This study aims to describe and compare nurses' willingness to provide care for patients with HIV/AIDS and factors associated with this in three countries. An international cross-sectional survey was conducted among nurses working in medical, surgical and gynaecology units in Finland (n =427), Estonia (n =221) and Lithuania ( n =185) in early 2006. The response rates were 75% (n = 322) in Finland, 54% (n =119) in Estonia and 86% (n = 160) in Lithuania. A modified version of a (...) scale developed in 1994 by Dubbert et al. was applied. Our findings showed a general willingness of the nurse participants to provide care for patients with HIV/AIDS. However, this willingness varied both among and within countries and was also related to specific nursing interventions. The results underline the importance of providing education on ethical issues related to HIV/AIDS care in Europe and tailoring the content of this education to meet nurses' national educational needs. In jenem Rahmen dokumentiert sie die Geschichte dieses Begriffes und würdigt aktuelle Theorien - um deren Beschränkungen zu überwinden. Auch wenn wir über das Gebotene hinausgehen, so ihre These, können wir fehlen; wir können uns neue Pflichten einhandeln.

The prevention, treatment and management of disease are closely linked to how the causes of a particular disease are explained. For multi-factorial conditions, the causal explanations are inevitably complex and competing models may exist to explain the same condition. Selecting one particular causal explanation over another will carry practical and ethical consequences that are acutely relevant for health policy. In this paper our focus is two-fold; the different models of causal explanation that are put forward within current scientific literature for (...) the high and rising prevalence of the common complex conditions of coronary artery disease and type 2 diabetes mellitus ; and how these explanations are taken up within national health policy guidelines. We examine the causal explanations for these two conditions through a systematic database search of current scientific literature. By identifying different causal explanations we propose a three-tier taxonomy of the most prominent models of explanations: evolutionary, lifecourse, and lifestyle and environment. We elaborate this taxonomy with a micro-level thematic analysis to illustrate how some explanations are semantically and rhetorically foregrounded over others. We then investigate the uptake of the scientific causal explanations in health policy documents with regard to the prevention and management recommendations of current National Service Frameworks for CAD and T2D. Our findings indicate a lack of congruence between the complexity and frequent overlap of causal explanations evident in the scientific literature and the predominant focus on lifestyle recommendations found in the mainstream health policy documents. Respect for autonomy and self-determination is a central principle in nursing ethics. Autonomy and quality of life are strongly connected, and, at the same time, autonomy is an important quality indicator on how older persons' housing functions. In this study, autonomy was conceived as self-determination. The aim of the study was to describe how older people living in sheltered housing experience self-determination and how they are valued as human beings. Eleven persons living in five different housing facilities for older people (...) in southern Sweden were interviewed. The data were analysed by manifest and latent qualitative content analysis. The overall theme expressing the latent content in the interviews emerged as disempowerment, which implied an environment that does not strengthen individual self-determination. The results showed a negative experience of how these older people thought they were valued in the sheltered housing where they lived. In sheltered housing, more attention should be paid to residents' self-determination and sense of value. (shrink)

Merchants in late medieval Antwerp worked with a number of different representatives: They appointed proxies to trade on their behalf, to collect debts in their name or to represent them in court. If a merchant wanted to authorize another person to act on his behalf, he went to a notary who issued a letter of procuration. The article discusses the role of Roman law in these procurations. It contrasts the procurations issued by the notaries with other models for procurations found (...) in a form book for notaries in Antwerp. The analysis comes to the conclusion that the basic idea of installing a proxy did not rely on Roman law. However, knowledge of Roman law was crucial if someone wanted to be represented in a Roman law court. Furthermore, concepts derived from Roman law were applied when complicated problems or phenomena had to be shortened to a few words. In sum, the practice of including phrases and concepts of Roman law wherever they proved useful can be regarded as a part of the lex mercatoria, the customary law of merchants. (shrink)

Even though teacher education has been successful in preparing students for their future profession, the classroom reality can differ greatly from the inservice training. Many novice teachers therefore find the transition from student teacher to inservice teacher overwhelming To support beginning teachers, mentoring programs—where more experienced teachers support novice teachers—have become commonplace in many schools worldwide. In Sweden, mentoring for beginning teachers has been a frequent feature of support since 2001. This study, conducted in Sweden, examines seven novice teachers and (...) the impact the mentoring process had upon them during their first‐year teaching. Based on interviews, it was found that these experienced both professional and personal support from their mentors. The study also showed the significance of observant leaders within the mentorship program following up on the development of the mentor–mentee relationship. (shrink)

