Aphasia is a language disorder that occurs after a stroke and impairs listening, speaking, reading, writing, and calculation skills. Patients with post-stroke aphasia in Japan are increasing due to population aging and the advancement of medical treatment. Opportunities for adequate speech therapy in chronic stroke are limited due to time constraints. Recent studies have reported that intensive speech therapy for a short period of time or continuous speech therapy using high-tech equipment, including speech (...) applications (apps, can improve aphasia even in the chronic stage. However, its underlying mechanism for improving language function and its effect on other cognitive functions remains unclear. In the present study, we investigated whether intensive speech therapy using a newly developed speech support app could improve aphasia and other cognitive functions in patients with chronic stroke. Furthermore, we examined whether it can alter the brain network related to language and other cortical areas. Thus, we conducted a prospective, single-comparison study to examine the effects of a new speech support app on language and cognitive functions and used resting state functional MRI (rs-fMRI) regions of interest (ROI) to ROI analysis to determine changes in the related brain network. Two patients with chronic stroke participated in this study. They used the independent speech therapy system to perform eight sets of 20 randomly presented words/time (taking approximately 20 min), for 8 consecutive weeks. Their language, higher cognitive functions including attention function, and rs-fMRI, were evaluated before and after the rehabilitation intervention using the speech support app. Both patients had improved pronunciation, daily conversational situations, and attention. The rs-fMRI analysis showed increased functional connectivity of brain regions associated with language and attention related areas. Our results show that intensive speech therapy using this speech support app can improve language and attention functions even in the chronic stage of stroke, and may be a useful tool for patients with aphasia. In the future, we will conduct longitudinal studies with larger numbers of patients, which we hope will continue the trends seen in the current study, and provide even stronger evidence for the usefulness of this new speech support app. (shrink)

Selon l'étude européenne translinguistique Neurodys (Becker et al., 2013), la prévalence de la dyslexie touche entre 5 et 10% des enfants âgés de 8 à 12 ans. L’apprentissage du langage écrit revient à maîtriser conjointement ses deux versants : lecture et écriture. La présente étude vise l’analyse des processus d'écriture utilisés par des scripteurs dyslexiques (DL) et dysorthographiques (DO). Afin de favoriser les collaborations chercheurs-praticiens, comme le proposent Hayes et Berninger (2014), nous étudions les données chronométriques relatives à un corpus (...) de textes recueillis auprès de 7 adolescents dyslexiques et dysorthographiques, recrutés dans le cadre de situations de soin courant orthophonique, afin de décrire les opérations de traitement cognitif du langage écrit en contexte d’activité rédactionnelle. Les corpus de productions écrites d’adolescents DL/DO suivis en orthophonie apportent par le biais du relevé chronométrique (vitesses d’écriture, durées des pauses en fonction de leur localisation dans le texte, longueur des passages d’écriture sans pause longue) des éléments sur l’activité de rédaction et les modalités de révision du texte. Ce travail ouvre des perspectives quant au recueil de données en contexte professionnel, de manière à interroger les données cliniques par des outils de recherche développés en linguistique de corpus. The purpose of the study is to theoretically and experimentally investigate the motivational aspect of training future speech therapists to use information and communication technologies in professional activities. The article analyzes various studies, as well as the regulatory framework that consider the problems of training future speech therapists, as well as the motivational aspect of preparing students for the use of information and communication technologies. As part of this study, a survey with students was conducted and analyzed. The (...) scientific novelty lies in a more detailed study of the specific motivational aspect of the training of future speech therapists. Cet article rend compte d’une expérience pédagogique consistant à proposer à des étudiants de 2e année en orthophonie de constituer leur propre corpus de productions orales ou multimodales (enregistrement audio ou vidéo et transcription de cet enregistrement). Il met en évidence un certain nombre de questionnements auxquels cet exercice conduit les étudiants et souligne les liens avec leur pratique future.

PurposeTo evaluate the prevalence and treatment patterns of speech and language disorders in Germany.MethodsA retrospective analysis of data collected from 32% of the German population, insured by the statutory German health insurance. We used The International Statistical Classification of Diseases and Related Health Problems, 10th revision, German Modification codes for stuttering, cluttering, and developmental disorders of speech and language to identify prevalent and newly diagnosed cases each year. Prescription and speech therapy reimbursement data were used to (...) evaluate treatment patterns.ResultsIn 2017, 27,977 patients of all ages were diagnosed with stuttering. Stuttering prevalence peaks at age 5 years. Cluttering was diagnosed in 1,800 patients of all ages. Developmental disorders of speech and language were identified in 555,774 AOK-insurants. Treatment data indicate a substantial proportion newly diagnosed stuttering individuals receive treatment, with slightly fewer than 20 sessions per year, on average. We confirmed a previous study showing increased rates of atopic disorders and neurological and psychiatric comorbidities in individuals with stuttering, cluttering, and developmental disorders of speech and language.ConclusionThis is the first nationwide study using health insurance data to analyze the prevalence and newly diagnosed cases of a speech and language disorder. Prevalence and gender ratio data were consistent with the international literature. The crude prevalence of developmental disorders of speech and language increased from 2015 to 2018, whereas the crude prevalence for stuttering remained stable. For cluttering, the numbers were too low to draw reliable conclusions. Proportional treatment allocation for stuttering peaked at 6 years of age, which is the school entrance year, and is later than the prevalence peak of stuttering. (shrink)

