Harmony is one of the most fundamental Japanese values. It is derived from Confucianism and encompasses a state of mind, an action process and outcomes of the action. This article draws on research data and discusses Japanese nurses’ perceptions of harmony as reflected in their everyday practice. The most important virtues for these nurses were reported as politeness and respect for other persons. The outcome from the nurses’ harmonious practice, it is claimed, benefited patients and created peaceful, harmonious relationships for (...) all. Because of the unique link between harmony and the location of interaction, the ideal ‘workplace harmony’ threatened some nurses’ professional decision making. These nurses confused harmony with conformity by superficial agreement. The Japanese seniority system could be a major factor contributing to this problem. Ethics education that includes traditional values and concepts in Japanese culture is strongly urged. (shrink)

There has been a long-standing controversy in the context of language evolution on whether the original function of human language was internal thought or external communication. However, given the fact that language clearly serves both functions, internalization and externalization must have been co-evolutionarily acted in the emergence of human language. This article proposes a theoretical hypothesis about this co-evolutionary relationship of internalization and externalization, which especially explains the emergence of the human lexicon. To discuss the evolution of language from a (...) comprehensive perspective, this article proposes a promising model that integrates two approaches with different standpoints: generative grammar and cognitive linguistics. This paper also examines the definition and nature of the lexicon and lexical items based on this integrated model. The hypothesis presented here demonstrates that the co-evolutionary work of internalization and externalization has been involved in two processes in the development of the lexicon: the establishment of syntactic objects (lexical items) and the improvement of creativity responsible for the expansion of lexicon size. The main conclusion is that these processes have formed a positive feedback loop and provided our lexicon with complex and unique properties. (shrink)

Robert Brandom’s inferentialism notoriously entails meaning holism, which has often been seen as unacceptable because it seems to make communication impossible. This paper aims to improve Brandom’s conception of communication as “navigation-across-perspectives” to reconcile meaning holism and the possibility of communication. The conception proposed here entails keeping track of speakers’ own and the other’s scores of commitments and entitlements. I argue that the whole of commonly endorsed inferences in each practice should determine the contents of utterances and those of the (...) commitments of the participants. The local norm defined by such inferences is holistic and can make sense of each other’s commitments. Contents or meaning can be primarily determined at the level of each practice, not that of an individual, the whole community, or the objective world. (shrink)

The objective of this paper is to establish a theoretical foundation for food education in schools. Amartya Sen’s capability approach (CA), an ethical theory concerning the freedom required to achieve one’s well-being, was applied to define the previously unchallenged ethical nature of food education. The analyses were informed by foundational CA concepts, Sen’s own perspectives on ‘food’ and ‘education’, and CA-based education studies. Through these analyses, the fundamental nature of food education was defined as follows: ‘Capability to eat well’ is (...) the freedom to convert commodities (such as food or pocket money) into the ‘functionings to eat well’ that constitute children’s ‘well-eating’ (Definition 1); and the objective of food education as public policy is to expand such capability to eat well (Def. 2). However, if an appropriate pedagogy is not employed, children’s capability can be reduced (Def. 3); therefore, to avoid this, the pedagogy has to enable them to perceive more ‘associations’ (in a Deweyan sense) between related conversion factors (Def. 4), as well as to choose and participate in the educational process (Def. 5). This theory not only elucidated the ethical nature of food education but also expanded the scope of the application of the CA to food issues. (shrink)

The objective of this article is to gain an in-depth understanding of the eating lives of low-income single mothers in Japan. Semi-structured interviews were conducted with nine low-income single mothers living in the three largest urban areas (Tokyo, Hanshin [Osaka and Kobe] and Nagoya) in Japan. Framed by the capability approach and sociology of food, their dietary norms and practices, as well as underlying factors that impact the norm-practice gap were analysed across nine dimensions: meal frequency, place of eating, meal (...) timing, duration, persons to eat with, procurement method, food quality, meal content and pleasure of eating. These mothers were deprived of various types of capabilities, extending not only from the quantity and nutritional aspects of food, but also to spatial, temporal, qualitative and affective aspects. Aside from financial constraints, eight other factors (time, maternal health, parenting difficulties, children’s tastes, gendered norms, cooking abilities, food aid and local food environment) were identified as influencing their capabilities to eat well. The findings challenge the view that food poverty is the deprivation of economic resources required to ensure a sufficient amount of food. Social interventions that go beyond monetary aid and food provision need to be proposed. (shrink)

