Engaging communities in community-based health research is increasingly being adopted in low- and middle-income countries. The use of community advisory boards is one method of practicing community involvement in health research. To date, few studies provide in-depth accounts of the strategies that CAB members use to practice community engagement. We assessed the perspectives, experiences and practices of the first local CAB in Eswatini, which was implemented as part of the MaxART Early Access to ART for All study. Trained Swazi research (...) assistants conducted two focus group discussions and 13 semi-structured interviews with CAB members who had been part of the MaxART study for at least 2.5 years. Interviews explored CAB composition and recruitment, the activities of CAB members, the mechanisms used to engage with communities and the challenges they faced in their role. The MaxART CAB played an active role in the implementation of the Early Access to Art for All study, and activities mainly focused on: promoting ethical conduct, in particular privacy, consent and confidentiality; communication and education, communicating about the study and educating the community on the benefits of HIV testing and early access to HIV treatment; and liaising between the community and the research team. Strategies for interacting with communities were varied and included attending general community meetings, visiting health facilities and visiting public places such as cattle dipping tanks, buses, bars and churches. Differences in the approach to community engagement between CAB members living in the study areas and those residing outside were identified. The experiences of the first CAB in Eswatini demonstrate that community engagement using CABs is a valuable mechanism for engaging communities in implementation studies. Considerations that could impact CAB functioning include clearly defining the scope of the CAB, addressing issues of CAB independence, the CAB budget, providing emotional support for CAB members, and providing continuous training and capacity building. These issues should be addressed during the early stages of CAB formation in order to optimize functioning. (shrink)

This article describes challenges of conducting an HIV prevention program involving 40 male and female participants ages 12–18 in Hammanskraal, South Africa, aimed at increasing awareness and knowledge of laws protecting children’s sexual health rights and access to services through a culturally based “study circle” format. Challenges highlighted by the project included Institutional Review Board approval of youth consent procedures, cooperation and coordination with local policymakers, the need to modify presentation materials to youths’ comprehension levels, availability of youth-based sexual health (...) service providers, and cultural ambiguity over parental involvement in youth health care decisions and laws pertaining to sexual relationships among minors. It would take much work to sift the fabricated references in Borges' works from the ones deliberately misread from the over-emphasis on an author's casual remark, etc.

Multivariate pattern analysis can reveal new information from neuroimaging data to illuminate human cognition and its disturbances. Here, we develop a methodological approach, based on multivariate statistical/machine learning and time series analysis, to discern cognitive processing stages from functional magnetic resonance imaging (fMRI) blood oxygenation level dependent (BOLD) time series. We apply this method to data recorded from a group of healthy adults whilst performing a virtual reality version of the delayed win-shift radial arm maze (RAM) task. This task has (...) been frequently used to study working memory and decision making in rodents. Using linear classifiers and multivariate test statistics in conjunction with time series bootstraps, we show that different cognitive stages of the task, as defined by the experimenter, namely, the encoding/retrieval, choice, reward and delay stages, can be statistically discriminated from the BOLD time series in brain areas relevant for decision making and working memory. Discrimination of these task stages was significantly reduced during poor behavioral performance in dorsolateral prefrontal cortex (DLPFC), but not in the primary visual cortex (V1). Experimenter-defined dissection of time series into class labels based on task structure was confirmed by an unsupervised, bottom-up approach based on Hidden Markov Models. Furthermore, we show that different groupings of recorded time points into cognitive event classes can be used to test hypotheses about the specific cognitive role of a given brain region during task execution. We found that whilst the DLPFC strongly differentiated between task stages associated with different memory loads, but not between different visual-spatial aspects, the reverse was true for V1. Our methodology illustrates how different aspects of cognitive information processing during one and the same task can be separated and attributed to specific brain regions based on information contained in multivariate patterns of voxel activity. In this ground-breaking work, Deborah Cowen makes the first book-length critical intervention into the field of business logistics. Tracing the social, spatial and political transformations wrought by the ‘logistics revolution’, Cowen argues that logistical systems have blurred the boundaries between production and circulation, civilian and military life, and geopolitics and geo-economics, constructing an architecture of the supply chain animated by both the art of war and the science of business. This review considers the political stakes of Cowen’s argument that logistics (...) has reconfigured capitalist social relations of production, especially in relation to the securitisation of spatial mobility and the restructuring of the labour process. In particular, Cowen’s wager that logistical supply-chains are characterised by their vulnerability – and that attendant possibilities for building solidarity emerge out of movements that disrupt such flows – is interrogated in order to question whether such interruptions effectively produce challenges to value-in-motion, or actually become integral to processes of value production. A standard objection to so-called ‘minimal semantics’ (Borg 2004, 2012, Cappelen and Lepore 2005) is that minimal contents are explanatorily redundant as they play no role in an adequate account of linguistic communication (those making this objection include Levinson 2000, Carston 2002, Recanati 2004). This paper argues that this standard objection is mistaken. Furthermore, I argue that seeing why the objection is mistaken sheds light both on how we should draw the classic Gricean distinction between saying and implicating, and (...) how we should think about the key philosophical notion of assertion. Specifically, it reveals that these ideas are best understood primarily in socio-linguistic terms (resting on the degree of liability a speaker is held to have for linguistically conveyed content). But I suppose Hume might be worked in also, because, (...) after all, Hume refutes Berkeley. Really, he comes from Berkeley—even if Berkeley comes from Locke. You might think of Locke, of Berkeley, and of Hume as being three links in an argument..... But I suppose that when someone refutes someone else in philosophy, he is carrying on the argument. Emma Borg examines the relation between semantics and pragmatics, and assesses recent answers to fundamental questions of how and where to draw the divide between the two. She argues for a minimal account of the interrelation between them--a 'minimal semantics'--which holds that only rule-governed appeals to context can influence semantic content.