It is well known that the unpredictable speech production brought on by stress from the task at hand has a significant negative impact on the performance of speech processing algorithms. Speech therapy benefits from being able to detect stress in speech. Speech processing performance suffers noticeably when perceptually produced stress causes variations in speech production. Using the acoustic speech signal to objectively characterize speaker stress is one method for assessing production variances brought (...) on by stress. Real-world complexity and ambiguity make it difficult for decision-makers to express their conclusions with clarity in their speech. In particular, the Neutrosophic speech algorithm is used to encode the language variables because they cannot be computed directly. Neutrosophic sets are used to manage indeterminacy in a practical situation. Existing algorithms are used except for stress on Neutrosophic speech recognition. The creation of algorithms that calculate, categorize, or differentiate between different stress circumstances. BackgroundResearch on interpersonal synchrony has mostly focused on a single modality, and hence little is known about the connections between different types of social attunement. In this study, the relationship between sympathetic nervous system synchrony, movement synchrony, and the amount of speech were studied in couple therapy.MethodsData comprised 12 couple therapy cases. Synchrony in electrodermal activity, head and body movement, and the amount of speech and simultaneous speech during the sessions were analyzed in 12 sessions (...) at the start of couple therapy and eight sessions at the end of therapy. Synchrony was calculated from cross-correlations using time lags and compared to segment-shuffled pseudo synchrony. The associations between the synchrony modalities and speech were analyzed using complex modeling.FindingsCouple therapy participants’ synchrony mostly occurred in-phase. Anti-phase synchrony was more common in movement than in sympathetic nervous system activity. Synchrony in sympathetic nervous system activity only correlated with movement synchrony between the client-therapist dyads. Movement synchrony and the amount of speech correlated negatively between spouses and co-therapists, meaning that the more time the dyad members talked during the session, the less bodily synchrony they exhibited.ConclusionThe different roles and relationships in couple therapy were associated with the extent to which synchrony modalities were linked with each other. In the relationship between clients and therapists, synchrony in arousal levels and movement “walked hand in hand”, whereas in the other relationships they were not linked. Generally, more talk time by the therapy participants was associated with anti-phase movement synchrony. If, as suggested, emotions prepare us for motor action, an important finding of this study is that sympathetic nervous system activity can also synchronize with that of others independently of motor action. AimsThis preliminary study aimed to investigate therapy-induced electrophysiological changes in persons with primary progressive aphasia. The investigated event-related potential components associated with language processing were the mismatch negativity, P300, N400, and P600.MethodsA linguistic ERP test battery and standardized language assessment were administered in four patients with PPA of which two received speech-language therapy and two did not receive therapy. The battery was administered twice with approximately 6 months in between in each patient. The results of the (...) follow-up assessments were compared to the results of the initial assessments.ResultsAlthough the results of the behavioral language assessment remained relatively stable between the initial and follow-up assessments, changes in the mean amplitudes, onset latencies, and duration of the ERP components were found in the four patients. In the two patients that did not receive SLT, an increased delay in 50% and a decreased mean amplitude in 25% of the measured ERP components were found. The electrophysiological changes found in the patients that received SLT were variable. Interestingly, the mismatch negativity and the N400 effect elicited by the categorical priming paradigm were less delayed and had an increased mean amplitude at the follow-up assessment in the patient with the non-fluent variant who received SLT. In this patient, the P600 component was absent at the initial assessment but present at the follow-up assessment.ConclusionAlthough no clear patterns in electrophysiological changes between patients who received SLT and patients who did not receive SLT were found by our preliminary study, it seems like the SLT induced improvements or compensation mechanisms in some specific language comprehension processes in the patient with the NFV. The results of this study are still preliminary because only four heterogeneous patients were included. Future studies should include larger patient groups of the three clinical variants because the therapy-induced electrophysiological changes might differ depending on the clinical variant and the underlying pathology. On campuses across the country in recent years, speech codes have been introduced, the “re-education” (...) of professors urged, visiting speakers on controversial subjects silenced, and classes monitored by administrators eager to do the bidding of various special interest groups. As universities have come more and more to see their role as that of some sort of multipurpose social project, education itself has become less pressing a priority. Thus, while money is found to hire sexual harassment officers, employment equity advisors and “wellness” mavens, academic posts disappear. Those that remain are frequently filled in accordance with crass gender and race-based quotas, and when that fails to guarantee positions for members of bureaucratically favored groups, contrived “schools” and pseudodisciplines proliferate. Many of these developments, however, are simply the manifestation of trickle-down deconstruction, trickle-down Rortyism, trickle-down “politics of difference,” of the sort that Charles Taylor is prone to rhapsodize. Notions of value, nature and merit, of equality, universality and objectivity are dismissed as mere inventions, “imperial” notions designed to favor the powerful and oppress the disenfranchised. The art [End Page 492] of spoken and written argument, with their ideals of detachment and discipline, are taken as evidence of a smug “androcentrism,” while rationality itself is derided as a “regime” that works “to maintain patriarchal control of knowledge and knowledge production.” Even the notion of evaluating students by means of essays and exams is ridiculed as scandalously “non-feminist.” While past attacks on academic independence have tended to come from outside the university, Canadian academics over the past decade have slavishly accepted their own domestication. For instance, Smith writes: “The textuality of public text-mediated discourse is essential to its peculiar temporal and spatial properties of ubiquity and constancy of replication across multiple and various actual situations of watching, reading, or hearing” (p. 25). Translation? The more one sees, reads or hears a view expressed, the more one tends to believe it’s true. Her concern here, of course, is to establish that whoever controls the means of public debate controls its terms as well, that those means happen to be controlled by neo-conservative interests bent on retarding social progress, and that growing public disquiet about educational reform simply confirms the success of that neo-conservative strategy. But the failure of Smith’s account goes far beyond the thoroughly rebarbative nature of her expression.According to Smith, the very term, “political correctness,” is shorthand for an “ideological code” that has been expressly designed by “neo-conservatives” to caricature the good works of the visionary left. An ideological code, she says, operates as a “free-floating form of control,” which, once established, is “self-reproducing” (p. 27). Aphasia is a complex disorder, frequent after stroke (with an incidence of 38%), with a detailed pathophysiological characterization. Effective approaches are crucial for devising an efficient rehabilitative strategy, in