While valuable work has been done addressing clinical ethics within established healthcare systems, we anticipate that the projected growth in acquisitions of community hospitals and facilities by large tertiary hospitals will impact the field of clinical ethics and the day-to-day responsibilities of clinical ethicists in ways that have yet to be explored. Toward the goal of providing clinical ethicists guidance on a range of issues that they may encounter in the systematization process, we discuss key considerations and potential challenges in (...) implementing system-wide ethics consultation services. Specifically, we identify four models for organizing, developing, and enhancing ethics consultation activities within a system created through acquisitions: train-the-trainer, local capacity-building, circuit-riding, and consolidated accountability. We note each model’s benefits and challenges. To our knowledge, this is the first paper to consider the broader landscape of issues affected by consolidation. We anticipate that clinical ethicists, volunteer consultants, and hospital administrators will benefit from our recommendations. (shrink)

The clinical ethicist met with Ms. H to clarify what information she wants and does not want to know. First, she wants to receive any treatment that could prolong her life, regardless of how the treatment affects her ability to engage in activities of daily living. Second, she wants to be included in the decision‐making process as much as possible, as long as clinicians use only “positive” language. Ms. H considers the words “dying,” “chemotherapy,” “radiation,” and “cancer” to be “bad (...) words.” For conversations in which these words cannot be avoided, she wants her clinicians to talk to her son. Her desired engagement includes hearing about risks, benefits, and alternatives to treatments if clinicians use only “positive” language. Finally, she says that she rarely sees doctors and that she is “very scared” of hospitals, despite exhibiting a comfortable demeanor.Should clinicians honor Ms. H's waiver of specified information and, if so, to what degree? An analysis is briefly presented of the possible causes of the failure of stochastic electrodynamics (SED) when applied to systems with nonlinear forces, on the basis that the main principles of the theory are correct. In light of this analysis, an alternative approach to the theory is discussed, whose postulates allow to establish contact with quantum mechanics in a natural way. The ensuing theory, linear SED, confirms the essential role of the vacuum–particle interaction as the source of quantum phenomena.

The present paper reveals (non-relativistic) quantum mechanics as an emergent property of otherwise classical ergodic systems embedded in a stochastic vacuum or zero-point radiation field (zpf). This result provides a theoretical basis for understanding recent numerical experiments in which a statistical analysis of an atomic electron interacting with the zpf furnishes the quantum distribution for the ground state of the H atom. The action of the zpf on matter is essential within the present approach, but it is the ergodic demand (...) what ultimately leads to the matrix formulation of quantum mechanics. The paper thus represents a step forward in the quest for an elucidation of the fundamentals of quantum mechanics. (shrink)

We discuss the main results of Linear Stochastic Electrodynamics, starting from a reformulation of its basic assumptions. This theory shares with Stochastic Electrodynamics the core assumption that quantization comes about from the permanent interaction between matter and the vacuum radiation field, but it departs from it when it comes to considering the effect that this interaction has on the statistical properties of the nearby field. In the transition to the quantum regime, correlations between field modes of well-defined characteristic frequencies arise, (...) which coincide with the transition frequencies of quantum mechanics and are therefore directly related with the energy quantization. The Heisenberg equations of motion of (non-relativistic) quantum electrodynamics are thus obtained. After a detailed consideration of the significance of the approximations made, we present a discussion on some of the most delicate or controversial features of quantum mechanics from the perspective provided by the present theory. We formulate from first principles a theory of stochastic processes in configuration space. The fundamental equations of the theory are an equation of motion which generalizes Newton's second law and an equation which expresses the condition of conservation of matter. Two types of stochastic motion are possible, both described by the same general equations, but leading in one case to classical Brownian motion behavior and in the other to quantum mechanical behavior. The Schrödinger equation, which is derived here with no (...) further assumption, is thus shown to describe a specific stochastic process. It is explicitly shown that only in the quantum mechanical process does the superposition of probability amplitudes give rise to interference phenomena; moreover, the presence of dissipative forces in the Brownian motion equations invalidates the superposition principle. At no point are any special assumptions made concerning the physical nature of the underlying stochastic medium, although some suggestions are discussed in the last section. (shrink)

