caused by SARS 17 years earlier, China’s Communist authorities made the long overdue decision on February 24, 2020 and (...) outlawed wildlife breeding and trade for the country’s exotic food market. This decision was commendable. Yet, breeding of wildlife for the exotic food market was only one of the five-piece captive farming operation that generated a revenue of $78 billion a year. What the Chinese authorities have retained is captive breeding for Traditional Chinese Medicine, the third largest component of the country’s controversial industry. Called a ‘national treasure’, TCM, in the minds of many, brooks no questioning. Liz P.Y. Chee’s Mao’s Bestiary: Medicinal Animals and Modern China steps in this minefield with questions not about the efficacy of TCM but about the drivers of its faunal medicalization in the last seven decades. The witch-hunt of the Burgundian town of Arras in 1459-1460 was the first large- scale, state-sponsored witch-hunt of Western Europe. However, immediately following this witch-hunt we still find evidence of a reluctance to accept the realities of witchcraft among the populace, made plain in the official appeal record of the accused Seigneur Colard de Beaufort at the parlement de Paris. Scepticism of this kind stirred the Dominican cleric Johannes Tinctor out of retirement to write a vicious demonological treatise to convince (...) the courts of Burgundy and France of the existence and dangers of a sect called vaudois, a term that had come to refer to witches. This essay closely examines Tinctor's heavy use of crusading imagery in his Invectives contre la secte de vauderie to justify and rationalize his arguments for duke Philip the Good of Burgundy and his court, a court renowned for consistent but empty promises of crusade and an elaborate culture bloated with an idealized infatuation with chivalric virtue and romance. In the autumn of the middle ages, when the traditional eastern crusade against "Saracens" had become frustratingly difficult to organize, what could be more appealing to a court so starved for crusade than a cry for war against an even greater enemy hiding amongst the populace, threatening Christendom from within? Though originally written for different occasions, all three develop the theme expressed by the title, that is, "the continued presence of medieval traits in the civilization of the Renaissance". As Kristeller himself explains in the Preface, the presence of medieval traits in the Renaissance does not militate against the acknowledged reality (...) of many nonmedieval and even antimedieval features of this same period.