Amartya Sen’s work has contributed to shifting our focus from food availability to food access and utilisation, together called ‘nutritional capability’. Existing food insecurity instruments have been informed partially by the capability approach, but remain focused on material deprivation and its economic sub-dimensions. This narrow focus has become problematic, particularly in high-income countries, where material deprivation is largely overcome and food poverty manifests itself differently from that in low-income countries. Food poverty in high-income countries should thus be approached from a (...) ‘food capability’ perspective that fully recognises the multidimensionality of eating lives and the multiple factors that affect their dietary standards. To demonstrate the usefulness of this perspective, the Alkire-Foster method for multidimensional food poverty was utilised to analyse food poverty in Japan. The Alkire-Foster measure has a dual cut-off approach and its adapted method can produce a multidimensional food poverty index by setting reasonable food deprivation and poverty thresholds. The national data used for empirical demonstration was obtained from a web-based questionnaire conducted in Japan, to which 973 participants (aged 20–60 years) responded. The measurement identified a 20.6% food-poor population and inequalities in food capabilities among subgroups. Although economic deprivation was tightly linked to food poverty, the results suggested that gender- and age-based inequalities were larger than socioeconomic status-based ones, which reflected much wider societal problems in Japan than growing economic poverty, such as gender inequality and population ageing. The proposed measurement is effective for monitoring food policy impacts and complementing existing food insecurity assessment tools. The results will be useful for promoting social debate about what the minimal dietary standard should be in high-income societies. (shrink)

Improving public understanding and acceptance are critical for promoting coronavirus vaccination. However, how to promote COVID-19 vaccine programs remains controversial due to various ethical issues. This study, thus, aimed to survey the acceptance of COVID-19 vaccines among Japanese citizens and discuss relevant ethical issues. A cross-sectional survey was conducted via an online platform. An anonymous, quantitative, self-administered online questionnaire was sent to 6965 registered Japanese residents, which included questions regarding the respondent’s general knowledge, experience, and opinions of vaccines, vaccine development, (...) COVID-19, and COVID-19 vaccines. Of the 1569 respondents, 730 and 839 were categorized into the younger and older groups, respectively. Most of the respondents possessed general knowledge of COVID-19 vaccines and their features. Of the respondents, 57.8% definitely agreed or somewhat agreed to receive COVID-19 vaccines. The older group showed significantly greater willingness to receive vaccines and higher literacy regarding vaccines in general. Possible reasons for the older group’s greater willingness to receive COVID-19 vaccines are a high risk of severe COVID-19 infection and their past accumulated experience of receiving various vaccinations. Although active public intervention could increase vaccination rates, most of the respondents did not agree with mandatory vaccination. Furthermore, a gap between the participants in the COVID-19 vaccine trials and the prioritized population in real-world vaccination should be adjusted in future vaccine development. (shrink)

In the Sorbonne lectures on the philosophical and psychological inquiry of child development, Merleau-Ponty offers a fundamental insight about imitation. Denying the representation-based explanation of imitation, he proposes that gestures occur without representation through the body-object relation, such as “precommunication” based on the works of body schema. Merleau-Ponty’s thought could be examined by way of more practical examples of body techniques. This paper describes the experience of object manipulation, in particular, Bunraku puppetry. Because three puppeteers manipulate a single puppet together (...) in Bunraku, this example might be a challenge to an ordinary assumption that a body is owned by an individual and that inner thoughts control the body. Merleau-Ponty’s insight suggests that the puppeteers share another type of body schema that is not internalized to their individual bodies but emerges afresh in each performance through collaborative movement. This article, written from research data, focuses on the possible meaning of the data rather than on detailed statistical reporting. It defines whistleblowing as an act of the international nursing ethical ideal of advocacy, and places it in the larger context of professional responsibility. The experiences, actions, and ethical positions of 24 Japanese nurses regarding whistleblowing or reporting a colleague for wrongdoing provide the data. Of these respondents, similar in age, educational level and clinical experience, 10 had previously reported another (...) nurse and 12 had reported a physician for a wrongful act. These data raise questions about overt actions to expose a colleague in a culture that values group loyalty and saving face. Additional research is needed for an in-depth understanding of whistleblowing, patient advocacy and professional responsibility across cultures, especially those that value group loyalty, saving face and similar concepts to the Japanese Ishin Denshin, where the value is on implicit understanding requiring indirect communication. Usually, being direct and openly discussing sensitive topics is not valued in Japan because such behavior disrupts the most fundamental value, harmony (wa). (shrink)