Arguments are given in favor of a stochastic theory of quantum mechanics, clearly distinguishable from Brownian motion theory. A brief exposition of the phenomenological theory of stochastic quantum mechanics is presented, followed by a list of its main results and perspectives. A possible answer to the question about the origin of stochasticity is given in stochastic electrodynamics by assigning a real character to the vacuum radiation field. This theory is shown to reproduce important quantum mechanical results, some of which are (...) presented explicitly to illustrate its potentialities. Finally the main problems and some perspectives of research within stochastic electrodynamics are discussed. (shrink)

The origin of the wave properties of matter is discussed from the point of view of stochastic electrodynamics. A nonrelativistic model of a charged particle with an effective structure embedded in the random zeropoint radiation field reveals that the field induces a high-frequency vibration on the particle; internal consistency of the theory fixes the frequency of this jittering at mc2/ħ. The particle is therefore assumed to interact intensely with stationary zeropoint waves of this frequency as seen from its proper frame (...) of reference; such waves, identified here as de Broglie's phase waves, give rise to a modulated wave in the laboratory frame, with de Broglie's wavelength and phase velocity equal to the particle velocity. The time-independent equation that describes this modulated wave is shown to be the stationary Schrödinger equation (or the Klein-Gordon equation in the relativistic version). In a heuristic analysis appled to simple periodic cases, the quantization rules are recovered from the assumption that for a particle in a stationary state there must correspond a stationary modulation.Along an independent and complementary line of reasoning, an equation for the probability amplitude in configuration space for a particle under a general potential V(x) is constructed, and it is shown that under conditions derived from stochastic electrodynamics it reduces to Schrödinger's equation. This equation reflects therefore the dual nature of the quantum particles, by describing simultaneously the corresponding modulated waveand the ensemble of particles. (shrink)

The connection between stochastic electrodynamics (SED) and the quantum theory of matter is further explored. The main result is that the Fokker-Planck-like equation of SED can be recast into the form of a Schrödinger equation with radiative corrections, when the system is close to a state of equilibrium. The phase-space distribution can be written as Wigner's pseudo-distribution plus corrections due to the nonlinearity of the external force and to radiative effects. The radiative corrections predicted by the theory, namely the Lamb (...) shift and the decay of excited atomic states, coincide with those predicted by QED. Moreover, the theory offers a clear physical interpretation of these phenomena as due to the coupling of the electric dipole of the system with the zero-point radiation field and to radiation reaction, respectively. By making use of the method of moments we study some aspects of the statistical behavior of the nonrelativistic harmonic oscillator according to stochastic electrodynamics. We show that the random rotations induced on the particle by the zero-point field account for the magnitude of the spin of the electron, the result differing from the correct one(3/4)h 2 by a factor of2. Assuming that the measurement of a spin projection may be effectively taken into account by considering the action of only (...) the subensemble of the field with the corresponding circular polarization, the calculated value of the spin projection comes out to be the correct one within a factor of order unity. The radiative corrections give rise to both the Lamb shift and the anomalous magnetic moment of the electron, the latter being evaluated to within a factor of2. The magnetic and gyromagnetic properties of the electron come out to be in agreement with quantum mechanics. Interference effects are shown to occur when evaluating the average value of the square of the angular momentum. (shrink)

