Drawing on a landscape analysis of existing data-sharing initiatives, in-depth interviews with expert stakeholders, and public deliberations with community advisory panels across the U.S., we describe features of the evolving medical information commons. We identify participant-centricity and trustworthiness as the most important features of an MIC and discuss the implications for those seeking to create a sustainable, useful, and widely available collection of linked resources for research and other purposes.

Nous proposons dans cette contribution une lecture spinoziste de la recommandation de bonne pratique « Voir la personne derrière la pathologie ». Il nous a semblé intéressant, en effet, de nous poser la question suivante : lorsqu’on recommande de prendre soin de la personne qu’est le patient derrière l’organe malade, qu’est-ce que cela implique, d’un point de vue anthropologique et psychologique, pour la personne qu’est le soignant? Est-ce que cela ne risque pas d’être vécu comme une mise en danger personnelle, (...) pour celui qui est au contact de situations dramatiques au quotidien de sa pratique professionnelle? Ces questionnements nous invitent à suivre deux pistes réflexives. Premièrement, penser une nouvelle anthropologie éthique de la relation de soins, par-delà les grands principes désincarnés de la bioéthique. Deuxièmement, redonner une place au corps organique et affectif du soignant dans la relation de soins, ainsi qu’à ce que ce dernier sent de la situation au-delà ou aux côtés de l’imagerie et des analyses médicales. Ces pistes réflexives ont elles-mêmes donné lieu à des expérimentations pratiques : proposer aux soignants de faire l’expérience d’autres manières d’être affectés, et leur proposer de contribuer activement à une réflexion éthique participative sur leur pratique. Cela implique en retour de sortir de l’immédiateté des effets attendus, et d’inscrire l’éthique en santé dans une temporalité longue, dans un devenir sans cesse réalimenté. (shrink)

Shared decision making has become the standard of care, yet there remains no consensus about how it should be conducted. Most accounts are concerned with threats to patient autonomy, and they address the dangers of a power imbalance by foregrounding the patient as a person whose complex preferences it is the practitioner's task to support. Other corrective models fear that this level of mutuality risks abdicating the practitioner's responsibilities as an expert, and they address that concern by recovering a nuanced (...) but genuinely directive clinical role. Cribb and Entwistle helpfully categorize models of shared decision making as ‘narrower’ and ‘broader’ and praise the latter's ‘open‐ended and fully dialogical ways of relating’. However, they stop short of providing a philosophical account of how that dialogue works. In this paper, a nurse–midwife and a philosopher collaborate to argue that the Socratic model of dialogue offers a solution to the practitioner–patient dilemma. In the Theaetetus, Socrates compares dialogical reasoning to ‘midwifery with all its standard features’. By means of a three‐way analogy, elements of midwifery practice are used to illuminate features of Socrates' claim that his dialogue is like midwifery; those features are then translated into an approach to shared decision making as the ‘midwifery of good thinking’ which both midwives and physicians would do well to adopt. A key concept that emerges is the need for practitioners to make a risk‐confidence assessment of the particular content of any decision to appropriately modulate their role in the practice of shared decision making. This book explores Goethe's ethics of happiness and the role of resignation within them. Prandi has carefully separated autobiographical material from literary expository of these themes in order to clarify the misunderstanding that has resulted from relying on Goethe's fictional works to document his personal ethical convictions. The book aims in part at working out in detail the usefulness of Spinoza's Ethics in evaluating ethical views expressed in poetry and fiction; and in part at correcting erroneous and confused ideas about (...) Goethean resignation. Prandi studies the 'natural morality' Goethe developed and practiced, using Lucretius and Spinoza as models of influence. All three define the good as what makes people rationally happy; each has his own resignation model to offer. From a deep analysis of views on happiness and resignation, the author's discussion leads to some surprising new conclusions. (shrink)