Business Ethics: A European Review, EarlyView.

In this paper I want to explore the arguments for so-called ‘unarticulated constituents’ (UCs). Unarticulated constituents are supposed to be propositional elements, not presented in the surface form of a sentence, nor explicitly represented at the level of its logical form, yet which must be interpreted in order to grasp the (proper) meaning of that sentence or expression. Thus, for example, we might think that a sentence like ‘It is raining’ must contain a UC picking out the place at which (...) the speaker of the sentence asserts it to be raining. In §1 I will explore the nature of UCs a little further, and, in §2, suggest that we can recognise two different forms of argument for them.. (shrink)

I propose and defend the conjecture that what explains why Gettiered subjects fail to know is the fact that their justified true belief depends essentially on unknown propositions. The conjecture follows from the plausible principle about inference in general according to which one knows the conclusion of one’s inference only if one knows all the premises it involves essentially.

John N. Williams argued that Peter Klein's defeasibility theory of knowledge excludes the possibility of one knowing that one has (first-order) a posteriori knowledge. He does that by way of adding a new twist to an objection Klein himself answered more than forty years ago. In this paper I argue that Williams' objection misses its target because of this new twist.

Minimal Semantics asks what a theory of literal linguistic meaning is for - if you were to be given a working theory of meaning for a language right now, what would you be able to do with it? Emma Borg sets out to defend a formal approach to semantic theorising from a relatively new type of opponent - advocates of what she call 'dual pragmatics'. According to dual pragmatists, rich pragmatic processes play two distinct roles in linguistic comprehension: as (...) well as operating in a post-semantic capacity to determine the implicatures of an utterance, they also operate prior to the determination of truth-conditional content for a sentence. That is to say, they have an integral role to play within what is usually thought of as the semantic realm. Borg believes dual pragmatic accounts constitute the strongest contemporary challenge to standard formal approaches to semantics since they challenge the formal theorist to show not merely that there is some role for formal processes on route to determination of semantic content, but that such processes are sufficient for determining content. Minimal Semantics provides a detailed examination of this school of thought, introducing readers who are unfamiliar with the topic to key ideas like relevance theory and contextualism, and looking in detail at where these accounts diverge from the formal approach. Borg's defence of formal semantics has two main parts: first, she argues that the formal approach is most naturally compatible with an important and well-grounded psychological theory, namely the Fodorian modular picture of the mind. Then she argues that the main arguments adduced by dual pragmatists against formal semantics - concerning apparent contextual intrusions into semantic content - can in fact be countered by a formal theory. The defence holds, however, only if we are sensitive to the proper conditions of success for a semantic theory. Specifically, we should reject a range of onerous constraints on semantic theorizing (e.g., that it answer epistemic or metaphysical questions, or that it explain our communicative skills) and instead adopt a quite minimal picture of semantics. This paper develops a framework for establishing rigour for a discourse analysis of professional transition in midwifery, theorised as a ‘female professional project’. Discourse analysis has gained recognition as a useful approach in nursing and midwifery research. It provides an alternative to those qualitative approaches that propose to reveal a ‘reality’ from the perspective of the individual experience, and that this lived experience can be directly represented in language. There are multiple discourse analytic approaches, and often researchers are not explicit (...) about what type they are employing. Furthermore, to date there are few clear guidelines for what constitutes rigour in a discourse analytic study. Rigour in qualitative research broadly falls into two categories: the