order to address the everyday life and professional disability. Several rehabilitative procedures are based on psycholinguistic, cognitive, psychosocial or pragmatic approaches, including amongst those with a neurobehavioral approach the Melodic Intonation Therapy (MIT). Van Eeckhout’s adaptation of MIT to French language (Melodic-Rhythmic Therapy: MRT) has implemented the training strategy by (...) adding a rhythmic structure reproducing French prosody. The purpose of this study was to adapt MRT rehabilitation procedures to Italian language and to verify its efficacy in a group of six chronic patients (five males) with severe non-fluent aphasia and without specific aphasic treatments during the previous 9 months. The patients were treated 4 days a week for 16 weeks, with sessions of 30–40 min. They were assessed 6 months after the end of the treatment (follow-up). The patients showed a significant improvement at the Aachener Aphasie Test (AAT) in different fields of spontaneous speech, with superimposable results at the follow-up. Albeit preliminary, these findings support the use of MRT in the rehabilitation after stroke. Specifically, MRT seems to benefit from its stronger structure than the available stimulation-facilitation procedures and allows a better quantification of the rehabilitation efficacy. (shrink)

This article applies Russon's principles of reading Plato's dialogues to solve a problem arising from both the dramatic and philosophical aspects of Plato's Republic: persuasive speech seems effective only when its audience is already willing to listen and be convinced. Yet if so, then either persuasive speech is powerless to persuade anybody truly, or it is unclear how it differs from simple manipulation or brainwashing. This article resolves this dilemma by using Russon’s insights about the kind of rationality (...) Plato invites us to assume, namely a “concrete rationality,” and by analyzing the ????irst three interlocutors of Socrates in the Republic: Polemarchus, Cephalus, and, of course, Thrasymachus. This approach enables us to differentiate these three interlocutors, explain Thrasymachus’s persistence in listening to the conversation until the end despite his unwillingness to listen, as well as the therapeutic function of theōria for providing him some momentary relief from his “hatred of speech” (misologia). This paper examines two realisations of the television talk show in North America: The Oprah Winfrey Show and Dr. Phil, looking specifically at how they function within the sub-genre of ‘therapeutic talk show’ in keeping with Livingstone and Lunt’s (1994) classification of talk shows. Talk shows are defined by Ilie (2001) as “semi-institutional discourse” having features of a given setting (TV studio), topic- and goal-oriented talk, high degree of topic control, as well as restrictions on time and turn-taking. Theorists examining (...) this sub-genre of therapeutic talk show have argued that it provides a valuable means by which stories otherwise unrecognized have a forum, or that values of truth and honesty can be conveyed (Livingstone & Lunt 1994; Masciarotte 1991; Carbaugh 1988). However, theorists such as Abt and Seesholtz (1994) view therapeutic talk shows as potentially demeaning to guests who appear on them as well as a means by which suffering can be trivialized. Through an examination and analysis of requests for information, both direct and indirect, which are the principal features of all interviews, I look at how Oprah Winfrey and Dr. Phil problematize the narratives told by their guests. Both hosts rely heavily on one type of request, B-Event assertions functioning as indirect requests for information, to provoke specific response. My findings indicate that these talk shows do not in fact provide a forum for talk that would otherwise not be considered as suggested by Livingstone and Lunt (1994). Both Oprah Winfrey and Dr. Phil appropriate the stories provided by their guests; they also participate in their own overarching meta-narrative whereby as ‘heroes’ they solve their guests’ problems within a given time frame. Guests’ narrative responses share more with the speech act of confession than with free self-expression. The speech act of confession encouraged by these therapeutic talk shows serves in turn to reinforce conservatively-held values on the part of the at-home audience for whom such confession is also entertainment. This article argues that the meaning of the word ‘autism’ experienced a radical shift in the early 1960s in Britain which was contemporaneous with a growth in epidemiological and statistical studies in child psychiatry. The first part of the article explores how ‘autism’ was used as a category to describe hallucinations and unconscious fantasy life in infants through the work of significant child psychologists and psychoanalysts such as Jean Piaget, Lauretta Bender, Leo Kanner and Elwyn James Anthony. Theories of autism (...) were then associated both with schizophrenia in adults and with psychoanalytic styles of reasoning. The closure of institutions for ‘mental defectives’ and the growth in speech therapy services in the 1960s and 1970s encouraged new models for understanding autism in infants and children. The second half of the article explores how researchers such as Victor Lotter and Michael Rutter used the category of autism to reconceptualize psychological development in infants and children via epidemiological studies. These historical changes have influenced the form and function of later research into autism and related conditions. (shrink)

I-BLOCKS are an innovative concept of building blocks allowing users to manipulate conceptual structures and compose atomic actions while building physical constructions. They represent an example of enabling technologies for tangible interfaces since they emphasise physicality of interaction through the use of spatial and kinaesthetic knowledge. The technology presented in this paper is integrated in physical building blocks augmented with embedded and invisible microprocessors. Connectivity and behaviour of such structures are defined by the physical connectivity between the blocks. These are (...) general purpose, constructive, tangible user interface devices that can have a variety of applications. Unlike other approaches, I-BLOCKS do not only specify a computation that is performed by the target system but perform at the same time the computation and the associated action/functionality. Manipulating I-BLOCKS do not only mean constructing physical or conceptual structures but also composing atomic actions into complex behaviours. To illustrate this concept, the paper presents different scenarios in which the technology has been applied: storytelling performed through the construction of physical characters exhibiting emotional states, and learning activities for speech therapy in cases of dyslexia and aphasia. The scenarios are presented; discussing both the features of the technology used and the related interaction design issues. The paper concludes by reporting about informal trials that have been conducted with children. It should be noted that, even if both trials represent application scenarios for children, the I-BLOCKS technology is in principle open to different kinds of applications and target users like, for example, games for adults or brainstorming activities. The pandemic had serious implications for university education, specifically due to the transition from face-to-face teaching to online methodologies. This article analyzes the