How do we teach medical students to protect patients? My initial reaction to the question posed by Taylor and Goodwin was like first glimpsing ‘Jaws’: we’re going to need a bigger boat. The authors’ answer makes two important claims: first, that safety should be ethically sourced by better integration between teaching of safety and ethics; second, that teaching should encourage students to think about organisational failure rather than focusing on individual blame and personal responsibility to whistleblow.1 On the first, they (...) highlight how ethics teaching often sits apart from that on safety. For the second, they use the concept normalisation of deviance, with its contained idea of structural secrecy, to depict organisational failure that is argued as largely immune to whistleblowing. Why wouldn’t these challenges warrant a bigger boat? On their first claim: it is not just for safety, but throughout the curriculum that ethics teaching needs to be integral. Given coming challenges, it should become the cake rather than the icing. Consider, for example, that artificial intelligence stands to transform many knowledge-based professions, medicine among them. A biography of the seventeenth-century French mathematician and philosopher whose revolutionary methods of reasoning greatly influenced future scientific thinking.

Two decades of attempts to model the emergence of language as a collective cognitive activity have demonstrated a number of principles that might have been part of the historical process that led to language. Several models have demonstrated the emergence of structure in a symbolic medium, but none has demonstrated the emergence of the capacity for symbolic representation. The current shift in cognitive science toward theoretical frameworks based on embodiment is already furnishing computational models with additional mechanisms relevant to the (...) emergence of symbolic language. An analysis of embodied interaction among captive, but not human‐enculturated, bonobo chimpanzees reveals a number of additional features of embodiment that are relevant to the emergence of symbolic language, but that have not yet been explored in computational simulation models; for example, complementarity of action in addition to imitation, iconic in addition to indexical gesture, coordination among multiple sensory and perceptual modalities, and the orchestration of intra‐ and inter‐individual motor coordination. The bonobos provide an evolutionarily plausible intermediate stage in the development of symbolic expression that can inform efforts to model the emergence of symbolic language. (shrink)

Of course, the rationale of PME is so different from what has been taught in “orthodox” statistics courses for fifty years, that it causes conceptual hangups for many with conventional training. But beginning students have no difficulty with it, for it is just a mathematical model of the natural, common sense way in which anybody does conduct his inferences in problems of everyday life.The difficulties that seem so prominent in the literature today are, therefore, only transient phenomena that will disappear (...) automatically in time. Indeed, this revolution in our attitude toward inference is already an accomplished fact among those concerned with a few specialized applications; with a little familarity in its use its advantages are apparent and it no longer seems strange. Nietzche claimed that once we know why to live, we’ll suffer almost any how.1 Artificial intelligence (AI) is used widely for the how, but Ferrario et al now advocate using AI for the why.2 Here, I offer my doubts on practical grounds but foremost on ethical ones. Practically, individuals already vacillate over the why, wavering with time and circumstance. That AI could provide prosthetics (or orthotics) for human agency feels unrealistic here, not least because ‘answers’ would be largely unverifiable. Ethically, (...) the concern is that AI stands to frack our humanity. We form a fragile ecosystem of ethical subjects, our responsiveness to others’ suffering, enabled by our own. To deliberate together for incapacitated others is among those solemn privileges that verify our humanity. Having AI mine these delicate pain-forests risks treating our suffering as the new oil—to be extracted and exploited, but beyond our vision and at our cost. Let’s briefly develop each idea, starting with the how/why distinction. This is palpable, even for more prosaic questions like how or why to drive. The former admits of increasingly sophisticated technological fixes and nudge; the latter often remains very particular and personal. How much greater then, the difference between …. (shrink)