ObjectiveTo investigate the association of pre-existing migraine in patients hospitalised and who recovered from severe acute respiratory syndrome coronavirus 2 infection with the presence of post-coronavirus disease symptoms.BackgroundNo study has investigated the role of migraine as a risk factor for development of post-COVID symptoms.MethodsA case-control study including individuals hospitalised during the first wave of the pandemic was conducted. Patients with confirmed previous diagnosis of migraine were considered cases. Two age- and sex-matched individuals without a history of headache per case were (...) also recruited as controls. Hospitalisation/clinical data were collected from hospital medical records. Patients were scheduled for a telephone interview. A list of post-COVID symptoms was systematically evaluated, but participants were invited to freely report any symptom. The Hospital Anxiety and Depression Scale and the Pittsburgh Sleep Quality Index were used to assess anxiety/depressive symptoms and sleep quality. Multivariable conditional logistic regression models were constructed.ResultsOverall, 57 patients with confirmed diagnosis of migraine and 144 non-migraine controls who had recovered from COVID-19 were assessed at 7.3 months after hospital discharge. The number of post-COVID symptoms in the migraine group was significantly greater than in the non-migraine group. Fatigue was significantly more prevalent in the migraine group. However, no between-groups difference in the prevalence of headache as a post-COVID symptom was detected.ConclusionPatients with a history of migraine who recovered from COVID-19 exhibited more long-term fatigue as post-COVID sequelae than those without migraine. Some of the pathophysiological changes associated with migraine could predispose to the occurrence of post-COVID symptoms. (shrink)

In December 2013, the Nonhuman Rights Project (NhRP) filed a petition for a common law writ of habeas corpus in the New York State Supreme Court on behalf of Tommy, a chimpanzee living alone in a cage in a shed in rural New York (Barlow, 2017). Under animal welfare laws, Tommy’s owners, the Laverys, were doing nothing illegal by keeping him in those conditions. Nonetheless, the NhRP argued that given the cognitive, social, and emotional capacities of chimpanzees, Tommy’s confinement constituted (...) a profound wrong that demanded remedy by the courts. Soon thereafter, the NhRP filed habeas corpus petitions on behalf of Kiko, another chimpanzee housed alone in Niagara Falls, and Hercules and Leo, two chimpanzees held in research facilities at Stony Brook University. Thus began the legal struggle to move these chimpanzees from captivity to a sanctuary, an effort that has led the NhRP to argue in multiple courts before multiple judges. The central point of contention has been whether Tommy, Kiko, Hercules, and Leo have legal rights. To date, no judge has been willing to issue a writ of habeas corpus on their behalf. Such a ruling would mean that these chimpanzees have rights that confinement might violate. Instead, the judges have argued that chimpanzees cannot be bearers of legal rights because they are not, and cannot be persons. In this book we argue that chimpanzees are persons because they are autonomous. (shrink)

We submit this brief in support of the Nonhuman Rights Project’s efforts to secure habeas corpus relief for the elephant named Happy. We reject arbitrary distinctions that deny adequate protections to other animals who share with protected humans relevantly similar vulnerabilities to harms and relevantly similar interests in avoiding such harms. We strongly urge this Court, in keeping with the best philosophical standards of rational judgment and ethical standards of justice, to recognize that, as a nonhuman person, Happy should be (...) released from her current confinement and transferred to an appropriate elephant sanctuary, pursuant to habeas corpus. In this brief, we argue that there is a diversity of ways in which humans (Homo sapiens) are ‘persons’ and there are no non-arbitrary conceptions of ‘personhood’ that can include all humans and exclude all nonhuman animals. To do so we describe and assess the four most prominent conceptions of ‘personhood’ that can be found in the rulings concerning Kiko and Tommy, with particular focus on the most recent decision, Nonhuman Rights Project, Inc v Lavery.

We submit this brief in support of the Nonhuman Rights Project’s efforts to secure habeas corpus relief for the elephant named Happy. The Supreme Court, Bronx County, declined to grant habeas corpus relief and order Happy’s transfer to an elephant sanctuary, relying, in part, on previous decisions that denied habeas relief for the NhRP’s chimpanzee clients, Kiko and Tommy. Those decisions use incompatible conceptions of ‘person’ which, when properly understood, are either philosophically inadequate or, in fact, compatible with Happy’s personhood.