This pilot study had two purposes: (1) to review recent Japanese nursing literature on nursing advocacy; and (2) to obtain data from nurses on advocacy. For the second purpose, 24 nurses at a nursing college in Japan responded to a questionnaire. The concept of advocacy, taken from the West, has become an ethical ideal for Japanese nurses but one that they do not always understand, or, if they do, they find it difficult to fulfil. They cite nursing leadership support as (...) necessary to enacting this role. Discussion on the meaning of and the rationale for advocacy in a society where goodness or badness is relative to social situations and its impact may reveal two parallel but overlapping views of morality. Such a situation would not only influence notions of advocacy but also possibly render them more complex. (shrink)

Two questionnaire surveys are reported describing the attitudes of 127 Japanese nurses towards developing a do not resuscitate policy. The background information features the Japanese health care situations: a lack of policies for end-of-life care decisions; frequent life-prolonging treatments initiated without the patient’s knowledge or consent; ethical dilemmas confronting nurses in relation to such treatments; and the public’s growing concern over end-of-life care. A hypothetical DNR policy was used in which a health professional asked patients about their decision regarding DNR. (...) The respondents supported this model DNR policy as an ideal. However, they noted that this policy was at variance with Japanese culture. Recommendations for Japanese nurses emerged from the data. In order to make an impact on end-of-life decision making, nurses should take culturally appropriate actions: teach patients about their rights; and work with physicians to provide patients with accurate information about their condition. End-of-life issues have become an urgent problem in Japan, where people are among the longest lived in the world and most of them die while connected to high-technology medical equipment. This study examines a sensitive end-of-life ethical issue that concerns patients, families and nurses: the withdrawal of artificial food and fluid from terminally ill patients. A sample of 160 Japanese nurses, who completed a questionnaire that included forced-choice and open-ended questions, supported this act under only two specific conditions: if the (...) patient requested it, and if it relieved the patient’s suffering. They considered that the doctor’s orders, the family’s request, or the patient’s advanced age did not ethically justify this act. A small number of people who had recently lost a relative took part in semistructured interviews focusing on their experiences of their terminally ill relatives being given artificial food and fluid. Ethical, social and cultural factors surrounding this issue are discussed. (shrink)

This paper examines how inner theories influence lexical choice. Subjects in one study completed auditorily presented sentence fragments, some of which contained nonhuman antecedents such as dog and cat. Subjects were more likely to use human pronouns rather than it for referring to pets rather than nonpets , and antecedents that were liked rather than disliked , familiar rather than unfamiliar , named rather than unnamed , rational rather than nonrational , and engaging in typically human rather than nonhuman activities (...) . The beliefs and attitudes underlying these pronoun choices intruded not just hypoconsciously , but contraconsciously . A second study examined rapidly generated inferences that influenced what pronoun subjects chose to complete visually presented sentences, such as "When a student practices basketball instead of studying" and "When a student practices ballet instead of studying." The results indicated that inferences from beliefs altered the choice of pronoun from predominately he to predominantly she. Other results indicated that pronouns have semi-autonomous functions of their own, and reflect beliefs and attitudes that vary with subject sex, and are context dependent, pragmatic in nature, and dynamic over time. This article reports a survey of nurses in different cultural settings to reveal their perceptions of ethical role responsibilities relevant to nursing practice. Drawing on the Confucian theory of ethics, the first section attempts to understand nursing ethics in the context of multiple role relationships. The second section reports the administration of the Role Responsibilities Questionnaire (RRQ) to a sample of nurses in China (n = 413), the USA (n = 163), and Japan (n = 667). Multidimensional preference analysis revealed (...) the patterns of rankings given by the nurses to the statements they considered as important ethical responsibilities. The Chinese nurses were more virtue based in their perception of ethical responsibilities, the American nurses were more principle based, and the Japanese nurses were more care based. The findings indicate that the RRQ is a sensitive instrument for outlining the embedded sociocultural factors that influence nurses’ perceptions of ethical responsibilities in the realities of nursing practice. This study could be important in the fostering of partnerships in international nursing ethics. (shrink)

