An individual's health, genetic, or environmental-exposure data, placed in an online repository, creates a valuable shared resource that can accelerate biomedical research and even open opportunities for crowd-sourcing discoveries by members of the public. But these data become “immortalized” in ways that may create lasting risk as well as benefit. Once shared on the Internet, the data are difficult or impossible to redact, and identities may be revealed by a process called data linkage, in which online data sets are matched (...) to each other. Reidentification, the process of associating an individual's name with data that were considered deidentified, poses risks such as insurance or employment discrimination, social stigma, and breach of the promises often made in informed-consent documents. At the same time, re-ID poses risks to researchers and indeed to the future of science, should re-ID end up undermining the trust and participation of potential research participants. The ethical challenges of online data sharing are heightened as so-called big data becomes an increasingly important research tool and driver of new research structures. Big data is shifting research to include large numbers of researchers and institutions as well as large numbers of participants providing diverse types of data, so the participants’ consent relationship is no longer with a person or even a research institution. In addition, consent is further transformed because big data analysis often begins with descriptive inquiry and generation of a hypothesis, and the research questions cannot be clearly defined at the outset and may be unforeseeable over the long term. In this article, we consider how expanded data sharing poses new challenges, illustrated by genomics and the transition to new models of consent. We draw on the experiences of participants in an open data platform—the Personal Genome Project—to allow study participants to contribute their voices to inform ethical consent practices and protocol reviews for big-data research. ABSTRACTEthical scandals involving journalists in English-speaking West African countries have been documented to include conflict of interest, freebies, intellectual theft, deception, carelessness, kowtowing to advertisers and politicians, use of dubious evidence, and outright bias. This study explores how pronounced and clear the rules relating to these breaches are in the codes of these countries and whether the similarities and dissimilarities in wording indicate the influence of individual actors involved in writing them. Relying on thematic and qualitative document analysis methods, the (...) study found that rules in the codes addressing the ethical breaches are pronounced and clear. Although largely similar in content, the codes also feature important differences that are strongly related to the composition of the groups that wrote them. This study discusses why ethical challenges in these countries persist in the midst well-written code of ethics. (shrink)

The required role of the psychiatric‐mental health nurse in primary health‐care: an augmented Delphi study An augmented Delphi study was employed to elicit the perceptions of CPNs, GPs, social workers, managers of psychiatric nursing services and health service purchasers in England, on the role required of a psychiatric (mental health) nurse in primary health care. In the final stage of the study, users of mental health service were enlisted in a verification study of the emergent findings. A broad consensus emerged (...) that a gap existed in mental health services in primary care. This was perceived as placing a large burden on GPs, which might be reduced by the appropriate development of community psychiatric‐mental health nursing. The study suggested the possibilities for development of a ‘mental health nurse consultant’ role, which might augment the current CPN role, within which the nurse might offer not only direct patient care, but also support to members of the primary care team. From Parmenides to the present, philosophers have been attracted by characterizations of being as being uttered or utterable, formulated or formulable. But what for Parmenides was presumably a valid co-entailment between antecedently understood concepts reappears in contemporary thought as a proffered explication of what it is to be. Without presuming to discredit Parmenidean views in general, my purpose here is to examine certain members of a modern family of theories of existence that fall into place around Quine’s. Depending on the (...) methodology of explication espoused, the theories in this family are either semantic commentaries on ideal languages, or they are themselves interpreted formalized languages. The former may be called, after Quine, theories of ontological commitment. The theories of the latter sort I wish to consider are called, after Karel Lambert, free logics, i.e., logic systems the singular terms of which are free of existential import. My conclusion about theories of both kinds is that, while unexceptionable on their own terms, from a broader point of view they beg the question of what it is to be. In arguing that these theories thus fail to clarify the concept of existence, I believe I am not so much propounding a new insight as articulating an objection many thinkers have felt in the face of views akin to Quine’s. This book, published in 2000, is a clear account of causation based firmly in contemporary science. Dowe discusses in a systematic way, a positive account of causation: the conserved quantities account of causal processes which he has been developing over the last ten years. The book describes causal processes and interactions in terms of conserved quantities: a causal process is the worldline of an object which possesses a conserved quantity, and a causal interaction involves the exchange of conserved quantities. Further, (...) things that are properly called cause and effect are appropriately connected by a set of causal processes and interactions. The distinction between cause and effect is explained in terms of a version of the fork theory: the direction of a certain kind of ordered pattern of events in the world. This particular version has the virtue that it allows for the possibility of backwards causation, and therefore time travel. (shrink)