perception of students undergoing speech therapy from a Chilean University about the E-portfolio incorporation as an evaluative tool during the emergency virtual teaching due to the COVID-19 pandemic. From quantitative research, a survey of 38 questions based on Likert scales was applied to 108 penultimate year undergraduate students. He had graduated from St Mary’s Seminary in Baltimore and was being ordained as a Roman Catholic priest. The event was attended by many of my family members. Several of the littlest attendees struggled with all the commotion, some were said to be shy, some didn’t want to be crowded, (...) and parents and caregivers did what they could to make it easier on them. Watching those kids trying to cope with a family event, which was different and a bit confusing, brought back a flood of memories of my own childhood. Nothing was simple about my childhood.My life has been “interesting” for sure. As a young child I faced many challenges. Until the age of two and a half I was nonverbal. Even though later I would gain speech, my developmental delays and sensory integration dysfunction issues caused great [End Page 202] difficulty for myself and my parents. I was evaluated by professionals and was diagnosed with an autism spectrum disorder–pervasive developmental disorder not otherwise specified (PDD–NOS) at the age of four.Thoughts of that difficult time filled my head when watching the children at this family function. My uncles, aunts and cousins couldn’t possibly know my struggles in those years. They weren’t present for all the interventions and therapy sessions I endured. Today, they don’t see me as someone different or disabled. I still have some obstacles to overcome in my development but rather than bemoan where I am or where I have been I have chosen to accept myself as I am. Although some may say I have a disability I choose to embrace my abilities and to celebrate them. I’m glad I’ve stayed on a path that has led me to become an independent adult. I am now a national motivational speaker, a Master’s graduate, owner of a non–profit corporation and soon to be an author.But what about this word “disabled”? Since I was six years old I seemed to have been tagged with this word by those around me. They all considered autism as a disability. I didn’t even know I had autism until I was 11. But I was very aware of people defining me as being disabled. It had to be the reason I was being placed in Special Ed classes, for the hours I had to travel to doctors and therapies: occupational therapy, physical therapy, speech therapy. I was being called disabled and I didn’t know why. Of course the signs were there for me that I had a problem. In daycare many things confused and frightened me: loud noises, wind, water, transitions. I cried constantly, a lot more than everyone else. I had terrible tantrums (lying on the floor kicking and screaming) to the confusion and consternation of the adults and caregivers around me. I attended two different daycare centers and was asked to leave both of them. I couldn’t even make the transition from the door of a daycare center to my parent’s car without fighting, screaming and crying uncontrollably.It’s now hard to believe that my difficulty with sensory integration dysfunction, transitions and social interactions lasted for almost 15 years of my life. During those years there were many confusing, frightening and painful episodes for me, my parents and others around us.Like most kids with these types of deficits, we tried to find splinter skills to help me compensate. I have hyper–mobility of the joints making buttoning and fine motor tasks almost impossible. I also have dysgraphia, which makes it difficult for me to hold a pen or pencil to write. Even with therapy I never learned to ride a bicycle. One bright side to the hyper–mobility was being able to learn to type 100 words per minute.My sensitivity to water, wind and loud sounds was extreme. I fought my parents each time they tried to get me into the bathtub. I hated going on the swings because I couldn’t feel my feet on the ground... (shrink)

This article proposes an analysis of the gestural production of people suffering from fluent or non-fluent aphasia in comparison with the gestures performed by speakers not suffering from any language disorder. The interest of such a study is twofold: firstly, it allows a better understanding of what happens to gestures during the numerous speech dysfluencies caused by the speech impairment. Among other things, it sheds light on the links between speech and gestures in multimodal face-to-face interaction. Despite (...) the profusion of work on aphasia, very little is still known about the characteristics of the gestures performed spontaneously by aphasic people. The study presented here is based on video examples from the AphasiaBank corpus to illustrate the specific features of the speech and gestures of atypical speakers and gives a more detailed description of the gestural production of people with aphasia. This knowledge can inform post-stroke rehabilitation procedures that rely on some form of multimodal speech therapy in order to adapt to the various communication needs of the patients. (shrink)

Since 2016, my embodied avatar performance (EAP) has explored healing rituals and life review at the intersection of arts, health and virtual reality (VR) for a variety of individuals, including older adults and cancer patients. EAP established a format in which the avatar mirrors the participant’s behaviours and speech, facing them during the life review process. The aspect of mirroring and facing is crucial in EAP for facilitating engagement, embodiment and empathy and a symbiotic relationship between avatar and human. (...) This article outlines the symbiosis between humans and avatars within the EAP process, focusing on the interactions where participants face and mirror their avatars. The avatars are presented as if participants are looking into a mirror. As the avatars face the participants and mimic their movements, they create an illusion similar to seeing one’s reflection in a virtual setting. The article discusses a range of mirroring aspects, including virtual mirrors in VR, echoing techniques in drama and movement therapy, and broader concepts like empathy, embodiment, we-ness and the Korean emotional concept of Cheong. This article presents a multimodal video corpus with the principal aim to model and predict the effects of aging in Mild Cognitive Impairment situation on pragmatic and communicative skills. We take as observable variables the verbal pragmatic markers and non-verbal pragmatic markers. This approach, at the interface of the psycholinguistics, cognitive sciences and rehabilitation medicine (speech-language pathology and therapy) is part of a longitudinal research process in an ecological situation (interviews conducted by close intimate of the elderly).In the (...) first part of the article we present the linguistic, cognitive and social characteristics of aging in its continuum up to mild cognitive impairment and pathological disorders such as Alzheimer’s disease. In the second part, we develop a multimodal approach, in particular to inform and enrich speech and language therapy knowledge. Finally, we present our experimental design and preliminary results on two female participants over 75 years of age with mild cognitive impairment.Our general findings indicate that with aging, verbal pragmatic markers acquire an interactive function that allows people with Mild Cognitive Impairment