Affect misattribution occurs when affective cues color subsequent unrelated evaluations. Research suggests that affect misattribution decreases when one is aware that affective cues are unrelated to the evaluation at hand. We propose that affect misattribution may even occur when one is aware that affective cues are irrelevant, as long as the source of these cues seems ambiguous. When source ambiguity exists, affective cues may freely influence upcoming unrelated evaluations. We examined this using an adapted affect misattribution procedure where pleasant and (...) unpleasant responses served as affective cues that could influence later evaluations of unrelated targets. These affective cues were either perceived as reflecting a single source , or as reflecting two sources suggesting source ambiguity. Results show that misattribution of affect decreased when participants perceived affective cues as representing one source rather than two. (shrink)

This research explored the ethical issues that nurses reported in the process of elaboration and further disclosure after an initial diagnosis of a terminal illness had been given. One hundred and six hospice nurses in Norway and Denmark completed a questionnaire containing 45 items of forced-choice and open-ended questions. This questionnaire was tested and used in three countries prior to this study; for this research it was tested on Danish and Norwegian nurses. All respondents supported the ethics of ongoing disclosure (...) to terminally ill patients based on ethical principles embedded in their country’s Patients’ Rights Acts. Truth, as an intrinsic value, proved foundational to patient autonomy, the most frequent ethical principle these nurses reported to justify their ethical position on information disclosure to terminally ill people. Telling the truth about a diagnosis was not the end of ethics in hospice care, but rather the beginning because what occurs ethically in dealing with prognosis issues became central to these hospice nurses, the patients and their families. Coupled with truth-telling, compassionate interaction and care become extensions of patients’ rights. (shrink)

We examined amygdala subregion volumes in patients with a first episode of major depression and in healthy subjects. Covariate-adjusted linear regression was performed to compare the MD and healthy groups, and adjustments for age, gender, and total estimated intracranial volume showed no differences in amygdala subregion volumes between the healthy and MD groups. Within the MD group, we examined the association between amygdala subregion volume and the 17-item Hamilton Rating Scale for Depression score and the HAMD subscale score, and found (...) no association in the left amygdala. In the right amygdala, however, there was an inverse linear association between the HAMD total and the HAMD core and lateral nucleus and anterior-amygdaloid-regions. Furthermore, an inverse linear association was seen between the HAMD psychic and the lateral nucleus, anterior-amygdaloid-regions, transition, and whole amygdala. The findings of this study suggest that the severity of MD and some symptoms of MD are associated with right amygdala volume. There have been few reports on the relationship between MD and amygdala subregional volume, and further research is needed to accumulate more data for further validation. (shrink)

This study is based on the self-reporting by circus artists’ concerning their injuries. We refer to the theoretical framework of emersiology and argue that circus artists may be able to soothe their distress and pain by learning through their body. We will draw further on the comparison between our therapeutic approach and the techniques of self-care introduced by Michel Foucault in his History of Sexuality.

This study examines public health nurses’ perceptions and concerns about the implications of Japan’s new long-term care insurance law concerning care provision for elderly people and their families. Respondents voiced their primary concern about this law as access to services for all elderly people needing care, and defined their major responsibility as strengthening health promotion and illness prevention programmes. Although wanting to expand their roles to meet the health care, social and public policy advocacy needs of elderly persons and their (...) families, respondents also stated their concern for the possible lack of enough resources for this expansion to support family caregivers adequately. They viewed their first function as developing collaborative relationships with local government officials to help to assure sufficient resources to provide the necessary foundation for long-term care programmes to deliver services to all those in need. These concerns fall within the larger ethical issue of distributive justice in a society based on the obligations of the state to citizens and the family to its members, especially elderly relatives, who, according to traditional Japanese values, retain respect.