replication perspective that argues for the maintenance of criteria of validity and reliability, usually with an underpinning assumption (often unquestioned) that a ‘truth’ can be revealed. The second is the parallel perspective that questions the ontological and epistemological assumptions of the replication perspective and argues for the development of criteria that reflect the assumed questionable nature of truth and reality. Transferring these criteria to a discourse analytic study revealed some problems. A common challenge identified in achieving rigour in discourse analysis is the maintenance of congruity between the epistemological and ontological basis of a piece of research and the actual analysis conducted or reported. A framework is proposed to address these incongruities and includes six elements. As nurses and midwives increasingly employ discourse analysis in their research, attempts to clarify how rigour will be determined are important if the outcomes of such research are to be seriously considered for their implications for theory, policy and practice. In this paper, the relationship between the e-value of a complex hypothesis, H, and those of its constituent elementary hypotheses, Hj, j = 1… k, is analyzed, in the independent setup. The e-value of a hypothesis H, ev, is a Bayesian epistemic, credibility or truth value defined under the Full Bayesian Significance Testing mathematical apparatus. The questions addressed concern the important issue of how the truth value of H, and the truth function of the corresponding FBST structure M, relate to (...) the truth values of its elementary constituents, Hj, and to the truth functions of their corresponding FBST structures Mj, respectively. Saul Kripke argued that the requirement that knowledge eliminate all possibilities of error leads to dogmatism . According to this view, the dogmatism puzzle arises because of a requirement on knowledge that is too strong. The paper argues that dogmatism can be avoided even if we hold on to the strong requirement on knowledge. I show how the argument for dogmatism can be blocked and I argue that the only other approach to the puzzle in the literature is mistaken.

There is a sense in which it is trivial to say that one accepts intention- (or convention-) based semantics.[2] For if what is meant by this claim is simply that there is an important respect in which words and sentences have meaning (either at all or the particular meanings that they have in any given natural language) due to the fact that they are used, in the way they are, by intentional agents (i.e. speakers), then it seems no one should (...) disagree. For imagine a possible world where there are physical things which share the shape and form of words of English or Japanese, or the acoustic properties of sentences of Finnish or Arapaho, yet where there are no intentional agents (or where any remaining intentional agents don’t use language). In such a world, it seems clear that these physical objects, which are only superficially language-like, will lack all meaning. Furthermore, it seems that questions of particular meaning are also settled by the conventions of intentional language users: it’s nothing more than convention which makes the concatenation of letters ‘a’^‘p’^‘p’^‘l’^‘e’ mean apple, rather than banana, in English.[3] So, understood as the minimal claim that intentional agents, who have a practice of using certain physical objects (written words, sounds, hand gestures, etc) to communicate certain thoughts, are a prerequisite for linguistic meaning, the idea that semantics is based on both intention and convention seems indisputable. I will label a theory which recognises this preconditional role for speaker intentions an A-style intention-based semantics and we will explore one such account in §1. (shrink)

In 'Local pragmatics in a Gricean framework', Mandy Simons argues that, contrary to the received view, it is possible to accommodate local pragmatic effects utilising just the mechanisms for pragmatic reasoning provided by Grice. Although I agree with this overarching claim, this paper argues that we need to be careful in our understanding of 'what is said', and the nature of communicated content in general, when deciding between local and global accounts of pragmatic effects.

In June 2002, the University of Minnesota hosted a conference to explore the implications of using genetic technologies and genealog.