to maintain intersubjective relationships with their interlocutor. In addition, at the non-verbal level, gestural manifestations are increasingly mobilized over time with a preference for non-verbal pragmatic markers with a referential function and an interactive function. One such non-verbal manifestation compensates for naming deficits, planning difficulties, discursive hitches; while another optimizes and maintains the interaction with the interlocutor.Clinicians have a duty to develop their professional practice through an evidence-based clinical approach whose main objective is to reconcile clinical practice with the best evidence from research (Dollaghan 2007). In the case of speech-language pathology, clinicians consider themselves very limited in this approach (Lof 2011; McCurtin 2011), especially for patients with Mild Cognitive Impairment (Mungas et al. 2010; Hopper 2013; Morello 2017) and more specifically when it comes to assessing or supporting language functions (Cummings 2014).The studies focusing on Mild Cognitive Impairment require longitudinal corpora i) to understand the naturally occurring evolutions in subjects, ii) the implication of the cognitive reserve in each individual, and iii) to take advantage of these parameters as evidence for research and earlier rehabilitation. I would extend Postman’s Technopoly to Technopolis, to give it more of a presence in time and space. Hence, I will use the term Technopoly when I am referring to Postman; the term Technopolis is my own. -/- In his book From Faith to Fun: The Secularisation of Humour, Russel Heddendorf argues that humour is a technology (Heddendorf 2009: 32-34), as it is understood and theorized by Neil Postman in Technopoly. Russel Heddendorf also argues that “Technopoly” is a term correlated with Wittgenstein’s use of the term “worldview” (Weltanschauung) (Heddendorf 2009: 11). -/- I would like to draw on these thoughts to propose that Technopoly (or Technopolis) could be the secularized Kingdom of God, namely that Technology is the Religion of the 21st century, and that Fun is a paradigm of such a Technology / Technique. God is yet another technique or Technology of and in Technopoly. -/- The godless individual in the 21st century, I will argue, is as much a believer as the faithful in the Sacred Religion (term used in opposition to Secular or rather Technological or Technologized Religion, which is Fun). He believes in Fun (as a post-modern form of pleasure), as much as the believer believes, or more accurately, believed in God. In the 21st century, the faithful in Sacred Religion believes in a fun God, as opposed to the serious God of the past. Fun is turning into the Technologized Religion for all; for the faithful in Godly / Sacred Religion, as well for as the unfaithful. Fun (pleasure, humour) is the definitive qualia of 21st century pleasure. The mechanics of Fun make it absolutely functional for controlling people; as Aldous Huxley wrote in Brave New World, people “are controlled by inflicting pleasure” (cited in Heddendorf 2009: 158). Now, indulging in pleasure is having Fun. In the 21st century, nobody is unfaithful, nobody disputes God, because God has been manufactured into Fun. We are living in the most religious century of all. -/- The commodity of God, now inextricably blended into work and leisure ethics and culture, is as unavoidable as survival. You don’t work, you don’t survive, you don’t exist. You work, you are entitled to leisure, or free time – the time (“busy-time”) when you are busy is work. This article examines the design and situated employment of reported `private thoughts' in both everyday and institutional interaction. By reported `private thoughts' we mean the `active voicing' of utterances characterized as `private thought' done in the first place for the speaker-feeler, rather than the listener. Examples are drawn from a large UK collection of over 240 instances from domestic telephone calls, interview talk, therapy sessions, and patient—provider interactions. Instead of treating reported `private thoughts' as neutral and transparent descriptions of (...) the inner mind, we focus on their `brought off ' nature. Drawing on the cumulative resources of conversation analysis and discursive psychology, we focus on lexical and non-lexical features of their design and its similarity to direct reported speech. We go on to illustrate the flexibilities of positioning reporting `private thoughts' affords, that is, how they can be done for the self as a `one-off ', as generalized or hypothetical and how they can be done for others. Our analyses draw attention to how `private thoughts' might be considered as a speaker's resource for handling everyday rational accountability in reporting and explaining actions and events. This article aims to present the problem of the impact of artificial intelligence on respect for human dignity in the sphere of care for people who, for various reasons, are described as particularly vulnerable, especially seniors and people with various disabilities. In recent years, various initiatives and works have been undertaken on the European scene to define the directions in which the development and use of artificial intelligence should go. According to the human-centric approach, artificial intelligence should be developed, used (...) and monitored with people in mind, their needs and rights. It is artificial intelligence that should adapt to the rules set by people, not the other way around. The source of all human rights and freedoms is respect for human dignity. This is evidenced by numerous European, international, national regulations and documents. Respecting this value is also one of the works on the AI development. One of the areas of our life into which AI enters more and more boldly and which in the near future, due to demographic reasons and aging population, may be almost dominated by modern technologies, is the medical and care system. The usefulness of industrial co-robots made us think about the possibility of using similar devices in the non-industrial sphere, including in the sphere of care, nursing and therapy. Actually, despite the impressive development of technology and AI, intelligent robots/devices used for care, nursing or therapy (so-called care/nursing or therapy robots) perform rather simple activities. They provide information, navigate, bring something and give it (e.g. medicines, food), help to get up. They are an important support for medical staff in this respect. Performing tasks that are more complex in terms of technology and movement, such as feeding, washing, intimate hygiene, dressing or undressing, is still beyond the reach of the currently used robots. The question we must ask ourselves is whether, in the future, care robots will be able to perform such tasks, and should they do so? Will this lead to the replacement of human medical personnel in the performance of the above-mentioned activities with such robots. Would anyone want to be "looked after" by a soulless machine? Should potential wards/patients, in other words all of us in fact, have the right to object to being taken into the care of an intelligent robot? There is no doubt that intelligent robots have great potential, especially when it comes to ensuring or maintaining the independence or mobility of people with various health limitations. However, we must not forget about the risks and dangers they may generate. Even a simple robot generates serious legal and ethical problems. Who should be liable for damage caused in connection with improper care performed with the participation of a robot? Robots based on AI systems are able to collect a significant amount of sensitive data, for example through the face and speech recognition function. There is considerable scope for abuse in this area, not only in terms of personal data protection but also in terms of informational self-determination, privacy and dignity. Bearing in mind the above risks, the creation of a new right to object to being looked after by a robot should be considered. (shrink)