Video recording is widely available in modern operating rooms. Here, I argue that, if patient consent and suitable technology are in place, video recording of surgery is an ethical duty. I develop this as aduty to protect,arguing for professional and institutional duties, as distinguished forduties of rescue.A professional duty to protect is described in mental healthcare. Practitioners have to take reasonable steps to prevent serious, foreseeable harm to their clients and others, even if that entails a non-consensual breach of confidentiality. (...) I argue surgeons have a similar duty to patients which means that, provided the patient consents, surgery should be routinely videoed. This avoids non-consensual breaches of patient confidentiality and is aligned with stated professional obligations.An institutional duty to protect means institutions have to take reasonable steps to prevent serious, foreseeable harm at the hands of their surgeons. Rulli and Millum highlighted how institutions can meet their duty using a more consequentialist approach that balances wider interests.To test the force and scope of such duties, I examine potential impacts of routine videoing on aspects of autonomy, justice, beneficence and non-maleficence. I find routine videoing can benefit areas including safety, candour, consent and fairness in access (to surgical careers and expertise). Countervailing claims, for example, on liability, confidentiality and privacy can be resisted—such that where consent and the technology are in place, routine videoing meets aduty of easy protection. This paper reports two cases in Hong Kong involving two native Chinese adolescent cancer patients (APs) who were denied their rights to consent to necessary treatments refused by their parents, resulting in serious harm. We argue that the dynamics of the ‘AP‐physician‐family‐relationship’ and the dominant role Chinese families play in medical decision‐making (MDM) are best understood in terms of the tendency to hierarchy and parental authoritarianism in traditional Confucianism. This ethic has been confirmed and endorsed by various Chinese writers from (...) Mainland China and Hong Kong. Rather than giving an unqualified endorsement to this ethic, based more on cultural sentimentalism than rational moral reasoning, we warn that a strong familism in MDM, which deprives ‘weak’ family members of rights, represents the less desirable elements of this tradition, against which healthcare professionals working in this cultural milieu need to safeguard. Specifically for APs, we suggest that parental authority and family integrity should be re‐interpreted in terms of parental responsibility and the enhancement of children's interests respectively, as done in the West. This implies that when parents refuse to consent to necessary treatment and deny their adolescent children's right to consent, doctors, as the only remaining advocates of the APs' interest, have the duty to inform the state, which can override parental refusal to enable the doctors to fulfill their professional and moral obligations. In so doing the state exercises its ‘parens patriae’ power to defend the defenseless in society and the integrity of the medical profession. (shrink)

This paper presents a probabilist paraconsistent theory of belief revision. This theory is based on a very general theory of probability, that fits with a wide range of classical and nonclassical logics. The theory incorporates a version of Jeffrey conditionalisation as its method of updating. A Dutch book argument is given, and the theory is applied to the problem of choosing a logical system.

The logic CE (for "Classical E") results from adding Boolean negation to Anderson and Belnap's logic E. This paper shows that CE is not a conservative extension of E.

ABSTRACT Objectives: To investigate whether Chinese adolescents in Hong Kong share similar perceptions with their Western counterparts regarding their capacity for autonomous decision‐making, and secondarily whether Chinese parents underestimate their adolescent children's desire and capacity for autonomous decision‐making. Method:‘Healthy Adolescents’ and their parents were recruited from four local secondary schools, and ‘Sick Adolescents’ and their parents from the pediatric wards and outpatient clinics. Their perceptions of adolescents' understanding of illnesses and treatments, maturity in judgment, risk‐taking, openness to divergent opinions, pressure (...) from parents and doctors, submission to parental authority and preference for autonomy in medical decision‐making are surveyed by a 50‐item questionnaire on a five‐point Likert scale. Results: Findings indicate that Chinese adolescents aged 14–16 perceive themselves to possess the necessary cognitive abilities and maturity in judgment to be autonomous decision‐makers like their Western counterparts. Paradoxically, although they hesitate to assert their autonomy, they are also unwilling to surrender that autonomy to their parents even under coercion or intimidation. Parents tend to underestimate their adolescents' preferences for making autonomous decisions and overestimate the importance of parental authority in decision‐making. Conclusion:‘14‐and‐above’ Chinese adolescents in Hong Kong perceive themselves as capable of autonomous decision‐making in medically‐related matters, but hesitate to assert their autonomy, probably because of the Confucian values of parental authority and filial piety that are deeply embedded in the local culture. Locke is often credited with having refuted the Cartesian account of the identity of persons, which locates their identity in the identity of immaterial substance. J. L. Mackie speaks for many when he writes that “Locke makes out a strong case for both his negative theses, that personal identity is to be equated neither with the identity of a soul-substance nor with that of a man …”. I will argue here that Locke’s attack on the immaterial substance theory is, if (...) viewed in the standard way, not only unsuccessful against the Cartesian theory but also unsuccessful even in its own terms. We will see, however, that Locke mingles this line of argument with another one which has not to my knowledge been remarked in the literature on Locke on personal identity; this line of argument is based on a deep agnosticism about the nature of thinking substance, and of substance generally, and it does succeed against a wide range of immaterial substance theories, but not, still, the Cartesian one. At the end of the paper I will make use of these results to sketch how two quite distinct strains of seventeenth-century thought about immaterial substance may be disentangled from each other. This should make us wary of the rather perfunctory characterizations of dualism, and particularly Cartesian dualism, which have been popular recently. (shrink)

The challenges of today's military operations place a new imperative upon Professional Military Education (PME) to maximize the quality of instruction on ethics in terms of both content and effectiveness. This volume presents the refined proceedings of two conferences of the European Forum on Military Pedagogy dealing with ethical issues of teaching and learning in (...) PME.
