Objectives Access to healthcare in most western societies is based on equality. Rapidly rising costs have fuelled debates about differentiation in access to healthcare. We assessed the public's perceptions and attitudes about differentiation in healthcare according to lifestyle behaviour. Methods A vignette study was undertaken in participants in a colorectal cancer screening pilot programme in the Netherlands. Screenees with a negative test result received a questionnaire in which nine hypothetical situations were described: three different healthcare settings (screening, lung cancer, chronic (...) obstructive pulmonary disease) combined with three forms of differentiation each: a difference in premium, waiting list ordering or copayment according to lifestyle. We evaluated the responses using a general hierarchical linear model. Results The percentage of participants in agreement with differentiation varied from 20% to 58% (overall mean of 40%). Significantly more participants were in favour of giving a premium discount to those who do not engage in unhealthy behaviour compared with supporters for higher payments for those who do. More differentiation was supported for non-smoking versus smoking cessation than for participation versus non-participation in screening. We observed in-group favouritism in smokers, but no significant effects of age or disease. There was no support for waiting list ordering based on lifestyle. Conclusions Results of this study show that Dutch citizens eligible for colorectal cancer screening are in favour of some form of financial differentiation in the distribution of healthcare, but that significant differences exist between type of setting and the type of behaviour. Our study can be used in the ongoing discussion about solidarity and behaviour in healthcare. (shrink)

The article starts from the questions: what is it to be an inhabitant or citizen of a globalised world, and how are we to think of education in relation to such inhabitants? We examine more specifically the so–called ‘European area of higher education’ that is on the way to being established and that can be regarded as a concrete example of a process of globalisation. In the first part of the paper we try to show that the discursive horizon, and (...) the concrete techniques and strategies that accompany the establishment of this space of higher education, invite the inhabitants of that space to see themselves as entrepreneurial and autonomous entities. In the second part we show how this specific kind of subjectivation (this production of subjects), related as it is to this globalised space, involves what we call an immunisation that also affects our thinking and our ideas in and about education. To refer to this as a kind of immunisation implies that globalisation could in fact be considered a closing or enclosing rather than an opening up. We argue, therefore, that this immunisation needs to be refused in favour of the invention of other kinds of subjectivity, other ways of speaking and writing about the world and about education, such that we relate to ourselves in a different way. This paper offers a new angle on the common idea that the process of science does not support epistemic diversity. Under minimal assumptions on the nature of journal editing, we prove that editorial procedures, even when impartial in themselves, disadvantage less prominent research programs. This purely statistical bias in article selection further skews existing differences in the success rate and hence attractiveness of research programs, and exacerbates the reputation difference between the programs. The terms “politics” and “political” have become so overdetermined that it is difficult to use them in any effective manner. We argue that this has dangerous political consequences, and that this could be addressed by providing a new, sounder, notion of politics. This paper argues that defining politics in relation to the notion of play can provide a notion both intuitively appealing and able to withstand the problematic overdeterminations. We argue that politics is the set of practices through which the (...) indeterminate of Spielraum is made more determinate. This suggests that politics is always partly a matter of play: it is about instituting values without making any claims about the legitimacy of this instituting act. With reference to Huizinga and Nietzsche’s analyses of play, we define play as the living unity of seriousness and frivolity, and non-play as either seriousness without frivolity or frivolity without seriousness. In order to illustrate this, we comparatively analyse the attitudes of Barack Obama and Donald Trump in the single context of the well-known yearly White House Correspondent’s Dinner. There, we see two opposed attitudes to playfulness. Our analysis allows us to apply our _Spielraum_ model of politics to show that the thrust of Obama’s attitude involves an embrace of the non-foundational nature of politics as play, whereas Trump’s attitude is politicidal: it is animated by a refusal to acknowledge its own lack of foundation, leading to an oscillation between over-seriousness and over-frivolity. Strategic ignorance is a widespread phenomenon. In a laboratory setting, many participants avoid learning information about the consequences of their behaviour in order to act egoistically. In real life, many consumers avoid information about their purchases or the working conditions in which they were produced in order to retain their lifestyle. The question is whether agents are blameworthy for such strategically ignorant behaviour. In this paper, I explore quality of will resources, according to which agents are blameworthy, roughly, depending on (...) their moral concern. The account I will propose—the Maximal Account—has three innovative features: it utilizes a suitable concept of maximal moral concern, it offers an accountability version of the account which significantly differs from the more familiar attributability variant, and it maintains that agents without maximal concern are blameworthy. Why act when the effects of one’s act are negligible? For example, why boycott sweatshop or animal products if doing so makes no difference for the better? According to recent proposals, one may still have a reason to boycott in order to avoid complicity or participation in harm. Julia Nefsky has argued that accounts of this kind suffer from the so-called “superfluity problem,” basically the question of why agents can be said to participate in harm if they make no difference (...) to it. Cancers often express hundreds of genes otherwise specific to germ cells, the germline/cancer (GC) genes. Here, we present and discuss the hypothesis that activation of a “germline program” promotes cancer cell malignancy. We do so by proposing four hallmark processes of the germline: meiosis, epigenetic plasticity, migration, and metabolic plasticity. Together, these hallmarks enable replicative immortality of germ cells as well as cancer cells. Especially meiotic genes are frequently expressed in cancer, implying that genes unique to meiosis may play a (...) role in oncogenesis. Because GC genes are not expressed in healthy somatic tissues, they form an appealing source of specific treatment targets with limited side effects besides infertility. Although it is still unclear why germ cell specific genes are so abundantly expressed in cancer, from our hypothesis it follows that the germline's reproductive program is intrinsic to cancer development. Criteria for the evaluation of most scholars’ work have recently received wider attention due to high-profile cases of scientific misconduct which are perceived to be linked to these criteria. However, in the competition for career advancement and funding opportunities almost all scholars are subjected to the same criteria. Therefore these evaluation criteria act as ‘switchmen’, determining the tracks along which scholarly work is pushed by the dynamic interplay of interests of both scholars and their institutions. Currently one of the most (...) important criteria is the impact of publications. In this research, the extent to which publish or perish, a long standing evaluation criterion, led to scientific misconduct is examined briefly. After this the strive for high impact publications will be examined, firstly by identifying the period in which this became an important evaluation criterion, secondly by looking at variables contributing to the impact of scholarly papers by means of a non-structured literature study, and lastly by combining these data into a quantitative analysis. Nevertheless, people in general, and politicians especially, make decisions and present arguments they believe are convincing. We studied politicians who made decisions in complex situations. They spoke about possible actions, their consequences, the probabilities of these consequences and their evaluations, but rarely indicated why their arguments led to their choice. We hypothesized implicit argumentation rules involved and checked whether they (...) predicted those choices. We found seven implicit informal logic rules involved. We also found a random sample of people made the same choices based on the same arguments, suggesting basic warrants by which people argue about decisions. (shrink)

