A dor do ponto de vista clínico é definida como estímulo e resposta a partir de fenômeno biológicos complexos e subjetivos nas dimensões sensorial-discriminativa, afetivo-motivacional e cognitivo-avaliativo. Sua definição tem sido um desafio ao longo da história, desde a sua percepção como uma experiência espiritual e mística nos primórdios tempos, até o seu entendimento em termos biológicos como uma sensação física e mecânica que passa pelas fibras e alcança o cérebro. A dor é um efeito extremamente necessário uma vez que (...) é um sinal de alarme de que algum dano ou lesão está ocorrendo, embora a dor possa também estar presente na ausência de qualquer estímulo, dano ou doença detectável. Preponderantemente a dor é eliminada uma vez que o estímulo nocivo é removido, no entanto, a dor pode persistir apesar da remoção do estímulo e cicatrização aparente do tecido sendo denominada de dor crônica, com influência multidimensional. O limiar de dor fisiológico é definido como o ponto ou momento em que um dado estímulo é reconhecido como doloroso, o que pode variar de um indivíduo para o outro, e ainda estar associado as experiências prévias com lesão tecidual real ou potencial e a fatores psicológicos. Dessa forma, esta análise aborda a etimologia, histórico e evolução na concepção da definição da dor do ponto de vista clínico baseada na antiga e recente definição de dor pela Associação Internacional para Estudos da Dor frente ao contexto biológico que inevitavelmente é influenciado pelas condições psicossociais. Pode-se finalmente, estabelecer que há fortes indícios de associação entre lesão tissular e dor biológica com o sofrimento, comportamentos dolorosos e incapacidade. É evidente que o sofrimento e o comportamento originados a partir da dor do ponto de vista clínico, não se devem inteiramente à nocicepção originada por uma lesão tissular, pois os comportamentos dolorosos podem perpetuar-se em decorrência de fatores ambientais e de influências que não podem ser objetivamente avaliadas. A partir desse contexto, os bons programas de controle de dor devem ter uma abordagem cognitivo-comportamental e incorporar estratégias terapêuticas físicas, farmacológicas e psicológicas. Nenhum tratamento isolado é tão eficaz para a redução da dor crônica refratária como a conduta multidisciplinar. (shrink)

The article takes into consideration human pain from a threefold approach. First, it considers pain as an anthropological reality; second, it shows how medical sciences have contributed to such a reality; and finally, it addresses spiritual pain, recognizing how relevant the latter is for the care of patients, mainly for palliative care. Thus, the aim of the article is to motivate a reflection on how to treat human pain in a comprehensive way.

The self-domestication hypothesis suggests that, like mammalian domesticates, humans have gone through a process of selection against aggression – a process that in the case of humans was self-induced. Here, we extend previous proposals and suggest that what underlies human social evolution is selection for socially mediated emotional control and plasticity. In the first part of the paper we highlight general features of human social evolution, which, we argue, is more similar to that of other social mammals than to that (...) of mammalian domesticates and is therefore incompatible with the notion of human self-domestication. In the second part, we discuss the unique aspects of human evolution and propose that emotional control and social motivation in humans evolved during two major, partially overlapping stages. The first stage, which followed the emergence of mimetic communication, the beginnings of musical engagement, and mimesis-related cognition, required socially mediated emotional plasticity and was accompanied by new social emotions. The second stage followed the emergence of language, when individuals began to instruct the imagination of their interlocutors, and to rely even more extensively on emotional plasticity and culturally learned emotional control. This account further illustrates the significant differences between humans and domesticates, thus challenging the notion of human self-domestication. (shrink)

In his published lectures Civilizations: Nostalgia and Utopia, Daya Krishna criticizes postmodern thought and especially the writings of Jacques Derrida. By outlining similarities between the two, I would claim that, indeed, it was Daya Krishna’s unexpected proximity to Derrida’s ‘deconstruction’ project that triggered his scathing critique of the latter. Moreover, Daya Krishna’s response to Derrida reveals an ongoing inner conflict in his own thinking. On the one hand, he provides us with a harsh critique of Derrida the ‘postmodern’; on the (...) other hand, he concedes that the ‘modern’ notion of knowledge has been totally transformed, and the ‘deconstruction’ of its old formulations was the major catalyst that provoked and directed his own philosophical enterprise in the last years of his life, reformulating knowledges. Reconstructing in this way, the dialogue which never happened might prove beneficial, not only for understanding the distinctive works of its participants, but also for carrying their writings and us one step