The blackbody radiation field is studied from different points of view. The existence of zero-point fluctuations is shown to be crucial in determining the form of the thermal part of the spectrum. The notion of a continuous field is seen to be compatible with a discrete structure for its interaction: The description normally used in the quantum context does not refer to the field but to its interaction with atomic systems, which involves statistically independent elementary acts of absorption and emission. (...) The same classical field has different effects on absorption and on emission, and these are conveniently, but not necessarily, described in terms of non-commuting operators. (shrink)

This paper deals with an (otherwise classical) two-(non-interacting) particle system immersed in a common stochastic zero-point radiation field. The treatment is an extension of the one-particle case for which it has been shown that the quantum properties of the particle emerge from its interaction with the background field under stationary and ergodic conditions. In the present case we show that non-classical correlations—describable only in terms of entanglement—arise between the (nearby) particles whenever both of them resonate to a common frequency of (...) the field. For identical particles the entanglement becomes maximum and must be described by totally (anti)symmetric states. (shrink)

El presente artículo se enmarca en el campo de la comprensión acerca del sentido que se le otorga a la escuela. En este caso se contextualiza la pregunta en estudiantes mapuche que asisten a un colegio rural de alta vulnerabilidad socioeconómica. A través de un enfoque cualitativo se buscó principalmente comprender el sentido que le dan niños y niñas a la escuela y las motivaciones que tienen para asistir. Participaron 20 niños y niñas en dos grupos focales. Luego de los (...) análisis iniciales se procedió a volver al campo realizando diez entrevistas individuales. Los resultados muestran que el sentido de la escuela se configura tanto por motivaciones internas como expectativas que tienen los padres sobre el futuro de sus hijos. Desde la mirada de los estudiantes, tanto profesores como padres motivan a los estudiantes a salir de la ruralidad, a seguir estudios superiores como una fórmula de éxito social. (shrink)

L’œuvre d’Edgar Morin n’est pas exclusivement la démarche d’un chercheur isolé. Elle fut, pendant une vingtaine d’années, acte de transmission de connaissance et d’échange de points de vue au sein de ses séminaires. Ici, il s’agissait alors d’essayer de faire apparaître la dimension d’une épistémologie de la complexité. Quel fut, dans le contexte des « séminaires de Morin », le processus d’interaction, d’osmoses, l’intensité du dialogue entre le penseur et les personnes qui fréquentaient les différentes séances ainsi organisées ? Enfin, (...) nous verrons dans ce texte que « l’épistémologie de la complexité n’a pas pour mission de retrouver la certitude perdue et le principe Un de la vérité » .The work of Edgar Morin is not only the process of an isolated researcher. It was, for twenty years, act of transmitting knowledge and exchange views in his seminars. Here, it was then trying to show the size of an epistemology of complexity. What was in the context of "seminars Morin," the process of interaction, osmosis, the intensity of the dialogue between the thinker and the people who attended the various sessions and organized? Finally, we see in this text that "the epistemology of complexity is not a mission to find the lost certainty and the principle of a truth". (shrink)

Cet article propose une nouvelle lecture de l’utopie et de l’éthique de notre ère planétaire, s’inspirant des principes de « terre-patrie » d’Edgar Morin, qui suggèrent une autre vision de notre état du monde. La validité et la pertinence du concept de « terre-patrie » émergent comme une alternative à l’heure où nos visions du monde basculent fortement vers le désespoir et où celui qui dépeint un avenir utopique s’expose à la moquerie.