BackgroundSexual boundary violations in healthcare are harmful and exploitative sexual transgressions in the professional–client relationship. Persons with mental health issues or intellectual disabilities, especially those living in residential settings, are especially vulnerable to SBV because they often receive long-term intimate care. Promoting good sexual health and preventing SBV in these care contexts is a moral and practical challenge for healthcare organizations.MethodsWe carried out a qualitative interview study with 16 Dutch policy advisors, regulators, healthcare professionals and other relevant experts to explore (...) their perspectives on preventing SBV in mental health and disability care organizations. We used inductive thematic analysis to interpret our data.ResultsWe found three main themes on how healthcare organizations can prevent SBV in mental health and disability care: setting rules and regulations, engaging in dialogue about sexuality, and addressing systemic and organizational dimensions.ConclusionOur findings suggest that preventing SBV in mental health and disability care organizations necessitates setting suitable rules and regulations and facilitating dialogue about positive aspects of sexuality and intimacy, as well as about boundaries, and inappropriate behaviors or feelings. Combining both further requires organizational policies and practices that promote transparency and reflection, and focus on creating a safe environment. Our findings will help prevent SBV and promote sexual health in mental health and disability care organizations. (shrink)

In this paper, I want to contribute to understanding and improving on Keenan'sintriguing equivalence result about reducible type quantifiers (Keenan, 1992).I give an alternative proof of his result which generalizes to type quantifiers, andI show how the reduction of a reducible type quantifier to (the composition of) ntype quantifiers can be effected.