Heidegger’s 1927 call to provide “a special hermeneutic of empathy” is linked with his later commitment at the Zollikon Seminars to engage explicitly with issues in psychodynamic therapy with psychiatrists. The task of providing a special hermeneutic of empathy is one that Heidegger assigns in Being and Time, but on which he does not deliver. Inspired by the assignment, this article applies the distinctions of Heidegger’s Daseinanalysis to human interrelations. This article generates a Heideggerian account of empathy as a (...) multi-dimensional process that delimits and illuminates the field of possibilities of authentic human relationships. The multiple dimensions of empathy include affectedness (Befindlichkeit), understanding of possibility, interpretation, and speech, the latter including listening. The result is a reconstruction of a Heideggerian account of empathic human relations in the sense that it goes beyond what Heidegger explicitly says to what the Heideggerian method of inquiry can contribute to understanding and implementing the process of empathy. In particular, a two-by-two matrix is built and engaged in detail, cross referencing the four possibilities of authentic and inauthentic relationships with the individual and the other. A specifically Heideggerian analysis of the multi-dimensional process of empathy is the result. The clinical relevance of Heidegger’s work is made explicit as empathy is positioned as the foundation of clinical practice as exemplified in psychodynamic psychotherapy. (shrink)

Many new mothers question the nature of their motherly love after birth. This affectionate relationship towards the infant is commonly called bonding in everyday speech, clinical practice and research. Bonding may not sufficiently describe the mother’s emotional response to the infant and does not capture the ambivalence and struggle to develop maternal affection of many women. This study aims to explore the phenomenon of disturbed maternal affection through the clinical case of one mother who experienced severe and prolonged disturbances. (...) Two in-depth interviews led to a descriptive phenomenological analysis. The mother developed depressive symptoms from not feeling enough for her child, not the opposite, as is often hypothesized. We describe and discuss crucial constituents of her experience, such as ambivalence, remoteness, boredom, guilt, and the looming repetition of parenting patterns, and a solution resulting from therapy-enhanced reflection on motherhood vis-à-vis early life patterns, sociocultural expectations and existential predicaments. Explanations of former South African President Thabo Mbeki’s public and private views on the aetiology of the HIV/AIDS epidemic in the country remain partial at best without the recognition that the latter presuppose and imply a postmodernist/postcolonialist philosophy of science that erases the line separating the political from the scientific. Evidence from Mbeki’s public speeches, interviews, and private and anonymous writings suggests that it was postmodernist/postcolonialist theory that inspired him to doubt the “Western” scientific consensus on HIV/AIDS and to implement (...) a public health policy that dragged its feet on full roll-out of antiretroviral therapy, causing thousands of avoidable deaths. A weak reductio ad absurdum allows us to conclude from this premise that postmodernist/postcolonial critique of “Western” science ought to be shunned. A comparative argument from consequences further suggests that in a situation where a misguided health policy has lead to a humanitarian catastrophe, and where postmodernist/postcolonialist critique of science can and has been used to justify this policy, an alternative theory ought to be preferred on which such justification would not be possible. The paper closes with a call for a non-relativist alternative to postmodernist/postcolonialist philosophy of science, and evaluates the potential of recent developments in ‘Studies of Expertise” to yield such. The development of speech language therapy students into clinicians is an area of increasing interest as educators focus on how knowledge, skills and attitudes are taught and learnt within the profession. The personal journeys of students through experiences of service learning have potential to further our understanding of the impact of civic engagement on the student experience and their learning. This paper explores the journeys of first year speech and language therapy students through a Thematic Analysis (...) of reflective letters written by students to themselves at the beginning and completion of a service learning module. Analysis demonstrates development of interpersonal and preclinical skills as well as an understanding of attitudes and values inherent in the social model of disability. The skills and attitudes developed by the students through participation in the Conversation Partner Scheme are consistent with social model principles that support therapists to fully address the long-term, real life needs of clients with aphasia (the acquired communication disorder that frequently follows stroke). The interface between the social model of disability and the role of service learning in nurturing the attitudes and values which underpin this model are explored. (shrink)

The effect of such questions is to force us to realize that our concept of color is more complex than we might have realized, or would want (...) it to be if we are committed to epistemologicaI theories in which color experience counts as basic. Other questions make us aware that the simple contrast of conceptual and empirical turns out to be hopelessly inadequate, and that this immensely complicates, if it does not vitiate, the task of drawing a comprehensive map of knowledge. We understand, in a way, what Wittgenstein is doing, but come to think we do not, since the effect of reading these remarks is to unsettle us about beliefs we had thought we could safely assume. The difficulties of Wittgenstein's questions pass over into the Companion, as was the case with Max Black's Companion to the "Tractatus" as well. Both works fortify standard ventures at interpretation, and both frustrate by leaving untouched one's worst perplexities. But perhaps one sweeping thought can be ventured. Wittgenstein once remarked that the present age is a time for popular science, and therefore not a time for philosophy. 