further towards a productive, ‘relevant’ philosophical discourse in the twenty-first century. The book suggests a new perspective on the essence of human language. This enormous achievement of our species is best characterized as a communication technology - not unlike the social media on the Net today - that was collectively invented by ancient humans for a very particular communicative function: the instruction of imagination. All other systems of communication in the biological world target the interlocutors' senses; language allows speakers to systematically instruct their interlocutors in the process of imagining the intended (...) meaning - instead of directly experiencing it. This revolutionary function has changed human life forever, and in the book it operates as a unifying concept around which a new general theory of language gradually emerges. Dor identifies a set of fundamental problems in the linguistic sciences - the nature of words, the complexities of syntax, the interface between semantics and pragmatics, the causal relationship between language and thought, language processing, the dialectics of universality and variability, the intricacies of language and power, knowledge of language and its acquisition, the fragility of linguistic communication and the origins and evolution of language - and shows with respect to all of them how the theory provides fresh answers to the problems, resolves persistent difficulties in existing accounts, enhances the significance of empirical and theoretical achievements in the field, and identifies new directions for empirical research. The theory thus opens a new way towards the unification of the linguistic sciences, on both sides of the cognitive-social divide. (shrink)

It echoed in my ears for the first time a very long time ago and far, far away: on the Roof of the World, the Afghan Pamir, more than 30 years ago. It was uttered by a Kirghiz shepherd following a herd of sheep. Even if it is not in fact possible to transcribe that (...) call accurately - the combination of the phonetic parameters that determine it would necessitate a representation using a graph or numerical matrices - I will attempt to give it a written shape, HAY, while admitting that this transcription denotes an abstraction that subsumes many possible realizations. But be that as it may. A few years later, when I was living in the Haut-Livradois, in the deepest heart of rural France, I was surprised to hear that call again on the lips of an Auvergnat peasant who was taking his sheep to the meadow. This unlikely coincidence led me to take a closer interest in the problem of communication between humans and animals. Often taken for granted or seen as the ideology of European states vying for supremacy and conquest, the concept of sovereignty remains underexamined both in the history of its practices and in its aesthetic and intellectual underpinnings. Using global intellectual history as a bridge between approaches, periods, and areas, The Scaffolding of Sovereignty deploys a comparative and theoretically rich conception of sovereignty to reconsider the different schemes on which it has been based or renewed, the public stages (...) on which it is erected or destroyed, and the images and ideas on which it rests. The essays in The Scaffolding of Sovereignty reveal that sovereignty has always been supported, complemented, and enforced by a complex aesthetic and intellectual scaffolding. This collection takes a multidisciplinary approach to investigating the concept on a global scale, ranging from an account of a Manchu emperor building a mosque to a discussion of the continuing power of Lenin's corpse, from an analysis of the death of kings in classical Greek tragedy to an exploration of the imagery of "the people" in the Age of Revolutions. Introduction With an ageing population, end-of-life care is increasing in importance. The present work investigated characteristics and time trends of older peoples' attitudes towards euthanasia and an end-of-life pill. Methods Three samples aged 64 years or older from the Longitudinal Ageing Study Amsterdam (N=1284 (2001), N=1303 (2005) and N=1245 (2008)) were studied. Respondents were asked whether they could imagine requesting their physician to end their life (euthanasia), or imagine asking for a pill to end their life if they became tired (...) of living in the absence of a severe disease (end-of-life pill). Using logistic multivariable techniques, changes of attitudes over time and their association with demographic and health characteristics were assessed. Results The proportion of respondents with a positive attitude somewhat increased over time, but significantly only among the 64–74 age group. For euthanasia, these percentages were 58% (2001), 64% (2005) and 70% (2008) (OR of most recent versus earliest period (95% CI): 1.30 (1.17 to 1.44)). For an end-of-life pill, these percentages were 31% (2001), 33% (2005) and 45% (2008) (OR (95% CI): 1.37 (1.23 to 1.52)). For the end-of-life pill, interaction between the most recent time period and age group was significant. Conclusions An increasing proportion of older people reported that they could imagine desiring euthanasia or an end-of-life pill. This may imply an increased interest in deciding about your own life and stresses the importance to take older peoples' wishes seriously. (shrink)