This introduction provides an overview of the current state of the debate on the epistemic condition of moral responsibility. In sect. 1, we discuss the main concepts ‘ignorance’ and ‘responsibility’. In sect. 2, we ask why agents should inform themselves. In sect. 3, we describe what we take to be the core agreement among main participants in the debate. In sect. 4, we explain how this agreement invites a regress argument with a revisionist implication. In sect. 5, we provide an (...) overview of the main responses to the regress argument. In sect. 6, we address the question why blameless ignorance excuses. In sect. 7, we describe further issues that are addressed in the book. As AI Systems become increasingly autonomous, they are expected to engage in decision-making processes that have moral implications. In this research we integrate theoretical and empirical lines of thought to address the matters of moral reasoning and moral uncertainty in AI Systems. We reconceptualize the metanormative framework for decision-making under moral uncertainty and we operationalize it through a latent class choice model. The core idea being that moral heterogeneity in society can be codified in terms of a small number of (...) classes with distinct moral preferences and that this codification can be used to express moral uncertainty of an AI. Choice analysis allows for the identification of classes and their moral preferences based on observed choice data. Our reformulation of the metanormative framework is theory-rooted and practical in the sense that it avoids runtime issues in real time applications. To illustrate our approach we conceptualize a society in which AI Systems are in charge of making policy choices. While one of the systems uses a baseline morally certain model, the other uses a morally uncertain model. We highlight cases in which the AI Systems disagree about the policy to be chosen, thus illustrating the need to capture moral uncertainty in AI systems. (shrink)

This article aims to explore a ‘lived discipleship’ by determining whether and how contemporary communities of faith could implement the norms and principles reflected in the Emmaus narrative of Luke 24:13–35 within a plausible epistemological framework that might facilitate a fresh understanding of Christian followership as discipleship. This was done through an empirical case study using two focus groups as co-researchers, in order to actively listen to their respective understandings of lived theology in their unique South African contexts. The two (...) focus groups consisted of (1) a contemporary Christian grouping of Afrikaans-speaking, active churchgoers situated in Hazeldean, a suburb in Pretoria East, Tshwane, Gauteng and (2) a contemporary Christian grouping of African, active churchgoers situated in Ivory Park, a suburb in Tembisa, Ekurhuleni, Gauteng. This article concluded that Luke 24:13–35 nudged the co-researchers to re-evaluate their contemporary understanding of discipleship and moved them to additional and new perspectives in terms of practical expressions thereof that can be best described as ‘lived followership’. A shift from perceiving Jesus in terms of an ‘act to follow’ by gaining the correct knowledge, to following Jesus as ‘a performative act’, a shift from ‘theoretical knowledge’ to ‘heart knowledge’. Contribution: This article is a part of the Festschrift for Prof. Stephan Joubert. This article plays into similar creative interdisciplinary relationship as seen in the work of Prof. Joubert, by looking at the relationship between New Testament and Practical Theology in order to improve practices of faith that is rooted in a biblical understanding of Jesus. This contribution aims at deconstructing a Christian master narrative that interprets Josephus as crucial support for the New Testament message that the Temple had to become a ruin, in line with the will of God. It argues for an alternative interpretation, namely that both Jesus of Nazareth and Josephus considered the Temple to be still relevant, albeit in different ways. For Jesus the Temple was the self-evident cultic centre of Judaism and a special place to experience his relationship with God. (...) None of Jesus’ statements about the Temple in their original context necessarily implies that Jesus assumed that the institution of the Temple would stop functioning in the near future or at the end of time. Josephus’s perspective on the Temple changes in his works. The elaborate description of Jerusalem and the Temple in War 5 reads as a written monument of the past, but several passages in Josephus’s Antiquities and Against Apion imply that the Temple was still important after 70 CE. Josephus may have reckoned with the possibility that the Temple was going to be rebuilt if the Romans allowed for it.This contribution is dedicated to Pieter G.R. de Villiers, a modest but sophisticated scholar and a good friend. The role tradition played in preindustrial societies has been supplanted by the decisions of countless specialists organized by means of an intellectual and professional division of labor shaping a knowledge infrastructure that sustains these decisions. Three limitations of this knowledge system are discussed: (a) on the macrolevel, it imposes an end-of-pipe approach for dealing with the undesired consequences of decision making, rarely getting to the root of any problem; (b) on the microlevel, individual practitioners of a specialty are trapped in (...) a triple abstraction, leading to a poor ratio of desired to undesired effects of their decision making; and (c) on the intermediate level, it bars the road to genuine solutions to many difficulties faced by contemporary civilization. In this first of four articles, the beginning of a response is developed for the profession of engineering, which will be paradigmatic for other professions, the social sciences, and the university as a whole. This third part continues the exploration of how we can overcome the limitations of the present knowledge system. In preparation, two aspects of current engineering theory and practice are examined because they are paradigmatic: the concept that engineering is essentially problem-solving, which goes against our understanding of human skill acquisition, and the existence of parallel modes of knowing technology derived from professional education and practice and from living in a society permeated by technology. The former suspends practitioners in the previously (...) examined triple abstraction as a primary microlevel characteristic of the current knowledge system and is thus separated from experience and culture. The second mode of knowing derives from daily life experience, symbolically represented by the organization of the brain-mind, which also functions as a mental map for skillfully coping with the world according to a culture. The practitioner primarily derives analytical exemplars from the former knowing and design exemplars from the latter. It is shown how this affects the building of cities and the design of production systems. Umbrella concepts and round tables are introduced as further steps toward transforming university departments, and (as we will see in Part 4) the operation of the university. (shrink)

