Objectives To characterize the prevalence and content of pediatric triage policies.Methods We surveyed and solicited policies from U.S. hospitals with pediatric intensive care units. Policies were analyzed using qualitative methods and coded by 2 investigators.Results Thirty-four of 120 institutions (28%) responded. Twenty-five (74%) were freestanding children’s hospitals and 9 (26%) were hospitals within a hospital. Nine (26%) had approved policies, 9 (26%) had draft policies, 5 (14%) were developing policies, and 7 (20%) did not have policies. Nineteen (68%) institutions shared (...) their approved or draft policy. Eight (42%) of those policies included neonates. The polices identified 0 to 5 (median 2) factors to prioritize patients. The most common factors were short- (17, 90%) and long- (14, 74%) term predicted mortality. Pediatric scoring systems included Pediatric Logistic Organ Dysfunction-2 (12, 63%) and Score for Neonatal Acute Physiology and Perinatal Extensions-II (4, 21%). Thirteen (68%) policies described a formal algorithm. The most common tiebreakers were random/lottery (10, 71%) and life cycles (9, 64%). The majority (15, 79%) of policies specified the roles of triage team members and 13 (68%) precluded those participating in patient care from making triage decisions.Conclusions While many institutions still do not have pediatric triage policies, there appears to be a trend among those with policies to utilize a formal algorithm that focuses on short- and long-term predicted mortality and that incorporates age-appropriate scoring systems. Additional work is needed to expand access to pediatric-specific policies, to validate scoring systems, and to address health disparities. (shrink)

Some of the most interesting recent work in philosophy of language and metaphysics is focused on questions about propositions, the abstract, truth-bearing contents of sentences and beliefs. The aim of this guide is to give instructors and students a road map for some significant work on propositions since the mid-1990s. This work falls roughly into two areas: challenges to the existence of propositions and theories about the nature and structure of propositions. The former includes both a widely discussed puzzle about (...) propositional designators as well as direct and indirect arguments against the existence of propositions. The latter is dominated by what is currently the central debate about the metaphysics of propositions, i.e. whether they are structured, composite entities or unstructured ontological simples. 'Designating Propositions.' The Philosophical Review 111 (2002): 341–71. Sometimes substituting a definite description for a corresponding 'that'-clause can lead to bizarre changes in truth-conditions: compare 'Bill fears that Hillary will be president' with 'Bill fears the proposition that Hillary will be president'. This puzzle about propositional designators threatens the relational analysis of propositional attitude reports, the view that 'believes' expresses a relation to the proposition designated by its 'that'-clause, and thereby poses an indirect threat to the existence of propositions. King's solution posits an ambiguity in verbs like 'fear' that embed both 'that'-clauses and definite descriptions. 3. Jubien, Michael. 'Propositions and the Objects of Thought.' Philosophical Studies 104 (2001): 47–62. A direct attack on the existence of propositions. Jubien deploys an analogue of the problem that Paul Benacerraf raised for set-theoretical reductions of numbers against metaphysical reductions of propositions. Just as numbers can be reduced to sets in many different ways, any reduction of propositions brings with it equally good variants, thus making any such reduction arbitrary and unmotivated. The only alternative is to treat propositions as abstract metaphysical primitives. As Jubien argues, however, abstract primitive entities are incapable of doing what propositions must do, i.e. represent objects and states of affairs on their own, without the input of thinking subjects. The upshot is the propositions cannot be reduced and they cannot be primitive, and so they must not exist. 4. Hanks, Peter. 'How Wittgenstein Defeated Russell's Multiple Relation Theory of Judgment.' Synthese 154 (2007): 121–46. Scepticism about propositions has recently led some philosophers, Jubien included, to resuscitate Russell's multiple relation theory of judgment, the idea that judgment is a many-place relation to objects, properties and relations. This paper explains why Russell himself abandoned that theory, and why the theory is still refuted by an objection due to Wittgenstein. 5. Hofweber, Thomas. What does it mean to be a good thinker? Which virtues work together in someone who possesses good intellectual character? Although recent research on virtues has highlighted the benefits of individual intellectual virtues, being an excellent thinker is likely a function of possessing multiple intellectual virtues. Specifically, a good thinker would both recognize one’s intellectual shortcomings and possess an eagerness to learn driven by virtues such as love of knowledge, curiosity, and open-mindedness. Good intellectual character may only successfully manifest when (...) individuals possess not just one or a few intellectual virtues, but a larger set of such virtues to different degrees. However, little is currently known about what combination of virtues are necessary for good thinking. We argue that it is important to identify and clarify the nature of the good thinker and outline a profile methodology for achieving this goal. This approach characterizes the good thinker in terms of a profile of multiple intellectual virtues. Understanding this profile can allow greater insight into the extent to which people possess such a profile, and the potential for societal benefits of educating for these qualities. (shrink)

Biomedical research is increasingly globalized with ever more research conducted in low and middle-income countries. This trend raises a host of ethical concerns and critiques. While community engagement has been proposed as an ethically important practice for global biomedical research, there is no agreement about what these practices contribute to the ethics of research, or when they are needed.