BackgroundCommentators believe that the ethical decision-making climate is instrumental in enhancing interprofessional collaboration in intensive care units. Our aim was twofold: to determine the perception of the ethical climate, levels of moral distress, and intention to leave one's job among nurses and physicians, and between the different ICU types and determine the association between the ethical climate, moral distress, and intention to leave.MethodsWe performed a cross-sectional questionnaire study between May 2021 and August 2021 involving 206 nurses and physicians in a (...) large urban academic hospital. We used the validated Ethical Decision-Making Climate Questionnaire and the Measure of Moral Distress for Healthcare Professionals tools and asked respondents their intention to leave their jobs. We also made comparisons between the different ICU types. We used Pearson's correlation coefficient to identify statistically significant associations between the Ethical Climate, Moral Distress, and Intention to Leave.ResultsNurses perceived the ethical climate for decision-making as less favorable than physicians. They also had significantly greater levels of moral distress and higher intention to leave their job rates than physicians. Regarding the ICU types, the Neonatal/pediatric unit had a significantly higher overall ethical climate score than the Medical and Surgical units and also demonstrated lower moral distress scores and lower “intention to leave” scores compared with both the Medical and Surgical units. The ethical climate and moral distress scores were negatively correlated ; moral distress and "intention to leave" was positively correlated ; and ethical climate and “intention to leave” were negatively correlated.ConclusionsSignificant differences exist in the perception of the ethical climate, levels of moral distress, and intention to leave between nurses and physicians and between the different ICU types. Inspecting the individual factors of the ethical climate and moral distress tools can help hospital leadership target organizational factors that improve interprofessional collaboration, lessening moral distress, decreasing turnover, and improved patient care. A 2011 National Academies of Sciences report called for an “Information Commons” and a “Knowledge Network” to revolutionize biomedical research and clinical care. We interviewed 41 expert stakeholders to examine governance, access, data collection, and privacy in the context of a medical information commons. Stakeholders' attitudes about MICs align with the NAS vision of an Information Commons; however, differences of opinion regarding clinical use and access warrant further research to explore policy and technological solutions.

The ambition of the "ethical anthropology" established by the author is to think about ethics as a process of becoming by way of a new and precise reading of Spinoza and to give a significant place to affects, desire, imagination, personal history, and encounters.

Antibiotic resistance has become a problem of global scale. Resistance arises through mutation or through the acquisition of resistance gene(s) from other bacteria in a process called horizontal gene transfer (HGT). While HGT is recognized as an important factor in the dissemination of resistance genes in clinical pathogens, its role in the environment has been called into question by a recent study published in Nature. The authors found little evidence of HGT in soil using a culture‐independent functional metagenomics approach, which (...) is in contrast to previous work from the same lab showing HGT between the environment and human microbiome. While surprising at face value, these results may be explained by the lack of selective pressure in the environment studied. Importantly, this work suggests the need for careful monitoring of environmental antibiotic pollution and stringent antibiotic stewardship in the fight against resistance. (shrink)

In my article, “Why Psychology Hasn’t Kept Its Promises” , I argued that psychology hasn’t become the science its practitioners had hoped because psychologists continue to focus on mentalistic constructs and they adhere to a methodology that emphasizes statistical inference over experimental analysis. I concluded that in order to better keep their promise of a psychological science, psychologists should return to studying the relationship between observed behavior and its context with the type of experimental analysis that characterizes the other experimental (...) sciences. In his reply, Lana suggests that there may be aspects of human social and verbal behavior that are so complex that we may not be able to carry out a solid experimental analysis, thus limiting what we can discover about our own nature. Lana concludes that the methodologies needed to understand these more complex social relationships are hermeneutic and historical rather than experimental in nature. I concur with Lana both that an experimental analysis of much of human behavior may not be possible and that psychologists must, therefore, rely on complementary descriptive, interpretive, and historical analyses. I argue, however, that the interpretive language and the historical hindsight must be based on a foundation of basic principles derived from the systematic experimental analysis of behavior. (shrink)