This paper explores the fact that linear opinion pooling can be represented as a Bayesian update on the opinions of others. It uses this fact to propose a new interpretation of the pooling weights. Relative to certain modelling assumptions the weights can be equated with the so-called truth-conduciveness known from the context of Condorcet's jury theorem. This suggests a novel way to elicit the weights.

The guarded fragment (GF) was introduced by Andréka, van Benthem and Németi as a fragment of first order logic which combines a great expressive power with nice, modal behavior. It consists of relational first order formulas whose quantifiers are relativized by atoms in a certain way. Slightly generalizing the admissible relativizations yields the packed fragment (PF). In this paper we investigate interpolation and definability in these fragments. We first show that the interpolation property of first order logic fails in restriction (...) to GF and PF. However, each of these fragments turns out to have an alternative interpolation property that closely resembles the interpolation property usually studied in modal logic. These results are strong enough to entail the Beth definability property for GF and PF. Even better, every guarded or packed finite variable fragment has the Beth property. For interpolation, we characterize exactly which finite variable fragments of GF and PF enjoy this property. (shrink)

This fourth part outlines a strategy for overcoming the limitations of the knowledge system for engineering by combining intellectual maps, preventive approaches, umbrella concepts, and round tables as described in the earlier parts. A discussion of the issues faced by modern medicine illustrates the paradigmatic nature of the diagnosis and prescription made for engineering. The social sciences face mirror-image problems. One response has been the rise of new disciplines such as communications, environmental studies, urban affairs, criminology, and policy studies. To (...) avoid the limitations of discipline-based knowing and doing, a similar strategy for their transformation will have to be implemented. Considerable synergies would result if parallel efforts to transform the present knowledge system were carried out throughout the university. Some suggestions are made as to how this can be supported by organizational and institutional changes. Finally, it is suggested that such a transformation of the university could make a critical and decisive contribution to overcoming the current economic, social, and environmental crises. (shrink)

This second part continues the search for ways of overcoming the three limitations of the current intellectual and professional division of labor and its knowledge infrastructure, which were shown to be at the root of the present economic, social and environmental crises. A complementary knowledge strategy is proposed to counterbalance the trade of breadth for depth, based on the creation of intellectual maps. One such map is described for engineering, showing how through the process of industrialization people change technology and (...) how through its influence on human life and society, technology changes people. Because industrialization cannot destroy the matter and energy it requires, it also transforms its relations with the biosphere. Once the connections between technology and everything else are mapped, specialists can inquire into the consequences of their design and decision making that fall beyond their domains of expertise, to introduce a preventive orientation into their work to achieve a better ratio of desired to undesired effects. This is shown for materials and production, energy, work, and cities. In subsequent parts, it will become apparent that this example is paradigmatic for other professions, the social sciences, and the university. (shrink)