El consumo de energía y el crecimiento económico han llevado a la degradación del medio ambiente. Las centrales eléctricas son una de las principales fuentes de contaminación. Existe una necesidad de utilizar energías renovables para reducir el impacto ambiental. Sin embargo, en los países en desarrollo, la falta de recursos económicos limita la adopción de energías limpias. Las energías limpias, como la solar, eólica, hidroeléctrica y geotérmica, son fuentes sostenibles de energía. La energía hidroeléctrica puede ser controvertida debido a sus (...) impactos ambientales y sociales. La biomasa y el biogás también son fuentes de energía renovable. La eficiencia energética es otra área importante para reducir la dependencia de los recursos no renovables. El artículo discute la importancia de las energías limpias y el desarrollo sostenible en el Perú. Se menciona el potencial de la energía de las olas marinas, el bioetanol y el biodiesel como fuentes de energía renovable. También se abordan los impactos ambientales de los "negocios sucios" y las implicaciones socioeconómicas de las energías limpias. Se discuten los conflictos de interés y la regulación normativa en el país, así como las perspectivas futuras y la necesidad de un desarrollo energético estable y sostenible. The ArgumentErnst Cassirer's theory of language as a symbolic form, one of the richest and most insightful philosophies of language of the twentieth century, went virtually unnoticed in the mainstreams of modern linguistics. This was so for what seems to be a good metatheoretical reason: Cassirer insisted on the constitutive role of meaning in the explanation of linguistic phenomena, a position which was explicitly rejected by both American Structuralists and Chomskian Generativists. In the last decade, however, a new and promising (...) linguistic framework has emerged — the framework of lexical semantics — which seems to bear close theoretical resemblance to Cassirer's theory. In this paper, I show how the empirical results accumulated within the framework of lexical semantics serve to validate Cassirer's most fundamental philosophical insights, and suggest that Cassirer's philosophy helps position these empirical results in their appropriate epistemological context. I discuss the following fundamental points, which, for me, constitute the backbone of both Cassirer's philosophy and the theory of lexical semantics: natural language grammars constitute structural reflections of a deeply-rooted, highly structured level of semantic organization; the representational level of linguistic meaning, which is prior to experience in the Kantian sense, comprises a partial set of semantic notions, which language selects as centers of perceptual attention; this partial set is potentially different from the sets selected by other symbolic forms, such as myth, science, and art; and linguistic variability is to be explained in universalistic terms, thus allowing for specific patterns of variability within universally-constrained limits. This view helps characterize language as a unique and functional system — a cognitive system whose function is defined at the level of Linguistic Semantics. The thesis concerns itself with the selection constraints governing the basic distributional patterns of five complement constructions in English--the bare clause, the that-clause, the interrogative, the concealed question construction and the exclamative complement--across a wide array of knowledge, belief and communication predicates. The relevant distributional phenomena--which predicates are capable of embedding which complement types--have traditionally been captured by stipulative grammatical markings such as subcategorization frames, semantic selection frames and case-theoretic lexical markings. These theoretical tools, even to the extent that they (...) successfully reduplicate the observed phenomena, are of no explanatory value, as they do not derive the different behavioral patterns from some independently established properties of the embedding predicates or their complements. In the thesis, I present an epistemically-based theory, which attempts to derive the relevant distributional phenomena, including those traditionally assumed to reflect hard-core syntactic idiosyncrasies, from a linguistic-epistemic theory of knowledge, belief and communication. ;The theory is based on a lexical-semantic analysis of the embedding predicates, according to which they denote ternary relations between agents, objects of attitude and objects of representation. Thus, to give an example, the predicate think denotes a ternary relation between a thinker, a thought mentally represented by the thinker, and a proposition expressed by the thought and believed by the thinker to be true. This analysis, developed on linguistic grounds for linguistic purposes, echoes some of the recent insights in the philosophy of mind, especially Crimmins . Coupled with a theory of the meanings of the different complement types--partially based on the formal semantic insights of Karttunen , Groenedijk and Stokhof and Ginzburg --this account derives