Implicit and explicit memory tasks are interpreted within a traditional memory theory that distinguishes associations between different classes of memory units . Associations from specific sensory features to logogens are strengthened by perceptual experiences, leading to specific perceptual priming. Associations among concepts are strengthened by use, leading to specific conceptual priming. Activating associations from concepts to logogens leads to semantic and associative priming. Item presentation also establishes a new association from it to a representation of the personal context, comprising an (...) “episodic memory.” Such contextual associations play a major role in explicit memory tasks such as recall or recognition. A critical assumption of the theory is that presentation of a given item strengthens its sensory and contextual associations independently; this permits the theory to explain various dissociations of implicit and explicit memory measures. Furthermore, by assuming that brain-injured patients with global amnesia have a selective deficit in establishing novel associations to the context, the theory can explain their deficits in explicit memory along side their intact implicit memory. Increasing complexities facing physicians negotiating the bedside decision continue to fuel the debate over who is the appropriate party to offer ethics consults, should one be needed, during the decision-making process. Some very good arguments have been put forth on behalf of clinical ethicists as being the proper and best party to engage in ethics consultations. However, serious questions remain about the role of the clinical ethicist and his ability to provide the necessary level of objectivity called for in an (...) ethics consult.I argue that the clinician's professional psyche, or mode of thinking as a professional, leaves him little room to maneuver as an objective and detached third party ethics consultant. Several factors are cited and discussed that greatly influence the analyses applied to a case problem by physicians. The most formidable of these factors are habits and the practice of defensive medicine. I conclude that clinical ethicists are less suited for the overall tasks required of an objective consultant in medical cases that appear to involve insurmountable ethical issues. (shrink)

One approach to the analysis of ethical dilemmas in medical practice uses the “four principles plus scope” approach. These principles are: respect for autonomy, beneficence, non-maleficence and justice, along with concern for their scope of application. However, conflicts between the different principles are commonplace in psychiatric practice, especially in forensic psychiatry, where duties to patients often conflict with duties to third parties such as the public. This article seeks to highlight some of the specific ethical dilemmas encountered in forensic psychiatry: (...) the excessive use of segregation for the protection of others, the ethics of using mechanical restraint when clinically beneficial and the use of physical treatment without consent. We argue that justice, as a principle, should be paramount in forensic psychiatry, and that there is a need for a more specific code of ethics to cover specialised areas of medicine like forensic psychiatry. This code should specify that in cases of conflict between different principles, justice should gain precedence over the other principles. (shrink)