Clinical responses to dopamine replacement therapy for individuals with Parkinson’s disease are often difficult to predict. We characterized changes in MDS-UPDRS motor factor scores resulting from a short-duration L-Dopa response, and investigated how the inter-subject clinical differences could be predicted from motor cortical magnetoencephalography. MDS-UPDRS motor factor scores and resting-state MEG recordings were collected during SDR from twenty individuals with a PD diagnosis. We used a novel subject-specific strategy based on linear support vector machines to quantify motor cortical oscillatory frequency (...) profiles that best predicted medication state. Motor cortical profiles differed substantially across individuals and showed consistency across multiple data folds. There was a linear relationship between classification accuracy and SDR of lower limb bradykinesia, although this relationship did not persist after multiple comparison correction, suggesting that combinations of spectral power features alone are insufficient to predict clinical state. Factor score analysis of therapeutic response and novel subject-specific machine learning approaches based on subject-specific neuroimaging provide tools to predict outcomes of therapies for PD. RésuméL'article examine la thèse leibnizienne de la tendance à l'existence de toutes les essences et de la réalisation de la série des êtres possibles qui, ensemble, ont le plus d'entité. Bien qu'elle offre une réponse à la question de savoir pourquoi il y a un monde – et pourquoi il s'agit du monde réel et non d'un autre –, la thèse se heurte à de nombreux obstacles. On tend actuellement àécarter les lectures nécessitaristes. L'article s'inscrit en faux contre cette tendance; (...) il vise à montrer les difficultés qui entourent l'interprétation de la nécessité fondée en théorie de la preuve, non seulement en général mais aussi en particulier par rapport à Leibniz. Une issue aux difficultés pourrait consister à développer la notion leibnizienne des degrés de réalité, en concevant aussi des degrés d'existence. Cela permettrait d'éviter la rupture entre l'ordre des possibles et celui des existants. Alors que Leibniz reconnaît des variations dans le degré d'entité des possibles, il est mené par son attachement à la logique aristotélicienne à faire du passage de la possibilitéà l'existence un bond métaphysique, l'actualitéétant ainsi rque comme une question de tout ou rien. Une approche gradualiste permettrait de concilier une partie des motivations métaphysiques de Leibniz avec un réalisme modal apparentéà celui de David Lewis, mais dûment nuancé. (shrink)

El artículo asume que el complejo proceso privatizador y descentralizador que vivió el sistema educativo chileno desde el año 1981 ha tenido consecuencias en el sujeto que se educa, a través de problemáticas que se concentran en los actores educativos –específicamente los estudiantes–. Estas problemáticas se han definido como la “psicologización” de los problemas del sistema educativo, la supuesta “clientelización” de los alumnos y el lugar que le cabe a La Familia como figura preponderante en el proceso privatizador. Las consecuencias (...) de esto dicen relación con la consolidación del sistema privatizado en la forma de enfrentar las subjetividades y, muy especialmente, en el quiebre del rol público que le cabe a la escuela y a la educación. This essay is focused on a difficult passage in Varrò, rust. 2,5,3-4, which is quoted, explained and partially corrected by Columella. Both passages are for us a valuable source of information about the capital sentence inflicted, in a very early age, on the person who caused the death of the ploughing ox. However, a contradiction or apory can be observed in the attitude of the two authors. On the one hand - as admirers of the mos maiorum - they seem (...) to praise the innocence and the restraint of the ancients, who showed so much respect for so beneficent an animal. On the other hand, in their roles as instructors of the most efficient farming techniques, they are keen on the profits assured by the raising of farm animals, and discuss without embarrassment the slaughter of oxen and the eating of beef. In Greece, the Attic Bouphonia, with all their functional ambiguities, marked the transition from the age of the bloodless sacrifice - praised and placed forever in the archives of history - to the "modern" age of slaughter; on the contrary, Roman culture lacked a similar rite that could somehow reconcile the two antithetical ideas, the ancient taboo about the slaying of domestic animals and the actual killing as performed in historical times. The article looks into the recent magisterium of the Church on the death penalty and deems it an instance of the development of doctrine rather than a circumstantiated reaction. It then dwells on the thinking that constitutes the backbone of the Encyclical Evangelium Vitae and compares and contrasts it with the various pronouncements of the teaching office of the Church, and with the two editions of the Cathechism of the Catholic Church. When one carefully examines the issue, human dignity and (...) its consequent human rights - among which one must mention the inalienable right to life - emerge as the hermeneutical cog of the problem. Together with the personalistic views of Vatican II, such a hermeneutic asks for a rethinking of the ethical norms in penal law which, in a clearer and more definitive way - per se, and not simply per accidens -, must then declare the moral implausibility of death whenever it is meted out as punishment. A death inflicted on someone else can only be justified within the strict and rigorous condition of legitimate self-defence. (shrink)