the distributional patterns from simple considerations of semantic compatibility: those predicates which are capable of embedding a certain complement type are exactly those whose lexical semantics is compatible with the semantics of the complement. ;Some of the distributional patterns discussed in the thesis, especially the one involving concealed questions, have traditionally been taken to provide empirical support for the hypothesis of the autonomy of syntactic representations . The analysis presented in this thesis reveals the dramatic extent to which the semantics and the syntax of lexical selection are transparent to each other, and thus implies, at least with respect to the core domain of lexical selection, that the autonomy hypothesis is empirically unjustified. Existential threat lies at the heart of intergroup conflict, but the literature on existential concerns lacks clear conceptualization and integration. To address this problem, we offer a new conceptualization and measurement of existential threat. We establish the reliability and validity of our measure, and to illustrate its utility, we examine whether different existential threats underlie the association between political ideology and support for specific political policies. Study 1 (N = 798) established the construct validity of the scale, and revealed four (...) distinct existential threats: personal death, physical collective annihilation, symbolic collective annihilation, and past victimization. Study 2 (N = 424) confirmed the 4-factor structure, and the convergent and discriminant validity of the scale. Study 3 (N = 170) revealed that the association between a hawkish political ideology and support for hardline policies was mediated by past victimization, whereas the association between a dovish political ideology and conciliatory policies was mediated by concerns over collective symbolic annihilation. Study 4 (N = 503) conceptually replicated the pattern of findings found in Study 3, and showed that at times of conflict, physical collective annihilation concerns also mediate the relationship between hawkish ideologies and support for hardline policies. In both Studies 3 and 4, when controlling for other threats, personal death did not play a significant role. These results underscore the need to consider the multidimensional nature of existential threat, especially in the context of political conflict. (shrink)

This article reflects on the received view of the rupture which constitutes the beginning of a critical, ethical, political and legal opening, the understanding of which inhabits the cry of, and response to, injustice. It takes the very critique that feeds into, and is distorted by, practical reasoning, as its point of departure. Grasping this rupture as the complementary relation between deconstruction and radical alterity, would entail unreflectively accepting a certain kind of truthfulness—truthfulness as [in]correctness, manifesting in a relationship that (...) involves rootless and controlling movement of making and unmaking of world. In closely reading Wittgenstein and Heidegger on the level of seeing, showing and saying, truthfulness is shown to contain an essential tension between, on the one hand, the Socratic, metaphysically-bound notion of beingness, correctness and meaning-steering and, on the other hand, the pre-Socratic notion of unconcealment (a-lethia), which, pointing even earlier than pre-Socratics into aboriginality, involves attentive letting of gliding in the inexpressible saying of language. While steering is about generating new possibilities of expressibility, gliding is about poetic dwelling, or enduring inexpressibility as a constitutive part of saying. Although aletheia is taken to be the key influence on rootless post-foundational thinking, it is argued that unconcealment involves letting and enduring the presencing inexpressibility of place and home-coming, that is, worlding-rootedness; thus showing Heidegger’s originary politics as the district of the uncanny to be about worlding that attentively lets the presencing inexpressibility of earth be as place. In reading Heidegger’s views on humanism, beginning and language, the argument links inexpressibility—essentially and historically—to the grasping of the belongingness together of world, earth and place, viewing this belongingness as key to both the saying of art and of mortals dwelling together temporally, spatially, materially in a manner always strange to, and nearer than, the steering/controlling of beingness, time, space and place that the very gesture and emergence of critique is captive of and is not capable of attuning to and capturing. Art always estranges the metaphysical cycle of correctness which preserves pain and suffering—a cycle that inhabits a double bind of responding to violence and injustice generated by the violence of metaphysics with metaphysical violence and justice. In showing essential strife within truthfulness itself, Heidegger points to even greater and earlier problematic than the pre-Socratics—to the painful core of inexpressibility between the ontology of steering time, spaces and material—steering places—and the gliding temporality, spatiality and materiality of ontology of place.