The majority of recent work on the moral standing to blame (the idea that A may be unable to legitimately blame B despite B being blameworthy) has focused on blamers who themselves are blameworthy. This is unfortunate, for there is much to learn about the standing to blame once we consider a broader range of cases. Doing so reveals that challenged standing is more expansive than previously acknowledged, and accounts that have privileged the fact that the blamers are themselves morally (...) culpable likely require revision. One such account figures in Patrick Todd’s (2012) argument for incompatibilism, which ostensibly depends on considerations involving the standing to blame. I believe this argument fails. But its failure is instructive, for it allows us to appreciate the numerous ways in which one’s blame can be morally problematic, and hence ways in which one’s standing to blame can be challenged. Thus, while one objective of this paper is to show why Todd’s argument fails, the larger aim is to use that argument to frame discussion of some important (and novel) ways in which the standing to blame can be compromised. Just as humans can draw conclusions responsibly or irresponsibly, so too can computers. Machine learning systems that have been trained on data sets that include irresponsible judgments are likely to yield irresponsible predictions as outputs. In this paper I focus on a particular kind of inference a computer system might make: identification of the intentions with which a person acted on the basis of photographic evidence. Such inferences are liable to be morally objectionable, because of a way in which they (...) are presumptuous. After elaborating this moral concern, I explore the possibility that carefully procuring the training data for image recognition systems could ensure that the systems avoid the problem. The lesson of this paper extends beyond just the particular case of image recognition systems and the challenge of responsibly identifying a person’s intentions. Reflection on this particular case demonstrates the importance (as well as the difficulty) of evaluating machine learning systems and their training data from the standpoint of moral considerations that are not encompassed by ordinary assessments of predictive accuracy. Sarah McGrath has recently defended a disagreement-based argument for skepticism about moral knowledge. If sound, the argument shows that our beliefs about controversial moral issues do not amount to knowledge. In this paper, I argue that McGrath fails to establish her skeptical conclusion. I defend two main claims. First, the key premise of McGrath’s argument is inadequately supported. Second, there is good reason to think that this premise is false.

In his critique of the extended mind hypothesis, Robert Rupert suggests that we have no reason to move from the claim that cognition is deeply embedded in the environment to the more radical claim that, in some cases, cognition itself extends into the environment. In this paper, I argue that we have strong normative reasons to prefer the more radical extended mind hypothesis to Rupert’s modest embedded mind hypothesis. I take an agnostic position on the metaphysical debate about the ultimate (...) nature and location of the mind, and instead argue in favor of the extended mind framework on the basis of its ability to better capture normative concerns about the way we evaluate the cognitive capacities of learning disabled individuals. In light of the commitments of the embedded and extended mind frameworks, defenders of the embedded mind framework are committed to conclusions about learning-disabled individuals that we have good normative reason to reject, whereas the extended mind framework avoids such problematic conclusions. Thus, if we find these normative concerns persuasive, we have good reason to prefer the extended mind position. (shrink)

In what ways have our Western biases and philosophical underpinnings influenced and changed how shamanism is practiced, both in the West and in the traditional cultures out of which they emerged? Is it time to stop using the umbrella term “shamanism” to refer to such diverse cross-cultural practices? What are our responsibilities, both as researchers and as spiritual seekers? The authors examine the nursing practice of disposing unaltered controlled substances into public water systems as an issue for nurses concerned with the environmental harm it can cause. A summary of the history of controlled substance management reveals inconsistencies in the interpretation of current regulations that have led to disposal policies that vary by institution, according to a benchmarking survey of regional hospitals. Much attention has been given to the phenomenon of conscientious objection in the context of patient care that (...) conflicts with a nurse’s moral integrity. Nurses who are compelled to dispose narcotics down drains may also face this struggle. The authors submit that this disposal method is based on a type of double effect logic where preventing diversion is viewed as a good effect of flushing unused controlled substances that outweighs any harm it may cause, though there is little evidence to support this conclusion. Consequently, the topic deserves further study so that nurse managers and administrators can support and offer alternatives to nurses who object to flushing drugs down the drain. (shrink)

In approaching the subject of this symposium we have both worked from an assumption about the future of philosophia perennis. This is that a better general assessment of Ludwig Wittgenstein’s philosophy of language in his Philosophical Investigations is likely to lead to better work in the crucial areas of metaphysics and epistemology. We shall not argue for this assumption here. But we begin by stating it to make clear why we shall attempt some particular assessments of a few passages early (...) in the Investigations and of a few famous remarks offered about them by Wittgenstein’s close disciple and critical commentator, Rush Rhees. For we are flirting with at least two further assumptions. A stronger and riskier one is that the strengths and weaknesses of the Investigations’ initial discussions may well have had a profound effect on the wisdom of the work as a whole. A weaker one is that if we can find sources of error as well as inspiration in some of Wittgenstein’s beginning remarks and in some of his dedicated followers’ comments, then other philosophers will be less intimidated by his philosophical and literary genius or by the obvious talents of many who are called Wittgensteinians. Thus we hope that our particular investigations can contribute to a more effective general assessment of his philosophy of language and so stimulate further hopes among those who believe—as we do—that the most central problems of philosophy can gradually be solved but never be dissolved. By examining their thoughts on this particular situation, we may draw out, by the use of “right reason,” ways to handle everyday situations that causes us to face the tragic choice between two goods that lying presents.