The enrolment of children and adolescents in health research requires that attention to be paid to specific assent and consent requirements such as the age range for seeking assent; conditions for parental consent (and waivers); the age group required to provide written assent; content of assent forms; if separate assent and parental consent forms should be used, consent from emancipated young adults; reconsent at the age of adulthood when a waiver of assent requirements may be appropriate and the conditions for (...) waiving assent requirements. There is however very little available information for researchers and ethics committees on how to navigate these different issues. To provide guidance to research initiatives, the SickleInAfrica consortium conducted a thematic analysis of a sample of research ethics guidelines and procedures in African countries, to identify guidance for assent requirements in health research. The thematic analysis revealed that 12 of 24 African countries specified the age group for which assent is required. The minimum age for written assent varied across the countries. Five countries, Algeria, Botswana, Cameroon, Nigeria and The Democratic Republic of Congo require consent from both parents/family council in certain circumstances. Botswana, Nigeria, South Africa and Uganda have specific assent/consent requirements for research with emancipated minors. South Africa and Algeria requires re-consent at onset of adulthood. Five countries (Botswana, Cameroon, Nigeria, South Africa and Tanzania) specified conditions for waiving assent requirements. The CIOMS and the ICH-GCP guidelines had the most comprehensive information on assent requirements compared to other international guidelines. An interactive map with assent requirements for different African countries is provided. The results show a major gap in national regulations for the inclusion of minors in health research. The SickleInAfrica experience in setting up a multi-country SCD registry in Africa highlights the need for developing and harmonising national and international guidelines on assent and consent requirements for research involving minors. Harmonisation of assent requirements will help facilitate collaborative research across countries. (shrink)

The present article introduces and describes an innovative educational approach to learning processes, the Imagineering collaborative methodology. The methodology has been developed as a part of the learning curriculum for the Imagineering masters course. The theoretical and practical framework is explained, offering some illustrations in order to demonstrate the possibilities and implications of embracing such an approach in an educational environment. This article proposes a systemic, collaborative, and relational view on education that avoids reductive and disjunctive learning processes, inviting students (...) to be designers of their learning, preparing them to deal professionally with the demands of the twenty-first century, more aligned with a radically changing landscape in today's society. Traditional approaches in healthcare have been challenged giving way to broader forms of users’ participation in treatment. In this article we present the Health 2.0 movement as an example of relational and participatory practices in healthcare. Health 2.0 is an approach in which participation is the major aim, aspiring to reshape the system into more collaborative and less hierarchical relationships. We offer two illustrations in order to discuss how Health 2.0 is related and can contribute to a positive uptake of (...) patient’s knowledge, which implies challenging healthcare practices exclusively focused on scientific expertise. In contrast, the illustrations we discuss focus on relations and cultural practices, searching for responsive and context-sensitive interventions, entertaining multiple views and allowing space for creativity. Finally we introduce two relational resources to contribute with the development and sustainability of Health 2.0 practices: Relational being and edge of fluidity. Those are resources aiming to engage professionals in a type of conversation with their clients, which is different from the hierarchical, linear and fact-oriented approach. This conversation aims at creating a space where the voices of all involved are welcomed, raising different opinions and points of view, bringing up new light and possibilities to the problem being investigated. These resources may be useful for those who are interested in improving quality in healthcare by investing in collaboration, contextual sensitivity and relational engagement. (shrink)

Pero, los medios de comunicación hacen algo más que informarnos: eligen y organizan jerárquicamente nuestro menú cotidiano de informaciones relevantes. Vardanyan’s Theorems [36, 37] state that $\mathsf {QPL}(\mathsf {PA})$ —the quantified provability logic of Peano Arithmetic—is $\Pi ^0_2$ complete, and in particular that this already holds when the language is restricted to a single unary predicate. Moreover, Visser and de Jonge [38] generalized this result to conclude that it is impossible to computably axiomatize the quantified provability logic of a wide class of theories. However, the proof of this fact cannot be performed in a strictly positive signature. The system $\mathsf (...) {QRC_1}$ was previously introduced by the authors [1] as a candidate first-order provability logic. Here we generalize the previously available Kripke soundness and completeness proofs, obtaining constant domain completeness. Then we show that $\mathsf {QRC_1}$ is indeed complete with respect to arithmetical semantics. This is achieved via a Solovay-type construction applied to constant domain Kripke models. As corollaries, we see that $\mathsf {QRC_1}$ is the strictly positive fragment of $\mathsf {QGL}$ and a fragment of $\mathsf {QPL}(\mathsf {PA})$. (shrink)