6 Reading Wittgenstein, or his Companion, restores vitality to ways of thought and speech more primitive, and more basic, than science. Surely the typical object of his therapy is the muddle we get into by borrowing the language and method of science to describe and explain the relation of the perceiver to the world, or the agent to influences on his conduct. Moreover, the language of thought, act, and agency is presupposed in a description of how scientists can and do proceed. The temptations of a religious picture of the world for this point of view are strong--and reflected in the positive interest Wittgenstein evinced for religious thinkers. For religion expresses the limits of science by means of imagery that is not alien to a description of the world in terms of things and processes. But to say this is to come to a halt before the apparatus of language games and deep grammar as one comes to exegetical dead ends before Plato's Forms or Spinoza's infinite attributes. They are the imperfect means of communicating essentially private visions. One final cavil: The reader is sometimes teased rather than appeased by numbered citations referring to unpublished manuscripts. To quote them in full would of course have made the Companion impossibly tong. But for many readers Hallett's citations wilt be, at best, promissory notes to be cashed only when it suits Wittgenstein's executors to publish this material. It is to be hoped that when that day arrives it will be possible to correlate the manuscripts with Hallett's citations. As the Remarks on Colour show, however, we cannot be too sanguine. Would it not be wiser to publish the manuscripts as they were written, rather than to select passages which in the editor's judgment are related by theme? Jaeger Keywords embodiment, dialogical consciousness, interpersonal communication, epistemic responsibility, self-knowledge, understanding IN HER ARTICLE "Moral Tourists and World Travelers," Nancy Potter suggests a way in which psychiatrists and psychologists could gain a better understanding of their mentally ill patients' experiences. Rather than assuming that hallucinations and incoherent (...) speech and behavior are merely senseless disconnections from reality, Potter argues that mental health care clinicians ought to spend time engaging patients on their own terms. Even when mental illness is a result of brain disease, much can be learned about the patient's experiences of their disease by temporarily setting aside the scientific frameworks of diagnosis and medical treatment. Potter uses Maria Lugones' concept of world traveling and recommends playfully accepting, and even entering into the patient's world (Lugones 1990). She calls world traveling a methodology and explains it in some detail. Moreover, at the close of her essay, Potter suggests that clinicians have a moral obligation to understand the mentally ill patient's world from the patient's perspective. This obligation is based on an appeal to our sense of what it is to be a responsible knower who attends to issues of trust and truth. Relying on arguments made by Lorraine Code (1987), Potter asserts that to try to understand the other's perspective is the responsible thing to do.My discussion focuses on the notion of play to which Potter appeals, as well as on what she sees as the moral obligation of clinicians to communicate with their mentally ill patients in extra-professional ways. This moral obligation arises because (a) the theoretical and professional frameworks of medical science are seen as inadequate, and (b) playfulness opens up mutually respectful, authentic interaction between persons. I generally agree with Potter's claims, and I want to situate them in the wider framework of an epistemological debate about the status of our knowledge of others. A broader context will help to elucidate the strengths of Potter's analysis as well as some points for critical discussion. I begin with the relationship of trust between clinicians and patients.Few people would disagree with the claim that patients need to be able to trust that their physicians are not harming them, but helping them to get better. If patients feel misunderstood, they will not trust their caretakers. This is probably even truer for psychiatric patients, whose conditions and loss of autonomy make establishing trust even more challenging. Psychiatric patients [End Page 227] often have their own ways of compensating for the symptoms of their mental illness. If either their will to survive or their compensatory strategies go unrecognized or misinterpreted, patients may respond by rejecting the well-intentioned efforts of caretakers to restore them to a degree of ease, measured as it must be by the standards of the professionals under whose care the patient remains. 1 Despite the fact that these standards are established through scientific testing procedures for the treatment of diseases, they may be experienced by patients as indifferent to the particular and unique features of their condition. Nestled within the issue of trust is thus a question of truth.Although current scientific knowledge of the human brain, and of human behavior, has made available a range of sophisticated therapies and psychotropic drugs, there are still many mental illnesses that leave families and clinicians wondering whether the treatment plans are providing any help. Alzheimer's disease and schizophrenia are two examples. There is a difference between, on the one hand, containing the patient's behavior by admitting the patient to a psychiatric ward and managing symptoms with psychotropic medications, and, on the other hand, understanding what is happening for the patient, from his or her perspective. In May 2019, the Vatican held a conference in Rome that focused on perinatal palliative care. In these troubled times, that would seem to be an arcane topic for the church to address. The speeches at the conference made it clear why the topic was timely and relevant. Speakers included scientists, clinicians, theologians, and advocates for a humane approach to clinical decisions in situations of prenatal diagnosis of fetal anomalies. There is currently a movement advocating “philosophical counseling.” My own development as a philosopher, then a human services professional, then a psychoanalyst, charts how I came to believe that philosophical training was underrated, and training in psychology was overrated, as an appropriate intellectual foundation for psychotherapy. However, these fields are not distinct. Laws governing the practice of psychology are arrogant in their scope, and make virtually everything out to be the practice of psychology. The scope and nature of philosophy isn’t (...) any clearer. The kind of thinking encouraged in psychology is liable to be exactly the wrong sort of thing for training therapists. Unfortunately, philosophers are liable to not be good therapists either. The lack of neat distinctions between philosophical counseling and psychotherapy provides an argument against a monopoly on therapy-like activities by psychologists. On the liberal side this is an argument in favor of freedom of speech, of belief, and trade, for the applied philosopher. On the conservative side, it may also be an argument for certification (as opposed to licensure) for both psychologists and philosophers, in the interest of protecting the vulnerable by promoting truthful self-representation. The empirical slippery slope arguments are arguments of the following form: if we take a first step A, as a result of a sequence of events, step N will necessarily or very likely follow; N is clearly not acceptable; therefore we must not take step A. Such arguments are often used in the discussions concerning abortion, assisted suicide, human gene therapy, free speech, decriminalizing marijuana, gun control and other ethical or social issues. In this article, I am trying (...) to argue that although slippery slope arguments are not deductive, they need not to be considered as fallacies. There are some mechanisms that make the realization of the scenarios sketched in the slippery slope arguments much more probable than one could think. I analyze three different examples of such mechanisms. The first one originates in the psychological phenomenon of cognitive dissonance. The second is connected with the use of precedents. The third mechanism depends on so-called multi-peaked preferences which some people might have when they make their decisions. (shrink)