The embodied cognition thesis claims that cognitive functions cannot be understood without making reference to the interactions between the brain, the body, and the environment. The meaning of abstract concepts is grounded in concrete experiences. This book is the first edited volume to explore the impact of the embodied cognition thesis on the scientific study of film. A team of scholars analyse the main aspects of film (narrative, style, music, sound, time, the viewer, emotion, perception, ethics, the frame, etc.) from (...) an embodied perspective. By combining insights from various disciplines such as cognitive film theory, conceptual metaphor theory, and cognitive neuroscience, they show how the process of meaning-making in film is embodied and how empathy and embodied simulation play a role in understanding the way in which the viewer interacts with the film. Research on expert‐novice differences has mainly focused on how experts solve familiar problems. We know far less about the skills and knowledge used by experts when they are confronted with novel problems within their area of expertise. This article discusses a study in which verbal protocols were taken from subjects of various expertise designing an experiment in an area with which they were unfamiliar. The results showed that even when domain knowledge is lacking, experts solve a novel problem within their (...) area of expertise by dividing the problem into a number of subproblems that are solved in a specified order. The lack of domain knowledge is compensated for by using abstract knowledge structures and domain‐specific heuristic strategies. However, the quality of their solutions is considerably lower than the quality attained by experts who were familiar with the type of problem to be solved. The results suggest that when experts are confronted with novel problems as compared with familiar problems, their form of reasoning remains intact, but the content of their reasoning suffers due to lack of domain knowledge. The energy domain is still dominated by equilibrium models that underestimate both the dangers and opportunities related to climate change. In reality, climate and energy systems contain tipping points, feedback loops, and exponential developments. This paper describes how to create realistic energy transition management models: quantitative models that can discover profitable pathways from fossil fuels to renewable energy. We review the literature regarding agent-based economics, disruptive innovation, and transition management and determine the following requirements. Actors must be detailed, heterogeneous, interacting, (...) learning, and strategizing. Technology should be represented as a detailed and heterogeneous portfolio that can develop in a bottom-up manner, using endogenous feedback loops. Assumptions about discount rates and the social cost of carbon should be configurable. The model should contain interactions between the global, national, local, and individual level. A review of modelling techniques shows that equilibrium models are unsuitable and that system dynamics and discrete event simulation are too limited. The agent-based approach is found to be uniquely suited for the complex adaptive sociotechnical systems that must be modelled. But the choice for agent-based models does not mean a rejection of other approaches because they can be accommodated within the agent-based framework. We conclude with practical guidelines. (shrink)

What's wrong with joining corona parties? In this article, I defend the idea that reasons to avoid such parties come in degrees. I approach this issue from a participation-based perspective. Specifically, I argue that the more people are already joining the party, and the more likely it is that the virus will spread among everyone, the stronger the participation-based reason not to join. In defense of these degrees, I argue that they covary with the expression of certain attitudes.

In many logics dealing with information one needs to make statements not only about cognitive states, but also about transitions between them. In this paper we analyze a dynamic modal logic that has been designed with this purpose in mind. On top of an abstract information ordering on states it has instructions to move forward or backward along this ordering, to states where a certain assertion holds or fails, while it also allows combinations of such instructions by means of operations (...) from relation algebra. In addition, the logic has devices for expressing whether in a given state a certain instruction can be carried out, and whether that state can be arrived at by carrying out a certain instruction. This paper deals mainly with technical aspects of our dynamic modal logic. It gives an exact description of the expressive power of this language; it also contains results on decidability for the language with 'arbitrary' structures and for the special case with a restricted class of admissible structures. In addition, a complete axiomatization is given. The paper concludes with a remark about the modal algebras appropriate for our dynamic modal logic, and some questions for further work. The paper only contains some sketchy examples showing how the logic can be used to capture situations of dynamic interest, far more detailed applications are given in a companion to this paper (De Rijke [33]). In fact, to every proper normal extension L of S52 corresponds a natural number b(L) - the bound of L. For every L, there exists a polynomial P(·) of degree b(L) + 1 such that every L-consistent formula ϕ is satisfiable on an L-frame whose universe is bounded by P(|ϕ|), where |ϕ| denotes the number of subformulas of ϕ. We show that every extension of the basic hybrid logic with modal Sahlqvist axioms is complete. As a corollary of our approach, we also obtain the Beth property for a large class of hybrid logics. Finally, we show that the new completeness result cannot be combined with the existing general completeness result for pure axioms.

This note contains a corrective and a generalization of results by Borsboom et al. (2008), based on Heesen and Romeijn (2019). It highlights the relevance of insights from psychometrics beyond the context of psychological testing.

The German end-of-life vehicle take-back system is described and analyzed in terms of its impact on the environment and the car companies involved. It is concluded that although this system is often cited as an example of a successful take-back scheme, it is not one that maximizes the value recovered from end-of-life vehicles. As a result, corporations do not achieve the potential benefits that can be realized from an alternate value chain based on recovering value from end-of-life products. Neither is (...) society benefiting from the potential reduction in pollution and from making its way of life more sustainable. Without bringing these potential benefits to all stakeholders, the system is not likely to have a future. (shrink)