John Searle has argued that the aim of strong AI of creating a thinking computer is misguided. Searle’s Chinese Room Argument purports to show that syntax does not suffice for semantics and that computer programs as such must fail to have intrinsic intentionality. But we are not mainly interested in the program itself but rather the implementation of the program in some material. It does not follow by necessity from the fact that computer programs are defined syntactically that the implementation (...) of them cannot suffice for semantics. Perhaps our world is a world in which any implementation of the right computer program will create a system with intrinsic intentionality, in which case Searle’s Chinese Room Scenario is empirically (nomically) impossible. But, indeed, perhaps our world is a world in which Searle’s Chinese Room Scenario is empirically (nomically) possible and that the silicon basis of modern day computers are one kind of material unsuited to give you intrinsic intentionality. The metaphysical question turns out to be a question of what kind of world we are in and I argue that in this respect we do not know our modal address. The Modal Address Argument does not ensure that strong AI will succeed, but it shows that Searle’s challenge on the research program of strong AI fails in its objectives. (shrink)

However, it seems that the majority of demonstrative occurrences do not follow this model. ‘Complex demonstratives’ is the collective term I shall use for phrases formed by adjoining one or more common nouns to a demonstrative expression (e.g. ‘that cat’, ‘this happy man’) and I will call the combination of predicates immediately concatenated with the demonstrative in such phrases (...) the ‘matrix’ of the expression. The question, then, is how we should construe the logical form of such expressions within a semantic theory for our language; and I wish to suggest that some recent answers to this question are, in fact, mistaken. The structure of the paper is as follows: first, I wish to highlight two (often underlying) assumptions about the nature of noun phrases in general, and suggest that, if they are both adopted, they apparently constrain the possible accounts of the logical form of complex demonstratives to just three options. The second and third parts of the paper will be concerned with expanding these options and arguing that none of them are adequate; thus the major part of the paper is concerned with the negative claim that the most obvious moves to make in this area must actually fail on closer inspection. The fourth and final section will then (very briefly) sketch the positive thesis of the paper: that we can deliver a clear and cohesive account of complex demonstratives, armed simply with elements which we will draw from David Kaplan’s theory of demonstratives, but only at the cost of rejecting (or at least refining) one assumption we recognised initially. A direct implication of E=K seems to be that false beliefs cannot justify other beliefs, for no false belief can be part of one’s total evidence and one’s total evidence is what inferentially justifies belief. The problem with this alleged implication of E=K, as Comesaña and Kantin :447–454, 2010) have noted, is that it contradicts a claim Gettier cases rely on. The original Gettier cases relied on two principles: that justification is closed under known entailment, and that sometimes one is (...) justified in believing a falsehood. In this paper I argue that E=K, contrary to what Comesaña and Kantin would want us to believe, is compatible with the agent being justified in believing a falsehood. (shrink)

This paper deals with the question of whether nature sports are to be counted among the (traditional) sports and Kevin Krein’s recent argument, based on sporting excellence, as to why they should. Krein argues that sports as such are ultimately about sporting excellence and because both so-called traditional sports and nature sports fulfil that criterion, nature sports belong in the sport domain. Here, I show that Krein’s argument rests on an equivocation between two concepts of sporting excellence. Sporting excellence in (...) traditional sports is connected to sporting success. Here agon (contest or competition) dictates or frames areté (striving for excellence). This is not the case for sporting excellence in nature sports, as these are not competitions. We are dealing with two different concepts of sporting excellence and Krein’s argument, based on sporting excellence, must be rejected. It gives no basis for including nature sports in the sport domain. (shrink)

This study looked for evidence of biases in the allocation and disengagement of attention in dysphoric individuals. Participants studied images for a recognition memory test while their eye fixations were tracked and recorded. Four image types were presented (depression-related, anxiety-related, positive, neutral) in each of two study conditions. For the simultaneous study condition, four images (one of each type) were presented simultaneously for 10 seconds, and the number of fixations and the total fixation time to each image was measured, similar (...) to the procedure used by Eizenman et al. (2003) and Kellough, Beevers, Ellis, and Wells (2008). For the sequential study condition, four images (one of each type) were presented consecutively, each for 4 seconds; to measure disengagement of attention an endogenous cuing procedure was used (Posner, 1980). Dysphoric individuals spent significantly less time attending to positive images than non-dysphoric individuals, but there were no group differences in attention to depression-related images. There was also no evidence of a dysphoria-related bias in initial shifts of attention. With respect to the disengagement of attention, dysphoric individuals were slower to disengage their attention from depression-related images. The recognition memory data showed that dysphoric individuals had poorer memory for emotional images, but there was no evidence of a conventional mood-congruent memory bias. Differences in the attentional and memory biases observed in depressed and dysphoric individuals are discussed. (shrink)