Although developmental dyslexia is frequently diagnosed in Poland, little knowledge of effective treatment for this disorder is available in Polish society. Remedial teachers for many years have applied traditional methods aimed at correction and compensation for affected cognitive functions and academic skills. Otherwise, although western therapies are regularly advertised in the media, their effectiveness has rarely been subject to scientific investigation. Since the assumptions underlying some approaches are not consistent with current understanding of cognitive function, they may attract negative expert (...) appraisal. Unfortunately, it seems that fashion in dyslexia therapy is resistant to rare expressions of criticism from the scientific community. The purpose of this article is to promote awareness about effective treatment for specific reading disorders. Teachers’ greater understanding in this area should help Polish dyslexic children and their parents, who may be confused by offers of misguided therapy and sharp business practise. It is hoped that this article will clarify the situation. In this review of modern English journal articles, focus is on remedial teaching of reading, and more specifically, support for dyslexic students experiencing difficulties in decoding. Here we are concerned only with therapy in alphabetic languages in which individual speech sounds correspond to letters. Analysis of articles from the last six years leads to the conclusion that the most effective therapy for the reading disorder is training in phonological awareness and consolidation of letter-sound knowledge. However, these skills should be practised in the context of reading. (shrink)

There is much to admire in this paper. The treatment of speech and silence in the thought of Hegel (...) and Kierkegaard, with many well-chosen quotations, offers much for reflection, both with respect to these philosophers and more generally with respect to the human condition. This is a paper that deserves more than one reading. I find nothing to criticize here and intend the following as further discussion of the themes Berthold has brought to us. Taking his cue from Lacan and reading back into the nineteenth-century figures from the perspective of Lacan's emphasis on language and what he (Lacan) terms the Symbolic, Berthold explores the question of madness in Hegel and Kierkegaard. In my remarks, I connect Berthold's analysis with older traditions of madness, distinguishing what I call the madness of the philosophers and the madness of the clinic.With the phrase, madness of the philosophers, I refer to a longstanding philosophic tradition that opposes reason and passion and attributes madness to an excess of the latter. In Berthold's paper, Hegel is the representative of this position, with his description of madness as a regression into the primitive "life of feeling." As Berthold puts it:For Hegel, madness is a "reversion" or "withdrawal" or "sinking back" of the rational consciousness into the primitive world of instincts and drives, or what Hegel calls "the life of feeling" (Gufühlsleben), which he locates in "the soul," where the "mind is still in the grip of nature"... The soul is "the form of the dull stirring, the inarticulate breathing of the spirit through its unconscious individuality" (in seiner besusstlosen Individualität)... The mind becomes "mastered" by the archaic, unconscious drives of the soul: "the natural self... gains mastery over the... rational consciousness."(2009, 301)Taking off from Lacan in his interpretation of Hegel, Berthold associates reason with language—or, in Lacan's terminology—the register of the Symbolic. Hegel's "life of feeling"—or of passion—is inarticulate, primitive, and vulnerable to madness.Berthold analyzes Kierkegaard in these categories. On the one hand, Kierkegaard is driven to silence, subjectivity, and inwardness as the native [End Page 313] ground of faith. On the other hand, he recognizes something maddening in his extreme solitude, and acknowledges his need to write as a form of self-therapy: "If I stop [writing] for a few days, right away I become ill.... By Berthold's account, Kierkegaard's silence does not rate very high for Hegel and Lacan. For Hegel it represents a regression into the primitive "life of feeling." For Lacan, Kierkegaard's silent inner self is "a myth—a 'shapeless vapor,' 'absolutely nothing.'" As we shall see, Kierkegaard's ambivalence toward silence and speech (or writing) does not fit neatly into the equation of silence and madness.What, however, in contrast to this madness of the philosophers, is the madness of the clinic?1 It is madness caused by disease, whether the latter be understood in ancient terminology of humors, or more modern terminology of brain disease. For a quick review of how clinical madness has played off against the madness of the philosophers, we can turn to the philosophers themselves, invoking Plato as our first guide, because he speaks of both kinds of madness. In this essay, I contend that Polanyi’s view of metaphor as outlined in Meaning (1975), has important heuristic implications for understanding the way metaphor functions in trauma therapy. I also contend that in his seminal book on trauma, The Body Keeps the Score (2014), Bessel van der Kolk, M.D., although he rarely uses the term, relies on metaphor as a vital element in his treatment of trauma victims. Analysis of Van der Kolk’s practice further confirms and extends Polanyi’s view (...) of the bodily roots of all knowledge. Juxtaposing Polanyi’s theory and Van der Kolk’s practice demonstrates how unspeakable trauma can be overcome through the embodied metaphoric/linguistic matrix of human speech. (shrink)

There are currently no instruments available to measure ethical sensitivity in the therapeutic sciences. This study therefore aimed to develop and implement a measure of ethical sensitivity that would be applicable to four therapeutic professions, namely audiology, occupational therapy, physiotherapy, and speech-language pathology. The study followed a two-phase, sequential exploratory mixed-methods design. Phase One, the qualitative development phase, employed six stages and focused on developing an instrument based on a systematic review: an analysis of professional ethical codes, focus (...) group discussions, in-depth interviews, a review of public complaints websites, and an expert panel review. The development phase culminated in the Measuring Instrument for Ethical Sensitivity in the Therapeutic Sciences (MIEST), a pen-and-paper measure for studying ethical sensitivity in the therapeutic sciences.Phase Two, the quantitative stage, focused on implementing the MIEST in two different stages. A total of 100 participants completed the instrument. MIEST scores were found to be comparable for all four professions, which confirmed the multidisciplinary usability of the instrument. Participants tended to base decisions on the ethical principle of beneficence. The MIEST is effective to assess and describe the ethical sensitivity of professionals in the four specified therapeutic sciences. The constructed vignettes also make the MIEST appropriate for use in problem-based learning programs. (shrink)