The "public sphere" -- an idea with deep roots in the European enlightenment -- has always been a contested concept in American culture and society. American intellectuals, artists, politicians, and activists have stressed the non-unitary, diversified, and oppositional dynamics of all things public. From the early days of the American republic, competing interest groups and commercial mass media (first newspapers, novels, and the theater, then radio, television, and the internet) have worked to pluralize public speech and public action -- and (...) ultimately the notion of "publicness" itself. This essay collection explores the public sphere in North America as a multi-agential, commercially embattled, highly mediated, and ultimately trans-nationalized aggregate of publics and counterpublics. Weizsäcker’s notion of a „mental illness of peacelessness“ is the starting point for a psychological theory of a „culture of constructive debates“ (...) based on dialogue as well as resonance instead of enforcement. Intimacy and intimate dialogue are keywords for formulating a “paradigm of love” . I develop an externalist perspective and analysis of the relatedness of loneliness and (harmful) alcohol use and the concept of loneliness. I depart from twenty qualitative interviews with people undergoing inpatient treatment for alcohol dependence. Both, loneliness and its relatedness to alocohol dependence turn out to be complex relational and interactional phenomena whose occurrence and dynamics depend on the social and situational conditions under which they arise. Despite huge variations in interviewees’ experiences of loneliness, they share a common phenomenological and (...) analytical structure. Loneliness arises when instances of social interaction fail to arrive at mutual understanding within a certain social context. Loneliness is neither reducible to individual experiences nor to distinctive characteristics of a person. Rather, it presents an evaluative and interactional phenomenon, a person’s awareness of his/her failure to establish mutual understanding with others in social interaction. The relatedness of alcohol dependence and loneliness is neither conceptual nor causal nor explainable by facts about the individuals concerned, but depends on the kind of loneliness involved and the function habitual ways of drinking alcohol have in a person’s everyday life and social environment. La thèse d’indétermination de Quine est souvent considérée comme découlant de l’adoption d’un point de vue naturaliste. Contre cette lecture, qui manque la distinction entre empirisme et naturalisme, nous montrons comment l’indétermination est fondée dans une philosophie demeurant empiriste malgré la critique des deux dogmes de l’analyticité et du réductionnisme. Cet empirisme entre en conflit avec certaines thèses de l’épistémologie naturalisée défendue par Quine.Quine’s thesis of indeterminacy is often seen as grounded in his naturalism. Against this interpretation, which confuses empiricism (...) with naturalism, we show how indeterminacy is grounded in a philosophy which remains empiricist albeit the rejection of analyticity and reductionism. Nous montrons comment Quine hérite de Hume certains aspects de sa conception de l’objet physique. L’un et l’autre, examinant l’étendue de la garantie empirique de nos concepts, soutiennent que l’identité de l’objet n’est pas suffisamment déterminée par l’expérience. Chez Quine, cette thèse est fondée sur l’indétermination de la référence, chez Hume sur une analyse de l’identité comme fiction de l’imagination. Quine, déterminant ainsi sa situation propre dans une histoire subjective de l’empirisme, voit cette analyse comme la perception confuse du fait (...) que l’identité apparaît dans le système conceptuel seulement lors de l’acquisition de la référence divisée. Nous montrons comment les deux philosophes font alors face à une même tension, entre les exigences d’une construction empiriste des concepts à partir du matériau empirique et la dénégation d’une détermination complète. Nous montrons ainsi comment le rejet quinien du dogme du réductionnisme trouve une de ses racines dans le Traité de Hume. (shrink)

La solución de los problemas de salud requiere de la interrelación entre diferentes ramas científicas y tecnológicas, tales como la Biomecánica, las tecnologías de la información, la ingeniería y las ciencias médicas. La importancia de la comunicación interdisciplinaria en las universidades fundamenta la creación de un grupo de investigación que fusiona el conocimiento de la mecánica clásica con las disciplinas de la salud y la participación de pacientes, técnicos, ingenieros, médicos y estudiantes. En el trabajo se realiza una reflexión sobre (...) el carácter interdisciplinario de las investigaciones, cómo se inserta la Ingeniería Mecánica desde la modelación en la solución de problemas de salud y se brindan algunos resultados obtenidos por los miembros del Grupo de Biomecánica y Bioingeniería de la Universidad "Ignacio Agramonte y Loynaz" de Camagüey. The solution of health problems require an interrelation between different scientific and technological branches, such as biomechanics, information technologies, engineering and medical sciences. The importance of the interdisciplinary communication in universities lays the base on the creation of an investigative group that fuses the knowledge of classic mechanic with the disciplines of medicine and the participation of patients, technicians, engineers, doctors and students. A reflection about the interdisciplinary character of investigations and how mechanic engineering is inserted in the model to solve health problems is carried out in this work. Some results obtained by the members of the Biomechanics and Bioengineering Group of "Ignacio Agramonte y Loynaz" University are offered. (shrink)