Resumo: A acessibilidade como tema transversal à formação dos egressos da Universidade Federal do Pampa se faz presente tanto no projeto institucional da universidade quanto nos Projetos Pedagógicos dos Cursos. No entanto, embora tal intencionalidade constar nos documentos institucionais, a materialização de tal temática - para além da garantia dos requisitos legais de acessibilidade presentes nos instrumentos de avaliação do ensino superior - se mostra incipiente. No curso de Medicina, apesar do tema da diversidade e, da garantia de equidade sem (...) qualquer tipo de distinção, no atendimento da rede básica, compor as Diretrizes Curriculares Nacionais para a formação dos futuros médicos, a discussão sobre a temática da deficiência é recente, sendo atualmente impulsionada pela demanda de recursos para este público, em específico, na atenção básica de saúde, em função da pandemia da COVID-19. Neste artigo, apresentamos as práticas interdisciplinares que vêm sendo protagonizadas entre o Curso de Medicina e o Programa de Mestrado Profissional em Ensino de Ciências. Dentre estas práticas estão a produção de pranchas em Comunicação Alternativa Aumentativa (CAA) para apoio ao diagnóstico do COVID-19 em pacientes com deficiência; materiais instrucionais acessíveis sobre temas envolvendo a pandemia; curso de CAA para estudantes da área da saúde e formação de profissionais dos serviços de saúde para o atendimento de usuários com deficiência. Estas práticas e suas consequências para a formação dos estudantes de Medicina, constitui o objetivo deste trabalho. However, although such intentionality appears in the institutional documents, the materialization of such theme – apart from the guarantee of the accessibility legal requirements present in the evaluation instruments of the university education – is still incipient. Even in the medical course, in spite of the diversity theme and the guarantee of equality without any kind of distinction in the basic system assistance compose the National Curricular Guidelines to the education of the futures doctors, is recent the discussion about the disability theme, being nowadays stimulated by the demand of resources required for this specific type of public in the primary health care, arising from the COVID-19 pandemic. In this article, we present the interdisciplinary practices that have been executed in the Medical Course and Professional Master’s Degree on Sciences Teaching. Among these practices we have the production of boards in Augmentative Alternative Communication (AAC) for supporting the COVID-19 diagnosis in disabled patients; accessible instructional materials about themes concerning the pandemic; AAC courses for health area students and training of health services professionals for the assistance of disabled users. These practices and their consequences for the education of Medical students are the objective of the present work. Sin embargo, aunque tal intencionalidad está incluida en los documentos institucionales, la materialización de tal tema, además de garantizar los requisitos legales de accesibilidad presentes en los instrumentos de evaluación de la educación superior, es aún incipiente. En el curso de medicina, a pesar del tema de la diversidad y la garantía de equidad sin ningún tipo de distinción, en la atención de la red básica, componiendo los Lineamientos Curriculares Nacionales para la formación de futuros médicos, la discusión sobre el tema de la discapacidad es reciente., siendo impulsado actualmente por la demanda de recursos para este público, específicamente, en la atención primaria de salud, debido a la pandemia de COVID-19. En este artículo presentamos las prácticas interdisciplinarias que se vienen dando entre el Curso de Medicina y el Programa de Maestría Profesional en Enseñanza de las Ciencias. Entre estas prácticas se encuentran la producción de tableros en Comunicación Aumentativa Alternativa (CAA) para apoyar el diagnóstico de COVID-19 en pacientes con discapacidad; materiales educativos accesibles sobre temas relacionados con la pandemia; Curso CAA para estudiantes del área de la salud y formación de profesionales de los servicios de salud para la asistencia a usuarios con discapacidad. Estas prácticas y sus consecuencias para la formación de los estudiantes de medicina, constituyen el objetivo de este trabajo.
